1.Effects of Shen'an decoction on immunity,renal function and lipid metabolism in patients with chronic renal insufficiency

Objective To explore the clinical efficacy of Shen'an decoction in treating chronic renal insufficiency and systematically assess its effects on immune regulation,renal function and lipid metabolism.Methods A prospective randomized controlled study was conducted,and 102 patients with chronic renal insufficiency were selected.Patients were randomly divided into the control group(51 cases,receiving conventional Western medical treatment)and the observation group(51 cases,receiving conventional Western medical treatment combined with Ren'an decoction).The traditional Chinese medicine(TCM)syndrome scores,clinical efficacy,levels of immunoglobulin(Ig)G,IgM,IgA,24-hour urine protein quantification(24 hUP),cystatin C(Cys C),serum creatinine(Scr),blood calcium,blood phosphorus,triglycerides(TG)and total cholesterol(TC)were evaluated before treatment and 8 weeks after treatment.The levels of reactive oxygen species(ROS)and advanced oxidation protein products(AOPPs)were measured by enzyme-linked immunosorbent assay(ELISA).Adverse reactions such as nausea,diarrhea,dizziness and rash during the treatment period were recorded.Results After 8 weeks of treatment,the TCM syndrome scores,24 hUP,Cys C,Scr,blood phosphorus,TG,TC,ROS and AOPPs were significantly lower in both groups than those before treatment,and the observation group was significantly lower than that of the control group(P<0.05).The levels of IgG,IgM,IgA and blood calcium were significantly higher in both groups than those before treatment,and the observation group was significantly higher than those of the control group(P<0.05).The total clinical effective rate of the observation group was higher than that of the control group(94.12%vs.80.39%,P<0.05).There was one case of nausea,one case of dizziness and one case of rash in the observation group,and one case of nausea,one case of diarrhea and two cases of dizziness occurred in the control group.There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion The addition of Shen'an decoction in the treatment of patients with chronic renal insufficiency on the basis of conventional Western medical treatment can improve the clinical efficacy,enhance immune function,renal function,lipid metabolism and oxidative stress indicators,with good safety.

3.Clinical nursing of vaginal adhesions in patients treated with radiotherapy for cervical cancer

Objective To explore the nursing method for the prevention and treatment of vaginal adhesions in patients treated with full-dose radiotherapy for cervical cancer. Methods From 2015 to 2016, 80 cervical cancer patients who received radiotherapy were treated with domestic vaginal washing, vaginal local use of"Fufangbao", infection control, diet and sexual rehabilitation guidance, and so on, and the nursing outcome was evaluated. Results Of the 80 patients, 6 were lost during follow-up, resulting in a follow-up rate of 925%. Ten, five, and three patients were found to have grade 1, 2, and 3 vaginal toxicity, respectively. The patients with grade 1 vaginal toxicity were not treated with special treatment and were asked to do more vaginal washing. The patients with grade 2 and 3 vaginal toxicity were treated with daily vaginal washing and"Fufangbao" for repairing the vagina;vaginal adhesions disappeared or improved 3 months later. The patients with no vaginal adhesions had regular sexual intercourse and thus had improved quality of life. Conclusions Effective clinical nursing can reduce the incidence of vaginal adhesions and stenosis, improve patients' quality of life, and promote the harmony of family.
4.Clinical Observation of Warm Needling plus Herbal Medicine for Ocular Myasthenia Gravis

Objective To compare the therapeutic efficacy between warm needling and Pyridostigmine bromide in treating ocular myasthenia gravis, for seeking the optimal treatment method. Method Seventy patients with ocular myasthenia gravis were randomized into a treatment group intervened by warm needling and control group intervened by Pyridostingmine bromide, 35 in each group; meanwhile, fortified Yi Qi Tang decoction was given to both groups. The observation lasted 6 weeks and a 3-month follow-up was conducted. The clinical absolute and relative scoring system for myasthenia gravis, and the Chinese medicine syndrome scale for spleen-qi deficiency were adopted for quantitative comparison and efficacy evaluation. Result The total effective rate was 85.7% in the treatment group, higher than that in the control group. Conclusion Warm needling plus herbal medicine is effective in treating ocular myasthenia gravis.
5.Survival prognostic analysis in patients with hepatocellular carcinoma after radical resection

Objective To investigate the survival prognostic influencing factors of patients with hepatocellular carcinoma (HCC) after radical resection. Methods Clinical data of 1 102 patients with HCC undergoing radical resection in Sun Yat-sen University Cancer Center from January 2006 to June 2012 were analyzed retrospectively. There were 872 males and 230 females with a mean age of (55±15) years old. The informed consents of all patients were obtained and local ethical committee approval had been received. All the patients were followed up after operation. The cumulative survival rate was estimated using Kaplan-Meier method according to the follow-up results. Clinical indexes such as age, gender, history of viral hepatitis B, family history of HCC, serum alpha fetoprotein (AFP), serum alkaline phosphatase (ALP), Child-Pugh liver function grading, tumor diameter, portal vein tumor thrombus, tumor differentiation, vessel invasion, lymph node metastasis, distant metastasis, tumor node metastasis (TNM) staging were enrolled as prognostic impact factors and were analyzed using Log-rank test and Cox proportional hazards model. Results The survival time was 4-114 months with a median of 70 months for the patients with HCC after radical resection. The 1-, 3-, 5-year cumulative survival rates were 88.9%, 73.2%, 52.3%. Tumor diameter>4 cm, positive serum AFP, poor tumor differentiation were the independent risk factors for the prognosis of patients with HCC after radical resection (RR=1.951, 3.498, 3.781;P<0.05). Conclusion Tumor diameter>4 cm, positive serum AFP, poor tumor differentiation are the independent risk factors inlfuencing the prognosis of patients with HCC after radical resection.
