1.Sarcopenia: From Global Consensus to Korean Implementation — A Narrative Review and Standpoint

Sarcopenia is a major geriatric syndrome characterized by progressive loss of muscle mass and strength, resulting in disability and mortality. This narrative review synthesizes international consensus recommendations and Korean evidence to guide context-specific sarcopenia management strategies. PubMed, Embase, Cochrane Library, and KoreaMed (January 2000–November 2025) were searched, focusing on randomized trials, meta-analyses, systematic reviews, clinical practice guidelines, and large observational studies. Global diagnostic frameworks have evolved from muscle mass-based definitions toward multidimensional models that incorporate muscle strength and physical performance. Exercise and nutrition remain the mainstay treatments, with resistance-based training and adequate protein intake. Currently, pharmacologic options with proven clinical benefit are limited. In Korea, growing evidence supports the effectiveness of community-based sarcopenia interventions, underscoring the need for standardized, integrated delivery models that bridge the fragmented healthcare system and enable sustainable implementation.
2.Loss of Inspiratory Augmentation as a Predictive Factor for the Development of Gastroesophageal Reflux Disease After Peroral Endoscopic Myotomy in Patients With Achalasia

Background/Aims:
Gastroesophageal reflux disease (GERD) after peroral endoscopic myotomy (POEM) is a significant challenge. Disruption of the lower esophageal sphincter during POEM and dysfunction of the crural diaphragm, which contributes to esophagogastric junction pressure, may lead to GERD after POEM. We aim to identify predictors of GERD after POEM and focus on physiological parameters of esophagogastric junction pressure in patients with achalasia before POEM.
Methods:
We retrospectively analyzed a prospectively collected database of patients who underwent POEM between July 2016 and August 2023. Ninety-two patients with achalasia who underwent high-resolution manometry and endoscopy before and after POEM were included.
Results:
Forty-nine (53.3%) patients were diagnosed with GERD. Patient demographics, achalasia subtypes, prior treatments, myotomy length, and myotomy orientation were not associated with GERD after POEM. Loss of inspiratory augmentation before POEM was associated with GERD after POEM (P < 0.05). GERD occurred more frequently in patients with a shorter intra-abdominal lower esophageal sphincter length before POEM. Type III esophagogastric junction morphology in the GERD group (37.1%) was more common than that in the non-GERD group (15.8%). Loss of inspiratory augmentation occurred more frequently in patients with type III esophagogastric junction pressure morphology.
Conclusion
Loss of inspiratory augmentation may predict GERD after POEM.

Burning mouth syndrome remains a frequent source of diagnostic delay, repeated consultations, and fragmented care because it has long been treated as a “diagnosis of exclusion.” Recent updates in orofacial pain classification and research diagnostic frameworks support a practical shift: from an exclusionary label toward a structured diagnostic algorithm and staged, phenotype-guided management. This narrative, algorithm-focused clinical review aims to summarize clinically actionable evidence and to propose a “minimum sufficient workup” that prioritizes safety, efficiency, and patient-centered communication.We first outline pragmatic diagnostic criteria and key symptom patterns, then integrate current concepts of mixed mechanisms—peripheral neuropathic features, centralociplastic amplification, and psychosocial modulators—that account for symptom fluctuation and discordance between symptom severity and objective findings. The core of this review is a one-page stepwise algorithm: (1) confirm chronicity and absence of visible mucosal disease, (2) screen for red flags requiring urgent evaluation, (3) address common local contributors (e.g., candidiasis, irritants, contact allergy, xerostomia), (4) perform a targeted systemic/medication/deficiency panel, and (5) phenotype patients to guide staged treatment choices and follow-up. Finally, we provide a practical management framework emphasizing education, trigger control, and individualized combinations of topical, systemic, and behavioral interventions with predefined reassessment intervals. Future priorities include phenotype-stratified randomized trials and implementation outcomes that quantify reductions in diagnostic delay, misdiagnosis, and unnecessary testing.
4.Trends in Widefield Fundus Photography and Optical Coherence Tomography Examination in South Korea (2015–2024)

Purpose:
To understand the nationwide utilization trends of widefield fundus photography (WFP) and optical coherence tomography (OCT) in South Korea from 2015 to 2024.
Methods:
This retrospective observational study analyzed national insurance claims data from the Health and Medical Big Data Open System. Data for WFP and OCT were retrieved using their procedure code. We examined the annual number of patients and total examinations, stratified by age, sex, and healthcare facility type. Medical expenses were also analyzed.
Results:
Between 2015 and 2024, the number of patients undergoing WFP and OCT increased 11.5-fold (from 334,034 to 3,842,168) and 4.8-fold (from 682,893 to 3,256,514), respectively. Examination volumes grew even more sharply, with a 14.1- fold increase (from 907,009 to 12,796,731) for WFP and 5.9-fold (from 1,821,858 to 10,820,336) for OCT. Women consistently had higher utilization rates than men. Patients aged 50 to 79 years accounted for the majority of both WFP (>60%) and OCT (>70%) procedures, with the 70 to 79 age group showing the highest usage. Medical costs rose 17.6-fold for WFP and 7.4-fold for OCT. The proportion of procedures conducted in primary care settings also rose significantly, from 45.2% to 65.9% for WFP and from 43.4% to 57.9% for OCT.
Conclusions
The use of WFP and OCT in South Korea has markedly increased over the past decade, especially among older adults and in primary care settings. This trend reflects aging demographics, improved access, and policy changes, highlighting the need for efficient resource planning.
6.Prognostic value of carcinoembryonic antigen kinetics in predicting distant metastatic recurrence of breast cancer:a multicenter cohort study

