1.A Case of Multidisciplinary Treatment for Inflammatory Myofibroblastic Tumor Complicated by ANCA-Associated Vasculitis

A 51-year-old male presented with nasal obstruction, followed by progressive hearing loss and blurred vision. Imaging identified space-occupying lesions in the paranasal sinuses, orbits, and paraspinal regions, while laboratory tests confirmed positive anti-proteinase 3 anti-neutrophil cytoplasmic antibody(PR3- ANCA) immunoglobulin G (IgG)and markedly elevated serum IgG4. Despite treatment with corticosteroids, immunosuppressants, and radiotherapy, the patient exhibited steroid dependency with relentless disease progression. Following multidisciplinary consultation, a diagnosis of inflammatory myofibroblastic tumor (IMT) coexisting with ANCA- associated vasculitis (AAV) was favored, though IgG4-related disease remained a critical differential. Ultimately, profound immunosuppression precipitated a severe herpesvirus infection, leading to disseminated intravascular coagulation and multiple organ dysfunction syndrome. This case underscores the rarity and diagnostic complexity of concurrent IMT and AAV, highlights the therapeutic dilemma of balancing primary disease control against fatal opportunistic infections, and emphasizes the critical role of multidisciplinary collaboration in the diagnosis and treatment of complex diseases.
2.Research progress on health effects of triclosan and triclocarban

Triclosan (TCS) and triclocarban (TCC) are widely used synthetic broad-spectrum antibacterial agents that can enter the human body through the skin, gastrointestinal tract, and other pathways. More and more studies have found that exposure to TCS and TCC can affect human health, but currently, review reports on the health effects of human exposure to TCS and TCC are limited. Therefore, this study reviewed population studies on the relationship between TCS and TCC exposure and health effects by searching the PubMed database, summarized the associated health outcomes, and elucidated the biological mechanisms. A total of 56 studies were retrieved, among which cross-sectional studies (25 studies, 44.64%) and cohort studies (25 studies, 44.64%) accounted for a relatively large proportion, while case-control studies (6 studies, 10.72%) were relatively few. Studies on TCS exposure (48 studies, 85.71%) were far more prevalent than those on TCC exposure (2 studies, 3.57%). The remaining 6 studies involved both TCS and TCC exposure. The research results revealed that TCS exposure was associated with male and female abnormal reproductive functions, fetal growth restriction, abnormal behavior development in children, obesity, gestational diabetes mellitus (GDM), and immune-related diseases. Although the results of different studies show significant differences, they have indicated that exposure to TCS is a potential risk factor for these health problems. Due to the limited number of studies, the evidence for the relationship between TCC exposure and most of the aforementioned health effects is insufficient. Population studies and in vitro and in vivo studies have shown that exposure to TCS and TCC can interfere with the microbial homeostasis, the endocrine system, oxidative stress and immune function of the body, which are potential mechanisms causing adverse health effects. In the future, large-scale prospective cohort studies, as well as in vivo and in vitro studies, are still needed to further clarify the associations between TCS and TCC exposure and health effects, and to deeply explore its mechanism of action. These efforts will provide references for clarifying the human health hazards of TCS and TCC exposure and formulating targeted prevention and control strategies.
3.Analysis of factors influencing insufficient hyperopia reserve and refractive parameters in preschool children in Hefei

