1.Summary of best evidence for breastfeeding guidance and management for pregnant women with gestational diabetes mellitus

Objective:To summarize the evidence related to the guidance and management of breastfeeding in pregnant women with gestational diabetes mellitus.Methods:UpToDate, BMJ Best Practice, CINAHL, PubMed, Cochrane Library, Embase, Wanfang database, China National Knowledge Infrastructure, and other databases and professional websites were searched by computer for evidence on breastfeeding guidance and management for pregnant women with gestational diabetes mellitus, including clinical decisions, guidelines, evidence summaries, systematic reviews, expert consensus and randomized controlled trials. The search period was from database establishment to April 1, 2023. Two researchers independently conducted literature quality evaluation, evidence extraction, and integration.Results:A total of 23 articles were included, including 4 clinical decision-making articles, 4 guidelines, 4 evidence summaries, 6 systematic reviews, 1 expert consensus and 4 randomized controlled trials. A total of 21 best pieces of evidence were summarized from six aspects of support and education, knowledge and skills, safety issues, feeding issues, dietary care, and blood glucose monitoring.Conclusions:This study summarizes the best evidence on breastfeeding guidance and management for pregnant women with gestational diabetes and suggests that the medical staff should apply the proof according to the clinical situation and the patient's wish.
2.Application of High Semi-decubitus Position in Epidural Labor Analgesia in the Second Stage of Labor

Objective:To investigate the efficacy and safety of high semi-decubitus position in the second stage of labor in epidural analgesic delivery of primipara.Methods:A total of 120 women who gave birth in Hang-zhou women's Hospital from May 1,2021 to July 30,2021 were randomly divided into experimental group(n=60)and control group(n=60)by random number table method.The control group adopted supine position in the sec-ond stage of labor,and the experimental group transferred to high semi-supine position after the supine position was used applied for 30 minutes when the fetal head was not exposed.To compare the effects of different postur-al positions on the duration of the second stage of labor,mode of delivery,postpartum injury,perineal tearing,la-bor force experience and labor control,and neonatal asphyxia.Results:The duration of uterine opening to fetal head exposure,the duration of uterine opening to fetal head crown and the duration of second stage of labor in experimental group were shorter than those in control group.The rate of natural delivery of experimental group was higher than that of control group.The episiotomy rate and the second degree perineal laceration rate were lower than that of control group.The amount of blood loss during delivery and 2h postpartum of experimental group was less than that of control group.The scores of birth experience and birth control of experimental group were higher than those of control group,and the differences were statistically significant(P<0.05).There was no significant difference in 1-minute Agpar score and 5-minute Agpar score between the two groups(P>0.05).Conclusions:The application of high semi-decubitus position under epidural labor analgesia promoted vaginal natural labor,shortened the time of second stage of labor,reduced episiotomy rate and perineal laceration de-gree,reduced postpartum hemorrhage,had positive labor force experience,and increased the sense of labor con-trol.
3.Tongmai Kaiqiao Pills Treat Vascular Dementia in Rats by Regulating Mitochondrial Autophagy via HIF-1α/BNIP3 Signaling Pathway

