1.Anti-lung Cancer Mechanisms of Yang-warming Herbs and Formulas: A Review

Lung cancer, particularly non-small cell lung cancer (NSCLC), is the malignant tumor with the highest incidence and mortality in China and worldwide. In 2022, the global number of deaths reached 1.8 million, accounting for 18.7% of all cancer-related deaths, seriously threatening human health and life, and posing a severe challenge for prevention and treatment. Although treatment strategies for lung cancer have been continuously enriched in recent years, and progress has been made in targeted therapy and immunotherapy, long-term survival benefits remain limited due to primary or acquired drug resistance, low immune responsiveness, and chemotherapy-related toxicities. Therefore, there is an urgent need to explore safe and effective adjunctive therapeutic strategies. Traditional Chinese medicine (TCM), with its advantages of holistic regulation and individualized syndrome differentiation, has played an increasingly prominent role in comprehensive cancer treatment. TCM holds that "Yang deficiency leads to accumulation" is a key pathogenesis of tumors. Based on the theory that "Yang transforms Qi, while Yin forms substance", deficiency of Yang Qi results in impaired warming and transformation functions, leading to internal accumulation of Yin-cold. This is closely related to dysregulation of the immune microenvironment, "cold tumor" characteristics, and dysfunction of the neuroendocrine system in modern medicine. Accordingly, the therapeutic strategy of "warming Yang, supporting healthy Qi, and combating cancer" has gained increasing attention. In recent years, commonly used Yang-warming Chinese herbs, including Aconiti Lateralis Radix Praeparata, Zingiberis Rhizoma, Cinnamomi Cortex, Epimedii Folium, and Psoraleae Fructus, as well as their active constituents, have achieved notable progress in anti-lung cancer research by regulating multiple signaling pathways, inducing apoptosis, inhibiting metastasis, and reversing drug resistance. In addition, Yang-warming formulae such as Sini Tang and Yanghe Tang have shown promising effects in alleviating myelosuppression, improving cancer-related fatigue, managing malignant pleural effusion, and relieving cancer pain. These therapies exhibit toxicity-reducing and efficacy-enhancing effects, significantly improving patients' quality of life and survival benefits. To systematically summarize the roles and mechanisms of Yang-warming Chinese herbal medicines and compound formulae in lung cancer, this paper provides a comprehensive review of recent advances, aiming to offer insights for the clinical practice of TCM in the prevention and treatment of lung cancer.

3.Left atrial volumetric/mechanical coupling index for predicting left atrial dysfunction in patients with chronic kidney disease

Objective To observe the value of left atrial volume/mechanical coupling index(LACI)for predicting left atrial(LA)dysfunction in patients with chronic kidney disease(CKD).Methods Totally 213 CKD patients(CKD group)and 50 healthy controls(control group)were enrolled.Clinical data,laboratory indicators and echocardiographic parameters were compared between groups.According to quartile values of LACI,patients in CKD group were further divided into 4 subgroups,and their clinical,laboratory indicators and echocardiographic parameters were compared.The correlations between LACI and laboratory markers of myocardial injury as well as echocardiographic parameters were analyzed.Taken LAVI>34 ml/m2 as LA dysfunction,the efficacy of LACI,LA strain and LA stiffness index(LASI)for predicting LA dysfunction in CKD patients were evaluated.Results Statistical differences of gender,blood pressure,creatinine,estimated glomerular filtration rate(eGFR),cardia troponinT(cTnT),creatine kinase isoenzymes(CK-MB)and N-terminal pro-B-type natriuretic peptide(NT-proBNP)were found between groups(all P<0.05).In CKD group,with the increase of LACI,the prevalence of blood pressure and diabetes,the levels of cTnT and NT-proBNP in different subgroups showed increasing trend,while eGFR showed a decreasing trend.LACI was correlated with cTnT,CK-MB,NT-proBNP and multiple echocardiographic parameters(all P<0.001).The AUC of LACI for predicting LA dysfunction in CKD patients was 0.884,higher than that of LA strain during reservoir phase,conduit phase,contraction phase and LASI(AUC=0.652,0.621,0.611,0.746,all P<0.05).Conclusion LACI could be used to effectively predict LA dysfunction in CKD patients.
4.Establishment of a competency-oriented evaluation system for oncology practice based on entrustable professional activities