Objective:
To investigate the current status of refractive errors and insufficient hyperopia reserve in preschool children aged 3-6 years in Hefei and to analyze influencing factors, so as to provide a scientific basis for formulating targeted myopia prevention policies and comprehensive interventions.
Methods:
In May 2022, a stratified cluster random sampling method was used to select 897 preschool children from 8 kindergartens across four districts (Baohe, Yaohai, Shushan, and Economic and Technological Development Zone) in Hefei, and Children’s Visual Health related Behavior Assessment Scale was used to collect personal information and environmental factors. Pre and post cycloplegic refraction tests were conducted to assess insufficient hyperopic reserve and refractive development levels. Group comparisons were conducted using 2 test, t-test or analysis of variance. Multivariate regression analysis was performed to identify key factors influencing hyperopic reserve, axial length and spherical equivalent in preschool children.
Results:
The detection rates of refractive errors among preschool children were 6.8% for hyperopia, 1.6% for myopia, and 11.1% for astigmatism. Notably, the prevalence of myopia was significantly higher in boys (2.3%) than in girls (0.7%) ( χ 2=3.88, P <0.05). Additionally, 8.8% of the children exhibited insufficient hyperopic reserve. The results of multiple regression analysis showed that preschool children with high myopia in the father, high myopia in the mother, longer daily duration of near work, and longer daily electronic product use time had increased risks of axial growth ( β =0.12, 0.09, 0.15, 0.11), SE reduction ( β =-0.10, -0.07, -0.18, -0.13), and insufficient hyperopic reserve ( OR=1.87, 2.22, 1.40, 1.28) (P <0.05). While, preschool children with longer sleep time and daily outdoor activity duration had lower risks of axial growth ( β =-0.11, -0.10 ), SE reduction ( β =0.39, 0.51), and insufficient hyperopia reserve ( OR =0.54, 0.51) in preschool children ( P <0.05).
Conclusions
The rates of refractive errors and insufficient hyperopia reserve in preschool children in Hefei are relatively low, which are influenced by many factors. Parents, kindergartens and relevant departments should implement early vision monitoring and intervention for preschool children, and cultivate their scientific eye use habits.

Vaccine immunization is the most effective and cost-efficient method for infectious disease prevention and control.Since the outbreak of novel coronavirus pneumonia(caused by the novel coronavirus,COVID-19)at the end of 2019,third generation mRNA nucleic acid vaccines has been applied to stop viral spread.mRNA vaccines,rather than relying on the immune activation mode of traditional vaccines,are an innovative breakthrough using the body's own cells to produce antigens,thereby activating double specific immunity,forming immune memory,and providing more lasting specific immunity.Com-pared with the traditional first-generation(inactivated)and second-generation(genetically engineered)vaccines,mRNA vac-cines,because of the advantages provided by this platform,play important roles in the prevention and control of major sudden infectious diseases.Consequently,mRNA vaccines were the world's first COVID-19 vaccines to be applied clinically,thus ser-ving as a barometer in the field of vaccine research and development.Herein,the molecular design and presentation of mRNA vaccines,and the molecular mechanisms of their activation of the immune response are reviewed,to provide a theoretical basis for future application of novel mRNA vaccines in the prevention and control of animal infectious diseases.
5.Drug resistance characteristics and whole genome polymorphisms of group A Streptococcus in Jiangsu Province during 2016 to 2023

This study was aimed at understanding the relationships among the drug resistance and genome characteristics of group A Streptococcus in Jiangsu Province.A total of 149 group A Streptococcus strains were collected from hospitals between 2016 and 2023.Thirteen antimicrobial minimal inhibitory concentrations were detected with the micro-dilution broth method.The GAS strains were typed with emm genotyping analysis and whole genome sequencing,to determine the carriage rates of drug resistance genes and the evolutionary relationships among strains.The resistance rates of 149 GAS strains to erythromy-cin,tetracycline,and clindamycin exceeded 90%,whereas the strains showed sensitivity to 8 different antibiotics,including penicillin.Notably,the resistance rates to erythromycin,tetracycline,and clindamycin consistently increased over time.All strains were classified into 9 emm types,among which emm12 accounted for the highest proportion(77/149;51.68%).Signifi-cant statistical differences were observed among emm types,in terms of the drug resistance rate,number of resistant species,and prevalence of drug resistance genes.Furthermore,SNP evolutionary tree analysis revealed 3 distinct clusters within the GAS strains:emm12,emm1,and other emm types.emm 12 and emm1 were the dominant GAS strains in Jiangsu Province.Most isolates were resistant to erythromycin,tetracycline,and clindamycin.Differences in phenotypes and genomic characteris-tics were observed among emm types.
6.Predictive value of serum NINJ1,CysC and D-D for unfavorable outcomes in patients with acute myo-cardial infarction

