OBJECTIVE:
To investigate the effects of Huayu Tongluo (transforming stasis and unblocking collaterals) moxibustion on cognitive function and insulin resistance in patients with type 2 diabetes mellitus (T2DM) and cognitive decline.
METHODS:
Ninety patients with T2DM and cognitive decline were randomly divided into a moxibustion group (n=45, 3 cases dropped out, 2 cases were eliminated) and a waiting moxibustion group (n=45, 2 cases dropped out). Both groups received routine hypoglycemic treatment for 12 weeks. The moxibustion group additionally received Huayu Tongluo moxibustion at Baihui (GV20), Shenting (GV24), and Dazhui (GV14). Pressing moxibustion was applied to Baihui (GV20) for 20 min, while suspended moxibustion was applied to Shenting (GV24) and Dazhui (GV14) for 20 min each. Treatments of moxibustion were administered every other day (three times per week) for 12 weeks. All patients were followed up for 12 weeks, during which their original hypoglycemic medication regimen was maintained. Before treatment, after 12 weeks of treatment, and at the 12-week follow-up, the scores of Montreal cognitive assessment (MoCA), mini-mental state examination (MMSE), Addenbrooke's cognitive examination Ⅲ (ACE-Ⅲ), symbol digit modalities test (SDMT), and Athens insomnia scale (AIS) and the insulin resistance index (HOMA-IR) were observed in the two groups.
RESULTS:
Compared with before treatment, the MoCA scores, MMSE scores, ACE-Ⅲ subscale scores (attention, memory, language fluency, language, visuospatial ability) and total scores, and SDMT scores were increased (P<0.01), while the AIS scores were decreased (P<0.05) in the moxibustion group after treatment and at follow-up. Compared with before treatment, the MMSE score, ACE-Ⅲ subscale scores (memory, attention) and total score after treatment, as well as the ACE-Ⅲ subscale scores (language, memory, attention) and total score, and SDMT score at follow-up were increased (P<0.05, P<0.01) in the waiting moxibustion group. Compared with before treatment, HOMA-IR was decreased in both groups after treatment and at follow-up (P<0.01). At follow-up, ACE-Ⅲ subscale scores (attention, memory), and the total score in the moxibustion group were lower than those after treatment (P<0.05, P<0.01), and the ACE-Ⅲ language subscale score, total ACE-Ⅲ score, and SDMT score in the waiting moxibustion group were higher than those after treatment (P<0.01, P<0.05). After treatment and at follow-up, compared with the waiting moxibustion group, the moxibustion group had higher MoCA scores, MMSE scores, SDMT scores, ACE-Ⅲ subscale scores (attention, memory, language fluency) and total scores (P<0.05, P<0.01), and lower HOMA-IR (P<0.05).
CONCLUSION
Huayu Tongluo moxibustion can effectively improve cognitive function in patients with T2DM and cognitive decline. This improvement may be associated with the reduction in insulin resistance.

