1.Glucocorticoid Discontinuation in Patients with Rheumatoid Arthritis under Background of Chinese Medicine: Challenges and Potentials Coexist.

OBJECTIVE:
To evaluate the dynamic changes of glucocorticoid (GC) dose and the feasibility of GC discontinuation in rheumatoid arthritis (RA) patients under the background of Chinese medicine (CM).
METHODS:
This multicenter retrospective cohort study included 1,196 RA patients enrolled in the China Rheumatoid Arthritis Registry of Patients with Chinese Medicine (CERTAIN) from September 1, 2019 to December 4, 2023, who initiated GC therapy. Participants were divided into the Western medicine (WM) and integrative medicine (IM, combination of CM and WM) groups based on medication regimen. Follow-up was performed at least every 3 months to assess dynamic changes in GC dose. Changes in GC dose were analyzed by generalized estimator equation, the probability of GC discontinuation was assessed using Kaplan-Meier curve, and predictors of GC discontinuation were analyzed by Cox regression. Patients with <12 months of follow-up were excluded for the sensitivity analysis.
RESULTS:
Among 1,196 patients (85.4% female; median age 56.4 years), 880 (73.6%) received IM. Over a median 12-month follow-up, 34.3% (410 cases) discontinued GC, with significantly higher rates in the IM group (40.8% vs. 16.1% in WM; P<0.05). GC dose declined progressively, with IM patients demonstrating faster reductions (median 3.75 mg vs. 5.00 mg in WM at 12 months; P<0.05). Multivariate Cox analysis identified age <60 years [P<0.001, hazard ratios (HR)=2.142, 95% confidence interval (CI): 1.523-3.012], IM therapy (P=0.001, HR=2.175, 95% CI: 1.369-3.456), baseline GC dose ⩽7.5 mg (P=0.003, HR=1.637, 95% CI: 1.177-2.275), and absence of non-steroidal anti-inflammatory drugs use (P=0.001, HR=2.546, 95% CI: 1.432-4.527) as significant predictors of GC discontinuation. Sensitivity analysis (545 cases) confirmed these findings.
CONCLUSIONS
RA patients receiving CM face difficulties in following guideline-recommended GC discontinuation protocols. IM can promote GC discontinuation and is a promising strategy to reduce GC dependency in RA management. (Trial registration: ClinicalTrials.gov, No. NCT05219214).

2.Clinical treatment strategy for pT3N0 laryngeal squamous cell carcinoma.

Objective:To investigate optimal treatment strategy for pT3N0 laryngeal squamous cell carcinoma(SCC). Methods:A retrospective study of 150 patients with pT3N0 laryngeal SCC treated in the First Affiliated Hospital of Chongqing Medical University was performed. The efficacies of partial laryngectomy and total laryngectomy, as well as surgery alone and postoperative radiotherapy were evaluated. The overall survival(OS), disease specific survival(DSS) and disease-free survival(DFS) were analyzed with statistical package from SPSS. Results:Among the 108 patients with glottic laryngeal SCC, there were no significant differences in OS, DSS and DFS between the partial laryngectomy group and the total laryngectomy group(Log-rank=0.184, 0.010 and 0.051, P>0.05). Similarly, there were no significant differences in OS, DSS and DFS between the surgery-alone group and postoperative radiotherapy group(Log-rank=0.214, 0.251 and 0.003, P>0.05). Among the 38 patients with supraglottic laryngeal SCC, the OS in the total laryngectomy group was significantly higher than that in the partial laryngectomy group(Log-rank=7.338, P=0.007). The DSS and DFS in the total laryngectomy group were higher than in the partial laryngectomy group, but the differences were not statistically significant(Log-rank=0.895 and 1.792; P>0.05). The DFS in the postoperative radiotherapy group was significantly higher than in the surgery-alone group(Log-rank=7.172, P=0.007), but there were no significant differences in OS and DSS between these two groups(Log-rank=0.010 and 0.876, P>0.05). Conclusion:For pT3N0 glottic laryngeal cancer patients, the efficacy of partial laryngectomy is comparable to total laryngectomy, same as surgery alone and postoperative radiotherapy. For pT3N0 supraglottic laryngeal cancer patients, total laryngectomy could improve the overall survival, and postoperative radiotherapy could reduce the recurrence. Prospectively randomized study with large samples is still needed.