ObjectiveTo observe the effects of Tongmai Kaiqiao pills on the hypoxia-inducible factor-1α (HIF-1α)/adenovirus E1B 19 kD-interacting protein 3 (BNIP3) signaling pathway and mitochondrial autophagy in the hippocampus of the rat model of vascular dementia (VD). MethodNinety male SD rats underwent adaptive feeding for one week before the study. Ten rats were randomly assigned to the sham group, where the common carotid artery was isolated without ligation. The remaining rats were subjected to sequential ligation of the common carotid artery for the modeling of VD. The successfully modeled rats were randomly assigned into the following groups: model, high-, medium-, and low-dose (27.6, 13.8, 6.9 g·kg-1, respectively) Tongmai Kaiqiao pills, donepezil hydrochloride (0.45 mg·kg-1), and combination (27.6 g·kg-1 Tongmai Kaiqiao pills + 2.5 mg·kg-1 HIF-1α inhibitor YC-1) groups. After 4 weeks of treatment, samples were collected. Nissl staining and hematoxylin-eosin staining were performed to observe the loss of neurons and pathological changes, respectively, in the hippocampal region. Western blot was employed to determine the protein levels of HIF-1α, BNIP3, Beclin-1, and microtubule-associated protein 1 light chain 3B (LC3B) in the hippocampal tissue. Transmission electron microscopy was used to observe the mitochondrial ultrastructure and the number of autophagosomes in the hippocampal tissue. Immunofluorescence was employed to observe the fluorescence intensity of HIF-1α, BNIP3, and LC3B in the hippocampal tissue. ResultCompared with the sham group, the model group showed prolonged escape latency (P<0.01), decreased number of platform crossings (P<0.01), reduced and disarranged neuronal layers in the hippocampal region, decreased number of Nissl bodies, disrupted mitochondrial cristae, damaged mitochondrial double-membrane structures, increased number of autophagosomes, upregulated expression of HIF-1α, BNIP3, beclin1, and LC3B (P<0.05, P<0.01), and enhanced fluorescence intensity of HIF-1α, BNIP3, and LC3B (P<0.05, P<0.01). Compared with the model group, Tongmai Kaiqiao pills and donepezil hydrochloride shortened the searching time for the platform (P<0.01) and increased the number of platform crossings (P<0.01). Moreover, the drugs increased the number of neurons with normal morphology and orderly arrangement and the number of Nissl bodies, alleviated the damage, increased the number of autophagosomes, upregulated the expression of HIF-1α, BNIP3, Beclin1, and LC3B (P<0.05, P<0.01), and enhanced the fluorescence intensity of HIF-1α, BNIP3, and LC3B (P<0.05, P<0.01). Compared with high-dose Tongmai Kaiqiao pills, the combination group prolonged the escape latency (P<0.01), reduced the number of crossing platforms (P<0.01), decreased the number of hippocampal neurons, aggravated the damage, decreased the number of Nissl bodies and autophagosomes, downregulated the expression of HIF-1α, BNIP3, beclin1, and LC3B (P<0.01), and decreased the fluorescence intensity of HIF-1α, BNIP3, and LC3B (P<0.01). ConclusionTongmai Kaiqiao pills may activate the HIF-1α/BNIP3 signaling pathway to promote the occurrence of mitochondrial autophagy, clear damaged mitochondria, provide energy for healthy cells, reduce neuronal cell death, and restore the brain function, thereby reducing ischemic damage to the hippocampal tissue, improving learning and memory abilities, and exerting therapeutic effects on VD in rats.
4.Factors affecting sleep disorders among pregnant women

Objective:
To investigate the current status and influencing factors of sleep disorders among pregnant women, so as to provide insights into health management during pregnancy.
Methods:
Pregnant women who underwent prenatal checkups at Hangzhou Obstetrics and Gynecology Hospital from January to October 2023 were selected as subjects, and general data including age, pregnancy period and exercise were collected through questionnaire surveys. Sleep quality, pregnancy stress, anxiety and depression were evaluated using Pittsburgh Sleep Quality Index, Pregnancy Stress Rating Scale, Pregnancy-related Anxiety Scale and Edinburgh Postnatal Depression Scale, respectively. Factors affecting sleep disorders among pregnant women were analyzed using a multivariable logistic regression model.
Results:
A total of 386 pregnant women was surveyed, with a mean age of (30.28±4.65) years, including 20.47% in the first trimester, 47.93% in the second trimester and 31.61% in the third trimester. Women with anxiety and depression accounted for 14.51% and 21.76%, respectively. Pregnancy stress was mainly moderate, accounting for 51.04%. There were 106 pregnant women with sleep disorders, accounting for 27.46%. Mutivariable logistic regression analysis showed that age (≥35 years, OR=1.656, 95%CI: 1.094-2.503), pregnancy period (third pregnancy, OR=2.097, 95%CI: 1.213-3.621), regular exercise in the past 6 months (OR=0.376, 95%CI: 0.210-0.670), anxiety (OR=2.794, 95%CI: 1.545-5.048), depression (OR=3.501, 95%CI: 1.877-6.529) and pregnancy stress (moderate, OR=1.355, 95%CI: 1.018-1.801; severe, OR=2.538, 95%CI: 1.417-4.540) were the factors affecting sleep disorders among pregnant women.
Conclusions
Sleep disorders of pregnant women is influenced by age, pregnancy period, pregnancy stress, anxiety, depression and exercise. It is necessary to identify high-risk individuals with sleep disorders early, and to provide psychological intervention and prenatal health guidance.
5.Single-cell transcriptomics reveals tumor landscape in ovarian carcinosarcoma

