1.Analysis of components absorbed into blood and brain of Lithocarpus litseifolius leaves

OBJECTIVE To analyze the prototype components absorbed into blood and brain of Lithocarpus litseifolius leaves, so as to provide a reference for clarifying the pharmacological material basis of its prevention and treatment of central nervous system dis eases. METHODS The ethanol extract of L. litseifolius leaves, as well as the gastric lavage fluid and perfusion solution were prepared. Using rats as subjects, plasma samples of intestinal wall metabolism, intestinal flora metabolism and hepatic metabolism were prepared via in situ intestinal perfusion and closed intestinal loop method; while comprehensive metabolic plasma samples, brain tissue samples, and cerebrospinal fluid samples were collected after intragastric administration. UPLC-HRMS technology was utilized to analyze and identify chemical components and prototype components absorbed into blood and brain of L. litseifolius leaves. RESULTS A total of 66 chemical constituents were identified in L. litseifolius leaves, primarily consisting of flavonoids, organic acids, and others. A total of 16, 13, 11, and 5 prototype components were identified in intestinal wall metabolism, intestinal flora metabolism, hepatic metabolism, and comprehensive metabolic plasma samples, respectively. Additionally, 4 prototype components were detected in brain tissue and 9 in cerebrospinal fluid. Phloridzin, trilobatin, phloretin-2- O -malonyl hexoside, and phloretin were identified as common components across all sample types. CONCLUSIONS Prototype components absorbed into blood and brain of L. litseifolius leaves, such as phloridzin, trilobatin, phloretin, and other components may serve as the pharmacological material basis for their therapeutic effects on central nervous system diseases.

This paper introduces a real-world cohort research model for community-acquired pneumonia (CAP) based on the Jiangsu Traditional Chinese Medicine (TCM) Dominant Diseases Diagnosis and Treatment Data Platform. Firstly, data cleaning is performed by standardizing diagnosis, symptoms, treatment and imaging, intelligently extracting unstructured information, and cleaning and constructing a standardized database. Secondly, for cohort establishment, CAP patients across the province are screened in accordance with CAP diagnostic criteria to build a high-quality disease-specific cohort. Lastly, in terms of protocol design, the characteristics of TCM research and the CAP disease profile are considered to determine appropriate inclusion and exclusion criteria, estimate sample size, define interventions, outcomes and economic evaluations, providing a reference for real-world TCM research on CAP.

4.Differentiation and Treatment of Hypertension Comorbidities from the Perspective of "Constitution-Syndrome-Disease Location Correlation"

Mainly based on the "constitution-syndrome-disease location correlation" approach, this study constructs a traditional Chinese medicine (TCM) framework for understanding hypertension comorbidities. From the perspective of shared constitutional predispositions and similar pathomechanistic characteristics, hypertension comorbidity should not be regarded as a simple aggregation of multiple diseases. Rather, it is generally rooted in biased constitutions such as yin-deficiency, qi-deficiency, yang-deficiency, phlegm-dampness, dampness-heat, and blood-stasis constitutions, accompanied by the long-term interplay of pathogenic mechanisms involving wind, fire, phlegm, stasis, and deficiency, and gradually manifests as comorbidities affecting different systems through the evolution of disease locations related to the liver, heart, spleen, and kidney. On this basis, cardiac-, cerebral-, renal-, and metabolic-related comorbidities are taken as the major categories, and their characteristics related to constitution, syndrome, and disease location are systematically summarized to clarify the correspondence between shared pathomechanisms and predominant disease locations, and to propose corresponding prevention and treatment strategies. This framework further incorporates constitution-based stratification into risk stratification for prospective intervention, and takes syndrome as the central link in developing multi-disease treatment strategies such as boosting qi and activating blood, as well as resolving phlegm and unblocking collaterals. A clinical pathway is thereby proposed, encompassing constitution assessment, syndrome differentiation, comorbidity risk stratification, integrated traditional Chinese and western medicine intervention, and follow-up feedback. This study aims to provide a reference for the theoretical development and collaborative management of hypertension comorbidities in TCM.
5.Staged Prevention and Treatment for Low Anterior Resection Syndrome Using Acupuncture

Low anterior resection syndrome (LARS) is a common complication of sphincter-preserving rectal cancer surgery, and its manifestations change dynamically at different stages. By analyzing and summarizing the clinical symptoms and pathomechanism evolution of LARS across different stages, this paper proposes a staged prevention and treatment strategy using acupuncture. During perioperative stage, the main principle is activating the transport function of the body, supplemented by regulating the mind, with the use of dredging the bowels as needed. During the stoma reversal stage, treatment focuses primarily on fortifying the spleen, with draining dampness as a supplementary method, to help consolidate the intestines. During the radiotherapy or chemotherapy stage, the main focus is reinfor-cing healthy qi, with reducing toxin as an adjunct, to achieve the effect of activating the transport of the pivot. During the survival management stage, treatment primarily focuses on tonifying the kidneys and secondarily on fortifying the spleen, with regulating the corporeal soul as the therapeutic emphasis. Acupoints are selected and combined in accordance with the treatment principles at each stage, and different stimulation methods such as electroacupuncture and moxibustion are applied. An analysis of the mechanisms underlying acupuncture for LARS is also provided, offering a theoretical basis and practical approach for the prevention and treatment of LARS with acupuncture.
6.Analysis of the current situation and influencing factors of osteoporosis knowledge among orthopedic nurses in 621 hospitals