3.Advances in oral distant targeted nanodelivery systems

Due to patient compliance and convenience, oral medication is likely the most common and acceptable method of drug administration. However, traditional dosage forms such as tablets or capsules may lead to low drug bioavailability and poor therapeutic efficiency. Therefore, with advancements in material science and micro/nano manufacturing technology, various carriers have been developed to enhance drug absorption in the gastrointestinal tract. In this context, we initially discuss the key biological factors that hinder drug transport and absorption (including anatomical, physical, and biological factors). Building on this foundation, recent progress in both conventional and innovative oral drug delivery routes aimed at improving drug bioavailability and targeting is reviewed. Finally, we explore future prospects for oral drug delivery systems as well as potential challenges in clinical translation.
4.The shared mechanisms of three common chronic diseases and the discovery of traditional Chinese medicine from the perspective of ageing

Objective To explore the shared mechanisms of genes related to three common chronic diseases non-alcoholic fatty liver disease(NAFLD),type 2 diabetes mellitus(T2DM)and atherosclerosis(AS)with ageing as well as potential therapeutic agents by using bioinformatics analysis,machine learning algorithms and molecular docking methods and techniques.Methods Ageing-related genes were collected and organized from AgeingAtlas,CellAge,GenAge,and MSigDB databases.After taking the intersection of genes related to NAFLD,T2DM and AS obtained from databases such as CTD,DisGeNET,GeneCards,OMIM,PharmGKB,and TTD and the gene sets obtained based on the GEO differential gene analysis,we obtained the set of related disease genes for these three common chronic diseases.The KEGG pathway enrichment analysis was performed on the ageing gene set and the three disease-related gene sets using the clusterProfiler package,and the intersection was taken.The enriched genes in the KEGG pathway were merged and imported into the STRING database;the PPI network was constructed.We analyzed the core sub-modules in the PPI network using the MCODE tool and calculated the essential values of Nim and Cim for each node and module.Meanwhile,three machine learning models were used to screen the feature genes:the Lasso regression model,the Boruta algorithm,and the random forest model.The HIT2.0 database was utilized to find the targeted TCM small molecules related to the key feature genes.Small molecules were evaluated and analyzed by ADMET using SwissADME and ADMETlab 3.0 online system.The molecular docking method was utilized to dock the key action targets and screen small molecules.Results A total of 1 325,616,78,and 597 genes related to ageing,NAFLD,T2DM,and AS were obtained.The KEGG pathway enrichment analysis results for ageing and the three diseases were taken to intersect to get two shared intersecting pathways containing 243 genes.The PPI network was constructed,and Cluster 2 had the highest Cim value among the three core submodules.According to the results of signature gene screening,combined with PPI network module analysis results,four signature genes related to ageing were found:CDK6,CDKN1A,MYC,and PTEN.These four targets have 94 potential TCM small molecule candidates,among which resveratrol(RSV)is a TCM small molecule common to these four targets.The ADMET evaluation showed that it had good drug-forming properties.The PTEN target had a high Nim value,and molecular docking of RSV with PTEN showed good binding stability.Conclusion A potential herbal small molecule,RSV,was identified from the perspective of ageing,which may prevent and treat three common chronic diseases,namely,NAFLD,T2DM and AS,by regulating the key gene PTEN.
5.Effect of functional electrical stimulation hand cycling on functional connection of brain networks in stroke pa-tients:a study based on functional near-infrared spectroscopy

