1.Meta-analysis of the diagnostic value of ultrasound and MRI in knee osteoarthritis

Objective To evaluate the value of ultrasound and magnetic resonance imaging(MRI)in the diagnosis of knee osteoarthritis(KOA)by Meta-analysis.Methods Multiple databases for literature on KOA ultrasound and MRI diagnosis were screened.After data extraction and study quality assessment,14 articles involving 1613 patients were finally included.Meta-analysis were used on the extracted data to comprehensively evaluate the effectiveness of ultrasound and MRI in the diagnosis of KOA.Results All included studies used arthroscopy or surgical pathology as the gold standard for diagnosis.The pooled sensitivity,specificity,and diagnostic odds ratio(DOR)for ultrasound in diagnosing KOA were 0.79,0.82,and 20.30,respectively,with area under the curve(AUC)was 0.89.In comparison,the pooled sensitivity,specificity,and DOR for MRI in diagnosing KOA were 0.88,0.85,and 42.35,respectively,with AUC of 0.92.Conclusion Ultrasound and MRI have good accuracy in the diagnosis of KOA,and MRI is superior to ultrasound.
2.Measurement and analysis of the alveolar bone in the maxillary canine region of adults with different sagittal skeletal patterns using CBCT

Objective To measure and analyze through cone-beam computed tomography(CBCT)of the incidence of alveolar bone fenestration and dehiscence,as well as the thickness and height of the alveolar bone in the maxillary canine region of adult patients with different sagittal skeletal patterns.Methods A total of 174 adult patients meeting the criteria were selected.In the first step,the inci-dence rates of fenestration and dehiscence were measured and statistically analyzed after grouping according to ANB.In the second step,the patients with bone fenestration and bone dehiscence were excluded,and the thickness of the alveolar bone at the apical,mid-dle and cervical regions on the labial and palatal sides of the maxillary canine area and the height of the alveolar crest were measured in the remaining samples.Then,the three groups of patients were further divided into three groups according to the FMA for comparison and analysis.Results The incidence of bone fenestration and bone dehiscence in the maxillary canine area was higher in adult patients with skeletal Class Ⅱ.The thickness of the labial alveolar bone at the apical region in skeletal Class Ⅲ was greater than that in skeletal Class Ⅱ and skeletal Class Ⅰ.The thickness of the palatal alveolar bone at the middle root in skeletal Class Ⅱ:average angle>low angle>high angle.In skeletal Class Ⅲ,the thickness of the palatal alveolar bone at the cervical root:low angle>average angle>high angle.Conclusion The incidence of bone fenestration and bone dehiscence was the highest in adult patients with skeletal ClassⅡ.The thickness of the labial alveolar bone at the apical region was greater in skeletal Class Ⅲ than in skeletal Class Ⅱ and skeletal Class Ⅰ.The thickness of the palatal alveolar bone at the middle root in skeletal Class Ⅱ high angle patients was thinner than that in the average angle and low angle groups,and the thickness of the palatal alveolar bone at the cervical region in skeletal Class Ⅲ was thinner in the high angle group than in the average angle and low angle groups.

4.Three-dimensional arch morphology differences in the invisible orthodontic treatment of extracted and non-extracted patients

Objective:
To investigate the accuracy of the mandibular curve of Spee and mandibular curve of Wilson curve, mandibular posterior crown inclination, maxillary and mandibular arch width, and mandibular incisor labial inclination changes in the invisible orthodontic treatment of extracted and non-extracted patients to provide a basis for the study of the therapeutic efficacy of the invisible aligner.
Methods:
This study was approved by the Unit Medical Ethics Committee. Adult bony Class I patients treated with invisible aligners between 2016 and 2023 were selected and categorized into 32 cases in a group with four first premolar extractions and 33 cases in a non-extraction group. The initial scanning model (T1), the orthodontic plan design model (T2), and the scanning model after alignment and leveling (T3) were collected, and the scanning data at different time points were exported to STL format files. They were then analyzed and measured using GOM Inspect 2019 software; the clinical effect (T1-T3) was defined as AC, and the expected result (T1-T2) was defined as CC. To explore the mandibular curve of Spee, the leveling accuracy of the mandibular Wilson curve (AC/CC×100%), the change in mandibular posterior crown inclination, the change in maxillary and mandibular arch width and the change in mandibular incisor labial inclination. The results were tallied using R4.3.2 software.
Results:
The leveling accuracy of the mandibular curve of Spee was 3.2% and 10.1% in the extraction and non-extraction groups, respectively; the leveling accuracy of the mandibular first molar in the extraction and non-extraction groups was 9.5% and 4.2%, respectively, and the leveling accuracy of the mandibular second premolar was 32.8% and 25%, respectively. The leveling accuracy of the mandibular curve of Wilson was 126% in the extraction group compared to 704% in the non-extraction group. The maximum values of AC for crown inclination of the mandibular posterior teeth were all found in the first molar and the minimum values in the second premolar (P< 0.05); CC was the greatest in the second premolar and the least in the first molar in the extraction group (P< 0.05), and in the second molar and the least in the second premolar in the non-extraction group (P< 0.05). There was no significant difference in the change of maxillary and mandibular arch widths in the extraction group (P> 0.05); the AC of the arch widths at the maxillary first molar and at the mandibular second premolar and first molar in the non-extraction group was significantly greater than the CC (P< 0.05). The AC of the mandibular second premolar crown inclination was significantly greater in the non-extraction group than in the extraction group (P < 0.05), showing a more pronounced buccal inclination of the crowns; in contrast, there was no significant difference between the mandibular molar crown inclination AC and CC in the two groups (P > 0.05). The CC of mandibular curve of Spee in the extraction group was significantly greater than that in the non-extraction group (P < 0.05), whereas there was no significant difference in AC between the two groups (P > 0.05). In addition, the AC of the labial inclination of the mandibular incisors in the extraction group and the non-extraction group was smaller than the CC, but both were positive, indicating an increase in the degree of labial inclination of the mandibular incisors.
Conclusion
The invisible aligner mandibular curve of Spee leveling was poorly expressed, and there was a tendency for overexpression of the mandibular curve of Wilson leveling. Increased labial inclination of the mandibular incisors was observed in patients. The extraction group was accurately predicted in arch width control, there was overexpansion in the non-extraction group.

