Background When live working line operators engage in upper limb operations, working for a long time with raising arms and the exposure to adverse ergonomic factors tend to increase muscle load, cause fatigue accumulation, and increase the risk of work-related musculoskeletal disorders (WMSDs). Objective To analyze work posture and associated muscle fatigue during executing breaking/connecting contact terminal, and identify adverse ergonomic factors of the work process. Methods This study recruited 10 volunteers to perform breaking/connecting contact terminal simulation. At a distance of 4.5, 3.5, and 2.5 m from the body to the wire, each performed the task 5 times. Visual 3D was used to analyze the kinematic data from motion capture. The surface electromyography (sEMG) signals of the deltoid, biceps, triceps, and brachioradialis were recorded during the simulation and analyzed for muscle fatigue using root mean square (RMS), median frequency (MF), and jointed EMG amplitude and spectrum analysis (JASA). After completion of each task, Borg scale was used to query the volunteers of their subjective fatigue. A 2-min rest was required between each distance. Results The kinematic data from motion capture showed that in the entire process of the task, the right upper limb was higher when lifting, with the right shoulder joint maintaining flexion and fluctuating periodically between abduction and adduction, external and internal rotation, the right elbow joint maintaining flexion and supination. The frequency of task cycle showed a significant effect on the Borg scale scores (P<0.001, partial \begin{document}$ {\eta }^{2} $\end{document}=0.464); the distance showed no significant effect on the Borg scale scores (P>0.05, partial \begin{document}$ {\eta }^{2} $\end{document}=0.002); their interaction effect was not significant (P>0.05, partial \begin{document}$ {\eta }^{2} $\end{document}=0.020). The results of the sEMG signals showed that at a distance of 4.5 m, the volunteers had the highest percentages of maximal voluntary electrical activation (MVE%) in the deltoid and right brachioradialis and the lowest MF values in the deltoid and left brachioradialis after completing 5 task cycles. The results of JASA showed that fatigue was observed in brachioradialis and deltoid after completing 5 task cycles at a distance of 4.5 m, the right deltoid and right brachioradialis at a distance of 3.5 m, in the brachioradialis, deltoid, and left bicep at a distance of 2.5 m. Conclusion The subjective fatigue of workers who engaged in breaking/connecting contact terminal elevates with more repetitive operations. Based on the sEMG and motion capture analysis, it can be found that improper distance between the workers and the target work area can increase the burden from poor posture and improper use of work equipment, and lead to muscle fatigue; the deltoid and brachioradialis are the key muscles of fatigue in this type of operation; changing the angle of the elbow joint and the shoulder joint during the process of the operation can affect the deltoid and brachioradialis exertion and loading.

Background Power grid is an important component of the national infrastructure. The occupational health issues among the workers in this industry are attracting great concern nationwide. Objective To investigate the prevalence of work-related musculoskeletal disorders (WMSDs) in neck and shoulder among the power distribution workers of power supply enterprises, and analyze the related influencing factors. Method In April 2023, a total of 1244 workers in 50 municipal power supply bureaus were randomly selected from the three provinces under China Southern Power Grid using cluster sampling method. The questionnaire used in the study was based on a revised Musculoskeletal Disorders Questionnaire and added ergonomic load of upper limb work. \begin{document}$ {\chi }^{2} $\end{document} test and logistic regression model were used to explore the influencing factors of neck pain and shoulder pain among the subjects. Results A total of 1244 valid questionnaires were recovered and the effective recovery rate was 90.87%(1244/1369). The positive rates of neck pain and shoulder pain were 45.5% (566 cases) and 36.3% (452 cases) respectively. The logistic regression analysis results showed that more than 20 years working age (OR=2.27, 95%CI: 1.39, 3.96; OR=2.56, 95%CI: 1.53, 4.26), often (OR=2.08, 95%CI: 1.35, 3.20; OR=2.40, 95%CI: 1.50, 3.58)/quite often (OR=2.96, 95%CI: 1.73, 5.04; OR=2.72, 95%CI: 1.49, 4.94) maintaining a forced posture, and working immediately after break (OR=1.72, 95%CI: 1.04, 2.85; OR=2.14, 95%CI: 1.19, 3.88) were associated with a higher positive rate of neck pain and shoulder pain respectively. Feeling sufficient rest (OR=0.61, 95%CI: 0.47, 0.80; OR=0.63, 95%CI: 0.50, 0.83) and opportunity to decide when to take breaks (OR=0.72, 95%CI: 0.53, 0.98; OR=0.62, 95%CI: 0.41, 0.93) were associated with a lower positive rate of neck pain and shoulder pain respectively. Conclusion The positive rates of neck pain and shoulder pain are high among power distribution workers. The main influencing factors included long working age, quite often maintaining a forced posture, and working immediately after break.

The effects of shikonin, a natural compound from the roots of Lithospermum erythrorhizon, on oxidative stress and blood–brain barrier (BBB) injury in SAH was evaluated in this study. A rat model of SAH was established by endovascular perforation to mimic the rupture of intracranial aneurysms. Rats were then administered 25 mg/kg of shikonin or dimethylsulfoxide after surgery. Brain edema, SAH grade, and neurobehavioral scores were measured after 24 h of SAH to evaluate neurological impairment. Concentrations of the oxidative stress markers superoxide dismutase (SOD), glutathione (GSH), and malondialdehyde (MDA) in the brain cortex were determined using the corresponding commercially available assay kits. Evans blue staining was used to determine BBB permeability. Western blotting was used to quantify protein levels of tight junction proteins zonula occludens-1, Occludin, and Claudin-5. After modeling, the brain water content increased significantly whereas the neurobehavioral scores of rats with SAH decreased prominently. MDA levels increased and the levels of the antioxidant enzymes GSH and SOD decreased after SAH. These changes were reversed after shikonin administration. Shikonin treatment also inhibited Evans blue extravasation after SAH. Furthermore, reduction in the levels of tight junction proteins after SAH modeling was rescued after shikonin treatment. In conclusion, shikonin exerts a neuroprotective effect after SAH by mitigating BBB injury and inhibiting oxidative stress in the cerebral cortex.

