Aim To examine the pathogenesis of disul-fide death gene in vascular dementia(VD)by bioin-formatics analysis of disulfide death differentially ex-pressed genes(DEGs)combined with experimental verification. Methods The death DEGs of disulfide were screened and their correlation was analyzed. The VD patients data in the data set were analyzed by clus-tering and typing and gene set variation. The clustering risk of DEGs was tested with a nomogram model, and the optimal learning model was predicted. After the es-tablishment of VD rat model, water maze test,HE stai-ning and RT-qPCR detection were performed to verify the results of health information. Results Four DEGs including SLC7A11 were obtained, which had antago-nistic or synergistic interaction with each other. The genetic data could be divided into two subtypes with significant differences. After typing,VD disulfide DEGs were mainly concentrated in GnRH signaling pathways. The accuracy of the nomogram prediction model was high.Generalized linear was the best ma-chine learning model.Compared with the sham opera-tion group, the escape latency of rats in the model group was prolonged, the number of crossing platforms decreased, the relative mRNA expression levels of Slc3a2 and Slc7a11 decreased, and LRPPRC in-creased.Conclusions SLC7A11 and other disulfide death DEGs and its related GnRH signaling pathway may be an important part of the pathogenesis of VD di-sulfide death.SLC3A2,LRPPRC and SLC7A11 can be used as characteristic genes in the regulation of VD by disulfide death, which may affect VD progression through the regulation of disulfide death.

Objective To explore the role of secreted phosphoprotein 1(SPP1)and myeloid differentiation primary response 88(MYD88)in morphine-induced cardiomyocyte apoptosis. Methods A morphine addiction model was established in Sprague-Dawley(SD)rats.Twelve SD rats were randomly assigned to the normal saline(NS)group or the morphine-dependent(DAM)group.Histopathological analysis was employed to observe and compare myocardial tissue morphology between the two groups.Terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling(TUNEL)staining was performed to assess the number of apoptotic cells in each group. The expression levels of SPP1 and MYD88 were evaluated using immunohistochemistry.Quantitative real-time poly merase chain reaction(RT-qPCR)and Western blot were used to detect the mRNA and protein expression of SPP1,MYD88,Bax,Bcl2,Caspase-3,and Caspase-9.Simultaneously,Western blot analysis was used to detected the expression of Cleaved Caspase-3 and Cleaved Caspase-9 proteins. In vitro,SPP1 expression was knocked down in primary neonatal rat cardiomyocytes(NRCMs),and cells were divided into three groups:control(CON),morphine treated(DA),and shSPP1#3+DA.Cell viability was assessed using the CCK-8 assay, and apoptosis rates were determined by flow cytometry. Results HE and TUNEL staining of myocardial tissues from morphine-addicted SD rats revealed that, compared with the NS group, myofibrils in the DAM group exhibited partial disruption and a significant increase in apoptotic cells(P<0.05).Western blot and RT-qPCR analyses demonstrated that, relative to the NS group, the mRNA and protein levels of SPP1,MYD88,Bax,Caspase-3,and Caspase-9 were significantly upregulated in the DAM group(P<0.05),whereas Bcl2 expression was significantly downregulated at both mRNA and protein levels(P<0.05),and the protein expression levels of Cleaved Caspase-3 and Cleaved Caspase-9 were also increased.with all differences being statistically significant. In NRCMs following morphine intervention, cell viability in the DA group was markedly reduced compared to the CON group(P<0.05),accompanied by a signifi-cant increase in apoptosis rate(P<0.05).Consistently,Western blot and RT-qPCR results showed elevated mRNA and protein expression of SPP1,MYD88,Bax,Caspase-3,and Caspase-9 in the DA group(P<0.05),along with decreased Bcl2 expression(P<0.05).The protein expression levels of Cleaved Caspase-3 and Cleaved Caspase-9 were elevated simultaneously. In contrast, the shSPP1#3+DA group exhibited opposing trends compared to the DA group, with statistically sig nificant differences(P<0.05).Conclusion SPP1 and MYD88 play critical roles in mediating morphine-induced cardiomyocyte apoptosis, and silencing SPP1 has been shown to significantly reduce the extent of cardiomyocyte apoptosis following morphine exposure.