Objective:The present study used single-cell RNA sequencing(scRNA-seq)to characterize the cellular composition of ovarian carcinosarcoma(OCS)and identify its molecular characteristics.Methods:scRNA-seq was performed in resected primary OCS for an in-depth analysis of tumor cells and the tumor microenvironment.Immunohistochemistry staining was used for validation.The scRNA-seq data of OCS were compared with those of high-grade serous ovarian carcinoma(HGSOC)tumors and other OCS tumors.Results:Both malignant epithelial and malignant mesenchymal cells were observed in the OCS patient of this study.We identified four epithelial cell subclusters with different biological roles.Among them,epithelial subcluster 4 presented high levels of breast cancer type 1 susceptibility protein homolog(BRCA1)and DNA topoisomerase 2-α(TOP2A)expression and was related to drug resistance and cell cycle.We analyzed the interaction between epithelial and mesenchymal cells and found that fibroblast growth factor(FGF)and pleiotrophin(PTN)signalings were the main pathways contributing to communication between these cells.Moreover,we compared the malignant epithelial and mesenchymal cells of this OCS tumor with our previous published HGSOC scRNA-seq data and OCS data.All the epithelial subclusters in the OCS tumor could be found in the HGSOC samples.Notably,the mesenchymal subcluster C14 exhibited specific expression patterns in the OCS tumor,characterized by elevated expression of cytochrome P450 family 24 subfamily A member 1(CYP24A1),collagen type XXIII α1 chain(COL23A1),cholecystokinin(CCK),bone morphogenetic protein 7(BMP7),PTN,Wnt inhibitory factor 1(WIF1),and insulin-like growth factor 2(IGF2).Moreover,this subcluster showed distinct characteristics when compared with both another previously published OCS tumor and normal ovarian tissue.Conclusions:This study provides the single-cell transcriptomics signature of human OCS,which constitutes a new resource for elucidating OCS diversity.
6.Cutaneous hypopigmented lymphoproliferative disorders: a clinicopathological study of 41 cases