Objective:To investigate the predictive value of serum levels of Ninjurin 1(NINJ1),cystatin C(CysC),and D-dimer(D-D)for unfavorable outcomes in patients with acute myocardial infarction(AMI).Methods:A total of 118 AMI patients hospitalized in the Chinese PLA Joint Logistic Support Force 901st Hospital between Feb-ruary 2020 and March 2023 were enrolled.According to prognosis within 6 months after treatment,patients were divided into favorable outcome group and unfavorable outcome group.Serum levels of CysC,D-D and NINJ1 were measured upon admission.Multivariate Logistic regression analysis was conducted to identify the factors influencing unfavorable outcome in AMI patients.Receiver operating characteristic(ROC)curves were used to analyze the pre-dictive value of serum NINJ1,CysC and D-D for the prognosis of AMI patients.A nomogram model for predicting unfavorable outcome in AMI patients was constructed and verified.Results:Compared to patients with favorable outcomes,those with unfavorable outcomes had significantly higher proportions of hypertension,diabetes,number of diseased coronary arteries ≥2,and serum levels of NINJ1,CysC and D-D,as well as a significantly lower pro-portion of Killip class Ⅰ-Ⅱ(P<0.01 all).Multivariate Logistic regression analysis showed that number of diseased coronary arteries ≥2(OR=3.817,95%CI 1.140~12.779,P=0.030),Killip class Ⅲ-Ⅳ(OR=5.639,95%CI 1.706~18.637,P=0.005),hypertension(OR=4.188,95%CI 1.392~12.600,P=0.011),diabetes(OR=3.480,95%CI 1.148~10.552,P=0.028),and serum levels of NINJ1(OR=1.055,95%CI 1.009~1.103,P=0.018),CysC(OR=75.232,95%CI 5.537~545.908,P=0.003),and D-D(OR=56.163,95%CI 1.610~958.586,P=0.026)were significantly associated with unfavorable outcomes at 6 months in AMI patients.ROC a-nalysis showed that the AUC of serum NINJ1,CysC and D-D combined predicting unfavorable outcomes was 0.801(95%CI 0.718~0.869),which was significantly higher than those of serum NINJ1(0.675,95%CI 0.582~0.758)and D-D(0.676,95%CI 0.583~0.759)alone(Z=2.344,P=0.019;Z=2.173,P=0.030,),but the difference was not significant comparing to CysC alone(Z=1.856,P=0.064).The C-index of the nomogram model predic-ting unfavorable outcomes was 0.973(95%CI 0.967~0.986),and the predicted values of calibration curve were consistent with the actual values,indicating good prediction accuracy.Conclusion:Elevated serum levels of NINJ1,CysC and D-D in AMI patients were significantly associated with unfavorable outcomes.Combined detection of these indicators may accurately predict unfavorable outcomes compared to single detection,and the constructed no-mogram model exhibits good prediction efficacy.
7.I-125 seed implantation improves the prognosis of pancreatic cancer patients treated with anti-PD-1 combined chemotherapy

Objective To evaluate the prognosis and safety of patients with advanced pancreatic ductal adenocarcinoma(PDAC)who received I-125 seed implantation in treatment with anti-PD-1 monoclonal antibody+chemotherapy.Methods A retrospective analysis was conducted on patients with stage Ⅳ metastatic PDAC who received anti-PD-1 combined chemotherapy treatment at Yixing Hospital,Jiangsu University from Jan 2021 to Jun 2023.Patients were divided into two groups based on whether they received I-125 seed implantation:the I-125 seed implantation+anti-PD-1 monoclonal antibody+Chemotherapy group(IPC group)and the anti-PD-1 monoclonal antibody+chemotherapy group(PC group).The follow-up period ranged from 2 to 24 months,with a median follow-up time of 9 months.The prognosis of patients was analysed in combination with peripheral blood biomarkers.The peripheral lymphocyte subsets of patients in different treatment groups were preliminarily analysed by flow cytometry.Results A total of 13 patients were included,with 5 in the IPC group and 8 in the PC group.Progression-free survival(PFS)and overall survival(OS)in the IPC group were significantly longer than those in the PC group.The treatment in the IPC group was relatively safe,adverse reactions were controllable.The neutrophil-lymphocyte ratio(NLR)and CD4/CD8 ratio indicated that the prognosis of the IPC patients was better.The levels of regulatory T cells(Treg)and active regulatory T cells(aTreg)cells in the IPC patients were reduced after treatment compared with those of the PC patients.Conclusion The addition of I-125 seed implantation can improve the prognosis of patients with advanced PDAC who receive anti-PD-1 monoclonal antibody+chemotherapy,the post-treatment levels of patients'circulating aTreg cells are reduced,and the combination therapy has good safety.
8.Impact of metabolic syndrome on perioperative and long-term outcomes after radical resection for perihilar cholangiocarcinoma