Purpose:
Distant metastasis of breast cancer significantly affects the prognosis. Serum markers such as CEA and cancer antigen (CA) 15-3 are used for surveillance. However, the nonspecificity and nonmalignant elevation of CEA limit its absolute value in predicting metastatic recurrence. We reevaluated the clinical value of CEA kinetics in predicting metastatic recurrence in breast cancer.
Methods:
In this multicenter retrospective cohort study, we utilized electronic medical record data from eight institutions (2008–2023), including 4,872 surgically treated patients with invasive breast cancer. Novel CEA kinetic indices were defined.Predictive capabilities were assessed using logistic regression, Kaplan-Meier survival curves, and Cox proportional hazards models.
Results:
CEA velocity (vCEA) and last-interval vCEA (vCEA-L) showed higher accuracy and sensitivity in predicting distant metastasis than absolute CEA indices. All tumor marker indices were independent predictors; vCEA, vCEA-L, and maximum vCEA (vCEAmax) showed the highest association. In the 5-year distant disease-free survival analysis of patients, vCEA showed the most significant difference based on whether its cutoff value was exceeded (84.1% vs. 47.2%) (P < 0.001).Cox analysis showed that vCEA was the strongest predictor (hazard ratio, 4.509; P < 0.001). vCEAmax remained prognostic even when CEAmax was below its cutoff value.
Conclusion
Analysis of dynamic CEA changes, particularly velocity-based indices, offers superior predictive power than the static-based CEA indices for metastatic recurrence in breast cancer. These kinetic markers enhance prognostic accuracy when combined with CA 15-3. Future research should integrate these markers with imaging, liquid biopsy, and artificial intelligence for personalized follow-up.
7.Association between thyroid function and 10-year cardiovascular disease risk of patients with diabetes: a cross-sectional study using KNHANES 2013–2014

Background:
This study investigated the association between thyroid function and the 10-year cardiovascular disease (CVD) risk in patients with diabetes.
Methods:
This retrospective cross-sectional study analyzed 246 patients with diabetes aged 30 to 79 years (weighted n=1,611,708) from the 2013 to 2014 Korea National Health and Nutrition Examination Survey. Individuals with a history of CVD, thyroid disease, or pregnancy were excluded. Thyroid function was categorized as euthyroid, hypothyroid, or hyperthyroid, based on serum thyroid-stimulating hormone (TSH) levels (reference in Koreans, 0.62–6.86 mIU/L). The 10-year CVD risk was estimated using the Predicting Risk of CVD EVENTs calculator.
Results:
The mean participant age was 54.3±10.1 years, with a male-to-female ratio of 1.41:1. Although patients with hypothyroid (14.1%) and hyperthyroid (15.7%) statuses showed a higher CVD risk than those with euthyroid status (12.3%), the difference was not statistically significant (p=0.337). However, in the hypothyroid group (n=11), TSH levels showed a moderate correlation with 10-year CVD risk (r=0.603, p<0.05), although this finding should be interpreted with caution owing to the small sample size. After adjusting for diabetes- and thyroid-related variables, TSH level was independently associated with higher estimated 10-year CVD risk, particularly in patients with a glycated hemoglobin level of <7% (adjusted coefficient, 0.17; 95% confidence interval, 0.05–0.29; p<0.05).
Conclusion
Monitoring TSH levels in patients with well-controlled diabetes may provide additional information regarding the risk of CVD; however, prospective studies are needed to confirm the long-term prognostic value.
8.A needs assessment for the development of a digital-based self-nutrition management education program for older adults in Korea: a cross-sectional study