Objective:To investigate clinicopathological features of hypopigmented mycosis fungoides (HMF) and hypopigmented interface T-cell dyscrasia (HITCD) .Methods:A total of 41 patients with cutaneous hypopigmented lymphoproliferative diseases, who had complete clinicopathological data, were collected from Department of Dermatology, the Third People′s Hospital of Hangzhou from January 2015 to September 2020, and the clinicopathological and immunophenotypic features were analyzed. Comparisons of normally distributed measurement data were carried out using t test, comparisons of categorical data using Chi-square test or Fisher′s exact test, and comparisons of ranked data between 2 groups using rank-sum test. Results:All of the 41 patients clinically presented with irregular hypopigmentation, some of which was accompanied by erythema or furfuraceous scales. In terms of pathological features, 21 patients showed infiltration and aggregation of atypical lymphoid cells in the epidermis, which was consistent with typical pathological features of mycosis fungoides, and they were diagnosed with HMF; 20 patients showed vacuolar degeneration of the basal layer, accompanied by infiltration of lymphoid cells and mild epidermotropism, and they were diagnosed with HITCD. All immune cells expressed T-cell phenotype, and epidermal lymphocytes expressed a CD8-dominated phenotype in 14 (67%) cases of HMF and 13 (65%) of HITCD. In the epidermis, the total number of lymphocytes was significantly higher in the HMF group than in the HITCD group ( t= 1.81, P= 0.012) ; in the dermis, the number of CD4 + lymphocytes and CD8 + lymphocytes, and the total number of lymphocytes were all significantly higher in the HMF group than in the HITCD group ( t= 2.64, 1.51, 2.60, P= 0.012, 0.002, 0.001, respectively) . All patients were treated with narrow-band ultraviolet B radiation. Among 34 patients who completed the follow-up, 30 achieved complete clearance of skin lesions without recurrence, including all patients with HITCD, and 4 with HMF achieved partial regression of the lesions. Conclusions:Compared with HMF, HITCD presents different pathological characteristics and benign biological behaviors. Thus, HITCD should be distinguished from HMF as an independent disease. Phototherapy alone is effective for the treatment of HITCD.
7.Analysis of influencing factors for the occurrence of neonatal scalp hematoma

Objective:To discuss the influencing factors of neonatal scalp hematoma, and provide a basis for reducing the occurrence of neonatal scalp hematoma.Methods:Retrospectively analyzed the data of 13 151 parturients and their newborn infants who delivered in the Hangzhou Women′s Hospital from January to December 2020, counted the incidence of neonatal scalp hematoma, and Logistic regression analysis was used to analyze its influencing factors.Results:Among 13 151 newborn baby, 68 cases of neonatal head hematoma occurred, the incidence was 0.52%. Vaginal delivery, forceps, intraspinal analgesia were statistically significant in the occurrence of neonatal scalp hematoma ( χ2=19.26, 19.59, 6.11, all P<0.05). Conclusions:Vaginal delivery, forceps, and intraspinal analgesia are the risk factors for neonatal scalp hematoma. Lateral perineal incision during delivery did not reduce the incidence of neonatal scalp hematoma. In this study, many cases of neonatal scalp hematoma occurred before the crown of the fetal head. Labor management, improve the rate of non-drug analgesia, promote natural delivery, early detection, early prevention, and reduce the incidence of neonatal hematoma.
8.Meta-analysis of Clinical Efficacy and Safety of Misoprostol versus Oxytocin in Preventing Postpartum Hemorrhage

OBJECTIVE:To systematically evaluate the c linical efficacy and safety of misoprostol versus oxytocin in preventing postpartum hemorrhage ,and to provide evidence-based reference for clinical drug use. METHODS :Retrieved from CNKI,VIP,Wanfang database ,CBM,PubMed,Embase and the Cochrane Library ,randomized controlled trials (RCTs) published at home and abroad about clinical efficacy and safety of misoprostol and oxytocin in preventing postpartum hemorrhage were collected during the inception to Feb. 15th,2021. According to the inclusion and exclusion criteria ,the included studies were screened and evaluated. RevMan 5.3 software was used for Meta-analysis. RESULTS :A total of 14 RCTs involving 25 732 lying-in women were included. The results of Meta-analysis showed that compared with oxytocin group ,there was no difference in the mean blood loss (MD=14.96,95%CI(-2.62,32.54),P=0.10),the time of the third stage of labor (MD=-0.03,95%CI (-0.19,0.13),P=0.69),the incidence of blood transfusion (RR=0.91,95%CI(0.74,1.12),P=0.37)or the incidence of nausea(RR=1.06,95%CI(0.85,1.31),P=0.60)in misoprostol group ;but the incidence of shivering(RR=2.99,95%CI (2.22,4.04),P<0.000 01),the incidence of fever (RR=4.31,95%CI(2.79,6.67),P<0.000 01),the incidence of vomiting (RR=2.16,95%CI(1.63,2.88),P<0.000 01)and the incidence of diarrhea (RR=2.67,95%CI(1.79,3.98),P<0.000 01)in misoprostol group were higher. CONCLUSIONS :The effect of misoprostol in the prevention of postpartum hemorrhage is similar to oxytocin ,but the incidence of ADR is higher. It is necessary to pay close attention to the adverse reactions during use. Due to the limitation of included studies ,multi-center,large-scale and high-quality RCTs are needed for further validation.
9.Performance of reflectance confocal microscopy in localization diagnosis of and monitoring of therapeutic efficacy in early-stage mycosis fungoides

