1.Modified probiotics and the related combinatorial therapeutics.

Probiotics have shown excellent application prospects in preventing and treating many diseases. However, their sensitivity to the harsh environment in vivo always leads to a massive loss of viability and insufficient therapeutic effect. Fortunately, modified probiotics have emerged and provide multiple possibilities for their use in various diseases. Modification not only endows probiotics with extra capacity to resist severe environments but also gives them exogenous characteristics, such as prolonged retention time and improved therapeutic effects. Modified probiotics could combine with other therapies, which has opened up new avenues to enhance the efficacy of probiotic-based therapy. In this review, we have summarized the current physicochemical and biological modification strategies of probiotics. In addition, the progress of research on probiotic-based combination therapy has also been extensively reviewed, which contributes to the enhanced delivery of probiotics or other active constituents and provides new ideas for disease treatment, bioimaging, and diagnosis.
2.Application of two different methods of visual acuity assessment for myopia screening among lower grade elementary school students

Objective:
To explore two visual acuity standards for examining uncorrected visual acuity (UCVA) to define poor vision in lower grade elementary school students, and to compare the difference of screening myopia rates when combined with non cycloplegic auto refraction (NCAR), so as to provide a scientific basis for standardizing UCVA examination methods using CAR as the gold standard of authenticity and reliability.
Methods:
From March 22nd to April 9th, 2023, a total of 549 first and second grade students aged 7-8 years from a primary school in Hefei City were selected for the study by convenient cluster sampling method. Two methods were employed for UCVA examination:the first method involved charts where the student could not make mistakes in identifying at least half of the characters per line (V1), and the second method used charts with character sizes ranging from 4.0 -4.5, 4.6-5.0 and 5.1-5.3, without allowing 1, 2 and 3 errors per line (V2). While NCAR was performed, then 187 students underwent CAR examination. Paired Wilcoxon rank-sum test and McNemar test were used to compare the differences between V1 and V2 methods in defining poor vision and screening myopia rates. Using CAR as the gold standard, the authenticity and reliability of defining screening myopia rates through the combination of V1 and V2 methods along with NCAR were evaluated.
Results:
The UCVA examination results for V1 and V2 showed statistically significant differences in both the right eye [5.0(4.9,5.0), 4.9(4.8,5.0)] and the left eye [ 5.0 (4.9,5.0), 4.9(4.8,5.0)] ( Z=-13.95, -13.34, P <0.01). The detection rates of poor vision for the right eye were 43.53% for V1 and 63.21% for V2, and the left eye with 44.08% for V1 and 62.11% for V2, with statistically significant differences ( χ 2= 106.01 , 95.09, P <0.01). When screening myopia rates were assessed for UCNA methods combined with NCAR, the right eye rates were 21.49% for V1 and 24.59% for V2, and the left eye rates were 21.31% for V1 and 23.13% for V2, with statistically significant differences ( χ 2=15.06, 8.10, P <0.01). Using CAR as the gold standard, the detection rates in the right eye and left eye were 16.58 % and 17.11%, respectively. The Youden indices for defining screening myopia in the right eye were 0.80 for V1 and 0.79 for V2, and the left eye with 0.85 for V1 and 0.83 for V2. The agreement rates for the right eye were 91.98 % for V1 and 89.30% for V2, and the left eye with 94.12% for V1 and 91.98% for V2. The Kappa values for the right eye were 0.73 for V1 and 0.67 for V2, and the left eye with 0.81 for V1 and 0.75 for V2.
Conclusions
Authenticity and reliability of two UCVA examination methods combined with NCAR in defining screening myopia are higher in V1 than V2 methods. It is recommended to unify the visual acuity examination methods by requiring the correct identification of more than half of the total number of visual markers in a row.
3.Treatment of Endometriosis from the Perspective of "Retention due to Deficiency Qi"

