1.Recent advances in mechanisms, evaluations and treatments of in-stent stenosis following flow diverter implantation from hemodynamics perspective

Flow diverter (FD) devices have gradually become the mainstream approach for interventional treatment of intracranial aneurysms. In-stent stenosis (ISS) is a common complication after FD implantation, which can lead to ischemic events and affect the prognosis of patients. Current studies have shown that ISS occurrence is closely related to hemodynamic changes. From the perspective of hemodynamics, this article reviews the research progress of mechanisms, evaluation methods and treatments of ISS after FD implantation, in order to provide reference for clinical practice.
2.Analysis of relevant factors for rupture of basilar tip aneurysms

Objective:To investigate the relevant factors for rupture of basilar tip aneurysms.Methods:This is a retrospective cohort study. Patients who underwent cerebrovascular digital subtraction angiography at Department of Cerebrovascular Disease Surgery,Zhujiang Hospital of Southern Medical University from July 2008 to February 2023 and were confirmed to have basilar tip aneurysms were retrospectively analyzed. There were 45 males and 56 females, aged (57.5±10.2) years (range: 36 to 76 years); the maximum diameter of the basilar tip aneurysm ( M(IQR)) was 6.85 (5.79) mm (range: 1.28 to 27.48 mm). Finally, 101 basilar tip aneurysms were included, and divided into two groups based on whether the basilar tip aneurysm ruptured or not. There were 37 cases (36.6%) in the ruptured group and 64 cases (63.4%) in the unruptured group. Data were analyzed using independent t-test, Mann-Whitney U test, chi-square test, or Fisher′s exact test, as appropriate, and predictive factors for rupture of basilar tip aneurysms were explored by Logistic regression analysis. Variance inflation factors was used for analysis of collinearity. Results:The univariate results showed statistically significant differences between the ruptured and unruptured groups in terms of aneurysm neck width, basilar artery diameter, mean diameter of the parental artery, the ratio of neck width to parent artery diameter, aneurysm width and maximum diameter of the aneurysm (all P<0.05). In multivariate analysis, basilar artery diameter ( OR=0.347, 95% CI:0.176 to 0.687, P=0.002), and NRP ( OR=0.366, 95% CI:0.192 to 0.696, P=0.002) were predictive factors for rupture of aneurysms occurring at the basilar tip. There was no multiple covariance between NRP and basilar artery diameter in the analysis of covariance (VIF=1). Conclusion:Smaller basilar artery diameter and smaller NRP are relevant factors for rupture of basilar tip aneurysms.

The therapeutic efficacy and possible mechanism of action of superficial needling therapy for knee osteoarthritis(KOA)are reviewed by analyzing relevant medical literature.Superficial needling therapy exerts certain efficacy in treating KOA,and the clinical efficacy could be further improved by combining it with conventional acupuncture,as well as Chinese medicine and other acupuncture methods alone.The therapy of stimulation of meridian qi and invigoration of yang qiare in line with the pathogenesis of KOA.Superficial needling therapy exerts certain effect on KOA by enhancing acupuncture stimulation,enlarging the treatment area,and releasing muscle and fascia.
4.Research progress on unplanned readmissions in patients with left ventricular assist devices

The implantation of left ventricular assist device (LVAD) has significantly improved the quality of life for patients with end-stage heart failure. However, it is associated with the risk of complications, with unplanned readmissions gaining increasing attention. This article reviews the influencing factors, prediction methods and models, and intervention measures for unplanned readmissions in LVAD patients, aiming to provide scientific guidance for clinical practice, assist healthcare professionals in accurately assessing patients' conditions, and develop rational care plans.

7.Intervention effect of nursing program based on CICARE communication model on breast cancer patients undergoing chemotherapy