Objective:To analyze consistency between histopathological and reflectance confocal microscopy (RCM) characteristics of early-stage mycosis fungoides (MF) , and to evaluate the value of RCM in assisting the pathological diagnosis of early-stage MF and the feasibility of dynamic monitoring of treatment response with RCM.Methods:From January 2014 to January 2018, 40 cases of clinically suspected MF were collected from Department of Dermatology, Third People′s Hospital of Hangzhou, including 26 males and 14 females, and their age was 47.0 ± 17.6 years. According to the summarized RCM characteristics of early-stage MF, biopsy sites were preliminarily located, and then a histopathological examination was performed. The RCM and pathological features of MF were compared. In addition, a combination therapy with narrowband ultraviolet B and interferon was performed in patients with confirmed MF. Targeted lesions were followed up with RCM for 9 months, and then therapeutic efficacy was evaluated.Results:Among the 40 cases of clinically suspected MF, 8 were preliminarily diagnosed as typical MF, 18 as suspected MF, and 14 were excluded according to the RCM characteristics; according to the pathological features, 12 could be diagnosed as typical MF, 14 as suspected MF, and 14 were excluded. Consistency analysis showed that the kappa coefficient between RCM classification and pathological diagnosis was 0.848 ( P < 0.01) . The consistency of epidermal infiltration of mildly refractive cells was the highest between RCM and pathological findings (kappa coefficient = 1, P = 0.005) , followed by dermal fibrosis at the erythema stage (kappa coefficient = 0.714, P = 0.035) . The RCM characteristics of MF gradually returned to normal during treatment, but atypical lymphocytes still existed when clinical lesions completely regressed. Conclusion:RCM can be used for pathological localization of suspected MF lesions in the early stage, and for dynamic monitoring of therapeutic efficacy in MF.
10.Application effect of mindful delivery exercise during pregnancy in primipara

Objective:To explore the application value of mindful delivery exercise method in the personalized nursing management program of negative emotions of primipara, so as to improve maternal emotion and ensure the safety of mothers and children.Methods:From January 12, 2018 to January 23, 2019, a total of 120 pregnant women who underwent antenatal examination in Hangzhou Obstetrics and Gynecology Hospital were selected as research objects. According to the random number table method, the patients were divided into the control group and the experimental group. During the study period, 10 cases were dropped, and 110 cases were eventually included in the study. Finally, there were 56 cases in the control group and 54 cases in the experimental group. Pregnant women in the control group received routine prenatal check-ups, and interventions were conducted in the form of courses. The experimental group implemented a mindful delivery exercise method based on the intervention of the control group. The Chinese version of Depression-Anxiety-Stress Scale (DASS-21) was used to evaluate the maternal depression-anxiety-stress status before and after intervention, the pain degree was evaluated by VAS, and the delivery outcomes of two groups were recorded.Results:The scores of stress, anxiety and depression in the DASS-21 of the experimental group one month after intervention were lower than those of the control group, the time of the first stage of labor was shorter than that of the control group, and the spontaneous labor rate was higher than that of the control group. The differences were statistically significant ( P<0.01) . The VAS score of 1 hour after delivery of the experimental group was lower than that of the control group, and the difference was statistically significant ( P<0.01) . Conclusions:The application of mindful delivery exercise in the personalized nursing management of negative emotions of parturient women can help improve maternal mood, relieve pain, shorten the first stage of labor and improve the rate of spontaneous delivery.