It is believed that retention due to deficient qi is an important pathogenesis of endometriosis (EMs). Deficient qi is the root of the disease, mainly manifested as spleen deficiency, while retention is the branch pathogenesis of the disease, mainly with blood stasis, complicated with constraint, phlegm, heat, toxin and other pathological factors. Therefore, it is proposed to follow the treatment principle of supplementing deficiency and unblocking stagnation, and take the methods of replenishing qi and fortifying the spleen, removing stasis and eliminating concretions. Self-made Fuzheng Huayu Formula (扶正化瘀方) is taken as the basic formula, and can be modified with the symptoms in menstrual and non-menstrual periods. Additionally, the methods of moving qi, dispelling phlegm, clearing heat, relieving toxin and others can be combined, and it is recommended to treat the root and the branch simultaneously.
4.Correlation between CT findings and expression levels of serum miR-497, CA24-2 and HBsAg in patients with colorectal cancer

Objective:To investigate the correlation between computed tomography (CT) findings and serum micrornA-497 (miR-497), carbohydrate polypeptide antigen 24-2 (CA24-2) and hepatitis B surface antigen (HBsAg) expression in patients with colorectal cancer (CRC).Methods:A total of 64 CRC patients admitted to the Jiangsu Cancer Hospital from January 2021 to December 2022 were prospectively selected, and the surgical pathological results were used as the gold standard. Preoperative CT examination and serum miR-497, CA24-2 and HBsAg levels were performed in all patients. The CT images of the patients were analyzed. Pearson correlation analysis was used to evaluate the correlation between CT findings and serum miR-497, CA24-2 and HBsAg expression levels in CRC patients.Results:The CT imaging features of CRC patients were as follows: The intestinal wall was semi-annular in 37.50%(24/64), annular in 32.81%(21/64), localized thickening in 29.69%(19/64), the intestinal lumen was irregular and narrow, and most of the periintestinal fat was blurred in 59.38%(38/64). 67.19%(43/64) showed uneven enhancement, 51.56%(33/64) had lymph node metastasis and 43.75%(28/64) had liver metastasis. Lymph node metastasis in CRC patients was correlated with serum miR-497 and CA24-2 levels (all P<0.05), but not with serum HBsAg levels ( P>0.05). Liver metastasis in CRC patients was associated with serum HBsAg expression ( P<0.05), but not with serum miR-497 and CA24-2 levels (all P>0.05). Other CT findings of CRC patients were not correlated with serum miR-497, CA24-2 and HBsAg levels (all P>0.05). Pearson correlation analysis showed that lymph node metastasis in CRC patients was negatively correlated with the expression level of serum miR-497 ( r=-0.491, P<0.05), while positively correlated with the expression level of serum CA24-2 ( r=0.506, P<0.05). There was no correlation with serum HBsAg expression level ( P>0.05). The occurrence of liver metastasis in CRC patients was negatively correlated with the expression level of serum HBsAg ( r=-0.529, P<0.05), but was not correlated with the expression levels of serum miR-497 and CA24-2 (all P>0.05). Conclusions:CT imaging findings can evaluate the biological behavior of tumor cells in CRC patients to a certain extent, and have a certain correlation with the expression levels of serum miR-497, CA24-2 and HBsAg, which can reflect the lymph node and liver metastasis of tumors, and provide a basis for clinical comprehensive treatment and prognosis prediction of patients.

