1.Overseas imported cystic echinococcosis misdiagnosed as pulmonary and hepatic cysts: a case report

Cystic echinococcosis, a zoonotic disease that poses a significant threat to human health and animal husbandry development, is prevalent across the world and predominantly occurs in agricultural and pastoral regions. However, cystic echinococcosis cases are rare in non-endemic areas, which is likely to cause misdiagnosis or missing diagnosis, resulting in delay in treatment. This report presents an overseas imported cystic echinococcosis case misdiagnosed as pulmonary and hepatic cysts, so as to provide insights into diagnosis and treatment of cystic echinococcosis in non-endemic areas.
2.Current status of indoor microbial pollution and evaluation of air purifier effectiveness in Xi'an

Objective To investigate the current situation and health effects of indoor microbial pollution in Xi'an, to analyze the purification effect of air purifiers on indoor microbial pollution, and to provide reference for improving the indoor environment. Methods Through stratified random sampling, 20 families from rural areas and 20 families from upwind and downwind urban areas respectively were selected from Xi'an. Data was collected by questionnaire surveys and on-site environmental sampling. Non-parametric analysis and correlation analysis were used for statistical analysis. Results Overall, the standard-exceeding rate of total count of bacteria was 5.00%. The medians of the total count of bacteria and fungi were 312.50 cfu/m3 and 260.00 cfu/m3, respectively. In terms of health effects, the correlation between rhinitis and cold with total bacterial count was statistically significant (P<0.05), with the correlation coefficients of 0.182 and 0.223, respectively. Purification effect of air purifiers on microbial pollution was statistically significant (P<0.05). After opening for 2 hours, the total numbers of bacteria and fungi decreased significantly. Conclusion The occurrence risk of colds and rhinitis is increased by indoor microbial pollution. Air purifiers have a certain effect on decreasing the total number of bacteria and fungi. It is recommended to use air purifiers with high CADR of particulate matter, double-layer filter and sterilization and dehumidification function, and replace the filters regularly to reduce indoor microbiological contamination.
3.Bridging the widening demand-supply gap of public medical service delivery: experience of the Hospital Authority of Hong Kong and its enlightenment

Due to factors such as an aging population, the Hospital Authority(HA) of Hong Kong is facing a contradiction between limited health resource supply and continuously increasing demand. In order to effectively address challenges, the HA prompted three measures to bridging the demand-supply gap. The HA relied on its management system advantages to continuously increase its capital construction to enhance the service capacity of public health institutions; transformed service delivery mode so as to improve the experience, quality, and efficiency of service delivery; established cooperation with private service providers and communities to shunt population health demand. The practices of HA can provide reference for public hospitals and their sponsors in other regions of China.
4.Regulatory effect of human umbilical cord mesenchymal stem cells on intestinal barrier function in diabetic nephropathy rats

BACKGROUND:Diabetic nephropathy is an important cause of end-stage renal disease,and intestinal barrier damage plays an important role in the occurrence and development of diabetic nephropathy. OBJECTIVE:To observe the protective effect of human umbilical cord mesenchymal stem cells on the intestinal barrier in rats with diabetic nephropathy. METHODS:Thirty 8-week-old male SD rats were randomly assigned to healthy control group,model group and human umbilical cord mesenchymal stem cell group,with 10 rats in each group.Rats in the human umbilical cord mesenchymal stem cell group were injected with 1×106 human umbilical cord mesenchymal stem cells through the tail vein once a week for 4 weeks after the model establishment of diabetic nephropathy.Rats in the healthy control group and the model group were injected with an equal volume of PBS at the same time.1 week after the last injection,the histomorphological changes in the kidney and colon were observed under a light microscope.The expressions of ZO-1 and Occludin in the colon tissue of rats were detected by immunohistochemistry.Serum D-lactic acid and lipopolysaccharide levels were detected by ELISA.In addition,the distribution of human umbilical cord mesenchymal stem cells labeled with DiR dye in rats was observed by in vivo imaging system.The expression of human mesenchymal stem cell surface marker antigens CD44 and CD90 in colon tissue was detected by immunohistochemistry. RESULTS AND CONCLUSION:(1)Compared with the model group,human umbilical cord mesenchymal stem cell transplantation significantly inhibited the increase of urea nitrogen,serum creatinine,24-hour urine protein level and urinary albumin/creatinine ratio in diabetic nephropathy rats(all P<0.05).(2)The expression of human mesenchymal stem cell surface markers CD44 and CD90 was found in the colon of diabetic nephropathy rats.(3)Compared with the healthy control group,the expression levels of tight junction proteins Occludin and ZO-1 in the colon tissue of the model group were significantly reduced,while the expressions of Occludin and ZO-1 were significantly increased after treatment with human umbilical cord mesenchymal stem cells.(4)Compared with the model group,human umbilical cord mesenchymal stem cell transplantation significantly reduced serum D-lactic acid and lipopolysaccharide levels in diabetic nephropathy rats.(5)The results suggest that human umbilical cord mesenchymal stem cells may protect the intestinal barrier function by enhancing the expression of intestinal tight junction proteins in diabetic nephropathy rats.
5.Correlation of circulating ceramides and residual cholesterol with intracranial atherosclerotic stenosis in elderly stroke patients