OBJECTIVE:
To analyze the effectiveness of single three-dimensional (3D)-printed microporous titanium prostheses and flap combined prostheses implantation in the treatment of large segmental infectious bone defects in limbs.
METHODS:
A retrospective analysis was conducted on the clinical data of 76 patients with large segmental infectious bone defects in limbs who were treated between January 2019 and February 2024 and met the selection criteria. Among them, 51 were male and 25 were female, with an age of (47.7±9.4) years. Of the 76 patients, 51 had no soft tissue defects (single prostheses group), while 25 had associated soft tissue defects (flap combined group). The single prostheses group included 28 cases of tibial bone defects, 11 cases of femoral defects, 5 cases of humeral defects, 4 cases of radial bone defects, and 3 cases of metacarpal, or carpal bone defects, with bone defect length ranging from 3.5 to 28.0 cm. The flap combined group included 3 cases of extensive dorsum of foot soft tissue defects combined with large segmental metatarsal bone defects, 19 cases of lower leg soft tissue defects combined with large segmental tibial bone defects, and 3 cases of hand and forearm soft tissue defects combined with metacarpal, carpal, or radial bone defects, with bone defect length ranging from 3.8 to 32.0 cm and soft tissue defect areas ranging from 8 cm×5 cm to 33 cm×10 cm. In the first stage, vancomycin-loaded bone cement was used to control infection, and flap repair was performed in the flap combined group. In the second stage, 3D-printed microporous titanium prostheses were implanted. Postoperative assessments were performed to evaluate infection control and bone integration, and pain release was evaluated using the visual analogue scale (VAS) score.
RESULTS:
All patients were followed up postoperatively, with an average follow-up time of (35.2±13.4) months. In the 61 lower limb injury patients, the time of standing, walk with crutches, and fully bear weight were (2.2±0.6), (3.9±1.1), and (5.4±1.1) months, respectively. The VAS score at 1 year postoperatively was significantly lower than preoperative one ( t=-10.678, P<0.001). At 1 year postoperatively, 69 patients (90.8%) showed no complication such as infection, fracture, prosthesis displacement, or breakage, and X-ray films indicated good integration at the prosthesis-bone interface. According to the Paley scoring system for the healing of infectious bone defects, the results were excellent in 37 cases, good in 29 cases, fair in 3 cases, and poor in 7 cases. In the single prostheses group, during the follow-up, there was 1 case each of femoral prostheses fracture, femoral infection, and tibial infection, with a treatment success rate of 94.1% (48/51). In lower limb injury patients, the time of fully bear weight was (5.0±1.0) months. In the flap combined group, during the follow-up, 1 case of tibial fixation prostheses screw fracture occurred, along with 2 cases of recurrent foot infection in diabetic patients and 1 case of tibial infection. The treatment success rate was 84.0% (21/25). The time of fully bear weight in lower limb injury patients was (5.8±1.2) months. The overall infection eradication rate for all patients was 93.4% (71/76).
CONCLUSION
The use of 3D-printed microporous titanium prostheses, either alone or in combination with flaps, for the treatment of large segmental infectious bone defects in the limbs results in good effectiveness with a low incidence of complications. It is a feasible strategy for the reconstruction of infectious bone defects.

10.Evaluation of the predictive performance for distant metastasis in gastric adenocarcinoma based on machine learning and the SEER database
Chongqing Medicine 2025;54(11):2643-2648
Objective To evaluate the performance of models constructed based on demographic infor-mation and tumor characteristics for predicting distant metastasis in gastric adenocarcinoma patients. Methods Data of 7 788 gastric adenocarcinoma patients from the Surveillance,Epidemiology, and End Results(SEER)database(2001-2021)were collected as the training set,and clinical data of 259 gastric adenocarci-noma patients from Nanchong Jialing District People's Hospital(January 2019 to October 2024)were collect-ed as the test set.Four machine learning algorithms were used to construct a risk prediction model for distant metastasis of gastric adenocarcinoma, and multivariate logistic regression was used to analyze the influencing factors of distant metastasis in patients with gastric adenocarcinoma. Results Distant metastasis was present in 801 cases(10.3%)in the training set and 25 cases(9.7%)in the test set.Multivariate logistic regression analysis showed that high differentiation, advanced T stage, and higher log odds of positive lymph nodes(LODDS)were risk factors for distant metastasis(P<0.05),while older age, primary tumor site at the gas-troesophageal junction, and higher total number of lymph nodes examined were protective factors(P<0.05).Validation on the test set using the four models showed that the predictive ability of the random forest model was superior to that of logistic regression,K-nearest neighbors(KNN),and support vector machine(SVM).The predictive ability of SVM was better than that of logistic regression and KNN,while logistic regression and KNN showed comparable performance.Delong's test showed that the predictive performance of the ran-dom forest model was superior to that of logistic regression,KNN,and SVM.Calibration curve analysis showed that the predictive accuracy of the random forest model was superior to that of logistic regression,KNN,and SVM.The top five important factors in the random forest model were negative lymph node count, tumor size, metastatic lymph node count,T stage and age. Conclusion The risk prediction model for distant metastasis in gastric adenocarcinoma constructed using the random forest algorithm demonstrats optimal per-formance.