[Objective]To introduce the experiences of prescription based on syndrome differentiation of Ningbo famous traditional Chinese medicine doctor CHEN Ninggang in using Baihua Sheshecao Decoction to treat acne.[Methods]Comparing analyse of the similarities and differences of the pathogenesis based on the ancient classics,combining Master CHEN's experience through both ancient and modern learning to explore the clinical pathogenic factors and pathogenesis characteristics of modern patients,the prescription structure and the connotation of the prescription of the Baihua Sheshecao Decoction are deeply analyzed and a medical case is attached to support.[Results]Generations of traditional Chinese medical doctors have held different ideas on the pathogenesis of acne,and the related theories are numerous.Master CHEN believes that Ningbo is located in the south of the Yangtze River,coupled with the influence of bad living habits and emotions of modern people,patients get lung and stomach dampness and heat as the main pathogenesis,so the treatment should clear heat and dampness to promote acne through the skin.Therefore,Master CHEN creates Baihua Sheshecao Decoction which based on many years of clinical experience.In the formula,Herba Hedyotis as king medicine is used to clear the dampness and heat and scatter knot to eliminate sores,Poria cocos as minister medicine is used to help Herba Hedyotis to alleviate water retention,Mustard as assistant medicine is used to reducing phlegm and resolving masses,Gleditsia sinensis as assistant medicine is used to break sores and Rhizoma Zingiberis is to warm the spleen and protect the stomach,Ephedra sinica as guiding medicine is to take all medicine through the skin and dissipate the Yin knot,Radix Glycyrrhizae as guiding medicine is to keep the whole formula in harmony.The case attached reviews Master CHEN's process of thought from prescription to syndrome,which solves the pathological factors such as dampness,heat,phlegm and blood stasis in the process of disease and get remarkable results.[Conclusion]Master CHEN takes lung and stomach dampness and heat as the core of the pathogenesis,self-creating Baihua Sheshecao Decoction which existing both powerful destruction to pathogenic factors and considerate protection to patient's body,coupling hardness with softness.The formula can be flexible modified according to the various changes of the state of illnesses and is effective in treatment.This thinking process of prescription to syndrome analysis is worth further exploration and research for clinical reference.
6.Changes of pulmonary function of pertussis in children aged 5-17 years

Objective To investigate the characteristics of pulmonary function changes in older children with pertussis.Methods Clinical data and pulmonary function date of older children diagnosed with pertussis in outpatient clinics from April 2021 to December 2023 were collected.The clinical data of the case group were collected.A group of healthy older children were included as the control group.Pulmonary function parameters included peak expiratory flow(PEF),forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),FEV1/FVC,expiratory flow at 50%of vital capacity(FEF50),maximum mid-expiratory flow(MMEF)and expiratory flow rate with 75%vigorous exhalation(FEF75).Results Seventy children(36 boys and 34 girls)with pertussis were recruited in the case group,including 54 children with pertussis only and 16 children with pertussis and asthma together.The incidence of paroxysmal cough was 40.0%(28/70)and inspiratory croup 8.5%(6/70)in the case group.Sixty healthy children(28 boys and 32 girls)were included in the control group.There were no significant differences in gender,age,height and body weight between children with pertussis alone group and the control group(P>0.05).The pulmonary function parameters were significantly lower in the children with pertussis alone group than those in the control group,and PEF had the most obvious decline:PEF%pred[80.5(62.6,85.9)vs.109.8(103.2,118.7)].Compared with the pertussis alone group,pulmonary function was not decrease further in the pertussis combined with asthma group.After the improvement of clinical symptoms of children in the pertussis alone group,the level of pulmonary function(PEF and FEF50)increased significantly,but they were still lower than those of the control group.Conclusion The pulmonary function declines slightly in loder children with pertussis.The decreased PEF is most significant.
7.Current status and influencing factors of intrinsic capacity in elderly patients with hospitalization-associated disability