Objective To explore the potential central mechanisms of functional electrical stimulation(FES)hand cycling in promot-ing upper limb motor recovery after stroke.Methods A total of 35 stroke patients hospitalized in the Rehabilitation Center of Changzhou De'an Hospital from May,2023 to December,2024 were enrolled.They sequentially completed 10-minute FES hand cycling and 10-minute sham stimulation(simple hand cycling)tasks.The task order was randomized via dice rolling.Functional near-in-frared spectroscopy(fNIRS)was used to monitor real-time cerebral hemodynamic changes during both tasks to calculate the overall functional connectivity(FC)strength and the FC strength within regions of interest.Results The overall FC strength was higher during the FES hand cycling task than during the sham task(t=2.591,P<0.05),as well as FC between the ipsilateral primary motor cortex(iM1)and ipsilateral somatosensory association cortex(iSAC),iM1 and contralateral somatosensory association cortex(cSAC),ipsilateral pre-motor and supple-mentary motor cortex(iPMC)and iSAC,iPMC and cSAC,iPMC and ipsilateral primary somatosensory cortex(iS1),iPMC and iM1,and contralateral pre-motor and supplementary motor cortex(cPMC)and iSAC(PFDR<0.05).Conclusion FES hand cycling can promote the cortical function remodeling between cerebral hemispheres and the af-fected hemisphere,to integrate sensory-motor function.
6.Role and mechanism of trpc6 in mice with ka-induced epileptogenesis

Aim To investigate the role and underlying mechanisms of transient receptor potential cation chan-nel 6(TRPC6)in epileptogenesis using a kainic acid(KA)-induced mouse model.Methods C57BL/6 and TRPC6-KO(KO)mice were divided into two groups and implanted with cannulas for microinjection of KA(0.03 g·L-1,5 μL)into the lateral ventricle to establish the acute epilepsy model group,with saline injection serving as the control group.The Racine score was used to record the uninterrupted seizure grade of mice within two hours after KA administration.Immunohistochemistry was used to detect neuronal loss and tissue damage in the hippocampus brain region of mice.Immunofluorescence staining,Western blot and qPCR were used to detect the expressions of TRPC6,NLRP3,ASC,Caspase-1,p62,Atg7,Atg5,Beclin-1,LC3b-Ⅱ/LC3b-Ⅰ.in the hippocampus.Results KA induced significant neuronal loss and tissue damage in the hippocampal CA3 brain region of epilep-sy mice,while the expression levels of TRPC6,NL-RP3,ASC and Caspase-1 and other proteins in the hippocampus brain area of epilepsy mice increased,and the protein expression of autophagy-related proteins Atg7,Atg5,Beclin-1,LC3b-Ⅱ/LC3b-Ⅰ increased,while the expression of p62 protein decreased.TRPC6 knockout exacerbated KA-induced epileptogenesis,neuronal injury,inflammatory response and autophagy activation.Conclusion TRPC6 is involved in KA-in-duced epileptigenesis,and the mechanism may be re-lated to the activation of NLRP3 inflammasome-autoph-agy signaling caused by TRPC6 deletion.

10.Combining balloon catheter expansion with swallowing training can better improve the swallowing of tracheotomy patients after pontine hemorrhage

Objective:To observe any effect of supplementing basic swallowing training with balloon catheter dilation on the swallowing function of tracheostomy patients with pontine hemorrhage.Methods:A total of 40 pontine hemorrhage patients with tracheostomy and swallowing disorders were divided randomly into an observation group and a control group, each of 20. Both groups were given nutritional neurodrugs and basic swallowing training, but the observation group also received 25 minutes of balloon catheter dilation, five times a week for 6 weeks. Before and after the 6 weeks of treatment one swallowing therapist evaluated the feeding ability and leakage-aspiration status of each subject assigning functional oral intake (FOIS) ratings and Rosenbek Leakage/Aspiration Rating Scale (PAS) ratings double-blinded. The Watian water swallowing test was also applied.Results:After the treatment the average FOIS and PAS scores of both groups had improved significantly, with those of the observation group then significantly better than among the control group on average. The total treatment effectiveness rate was 70% in the observation group, significantly better than the 30% in the control group.Conclusion:Supplementing swallowing training with balloon catheter dilation can better improve the swallowing of patients recovering from a tracheotomy after pontine hemorrhage.