Objective:To construct a competency-oriented assessment index system based on entrustable professional activities (EPAs) for 5-year undergraduate clinical medical students in oncology internship.Methods:From June to December 2023, the scoping review approach and Bicomb 2.0 were used to construct and manage an item pool. The draft of EPAs and competencies was designed based on truncated word frequency. SPSS 25.0 was used for cluster analysis and UCINET 6.0 was used for visualization. Combining the characteristics and consensus of oncology, a multi-center expert group used the KJ method to draft the framework of EPAs and competencies. Subsequently, the expert group defined milestones and mapped the milestones to the framework to establish the assessment system.Results:Based on 26 included studies, a draft was created containing 19 EPA indicators and 72 competency characteristic indicators. After cluster analysis, 13 experts from 6 medical institutions established a framework including 13 EPAs and 10 competencies as well as 50 milestones, leading to the construction of the "EPAs-competencies-milestones" assessment system.Conclusions:The "EPAs-competencies-milestones" assessment system aligns with the trend of reform, demonstrating universality, specificity, and scientificity. It provides a reference for the development and assessment of oncology internship courses in medical universities.
5.Detection of etomidate,metomidate,propoxate,and isopropoxate in human hair by UPLC-MS/MS

Objective To develop a rapid and accurate ultra-performance liquid chromatography tandem mass spectrometry(UPLC-MS/MS)method for the detection of etomidate,metomidate,propoxate,and isopropoxate in human hair.Methods Hair samples containing etomidate,metomidate,propoxate,and isopropoxate were extracted with methanol containing the internal standard orthoxine,filtered with a 0.22 μm organic filter membrane and detected vio UPLC-MS/MS.All components were separated by using a gradient elution procedure consisting of 0.01%formic acid(1 mmol/L ammonium acetate)and acetonitrile.Positive electrospray ionization was performed using multiple reaction monitoring(MRM)mode.Results The linear relationships of etomidate,metomidate,propoxate,and isopropoxate were good in the range of 0.01~1 ng/mg(r ≥ 0.997 9),with recovery rates ranging from 87.9%to 101.5%.The accuracy was between 80.0%and 110.0%.The intra-day and inter-day relative standard deviations(RSD)were 2.9%~9.6%and 3.6%~19.9%.Conclusion This method is easy to operate and has high recovery efficiencies.It is sufficiently simple and sensitive to be applied to detect etomidate,metomidate,propoxate,and isopropoxate in hair.

7.Direct measurement assessment study of internal exposure dose of nuclear medicine staff expose to 131I

Objective:To understand the current situation of occupational internal exposure dose research using direct measurement method in China, and to explore the occupational internal dose level of nuclear medicine staff.Methods:From 2023 to April 2024, search on platforms such as China National Knowledge Infrastructure and Chinese Medical Journal Network, to collect research literature on the internal exposure dose monitoring of nuclear medicine staff and discuss the internal exposure dose assessment method. According to the literature reported thyroid 131I activity level at the measuring time, the internal exposure dose level was estimated using the method recommended by GBZ 129-2016. Results:According to existing research reports on direct measurement using portable γ spectrometers, 6.1% of nuclear medicine workers may receive a 131I dose greater than 1 mSv/a. Conclusion:It is necessary to conduct periodic continuous monitoring for personnel whose internal exposure dose may greater than 1 mSv/a, and it is necessary to explore the feasibility of periodic internal exposure monitoring method carried out by hospitals.
8.Energy CT and color mapping for optimizing ossification after lumbar interbody fusion