Objective:To explore the current status of intrinsic capacity in elderly patients with hospitalization-associated disability (HAD) and explore its influencing factors.Methods:From November 2023 to January 2024, convenience sampling was used to select 203 elderly patients with HAD at Xuanwu Hospital of Capital Medical University as the study subjects. A survey was conducted on elderly patients using the General Information Questionnaire, Fried Frailty Phenotype, Barthel Index, Social Support Rating Scale, and Intrinsic Capacity Assessment Tool. Binomial Logistic regression was used to analyze the influencing factors of intrinsic capacity in elderly patients with HAD.Results:A total of 203 questionnaires were distributed, and 199 valid questionnaires were collected, with a valid response rate of 98.03% (199/203). The total score of intrinsic capacity in 199 elderly patients with HAD was 5.00 (4.00, 6.00), with scores for cognitive dimension, psychological dimension, motor dimension, vitality dimension, and sensory dimension being 1.00 (1.00, 2.00), 2.00 (1.00, 2.00), 0 (0, 1.00), 1.00 (1.00, 1.00) and 1.00 (1.00, 1.00), respectively. The binomial Logistic regression showed that department of medicine and surgery, self-rating health status, social support, serum albumin, and Barthel Index were the influencing factors of intrinsic capacity in elderly patients with HAD ( P<0.05) . Conclusions:The intrinsic capacity of elderly patients with HAD is at medium to low level, with the most severe impairment in the motor dimension. Medical and nursing staff should develop personalized rehabilitation measures for elderly HAD patients based on the influencing factors of their intrinsic capacity, enhance their intrinsic capacity, and reduce the burden of care on families and society.
8.Current situation and influencing factors of ageism among older adults in megacity communities

Objective:To explore the current situation of ageism among older adults in megacity communities and analyze its influencing factors.Methods:From November to December 2023, convenience sampling was used to select 200 older adults who visited the Niujie Community Health Service Center in Xicheng District, Beijing as the research subject. A survey was conducted on older adults using the General Information Questionnaire, Barthel Index, 15-Item Geriatric Depression Scale, Family Adaptation, Partnership, Growth, Affection and Resolve Scale (Family APGAR Scale), Rosenberg Self-Esteem Scale, Social Support Rating Scale, Lubben Social Network Scale, and Ageism Questionnaire. Multiple linear regression was used to analyze the influencing factors of ageism among older adults in the community.Results:A total of 200 questionnaires were distributed and 200 valid questionnaires were collected, with a valid response rate of 100.00% (200/200). The total score of ageism among 200 older adults in the community was (3.55±0.31), with objective and subjective scores of (3.59±0.28) and (3.50±0.48), respectively. Multiple linear regression showed that occupational status, pre-retirement or current work, family care, self-esteem, and social support were the influencing factors of ageism among older adults in the community ( P<0.05) . Conclusions:Ageism among older adults is influenced by various factors. Medical and nursing staff should focus on older adults who are retired, mainly engaged in physical work, and have poor family and social support when formulating intervention strategies. Community health workers should regularly organize activities to encourage older adults to actively participate, enhance their sense of social participation, reduce ageism, so as to promote healthy aging.
9.Colon-specific controlled release of oral liposomes for enhanced chemo-immunotherapy against colorectal cancer.

A colon-specific drug delivery system has great potential for the oral administration of colorectal cancer. However, the uncontrollable in vivo fate of liposomes makes their effectiveness for colonic location, and intratumoral accumulation remains unsatisfactory. Here, an oral colon-specific drug delivery system (CBS-CS@Lipo/Oxp/MTZ) was constructed by covalently conjugating Clostridium butyricum spores (CBS) with drugs loaded chitosan (CS)-coated liposomes, where the model chemotherapy drug oxaliplatin (Oxp) and anti-anaerobic bacteria agent metronidazole (MTZ) were loaded. Following oral administration, CBS germinated into Clostridium butyricum (CB) and colonized in the colon. Combined with colonic specifically β-glucosidase responsive degrading of CS, dual colon-specific release of liposomes was achieved. And the accumulation of liposomes at the CRC site furtherly increased by 2.68-fold. Simultaneously, the released liposomes penetrated deep tumor tissue via the permeation enhancement effect of CS to kill localized intratumoral bacteria. Collaborating with blocking the translocation of intestinal pathogenic bacteria from lumen to tumor with the gut microbiota modulation of CB, the intratumoral pathogenic bacteria were eliminated fundamentally, blocking their recruitment to immunosuppressive cells. Furtherly, synchronized with lipopolysaccharide (LPS) released from MTZ-induced dead Fusobacterium nucleatum and the tumor-associated antigens produced by Oxp-caused immunogenic dead cells, they jointly enhanced tumor infiltration of CD8+ T cells and reactivated robust antitumor immunity.
10.Mechanism of Xiangsha Liujunzi Tang in Regulation of RhoA/ROCK2/MYPT1 Pathway to Improve Gastric Motility in Rats with Functional Dyspepsia