Background and Aims:Perihilar cholangiocarcinoma(pCCA)is associated with poor prognosis.Radical resection remains the mainstay of treatment;however,high recurrence rates and limited overall survival(OS)after surgery.Metabolic syndrome(MetS)has been linked to unfavorable outcomes in various malignancies,but its impact on postoperative outcomes in pCCA is unclear.This study aimed to evaluate the influence of MetS on perioperative and long-term outcomes in patients undergoing radical resection for pCCA.Methods:A retrospective analysis was conducted on 223 patients who underwent radical resection for pCCA at the First Affiliated Hospital of Army Medical University between January 2018 and December 2023.Patients were categorized into a MetS group(n=50)and a non-MetS group(n=173)according to diagnostic criteria.Perioperative complications,overall survival(OS),and recurrence-free survival(RFS)were compared between groups.Prognostic factors were identified using multivariate analysis.Results:No significant differences were observed between the two groups regarding median hospital stay,overall complications,or severe complications(all P>0.05).The 1-,3-,and 5-year OS rates in the MetS group were 62.3%,22.3%,and 0,respectively,compared with 78.2%,39.5%,and 22.0%in the non-MetS group.Corresponding RFS rates were 46.2%,16.9%,and 0 in the MetS group vs.63.8%,29.6%,and 18.8%in the non-MetS group.Median OS and RFS were significantly shorter in the MetS group than in the non-MetS group(15.0 vs.27.0 months;12.0 vs.21.0 months;P=0.021 and P=0.037,respectively).Multivariate analysis identified MetS and major vascular invasion as independent predictors of OS,while MetS,jaundice,R0 resection,and major vascular invasion were independent predictors of RFS(all P<0.05).Conclusion:MetS is significantly associated with worse long-term survival and higher recurrence risk after radical resection for pCCA.Incorporating MetS into preoperative assessment and postoperative management strategies may help improve patient outcomes.
9.Development of rapid detection kit for infectious diseases

Objective To develop a new type of rapid detection kit of infectious diseases for quick screening,testing and identification of several infectious pathogens.Methods The rapid detection kit for infectious diseases were composed of two trolley boxes for sample processing and rapid detection.The sample processing box had a pipette gun embedded into its upper cover and an automated nucleic acid extractor,a vortex mixer,a centrifuge,a deep-well plate,a reagent kit for nucleic acid rapid extraction inserted into its lower part;the rapid detection box had a portable computer,a data line and a power cord at its upper cover and a fluorescence quantitative PCR instrument,a mixing instrument and a multi-pathogen detection reagent kit.Results The rapid detection kit could meet the testing needs of 16 persons at a time and took about 60 min for one-time nucleic acid extraction and detection,which realized rapid detection of five types of pathogenic microorganisms for respiratory,intestinal,insect-borne,blood-borne and mucous membrane-borne infectious diseases and common biological agents.Conclusion The rapid detection kit gains advantages in multi testing items,easy operation,high safety,precision and efficiency,and facilitates rapid on-site screening and detection of infectious diseases.[Chinese Medical Equipment Journal,2025,46(5):109-111]