[Objective] To explore the predictive value of HALP score for prognosis in patients with multiple myeloma (MM). [Methods] A retrospective analysis was conducted on laboratory indicators and related clinical data of newly diagnosed multiple myeloma (NDMM) patients, treated at the Affiliated Hospital of North Sichuan Medical College from January 2016 to October 2023, prior to their first treatment. The HALP score was calculated, and the optimal cutoff value for HALP was determined using X-tile software. Survival analysis was performed using Kaplan-Meier curves for high HALP and low HALP groups. Univariate and multivariate analyses were conducted using the Cox regression model, and a forest plot was generated using Graphpad Prism to illustrate factors that may impact patient prognosis. The predictive ability of HALP score combined with β2-microglobulin and ECOG score for prognosis in MM patients was evaluated using receiver operating characteristic curve (ROC) analysis. [Results] A total of 203 MM patients were included, with the optimal cutoff value for HALP score being 29.15 (P<0.05). Among them, 101 patients were in the low HALP score group, and 102 patients were in the high HALP score group. The results of univariate and multivariate analysis using the Cox regression model showed that a HALP score <29.15 was an independent risk factor for progression-free survival (PFS) and overall survival (OS) (P<0.05). ROC curve analysis indicated that the combination of HALP score with β2-microglobulin and ECOG score had a higher predictive value for prognosis in MM patients compared to using HALP score alone. [Conclusion] The HALP score is closely related to the prognosis of patients with NDMM. A low HALP score indicates a poorer prognosis, while the combination of HALP score with β2-microglobulin and ECOG score provides a higher predictive value when assessed together.

Vascular structure pattern，namely the maturity，morphology and distribution of blood vessels，as one of the pathological features of renal cell carcinoma (RCC)，cannot be ignored when the prognosis is concerned.The current mainstream classification of vascular structure patterns includes pseudoacini，Golgi，lacunar and dispersion patterns.Many studies have shown that the vascular structure patterns in RCC are closely related to the diagnosis and prognosis.Early features such as dendritic penetration of the endothelium contribute to the differential diagnosis of RCC，while different vascular structure patterns are not only related to tumor grade，but also affect the responsiveness of targeted therapy and prognosis.Evaluating these vascular structural patterns can help to understand the biological behaviors of tumors and guide the treatment options，but a unified assessment of vascular structural patterns is still lacking.This paper reviews the research progress on the relationship between vascular structure patterns and RCC prognosis，aiming to provide clinical reference.

Objective Evaluation of the clinical efficacy of f trochanteric flip osteotomy combined with Kocher-Langen-beck approach for high acetabular posterior wall fracture.Methods Between January 2020 and December 2022,20 patients with high acetabular posterior wall fractures were retrospectively analyzed,including 12 males and 8 females,aged 18 to 75 years old.They were divided into two groups according to the different surgical methods.Ten patients were treated with greater trochanteric osteotomy combined with Kocher-Langenbeck approach as the observation group,including 5 males and 5 fe-males,aged from 18 to 75 years old.Ten patients were treated with Kocher-Langenbeck approach alone as the control group,including 7 males and 3 females,aged from 18 to 71 years old.Matta reduction criteria were used to evaluate the reduction quality of the two groups,and Harris score was used to compare the hip function of the two groups at the latest follow-up.The operation time,blood loss and postoperative complications of the two groups were analyzed.Results All patients were followed up for 10 to 24 months.According to the Matta fracture reduction quality evaluation criteria,the observation group achieved anatomical reduction in 6 cases,satisfactory reduction in 3 cases,and unsatisfactory reduction in 1 case,while the control group only achieved anatomical reduction in 3 cases,satisfactory reduction in 3 cases,and unsatisfactory reduction in 4 cases.At the final follow-up,the Harris hip score ranged from 71.4 to 96.6 in the observation group and 65.3 to 94.5 in the control group.According to the results of Harris score.The hip joint function of the observation group was excellent in 6 cases,good in 3 cases,and fair in 1 case.The hip joint function of the control group was excellent in 2 cases,good in 3 cases,fair in 3 cases,and poor in 2 cases.In the observation group,the intraoperative blood loss ranged from 300 to 700 ml,and the operation dura-tion ranged from 120 to 180 min;in the control group,the intraoperative blood loss ranged from 300 to 650 ml,and the opera-tion duration ranged from 100 to 180 min.Complications in the observation group included 1 case of traumatic arthritis and 1 case of heterotopic ossification,while complications in the control group included 3 cases of traumatic arthritis,3 cases of het-erotopic ossification and 1 case of hip abduction weakness.Conclusions Trochanteric flip osteotomy combined with the Kocher-Langenbeck approach significantly improved anatomical fracture reduction rates,enhanced excellent and good hip joint function outcomes,and reduced surgical complication incidence compared to the Kocher-Langenbeck approach alone.Clinical application of this combined approach is promising,although larger studies are needed for further validation.
