1.Structural valve deterioration after transcatheter aortic valve replacement: a research update.

Structural valve deterioration (SVD) refers to intrinsic and irreversible pathological changes in the components of prosthetic heart valves, manifesting as fibrosis, calcification, wear and tear, loosening, as well as strut fracture or deformation of the valve framework. These changes ultimately lead to valve stenosis and/or regurgitation.The mechanisms may be related to mechanical stress, immune response and abnormal calcium-phosphorus metabolism. Studies have shown that risk factors for SVD include patient factors (such as age, underlying cardiovascular disease and comorbidities), valve factors (such as material properties, processing techniques, and valve type), and surgical factors (such as valve injury, suboptimal stent expansion, and irregular stent release morphology). Clinical imaging assessment of SVD demonstrates complementary advantages among echocardiography, multi-detector spiral CT and cardiac magnetic resonance imaging, with distinct diagnostic objectives. The primary management strategies for SVD after trans-catheter aortic valve replacement (TAVR) include drug therapy, redo-TAVR, surgical aortic valve replacement (SAVR) and the novel SURPLUS technique. Among them, redo-TAVR has become a common method because of its minimally invasive nature, but it is still necessary to further clarify the patient indications and optimize the surgical strategy. SAVR is reserved for young, low-risk patients; SURPLUS combines the advantages of SAVR and TAVR, making it suitable for cases where redo-TAVR is unfeasible or contraindicated, while the risk of SAVR is excessively high. This article reviews the latest progress of SVD following TAVR treatment to provide reference for research into the durability of bioprosthetic valve and clinical intervention of SVD.

2.Fingerprint-enhanced hierarchical molecular graph neural networks for property prediction.

Accurate prediction of molecular properties is crucial for selecting compounds with ideal properties and reducing the costs and risks of trials. Traditional methods based on manually crafted features and graph-based methods have shown promising results in molecular property prediction. However, traditional methods rely on expert knowledge and often fail to capture the complex structures and interactions within molecules. Similarly, graph-based methods typically overlook the chemical structure and function hidden in molecular motifs and struggle to effectively integrate global and local molecular information. To address these limitations, we propose a novel fingerprint-enhanced hierarchical graph neural network (FH-GNN) for molecular property prediction that simultaneously learns information from hierarchical molecular graphs and fingerprints. The FH-GNN captures diverse hierarchical chemical information by applying directed message-passing neural networks (D-MPNN) on a hierarchical molecular graph that integrates atomic-level, motif-level, and graph-level information along with their relationships. Additionally, we used an adaptive attention mechanism to balance the importance of hierarchical graphs and fingerprint features, creating a comprehensive molecular embedding that integrated hierarchical molecular structures with domain knowledge. Experiments on eight benchmark datasets from MoleculeNet showed that FH-GNN outperformed the baseline models in both classification and regression tasks for molecular property prediction, validating its capability to comprehensively capture molecular information. By integrating molecular structure and chemical knowledge, FH-GNN provides a powerful tool for the accurate prediction of molecular properties and aids in the discovery of potential drug candidates.

9.Distribution and antimicrobial resistance of Enterobacter cloacae complex isolated from a tertiary hospital in Shanghai

Objective To investigate the distribution and antimicrobial resistance of Enterobacter cloacae complex(ECC)isolated from a tertiary hospital in Shanghai.Methods Clinical ECC isolates were collected from 2018 to 2020.Antimicrobial susceptibility test was performed with broth microdilution and agar dilution methods.PCR was applied to detect five carbapenemase genes(blaKPC,blaNDM,blaIMP,blaVIM and blaOXA-48).Results A total of 222 ECC isolates were collected from 2018-2020,including 36 strains(16.2%)from general surgery department.The strains were mainly isolated from sputum(41.0%)and urine(20.3%).MIC results showed that the isolates were highly resistant to trimethoprim-sulfamethoxazole,the second-and third-generation cephalosporins,aztreonam and quinolones(31.1%-71.2%resistant),but low resistance rates to tigecycline,amikacin,mecillinam,and ceftazidime-avibactam(0.5%-9.0%resistant).About 9.5%and 10.4%of the strains were resistant to meropenem and imipenem,respectively.A total of 34 ECC strains were carbapenem-nonsusceptible strains,of which 23 strains were resistant to carbapenems.Among the 34 carbapenem-nonsusceptible strains,25(73.5%)strains were susceptible to mecillinam,including 15 strains producing metallo-β-lactamase.PCR assay identified carbapenemase genes in 21 of the 34 carbapenem-nonsusceptible strains,including blaNDM(14 strains),blaIMP(5 strains)and blaKPC(2 strains).Compared with the strains isolated in 2018,the ECC strains isolated in 2019 and 2020 showed significantly higher resistance rates to imipenem and ceftazidime-avibactam(P<0.05),primarily associated with the production of NDM.Among the ECC strains resistant to third-generation cephalosporins,20.4%were resistant to imipenem and meropenem,while 4.9%and 8.7%were resistant to amikacin and mecillinam,respectively,and 9.7%were nonsusceptible to tigecycline.Conclusions The ECC isolates in 2019 and 2020 showed increasing resistance rates to carbapenems and ceftazidime-avibactam due to the production of metallo-β-lactamases.Some of the metallo-β-lactamase-producing ECC isolates were susceptible to mecillinam.