To investigate the anti-atherosclerotic effects of dietary hesperidin and naringin on ather-osclerosis in rabbits.Twenty-four adult healthy male New Zealand Large White rabbits were ran-domly assigned to four groups based on one factor:control(Con),model(Mod),hesperidin(HP),and naringenin(NG),each containing six rabbits and housed in solitary cages.During the experi-mental period, the control group received normal feed;the model group was fed high-fat for 30 d to establish the atherosclerosis,(AS)model and continued to be fed high-fat feed;the hesperidin and naringenin groups were fed high-fat for 30 d and then underwent AS modeling, and at a later stage,150 mg of hesperidin(or naringenin)was added to the diets at a rate of 1.5 mg per kg of di-et,respectively,for 30 d.Relative body quality, blood lipids, oxidation, and inflammatory factor lev-els were all assessed. At the end of the test period, the test rabbits were sacrificed to obtain the common carotid artery for histopathological section HE staining,Oil red O staining, and Sirius red staining in order to observe vascular tissue structure, lipid and inflammatory cell infiltration, and e-lastic plate and elastic fiber damage. The results indicated that the relative body quality of the mod-el,HP and NG groups increased rapidly, and the difference with the control group within the same period was highly significant(P<0.01);The HP and NG interventions significantly decreased to-tal cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)levels(P<0.01),while also leading to a significant increase in high-density lipoprotein cholesterol(HDL-C)levels(P<0.01);MDA levels exhibited a significant decrease(P<0.01),while SOD levels showed a notable in-crease(P<0.01)following the HP and NG interventions; After HP and NG interventions,TNF-α and IL-1 were regressed(P<0.01)and IL-10 was significantly elevated(P<0.01);Compared with the control group, the model group showed approximately 41.8 and 2.5 fold thickening of the maximal endo-medial membrane, respectively, and the HP and NG interventions showed approxi-mately 13 and 15 fold thickening of the maximal endo-medial membrane, and both of the maximal medial membranes thickened by approximately 1.4 fold;Staining of pathological sections showed that HP and NG intervened to significantly reduce inflammatory cell infiltration, deposition of lipid components, slight thickening of the endothelium, and intact fibroblastic components. The results showed that the addition of HP and NG to diets could regulate blood lipids and exert anti-inflam-matory and antioxidant effects by inhibiting the expression of inflammatory factors and oxidizing factors, showing significant anti-AS effects, and the two effects were comparable.

Chinese Journal of Health Policy 2025;18(8):1-9
Objective:Accurately grasping the disability levels and characteristics of the elderly population in China is of significant importance for responding effectively to the care service needs of this demographic. Methods:Based on multi-wave data(2011-2023)from three nationally representative aging-related surveys CLHLS,CHARLS and CALSS,this study assessed, compared, and predicted the disability levels and characteristics of China's elderly population. Results:Direct estimates from existing survey data showed low reliability.Integrated multi-source projections indicate that the disability rate among China's population aged 65 and older will follow a phased upward trend, ranging approximately between 12.27%and 15.05%from 2025 to 2050,with the disabled elderly population growing continuously from 25.9 million to 58.64 million.Bathing, toileting, and dressing were the three activities with the highest disability rates.Discussion and Suggestions:Future efforts should focus on improving the construction of an elderly capacity assessment system and addressing the shortage of high-quality disability data.Research should emphasize the evaluation process of item-specific disability among older adults. It is essential to grasp the phased characteristics of changes in disability levels and identify critical windows of opportunity for response.Based on the item-specific disability characteristics of older adults, the focus of elderly care services, facilities, and product supply should be accurately targeted.

Objective To address the issue of motion misclassification caused by individual gait differences and fixed time window data extraction in motion intention recognition for intelligent lower limb prostheses, this study proposes a motion intention recognition method based on adaptive temporal alignment. Methods In lower limb motion analysis, for continuous gait cycle data,inter-class variability across different steady-state modes was utilized to detect gait pattern consistency through inter-cycle frame differencing. For samples identified as single steady-state modes, the dynamic time warping algorithm was introduced to align adjacent mo-tion sequences, thereby reducing individual variability.Haar wavelet 4-level decomposition was applied to ex-tract low-frequency coefficients for feature vector construction, and classification was performed using a support vector machine. The experimental protocol was designed as follows:three inertial measurement units were used to collect lower limb acceleration and angular velocity data from subjects performing thirteen locomotion modes. The test subjects included ten healthy participants and one transtibial amputee. The locomotion modes consisted of five steady-state modes(level walking, stair ascent, stair descent, ramp ascent, and ramp descent)and eight transition modes(mutual transitions between level walking and stair ascent/descent,as well as ramp ascent/de-scent).Results Simulation tests on ten healthy individuals and one amputee showed recognition accuracies of 99.24%and 100%for five steady-state modes, and 98.51%and 89.11%for all thirteen motion modes, respectively. Conclusion This study proposes an adaptive temporal alignment-based motion intention recognition method. The pro-posed approach effectively reduces the interference of individual gait variability on feature representation,en-hances the consistency and discriminability of gait features, and ultimately improves recognition performance.