Objective To investigate the current status and analyze the factors affecting the osteoporosis knowledge among orthopedic nurses,and to provide references for improving osteoporosis knowledge among orthopedic nurses.Methods We investigated 5 681 orthopedic nurses from 621 hospitals in 31 provinces(autonomous regions/municipalities)using a convenient sampling method from September 2023 to November 2023.We collected information using a general information questionnaire and osteoporosis knowledge assessment tool.We used univariate and multivariate logistic regression to identify risk factors.Results A total of 5 204 orthopedic nurses were included.The total mean score of osteoporosis knowledge assessment tool of nurses was[13.00(12.00,14.00)],and 23.48%nurses had a low level of osteoporosis awareness.Logistic regression analysis showed that the age,title and position of the nurses,location of the hospital,the level of the hospital,whether to conduct the fracture liaison service/to set up the refracture prevention team,whether to use calcium tablets/vitamin D,whether to carry out anti-osteoporosis drug therapy,and whether to conduct topical lectures on osteoporosis or secondary fractures,were the factors that affected orthopedic nurses'knowledge of osteoporosis(P<0.05).Conclusion The osteoporosis knowledge of orthopedic nurses needs to be improved.Nursing managers should carry out personalized interventions from both hospital and nurse levels to improve the osteoporosis knowledge of orthopedic nurses in our country.
7.Emerging role of lysosomal regulation in Alzheimer's disease

Alzheimer's disease(AD)is the most common neurodegenerative disease in the elderly,and its main pathological manifestations include senile plaques formed by β-amyloid deposition and neuronal fibrillar nodules formed by hyperphosphorylation of tau proteins.Lysosome is an important organelle in eukaryotic cells,containing a variety of hydrolytic enzymes that can break down proteins and other biomolecules.It is closely related to intracellular transport and autophagy,and is important for maintaining cellular homeostasis.This review summarizes the interaction between lysosomal dysfunction and the development and progression of AD and the potential therapeutic mechanisms in treating AD by regulating and restoring the functions of lysosomes.Lysosomal dysfunction can lead to neurodegenerative diseases such as AD.Modulation of lysosomal function is a promising treatment strategy for AD.It is expected that more drugs and therapeutic regimens based on this mechanism can be used in the clinical treatment for AD patients in the future.
8.Textual Research of Key Information of Classic Formula Xieqingwan Based on Ancient and Modern Literature

ObjectiveThis paper aims to systematically collect and organize ancient and modern clauses and studies containing Xieqingwan, excavate and analyze the key information of Xieqingwan, and provide a reference for facilitating the development of the classic formula Xieqingwan. MethodsThe composition, dosage, decocting methods, usage, and other key information of Xieqingwan in ancient traditional Chinese medicine books were collected and analyzed by means of literature research and metrological methods. The modern clinical application of Xieqingwan was summarized. ResultsA total of 42 pieces of effective data involving 32 ancient traditional Chinese medicine books were collected. Xieqingwan was first recorded in Xiaoer Yaozheng Zhijue. The drug origin of this formula is basically clear in the ancient traditional Chinese medicine books. The modern drug usage and decocting method were as follows: Angelicae Sinensis Radix, Gentianae Radix et Rhizoma, Chuanxiong Rhizoma, Gardenia seeds, Radix et Rhizoma Rhei, Notopterygii Rhizoma et Radix, and Saposhnikoviae Radix were grounded to fine powder, decocted with honey, and finally formed into pills with the size of a chicken head (1.5 g). It was suggested that half a pill or one pill were taken for one dose with warm Lophatheri decoction and sugar. The indications and clinical application had developed from the recordings in Xiaoer Yaozheng Zhijue and evolved from pediatrics to ophthalmic otolaryngology, neurology, dermatology, digestion, and respiratory diseases. The main pathogenesis of these diseases is heat in the liver meridian and is treated. The effect of Xieqingwan is "clearing away heat and toxicity, removing fire and relaxing the bowels, and dispersing swelling and relieving pain". It is recommended to use the corresponding preparation methods in the 2020 Edition of Pharmacopoeia of the People's Republic of China. Modern clinical studies are centered around the clinical application of Xieqingwan, which is often modified and used in treating Tourette syndrome, herpes, febrile convulsion, sleepwalking, and insomnia. ConclusionThis paper conducts a thorough textual research of the key information of Xieqingwan, induces its historic evolution, and confirms its key information, so as to provide a reference for the future development of Xieqingwan.

10.Recurrent pyoderma gangrenosum treated with adalimumab combined with tofacitinib:A case report and literature review

Pyoderma gangrenosum(PG)is a rare autoinflammatory disease,characterized mainly by painful and necrotic skin ulcers.The etiology of PG is unknown,and its treatment is quite challenging.This article reports a case of recurrent pyoderma gangrenosum successfully treated with adalimumab combined with tofacitinib,along with a review of relevant literature.The patient,a 54-year-old male,presented with scrotal ulcers accompanied by pain for 20 days,worsening and involving the groin area for 5 days,and fever for 1 day.Dermatological examination revealed two painful ulcers on the scrotum with raised edges,clear boundaries,and a small amount of purulent discharge on the surface;the right groin area and the medial left thigh showed irregular infiltrative erythematous plaques,with scattered clustered pustules on them,central necrosis appearing purplish-brown,surrounded by a red halo with clear boundaries.Based on the patient's past medical history,clinical manifestations,and auxiliary examinations,the diagnosis was consistent with pyoderma gangrenosum.After treatment with adalimumab combined with tofacitinib,the patient's symptoms were significantly improved,and the ulcers healed.At the 6-month follow-up,the skin lesions had not recurred.For refractory PG that is ineffective to conventional treatments,adalimumab combined with tofacitinib is an effective and safe treatment option,providing a new combination therapy regimen for PG.
