LL-37 is a widely-distributed human antimicrobial peptide with antimicrobial activity.In recent years,it has been found that LL-37 not only has antibacterial effect,but also can exert comprehensive immune regulatory function on different immune cells in different microenvironments,and then play a pro-inflammatory or anti-inflammatory role in a series of autoimmune diseases.This article focuses on immunomodulatory effect of LL-37 and its possible role in autoimmune diseases.
2.The effects of exercise based on the sports and medical care integration model on the community-dwelling persons with type 2 diabetes mellitus

Objective:To explore the effect of exercise on glycemic control and physical indicators among community-dwelling persons with type 2 diabetes mellitus (T2DM).Methods:A total of 42 T2DM patients undergoing regular follow-up at a community health center were divided into a control group and an exercise group. To minimize confounding factors, 1∶1 propensity score matching was performed, resulting in 21 patients in each group. Both groups received standardized diabetes management, including medication, dietary guidance and diabetes health education. The exercise group additionally performed exercises which integrated sports and medical care models. The experiment lasted 8 weeks. Before and after the intervention, glycosylated hemoglobin A1c (HbA1c) levels, body weight, body mass index (BMI), fat mass, visceral fat area (VFA), fat mass index (FMI) and appendicular skeletal muscle mass index (ASMI) were recorded and compared between the two groups. Moreover, the actual exercise compliance of the exercise group was observed, along with any adverse events such as hypoglycemia or exercise-related injuries, as well as changes in antidiabetic medication dosage.Results:After the intervention, significant improvement was observed in both groups in all of the measurements, but with the average HbA1c, BMI, fat mass, VFA and FMI values of the exercise group then significantly better than the control group′s averages. The average compliance rate in the exercise group was 60%. No cases of hypoglycemia or exercise-related injuries were reported during the intervention, and there were no changes in the use of anti-diabetes medications.Conclusions:Exercise effectively improves glycemic control and physical indicators among community-dwelling T2DM patients. However, exercise compliance needs improving.
3.Uterine-sparing U-shaped Reconstruction Surgery in the Treatment of Diffuse Uterine Adenomyosis

Objective To observe the clinical efficacy of uterine-sparing U-shaped reconstruction surgery in the treatment of diffuse uterine adenomyosis.Methods A total of 48 patients with diffuse uterine adenomyosis underwent surgical treatment from October 2019 to September 2022.All the patients underwent a modified lesion resection procedure via sagittal plane incision of the uterine corpus,enabling direct visualization for U-shaped excision of adenomyotic foci within the myometrium,followed by endometrial cavity reconstruction and uterine remodeling.The dysmenorrhea score,menstrual volume score,serum CA125,and uterine volume were assessed at 1,6,and 12 months postoperatively.Results All the 48 patients underwent surgery successfully.The average surgery time was(115.7±13.6)min,the average intraoperative blood loss was(109.4±30.9)ml,and the total length of hospital stay was(8.6±1.7)d.At 1,6,and 12 months after surgery,the dysmenorrhea score,menstrual volume score,serum CA125 levels,and uterine volume were significantly lower as compared to the pre-operative values(all P=0.000).The hemoglobin level at 1 month after surgery was significantly higher than that before surgery(P=0.000).No symptomatic or ultrasonic recurrences were observed in all the 48 cases.Two cases got unexpected pregnancy after surgery.Conclusions Uterine-sparing U-shaped reconstruction surgery is a safe and effective treatment for diffuse uterine adenomyosis.It does not require special instruments,and is simple and easy to perform.