Objective To observe the effect of energy CT and color mapping to optimize displaying of ossification after lumbar interbody fusion.Methods A total of 76 patients who underwent lumbar fusion were retrospectively enrolled.According to the way of rescanning CT,the patients were divided into energy CT group(n=30)and traditional CT group(n=46).Subjective scoring of imaging quality of both groups on anatomic details,i.e.ossification of bone graft particles in the interbody fusion zone,cancellous bone growth near the endplate in the vertebral body,bone bridge connecting the interface between the fusion zone and the endplate in the vertebral body,as well as image artifacts were performed using a five-point scale.The image noise(SD value),and radiation dose represented by dose length product(DLP)were measured and compared between groups.Results Compared with traditional CT group,energy CT group had elevated subjective scores for anatomical details(4.50[4.28,4.50]vs.3.80[3.50,4.00])and image artifacts(4.30[4.00,4.50]vs.3.60[3.50,4.00]),while significantly lower SD value(37.22[34.90,38.85]HU vs.57.50[53.30,68.59]HU)and DLP(255.28[177.57,321.26]mGy·cm vs.327.60[298.13,415.95]mGy·cm)(all P<0.05).Compared with grayscale images,images processed with color mapping demonstrated enhanced visualization of osseous details.Conclusion Energy CT after lumbar fusion could reduce image artifacts and improve the visualization of anatomical details at a lower radiation dose compared to conventional CT,while color mapping might further enhance the clarity of osseous details.
9.The effect of cytomegalovirus and EB virus activation on hematopoietic reconstitution after intensive immunosuppressive therapy for severe aplastic anemia

Objective:To investigate the infection rate of cytomegalovirus (CMV) and Epstein-Barr virus (EBV) in patients with severe aplastic anemia (SAA) after intensive immunosuppressive therapy in combination with a thrombopoietin receptor agonist (lST+TPO-RA) as well as assess the clinical impact of treatment.Methods:A retrospective, case series study was undertaken involving patients with SAA who were admitted to Soochow Hopes Hematonosis Hospital, The First Affiliated Hospital of Soochow University, and Zhengzhou Third People′s Hospital from June 2022 to February 2025. Thirty patients with complete CMV and EBV monitoring data after IST+TPO-RA treatment were enrolled. The first activation time of CMV and EBV, the maximum viral load, the first negative conversion time, and blood routine tests within 3 days before CMV and EBV positivity, during the positive period, and within 3 days after turning negative were recorded. The patients were followed up for 9 months after the completion of IST. One-way analysis of variance was used to compare the changes of blood routine before and after virus positivity and after turning negative. The χ2 test was used to compare the viral infection rate and the therapeutic effect of IST between the two groups. Results:The 30 SAA patients comprised 15 males and 15 females with an average age of (40.0±16.9) years. Of the 30 patients, 18 (60.0%) were infected with CMV and 6 (20.0%) with EBV. Among them, 17 cases received rabbit anti-human thymocyte immunoglobulin (r-ATG) treatment (r-ATG group), 13 cases received porcine anti-human lymphocyte immunoglobulin (p-ALG) treatment (p-ALG group). The CMV infection rate was significantly higher in the r-ATG group than in the p-ALG group (15/17 vs. 3/13, χ2=13.03, P<0.001); meanwhile, the rate of EBV infection was only slightly higher in the r-ATG group than in the p-ALG group, and the difference did not reach statistical significance (5/17 vs. 1/13, χ2=2.17, P=0.196). In patients infected with CMV, neutrophil, hemoglobin, and platelet counts were significantly decreased during the infection phase, followed by significant increases after CMV clearance ( F=14.48, 11.38, 4.73; all P<0.05). No significant differences in treatment efficacy were found between the r-ATG and p-ALG groups at 3, 6, and 9 months post-IST (all P>0.05). Conclusions:This preliminary study showed that the incidence of CMV and EBV infection in patients with SAA increased after IST, with CMV infections occurring significantly more frequently than EBV infections. The CMV infection rate was significantly higher in patients treated with r-ATG than in those receiving p-ALG. CMV infection was associated with notable alterations in hematological parameters, highlighting the need for close clinical monitoring.