Objective To investigate the correlation of ceramides(Cer),triglyceride-glucose(TyG)index,residual cholesterol(RC)and intracranial atherosclerotic stenosis(ICAS)in elderly with ischemic stroke(IS).Methods A total of 209 elderly IS patients admitted in the Affiliated Hospi-tal of Xuzhou Medical University from January to December 2022 were recruited,and according to intracranial artery stenosis,they were divided into stenosis group(n=122)and non-stenosis group(n=87).TyG index and RC levels were calculated after testing different types of Cer.Spearman correlation analysis and logistic regression analysis were used.Results Compared with the non-stenosis group,the stenosis group had significantly larger proportions of higher levels of LP(a),HbA1c,TyG index,RC and Cer16∶0,Cer18∶0,Cer24∶1,Cer16∶0/Cer24∶0,Cer24∶1/24∶0 and Cer18∶0/24∶0,and lower Cer24∶ 0 level(P<0.05,P<0.01).The levels of Cer16∶0,Cer18∶0,Cer24∶1,Cer16∶0/24∶0,Cer18∶0/24∶0,Cer24∶1/24∶0,LP(a),HbA1c,TyG index and RC were positively correlated with(r=0.290,0.343,0.383,0.436,0.434,0.481,0.166,0.248,0.140,0.204,P<0.05,P<0.01),and Cer24∶0 was negatively with ICAS(r=-0.247,P<0.01).Cer18∶0,Cer24∶1,Cer16∶0/Cer24∶0,Cer18∶0/Cer24∶0 and RC were important influencing factors for ICAS(P<0.05,P<0.01).Conclusion The levels of Cer,TyG index and RC are higher in elderly IS patients,and these indicators might be the risk factors for ICAS.
6.Time trends and disparities of obesity and related national policies and programs in Nepal:a systematic review

Objective:Overweight and obesity prevalence has increased in low-income countries.This study systematically reviewed the obesity trend,disparities,and prevention and control efforts in Nepal. Methods:We searched PubMed and Google Scholar for articles and reports published between January 1,2004 and December 31,2022.Additional information on National policies and programs related to obesity prevention was retrieved from governmental websites and consultation with relevant experts.Overweight and obesity were defined using the World Health Organization body mass index cut points.Thirty-two studies and reports were included. Results:Overall,overweight and obesity rates increased in all groups in Nepal although nationally representative data remained limited.The combined overweight and obesity(OW/OB)and obesity rates in women aged 15-49 years increased from 8.5%to 22.2%and from 0.9%to 5.1%between 2006 and 2016,respectively.OW/OB and obesity rates in men were 17.1%and 2.5%based on data from the 2016 Demographic and Health Survey.OW/OB rate in under-five children increased from 0.6%to 2.8%between 2006 and 2016.Obesity rates for school-age(5-9 years)boys and girls in 2016 were 2.4%and 2.8%,respectively,and were 1.1%and 1.4%for male and female adolescents aged 10-19 years,respectively.OW/OB prevalence was much higher among women,residents in urban areas and central provinces,and in higher socioeconomic status groups.Projected prevalence of OW/OB and obesity for 2030 in adults aged 15-49 was 44.7%and 8.3%,respectively,while it was 2.2%for OW/OB in preschool children.Policies and direct interventions that specifically focused on obesity prevention and control are limited. Conclusions:OW/OB prevalence in Nepal has increased during the past 1.8 decades,disproportionately affecting population groups.Existing interventions mostly focused on undernutrition with some indirect implications for obesity prevention.In the future,Nepal needs to develop population-based programs for obesity prevention.

