Chinese Health Economics 2025;44(10):63-68
Objective:It aims to improve the accessment indexes of medical insurance drug value in China,it introduces a societal value dimension by combining international frontier research with China's medical insurance practices, and explores methods to improve the evaluation of drug accessment for medical insurance negotiations in China.Method:Based on Multi-Criteria Decision Analysis(MCDA),it uses literature review and related international guidelines to establishing a pool of social value evaluation indicators for medicines negotiated under China's medical insurance system.Based on the results of expert interviews(qualitative analysis)and Delphi method questionnaire surveys(quantitative analysis),indicators were selected, and the Analytic Hierarchy Process was used to calculate indicator weights, clarify indicator connotations, and establish scoring criteria.Result:A set of societal value framework are developed, composing 5 core indicators,13 secondary indicators and 4 innovative tertiary indicators. Conclusion:drug value assessment of national insurance is proposed to introduce societal value such as unmet needs, disease severity, and public health impact. It is proposed to calculate the comprehensive societal value score for the drug based on MCDA,adjust the pathway for the threshold of Quality-adjusted Life-year,so as to conduct further discuss.

Objective:To construct service quality evaluation indicators for brain-heart health management outpatient clinics, providing a reference for the standardized, high-quality management and evaluation of brain-heart health management clinics. Methods:Based on Donabedian's "structure-process-outcome" three-dimensional quality evaluation theory, the initial evaluation indicators were drafted after reviewing relevant literature and conducting qualitative interviews. After two rounds of Delphi expert consultation, the final service quality evaluation indicators for brain-heart health management outpatient clinics were established. Results:The effective response rate for the questionnaires from both rounds of expert consultation was 100.00% (15/15). The experts' authority coefficients were 0.925 and 0.930, respectively, the variation coefficients of the indicators were 0 to 0.21 and 0 to 0.19, and the Kendall concordance coefficients were 0.231 and 0.219 ( P<0.01). The final service quality evaluation indicators for brain-heart health management outpatient clinics included three primary indicators, nine secondary indicators, and 23 tertiary indicators, with specific definitions for the tertiary indicators. Conclusions:The service quality evaluation indicators for brain-heart health management outpatient clinics constructed in this study are scientific and reliable. The indicators reflect the specialty of brain-heart health management and highlight the professional value of brain-heart health managers, providing a reference for the high-quality development of brain-heart health management.

ObjectiveTo investigate the effects of Jiaohong pills (JHP) and its prescription, Pericarpium Zanthoxyli (PZ) and Rehmanniae Radix (RR) cognitive dysfunction in scopolamine-induced Alzheimer's disease (AD) mice and its mechanism through pharmacodynamic and metabolomics study. MethodThe animal model of AD induced by scopolamine was established and treated with PZ, RG and JHP, respectively. The effects of JHP and its formulations were investigated by open field test, water maze test, object recognition test, avoidance test, cholinergic system and oxidative stress related biochemical test. Untargeted metabolomics analysis of cerebral cortex was performed by ultra-performance liquid chromatography-Quadrupole/Orbitrap high resolution mass spectrometry (UPLC Q-Exactive Orbitrap MS). ResultThe behavioral data showed that, compared with the model group, the discrimination indexes of the high dose of JHP, PZ and RR groups was significantly increased (P<0.05). The staging rate of Morris water maze test in the PZ, RR, high and low dose groups of JHP was significantly increased (P<0.05, P<0.01), the crossing numbers in the PZ, JHP high and low dose groups were significantly increased (P<0.05, P<0.01); the number of errors in the avoidance test were significantly reduced in the PZ and high-dose JHP groups (P<0.01), and the error latencies were significantly increased in the JHP and its prescription drug groups (P<0.01). Compared with the model group, the activities of acetylcholinesterase in the cerebral cortex of the two doses of JHP group and the PZ group were significantly increased (P<0.05, P<0.01), and the activity of acetylcholinesterase in the high-dose JHP group was significantly decreased (P<0.05), and the level of acetylcholine was significantly increased (P<0.01). At the same time, the contents of malondialdehyde in the serum of the two dose groups of JHP decreased significantly (P<0.05, P<0.01). The results of metabolomics study of cerebral cortex showed that 149 differential metabolites were identified between the JHP group and the model group, which were involved in neurotransmitter metabolism, energy metabolism, oxidative stress and amino acid metabolism. ConclusionJHP and its prescription can antagonize scopolamine-induced cognitive dysfunction, regulate cholinergic system, and reduce oxidative stress damage. The mechanism of its therapeutic effect on AD is related to the regulation of neurotransmitter, energy, amino acid metabolism, and improvement of oxidative stress.

Objective:To explore the health behavior of middle-aged and young stroke patients and analyze its influencing factors. Methods:From April to July 2023, convenience sampling was used to select 172 middle-aged and young stroke patients admitted to the First Affiliated Hospital of Naval Medical University as the research subject. A survey was conducted using the General Information Questionnaire, Health Behavior Scale for Stroke Patients, Health Belief Scale, and Multidimensional Scale of Perceived Social Support. Spearman correlation was used to analyze the correlation between health behavior, social support, and health beliefs among middle-aged and young stroke patients. Multiple linear regression was used to analysis the influencing factors of health behavior among middle-aged and young stroke patients. A total of 172 questionnaires were distributed, and 8 questionnaires with missing items and short response times were excluded, and 164 valid questionnaires were collected, with a valid response rate of 95.34%.Results:Among 164 middle-aged and young stroke patients, the total score of the Health Behavior Scale for Stroke Patients was 64.50 (57.00, 80.75), and the average score of the items was 2.58 (2.28, 3.23). Multiple linear regression analysis showed that factors affecting the health behavior of middle-aged and young stroke patients were whether it was the first onset, the time required to reach nearby medical institution, health belief, and social support ( P<0.05) . Conclusions:The health behavior of middle-aged and young stroke patients is above the medium level. In the process of intervening in the health behavior of middle-aged and young stroke patients, medical and nursing staff should pay attention to patients with recurrent stroke and poor access to medical services, while also improving patients' health belief and social support to promote patients' health behavior and reduce stroke recurrence.