Objective To explore the intervention effect of nursing program based on contact-introduce-communicate-ask-respond-exit(CICARE)communication model on breast cancer patients undergoing chemotherapy.Methods Sixty patients with breast cancer who received routine intervention in Huai an First People's Hospital from January 2019 to June 2020 were selected as routine intervention group.Sixty patients with breast cancer who underwent chemotherapy nursing program based on CICARE communication model on the basis of routine intervention in Huai'an First People's Hospital from July 2020 to December 2021 were selected as CICARE intervention group.The intervention was carried out until discharge in both groups.The psychological state,self-management ability,level of hope and quality of life were compared between the two groups at admission and discharge.Results Compared with those at admission,the scores of Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS)at discharge decreased in both groups,and the scores in the CICARE intervention group were lower than those in the routine intervention group at discharge(P<0.05).The scores of resilience rating,exercise,and cognitive symptom management implementation,communication with the physician ratings,reality and future of attitude,positive action of attitude,keeping intimate relationships with others and quality of life at discharge increased in both groups,and these scores in the CICARE intervention group were significantly higher than those in the routine intervention group(P<0.05).Conclusion The nursing program based on CICARE communication model can improve psychological state,self-management ability,hope level and quality of life in breast cancer patients undergoing chemotherapy by enhancing the communication between nurses and patients with good intervention effect.
8.Qualitative study on military nurses' coping with death anxiety during emergency public health incident response

Objective:To understand the status and coping strategies of death anxiety among military nurses participating in emergency public health incident response, and to provide references for improving their coping abilities.Methods:A purposive sampling and snowball sampling method were used to select 15 nurses from multiple military hospitals who supported the emergency public health incident. One-on-one semi-structured interviews were conducted, and the Colaizzi's seven-step analysis method was used to analyze the interview data.Results:Four themes were identified: the occurrence of death anxiety at different time points; the presence of significant outbreaks of death anxiety; multiple factors that help military nurses alleviate death anxiety; and the intrinsic motivation and gains of military nurses participating in the response.Conclusions:It is crucial to monitor the occurrence and changes in death anxiety among nurses throughout the entire cycle of emergency public health incidents. Managers should focus on value guidance, leverage individual positive strengths to cope with death anxiety, enhance information and social support, reinforce training on infectious disease prevention and death education, and reduce the level of death anxiety among military nurses and improve their coping abilities.
9.MRI texture features combined with apparent diffusion coefficient for differentiating uterine sarcoma and cellular uterine leiomyoma

Objective To observe the value of MRI texture features combined with apparent diffusion coefficient(ADC)for differentiating uterine sarcoma(US)and cellular uterine leiomyoma(CUL).Methods Pelvic MRI data of 27 US patients(US group)and 34 CUL patients(CUL group)were retrospectively analyzed.The texture features of lesions were extracted from T2WI and diffusion weighted imaging(DWI),the ADC value were measured,and the average ADC value(ADCmean),the minimum ADC value(ADCmin)and standard ADC value(ADCst)were recorded.Then logistic regression(LR)models were constructed based on ADC value,optimal texture features alone and their combination,respectively,including LRADC,LRtexture and LRADC+texture models.Receiver operating characteristic curves were drawn,and the area under the curves(AUC)were calculated to evaluate the efficacy of each model for differentiating US and CUL.Results The ADCmean,ADCmin and ADCst in US group were all lower than those in CUL group(all P<0.05).A total of 3 750 texture features were extracted from pelvic T2WI and DWI,5 optimal features were finally obtained,and the constructed LRADC+texture model and LRtexture model had similar efficacy of differentiating US and CUL(AUC=0.921,0.887;P>0.05),which were both higher than that of LRADC model(AUC=0.696;both P<0.05).The calibration curve of LRADC+texture model was basically consistent with the ideal curve,which had better clinical benefits than LRADC and LRtexture models.Conclusion MRI texture features combined with ADC value could improve efficacy for differentiating US and CUL.
10.Research progress on the effect of advanced maternal age on maternal psychology and neurobehavioral development of their offspring and related mechanisms

With the changes in lifestyle, marriages and childbearing concepts, and population policies in recent years, the proportion of women with advanced maternal age (AMA) in China is rising rapidly. AMA and maternal stress due to advanced age are considered as independent risk factors for maternal mental health and the neurobehavioral development of their offspring. In this paper, we summarized how the glial cell line-derived neurotrophic factor, the hypothalamic-pituitary-adrenal axis, the 5-HT system and the oxytocin affect the maternal mental health and the neurobehavioral development of their offspring, as well as some potential targeted interventions. Interventions currently being developed include artificial supplements, antidepressants, physical exercise, nutrient supplementation, etc., but they have a limited efficacy and a high risk of side effects, yet there is still a lack of specific and systematic interventions. The relevant pathogenesis should continue to be explored in the future research, and hopefully more effective and safe interventions will be given in the near future to improve the mental health of mothers with AMA and the long-term healthy development of their offspring.