6.Correlation between gallbladder polyps and colorectal polyps

Objective:To explore the relationship between gallbladder polyps and colorectal polyps, providing insights into whether the presence of gallbladder polyps can serve as an indicator for colonoscopy screening.Methods:Clinical data from 2 542 patients who underwent colonoscopy and abdominal ultrasound at the First Affiliated Hospital of Shihezi University between January and December 2022 were retrospectively analyzed. Patients were divided into colorectal polyp group ( n=1 266) and non-colorectal polyp group ( n=1 276) based on colonoscopy findings. Univariate and multivariate Logistic regression models were used to analyze the relationship between gallbladder polyps and colorectal polyps. Results:The prevalence rates of gallbladder polyp in colorectal polyp group and non-colorectal polyp group were 16.67% (211/1 266) and 11.21% (143/1 276). Multivariate Logistic regression analysis showed a lower risk of colorectal polyps in women ( P<0.001, OR=0.523, 95% CI: 0.440-0.622). Age ( P<0.001, OR=1.059, 95% CI: 1.050-1.068), and hypertriglyceridemia ( P=0.013, OR=1.350, 95% CI: 1.066-1.709), low level of high-density lipoprotein ( P<0.001, OR=1.588, 95% CI: 1.280-1.969), and gallbladder polyp ( P<0.001, OR=1.712, 95% CI: 1.344-2.180) were independent risk factors for colorectal polyp. There was no significant difference in hypercholesterolemia, elevated low-density lipoprotein, hyperuricemia, or cholecystectomy between colorectal polyp group and non-colorectal polyp group ( P>0.05). Conclusion:The identification of gallbladder polyps via abdominal ultrasound may indicate a higher likelihood of colorectal polyps in patients, underscoring the need for further colonoscopy screening in individuals with gallbladder polyps.
7.Study on the improvement of offspring development in chronic mild stress rats by Shuyu Capsules

Objective To simulate modern social stress using a pre-pregnancy chronic mild stress(CMS)model and to explore the mechanisms of emotional,behavioral,and neurodevelopmental changes in male offspring of pre-pregnancy liver qi stagnation female mice through corticosterone(CORT)-brain-derived neurotrophic factor(BDNF)extracellular signal-regulated kinase(ERK)1/2 signaling cascade-mediated hippocampal injury.This study aimed to elucidate the impact of negative life events on offspring and the interventional mechanism of Shuyu Capsules.Methods CMS stress was used to induce pre-pregnancy depression in female rats(liver qi stagnation state),followed by intervention with Shuyu and fluoxetine capsules.After screening,male rats were mated and 12 male offspring from each group were selected for behavioral testing and detection of serum CORT levels by enzyme-linked immunosorbent assay.BDNF,ERK1/2,phospho(p)-ERK1/2,cAMP-response element binding protein(CREB),and p-CREB protein levels in the hippocampus were detected by Western Blot,and BDNF,ERK1,ERK2,and CREB mRNA levels in the hippocampus were detected by reverse transcription-polymerase chain reaction(RT-PCR),to verify the effects of pre-pregnancy CMS on the BDNF-ERK1/2-CREB signaling pathway and to investigate the key micro-mechanisms of Shuyu Capsules on emotional and learning memory-related behaviors of male offspring of females with pre-pregnancy liver qi stagnation syndrome.Results The distance,number of entries,and duration of stay in the central area in open-field experiments were significantly reduced in offspring in the model group(all P<0.05).The escape latency during the exploration period of the water-maze experiment was significantly prolonged(P<0.05)and the swimming distance,duration of the target quadrant,and number of platform crossings were significantly reduced(P<0.05,P<0.05,P<0.01),the suspension time and frequency in the forced-swimming experiment were increased(P<0.05,P<0.01),and the incubation period was shortened(P<0.05)in offspring in the model group.Prophylactic treatment with Shuyu Capsules and fluoxetine improved the depression-like behavior and cognitive impairment in the offspring in the model group.Biochemical tests showed that CORT levels were increased in the CMS model group(P<0.05),BDNF,p-ERK1/2,and p-CREB protein levels in the hippocampus were decreased(all P<0.05),and BDNF,ERK1,ERK2,and CREB mRNA levels were significantly reduced(P<0.01,P<0.05,P<0.01,P<0.05).Treatment with Shuyu Capsules and fluoxetine increased the CORT content and BDNF,ERK1/2,and CREB protein and mRNA levels in male offspring to varying degrees.Conclusions High levels of CORT in offspring act selectively on the hippocampus,exerting adverse effects on the emotional and learning memory functions of rats by downregulating the BDNF-ERK1/2 signaling cascade.The Chinese medicine Shuyu Capsules can reduce the impact of an adverse intrauterine environment on offspring development by correcting abnormal levels and pathways of glucocorticoids.