ObjectiveTo explore the intervention mechanism of Xiangsha Liujunzi Tang in rats with functional dyspepsia (FD) based on the Ras homolog gene family member A (RhoA)/Rho-associated coiled-coil containing protein kinase 2 (ROCK2)/Myosin phosphatase target Subunit 1 (MYPT1) pathway. MethodSixty male SD suckling rats in SPF grades were randomly divided into blank group (n=10) and model group (n=50). The comprehensive modeling method (gavage administration of iodoacetamide+exhaustion of swimming+disturbance of hunger and satiety) was used to replicate the rat model of FD. After successful replication of the model, the rats in the model group were randomly divided into model group, mosapride group, and high, middle, and low-dose Xiangsha Liujunzi Tang groups, with 10 rats in each group. Rats in the blank group and model group were given 10 mL kg-1·d-1 normal saline, those in the mosapride group were given 1.35 mg·kg-1·d-1 mosapride, and those in the high, middle, and low-dose Xiangsha Liujunzi Tang groups were given 12, 6, and 3 g·kg-1·d-1 Xiangsha Liujunzi Tang, respectively. The intervention lasted 14 days. The general living conditions of rats were observed before and after modeling and administration, and the 3-hour food intake and body mass of rats were measured. After intervention, the intestinal propulsion rate of rats was measured, and the pathological changes in the gastric tissue were observed by hematoxylin-eosin (HE) staining. The content of choline acetyl transferase (ChAT) and vasoactive intestinal peptide (VIP) in the medulla oblongata and gastric tissue homogenate was determined by enzyme-linked immunosorbent assay (ELISA), the distribution of adenosine triphosphate (ATP) enzyme in gastric antrum smooth muscle was observed by frozen section staining, and the protein expression levels of RhoA, ROCK2, and phosphorylated-myosin phosphatase target subunit 1 (p-MYPT1) in the gastric tissue were detected by Western blot. ResultCompared with the blank group, the model group had withered hair, lazy movement, slow action, poor general living condition, lower 3-hour food intake, body mass, and lower intestinal propulsion rate (P<0.05), whereas no obvious abnormality in gastric histopathology. In the model group, the content of ChAT in the medulla oblongata and gastric tissue decreased, the content of VIP in gastric tissue increased, the distribution of ATP enzyme in gastric antrum smooth muscle decreased significantly, and the protein expression levels of RhoA, ROCK2, and p-MYPT1 in the gastric tissue decreased significantly (P<0.05). As compared with the model group, the general living condition of rats in each intervention group was significantly improved, and the 3-hour food intake, body mass, and intestinal propulsion rate were significantly increased (P<0.05). There was no significant difference in gastric pathology in the intervention groups. The content of ChAT in the medulla oblongata and gastric tissue increased significantly, the content of VIP in the gastric tissue decreased, the distribution of ATP enzyme in gastric antrum smooth muscle increased significantly, and the protein expression levels of RhoA, ROCK2, and p-MYPT1 in the gastric tissue increased significantly (P<0.05). The intervention effect of Xiangsha Liujunzi Tang group on the above indexes was dose-dependent. ConclusionXiangsha Liujunzi Tang can effectively improve the general living condition and gastric motility of rats with FD, and its specific mechanism may be related to the activation of the RhoA/ROCK2/MYPT1 pathway in the gastric tissue to regulate smooth muscle relaxation and contraction and promote gastric motility.