7.Predicting the risk of spontaneous hemorrhage conversion after acute ischemic stroke based on a columnar graph model

Objective To establish a quantitative and visual prediction model for spontaneous hemorrhagic transformation(sHT)after acute ischemic stroke(AIS)and validate the efficacy by nomogram.Methods A total of 240 patients with AIS were selected,and the general data,serological tests and imaging findings were collected.The patients were randomly divided into modeling group(175 cases)and validation group(65 cases).The patients were also divided into non-HT group and HT group according to the imaging results.The R 4.1.1 software and the rms package were used to build the column line graph model,while Bootstrap method was applied to repeat sampling 1000 times for internal and external validation,and the H-L goodness-of-fit test,clinical decision curve and ROC curve were used to assess the calibration and discrimination of the column line graph model,respectively.Results Among 240 patients with AIS,bleeding conversion occurred in 60 cases(25.0%).In the modeling group,the results of multifactorial Logistic regression showed that the presence or absence of previous history of atrial fibrillation,NIHSS score at the onset,Hb,high-density lipoprotein(HDL)and infarct area were significant influencing factors for sHT after AIS.The x2 values of the H-L goodness-of-fit test for the modeling and validation groups were 5.61 and 0.74,respectively,corresponding to P values of 0.13 and 0.69,indicating that the established column line graph model had good prediction accuracy;the area under the ROC curve for the column line graph prediction modeling group and validation group were 0.963(95%CI:0.926-1.000)and 0.977(95%CI:0.950-1.000),and the results suggested that the model had good discrimination.Conclusions Previous history of atrial fibrillation,NIHSS score size at onset,Hb,HDL and the size of infarct area are independent influencing factors of sHT after AIS.Establishing the visual nomogram model based on the above factors can effectively predict the risk of sHT after AIS.

Closed-loop hospital management can effectivly cope with the COVID-19 pandemic. In order to ensure the continuity of treatments for hemodialysis patients under closed-loop management and minimize possible medical and infection risks, Huashan Hospital affiliated to Fudan University and 9 hospitals in Shanghai established a hemodialysis alliance in January 2021.The alliance optimized hemodialysis resources within the region through overall planning by preparing sites, materials and personnel shifts in advance, and establishing management systems and work processes to ensure that patients could be quickly and orderly diverted to other blood dialysis centers for uninterrupted high-quality hemodialysis services, in case that some hemodialysis centers in the alliance under closed-loop management.From November 2021 to April 2022, 317 of 1 459 hemodialysis patients in the alliance were diverted to other centers for treatment, accumulating 1 215 times/cases of treatments without obvious adverse reactions. The practice could provide a reference for medical institutions to quickly establish mutual support mode under major public health events.
9.Acute laryngeal edema induced by doxorubicin liposome

A 69-year-old male patient with peripheral T-cell lymphoma received chemotherapy with intravenous doxorubicin liposome. In the first chemotherapy cycle, no obvious adverse reactions appeared. In the second chemotherapy cycle, the patient developed transient muscle soreness during the IV infusion of doxorubicin liposome. In the third chemotherapy cycle, dexamethasone and chlorphenamine were given to prevent anaphylaxis before doxorubicin liposome treatment and the infusion rate was controlled in a standardized way. However, at about 20 minutes of infusion, the patient developed nausea and vomiting. The infusion of doxorubicin was stopped immediately and replaced by IV infusion of 0.9% sodium chloride injection 250 ml. Then the patient developed facial numbness, laryngeal pain, neck discomfort, and multiple parts of skin rash with pruritus. The electrocardiogram monitoring showed heart rate 130 times/min, blood pressure 80/50 mmHg, and oxygen saturation 0.98. The patient was given oxygen inhalation and in half-lying position following the doctor′s advice, but the patient developed dyspnea, hoarseness, and slurred speech 20 minutes later. Physical examination showed the patient′s tongue was hypertrophic, his neck was swollen and thickened. Acute laryngeal edema induced by doxorubicin liposome was considered. Intravenous injection of dexamethasone 10 mg, IV infusion of 10% calcium gluconate, and aerosol inhalation of budesonide inhalation aerosol were given immediately and about 3 hours later, the symptoms gradually improved. Two days later, the allergic symptoms disappeared.