Biliary fibrosis (BF) is the result of pathological repair of bile tract injury, characterized by thickening and sclerosis of the bile duct wall and progressive stricture of the lumen, which may ultimately lead to serious adverse outcomes such as biliary obstruction, biliary cirrhosis, liver failure, and hepatobiliary malignancies. Current research describes BF as a pathological feature of certain bile tract diseases, lacking a systematic summary of its etiology, pathophysiology, molecular mechanisms, and treatment. BF is a common but easily neglected disease state in biliary system, which may promote the development and progression of hepatobiliary diseases through abnormal repair mechanism after pathological biliary tract injury. Based on the latest research progress from both domestic and international perspectives, the authors review the concept, clinical manifestation, etiology, pathogenesis, and therapeutic strategies of BF to provide a reference for clinical physicians.
8.Myopia control effect of orthokeratology lens and construction and evaluation of prediction model based on random forest and Logistic regression

Objective To explore the myopia control effect of orthokeratology(OK)lenses and build a prediction model of myopia control effect based on random forest and Logistic regression,so as to provide a basis for further impro-ving adolescent vision.Methods A total of 289 patients who wore OK lenses after seeing an ophthalmologist in Shiji-azhuang People's Hospital from January to June 2019 were selected as the OK lens group,and a total of 289 patients who wore framed glasses during the same period were selected as the framed glass group.After 2 and 4 years of wearing,the in-crease in axial length(AL)of the patients was observed.The patients were divided into a well-controlled myopia group and a poorly-controlled myopia group according to the changes in AL of the eyes after wearing OK lenses for 4 years.The fac-tors influencing the myopia control effect were analyzed based on random forest and Logistic regression,and a nomogram prediction model was constructed.The performance of the model was evaluated using the receiver operating characteristic(ROC)curve,calibration curve,and decision curve analysis(DCA).Results After 2 and 4 years,the diopter increase and AL increase of patients wearing OK lenses were lower than those of patients wearing framed glasses(all P<0.001).There were no significant changes in corneal endothelial cell parameters and central corneal thickness at the thinnest point before,2 years and 4 years after wearing between the two groups(all P>0.05).The random forest model showed that basic AL,daily wear time,correct eye health behavior,pupil diameter,daily outdoor activity time,homework time after school,and daily sleep time were the top 7 variables in importance ranking.Logistic regression analysis showed that home-work time after school was a risk factor for poor myopia control in OK lens wearers,while basic AL,pupil diameter,daily wear time,correct eye health behavior,daily outdoor activity time,and daily sleep time were protective factors for myopia control.The ROC curve,calibration curve,and DCA suggested that the nomogram prediction model constructed based on random forest and Logistic regression analysis had good accuracy,consistency and clinical effectiveness.Conclusion OK lens shows good myopia control effect,has little effect on corneal endothelial cells and corneal thickness,and demon-strates high safety.Homework time after school,basic AL,pupil diameter,daily wear time,correct eye health behavior,daily outdoor activity time,and daily sleep time are influencing factors of myopia control.Strengthening the construction of a prediction model is helpful in identifying high-risk groups with poor myopia control and guiding clinical interventions in time.
9.Correlation of Ceramide and Intracranial and Extracranial Atherosclerotic Stenosis in Elderly Patients with Ischemic Stroke