Objectives:
This study aimed to investigate the current status of nutrition management and digital utilization, the demand for e-learning nutrition education, and the factors influencing e-learning nutrition education among older adults.
Methods:
A total of 500 older adults aged 65–84 years, registered as panelists with a professional survey agency, participated in an online survey. Chi-square and independent samples t-tests were used to examine gender differences. Correlations between participants’ general characteristics and the major variables were analyzed. Based on the results, a moderated regression analysis was conducted to identify how the general characteristics significantly associated with the intention to use e-learning nutrition education functioned as moderators.
Results:
Significant gender differences were observed in nutrition education experience, interest and practice of nutrition management, barriers, and digital device utilization (all P < 0.05). Exploratory factor analysis identified four sub-factors for education needs: ‘Self-directed well-being practice education’, ‘Dietary therapy for major chronic diseases’, ‘Nutrition strategy for longevity and anti-aging’, and ‘Enhancing nutrition literacy education’. In Model 1, the need for digital education, interest in nutrition management, and gender were significantly associated with usage intention (all P < 0.05), whereas health-promoting behavior showed marginal significance (P = 0.06). Although the Model 2 interaction was significant, the change in explanatory power was negligible (Delta R2 = 0.003), and all interaction terms were non-significant (P > 0.05), indicating that the relationships are not moderated by gender.
Conclusion
Although older adults exhibited high interest in nutrition management, their experience with nutrition education was limited. Significant gender differences in educational needs and health-promoting behaviors highlight the importance of tailored strategies. Enhancing digital accessibility and providing gender-specific, competency-based content are essential to effectively translate interest into behavioral change. This study underscores the importance of integrating gender-specific approaches, individual digital competencies, and inducement of health-promoting behaviors when developing e-learning nutrition education programs for older adults.
9.Clinical Efficacy of Real-Time Artificial Intelligence-Assisted Colonoscopy in Colorectal Polyp Detection: A Prospective Multicenter Randomized Controlled Trial

Background/Aims:
Early detection and removal of colon polyps are critical for preventing colorectal cancer. Computer-aided detection (CADe) systems have been introduced to increase the polyp detection rate (PDR) during colonoscopy, potentially enhancing its effectiveness. This study aimed to evaluate the efficacy of a CADe system in colorectal neoplasm detection.
Methods:
This prospective, randomized controlled trial was conducted at two tertiary centers (May 2023 to April 2025). Patients were randomly assigned to CADe or conventional colonoscopy and underwent screening, surveillance, or diagnostic colonoscopy. The primary endpoint was the adenoma detection rate (ADR), while the secondary endpoints were the PDR, relative risk (RR) of polyp detection, adenomas per colonoscopy (APC), and factors influencing adenoma detection.
Results:
Of 1,004 enrolled patients, 998 were randomly allocated into CADe and conventional colonoscopy groups (497 CADe system and 501 conventional colonoscopy). The CADe group had greater polyp counts (2.2 per colonoscopy vs 1.4 per colonoscopy; p<0.001) and APC values (1.2 vs 0.8; p<0.001). The CADe group showed significantly higher PDRs (72.2% vs 54.5%;p<0.001; RR, 2.173; 95% confidence interval [CI], 1.669 to 2.828) and ADRs (52.3% vs 36.1%;p<0.001; RR, 1.940; 95% CI, 1.505 to 2.499). CADe also significantly increased the detection rate of hyperplastic polyps (p=0.007; RR, 1.474; 95% CI, 1.113 to 1.952) and increased the detection rates across all sizes and locations. In multivariable analysis, CADe use was the strongest independent predictor of adenoma detection (odds ratio, 1.914; 95% CI, 1.467 to 2.496), outweighing male sex, older age, diagnostic indication, and withdrawal time.
Conclusions
Real-time CADe-assisted colonoscopy significantly increased PDR and ADR and proved to be a strong independent predictor of adenoma detection (cris.nih.go.kr, KCT0009664).
10.Whole-Exome Sequencing Improves Risk Assessments of Adult Moyamoya Disease

Background:
and Purpose Whole-exome sequencing (WES) is a valuable tool for identifying causative mutations in adult moyamoya disease (MMD), thereby advancing our understanding of the genetic mechanisms underlying this condition. Here, we conducted the first WESbased association study aimed at identifying genetic modifiers implicated in MMD.
Methods:
This WES study involved 160 patients with MMD and 189 controls from a multicenter hospital-based biobank, and evaluated combined annotation-dependent depletion (CADD) scores. Mutant-allele frequencies were compared in 369,121 individuals derived from the UK Biobank (UKB) WES. Mutant-allele risk scores (MARSs) were created based on WESidentified mutations. Gene-based association analyses and pooled analyses in East-Asian populations were further performed.
Results:
Fourteen mutations reached the genome-wide significance criterion (p<5×10-8 ), among which the p.R4810K mutation in the ring finger protein 213 gene (RNF213) showed the strongest significance (odds ratio=117.4, p=8.54×10-24 ). Notably, two mutations—p.G576S (alpha-glucosidase [GAA]) and p.D54N (charged multivesicular body protein 6 [CHMP6])— exhibited high CADD scores of 32 and 25, respectively, whereas the RNF213 p.R4810K mutation demonstrated a moderate deleteriousness score of 10.63. Fourteen mutations exhibited significant differences in allele frequencies between patients and UKB controlled data (p<1×10-8 ).The MARS9 model (incorporating nine missense mutations) showed better predictability for MMD (90.89%). The analysis of gene-based associations revealed four candidate genes: GAA, RNF213, CHMP6, and CARD14 (p=5×10-19 to 4×10-7 ). The subsequent pooled analyses validated four mutations in East Asian populations: p.V1195M, p.D1331G, p.S2334N, and p.R4810K (p<3×10-8 ).
Conclusions
This pioneering study has corroborated the significance of p.R4810K and identified several causative mutations predisposing patients to MMD, which helps to improve the understanding of its polygenetic nature.