10.Role of CIP2A in preoperative sleep deprivation-induced aggravation of postoperative cognitive dysfunction in aged mice

Objective:To evaluate the role of cancerous inhibitor of protein phosphatase 2A (CIP2A) in preoperative sleep deprivation (PSD)-induced aggravation of postoperative cognitive dysfunction (POCD) in aged mice.Methods:One hundred and ten healthy C57BL/6J mice of either sex, aged 18-20 months, weighing 29-35 g, were divided into 5 groups ( n=22 each) using a random number table method: sham operation group (S group), abdominal surgery group (O group), PSD + abdominal surgery group (D+ O group), CIP2A shRNA + abdominal surgery group (CS+ O group), and CIP2A shRNA+ PSD+ abdominal surgery group (CS+ D+ O group). At 14 days before surgery, control shRNA lentivirus was injected into the hippocampus in S, O and CS+ O groups, and CIP2A shRNA was injected into the hippocampus in D+ O and CS+ D+ O groups. PSD was carried out for 3 consecutive days prior to surgery. Cognitive function was assessed using the Morris water maze test at days 7-11 after surgery. The mice were sacrificed under deep anesthesia at day 3 after surgery, and hippocampal tissues were obtained to determine the expression of CIP2A, high mobility group box 1 (HMGB1), ionized calcium-binding adapter molecule 1 (Iba-1), alpha subunit of protein phosphatase 2A (PP2Aa), catalytic subunit of protein phosphatase 2A (PP2Ac), phosphorylated tau protein (p-tau) (S396), and p-tau (S404) (by Western blot), levels of reactive oxygen species (ROS), malondialdehyde (MDA), and superoxide dismutase (SOD), and count of Iba-1 positive cells in the hippocampal CA1 region (using immunofluorescence staining). Results:Compared with S group, the escape latency was significantly prolonged, the frequency of crossing the platform was reduced, duration of stay in the target quadrant was shortened, the expression of CIP2A, Iba-1 and HMGB1 was up-regulated, PP2Ac expression was down-regulated, levels of ROS and MDA and count of Iba-1 positive cells were increased, and the activity of SOD was decreased in O group ( P<0.05). Compared with O group, the escape latency was significantly prolonged, the frequency of crossing the platform was reduced, duration of stay in the target quadrant was shortened, the expression of CIP2A, Iba-1 and HMGB1 was up-regulated, PP2Ac expression was down-regulated, levels of ROS and MDA and count of Iba-1 positive cells were increased, and the activity of SOD was decreased in D+ O group, and the escape latency was significantly shortened, the frequency of crossing the platform was increased, duration of stay in the target quadrant was prolonged, the expression of CIP2A, Iba-1 and HMGB1 was down-regulated, PP2Ac expression was up-regulated, levels of ROS and MDA and count of Iba-1 positive cells were decreased, and the activity of SOD was increased in CS+ O group ( P<0.05). Compared with D+ O group, the escape latency was significantly shortened, the frequency of crossing the platform was increased, duration of stay in the target quadrant was prolonged, the expression of CIP2A, Iba-1 and HMGB1 was down-regulated, PP2Ac expression was up-regulated, levels of ROS and MDA and count of Iba-1 positive cells were decreased, and the activity of SOD was increased in CS+ D+ O group ( P<0.05). There was no significant difference in PP2Aa expression among the five groups ( P>0.05). Conclusions:The mechanism by which PSD aggravates POCD is related to up-regulating the expression of CIP2A and promoting oxidative stress responses, neuroinflammatory responses and phosphorylation of tau protein in aged mice.