Objective To investigate the valuation of ceramide(Cer),trigly ceride-glucose index(TyG index)and residual cho-lesterol(RC)levels on intracranial and extracranial atherosclerotic stenosis in elderly patients with ischemic stroke(IS).Methods A total of 447 elderly patients with IS who were hospitalized in the Affiliated Hospital of Xuzhou Medical University from January 2022 to June 2023 were selected.According to the digital subtraction angiography(DSA)results,the patients were divided into non-stenosis group(n=144)and stenosis group(n=303).The stenosis group was further divided into moderate stenosis group(n=121),severe stenosis and occlusion group(n=182).The general information and baseline biochemical data of all patients were collected.High per-formance liquid chromatography tandem mass spectrometry was used to detect serum Cer,and TyG index and RC level were calculated.Spearman method was used to analyze the correlation between Cer,TyG index,RC and intracranial and extracranial atherosclerotic steno-sis.Multivariate Logistic regression was used to analyze the risk factors of intracranial and extracranial atherosclerotic stenosis.Results Compared with the non-stenosis group,Cer,TyG index,RC,Lp(a),and glycosylated hemoglobin(HbA1c)and other indexes were significantly increased in the stenosis group(P<0.05).Multivariate Logistic regression analysis showed that Cer16:0,Cer18:0,Cer24:1,Cer24:1/Cer24:0,Cer18:0/Cer24:0,Cer16:0/Cer24:0 were independent risk factors for intracranial and extracranial atherosclerotic stenosis in elderly patients with IS.Spearman correlation analysis showed that Cer24 was negatively correlated with intracranial and ex-tracranial atherosclerotic stenosis(r=-0.122,P<0.05).The results of receiver operating characteristic curve showed that,the areas under the curve of Cer24:1,Cer24:1/Cer24:0,Cer18:0/Cer24:0,Cer16:0/Cer24:0,RC,TyG index in the diagnosis of intracranial and extracranial atherosclerotic stenosis in elderly patients with IS were 0.832,0.846,0.826,0.829,0.578,0.586(P<0.05).Conclusion The serum levels of serum Cer,TyG index and RC are higher in elderly patients with IS,which may be the risk factors for intracranial and extracranial atherosclerotic stenosis in elderly patients with IS.
10.Method for non-invasive prenatal paternity testing based on SNP calculation through high-throughput sequencing

Objective To derive the parentage index(PI)and power of exclusion(PE)formulars for non-invasive prenatal paternity testing(NIPPT),empirically validate the PE formula,and conduct statistical analysis on the PI formula.Methods Firstly,NIPPT was divided into 6 types,including Prenatal parentage testing of Alleged Father with biological mother(PAF),prenatal parentage testing of low fetal concentration Alleged Father with biological mother(PLAF),Prenatal parentage testing of Father-Child(PFC),Prenatal parentage testing of Mother-Child(PMC),Prenatal parentage testing of Alleged Parents(PAP),Prenatal parentage testing of Alleged mother with biological father(PAM).Then the formulas of PI in 6 types of NIPPT both in accordance with genetic laws and in the presence of mutation were derived.The calculation formulas of PE were mathematically derived and applied to the system of 10 SNP loci.And for the same 10 SNP sites,PE was simulated and measured through multiple groups of operation.In principle,the PE formula should be empirically verified using the correlation method for the corresponding values of the formula and simulation.Finally,models of simulated kinship groups and unrelated individual groups were established respectively with 30 to 100 SNP loci.The geometric mean(Π)and 95%confidence interval(CI)of cumulative parentage index(CPI),the percentages of cases where CPI>104 in kinship groups,and the percentages of cases where CPI<10-4 in unrelated individual groups were calculated.Results The formula of PE was all in accord with the simulation experiments.The novel formulas can be used to calculate all kinds of PI in all situations.All types of NIPPT cases in kinship group and unrelated individual group can be correctly identified with 100 SNP loci.In PLAF,75.90%cases in kinship group and 100%cases in unrelated individual group can be correctly identified in 50 SNP loci.Conclusion The formula in this paper can be applied to the calculation of PI and PE of NIPPT and has forensic application value.
