Postoperative infection of internal fixation of closed fractures the lower limbs in adults represents a devastating complication, characterized by diagnostic challenges, prolonged treatment duration and high disability rates. Current management of these infections faces multiple challenges, such as difficulties in early accurate diagnosis, and various controversies about the treatment plan, leading to poor overall diagnosis and treatment results. To address these issues, based on evidence-based medicine and principles with emphasis on scientific rigor, clinical applicability and innovation, the Trauma Branch of the Chinese Medical Association, Orthopedic Branch of the Chinese Medical Doctor Association, Orthopedics Branch of the Chinese Medical Association, and Trauma Orthopedics and Polytrauma Group of the Resuscitation and Emergency Committee of the Chinese Medical Doctor Association have collaboratively organized a panel of relevant experts to develop the Guideline for diagnosis and treatment of infection after internal fixation of closed lower limb fractures in adults ( version 2025). The guideline proposed 10 recommendations, aiming to provide a foundation for standardized diagnosis and treatment of postoperative infection in adults with closed lower limb fractures.
2.Ganoderma Lucidum Extract Nano-emulsion:Preparation,Characterization and Pharmacodynamics Study in Anti-skin Aging

Objective To screen the prescription,preparation,quality evaluation of Ganoderma lucidum extract nano-emulsion(GLEN)and study its anti-skin aging effect.Methods The emulsifier and oil phase of the nano-emulsion were preliminarily screened by solubility method.The pseudo-ternary phase diagram method was used to screen the nano-emulsions,and the prepared GLEN were evaluated for quality in terms of the appearance,particle size,structure,drug loading,stability and skin safety.A mouse skin aging model was constructed by injecting D-galactose combined with ultraviolet radiation.GLEN was applied to the skin at the modeling site and investigated for their anti-skin aging efficacy.Results The optimal GLEN prescription was 30%polyoxymethylene castor oil,14.4%glycerol,11.1%medium chain triglycerides,and 44.5%water.GLEN was a dark brown and transparent liquid at room temperature,O/W type nano-emulsion with an average particle size(32.20±2.89)nm,polydispersity coefficient 0.38±0.03,zeta potential value(-4.05±0.27)mV.The content of Ganoderma lucidum polysaccharides was(36.35±0.55)mg·mL-1,and the content of Ganoderma lucidum triterpenes was(5.99±0.46)mg·mL-1,which possessed good stability.Pharmacodynamic experiments showed compared with the model group,the skin scores of mice in the GLEN group significantly improved(P﹤0.05),the water content increased(P﹤0.01),and the contents of SOD,GSH-PX and Hyp increased(P﹤0.05),while the content of MDA was decreased significantly(P﹤0.01).HE,Masson and Sirius crimson staining further indicated that the pathological changes in mice were alleviated to varying degrees in GLEN group.And GLEN had high safety for skin,could be applied to human body.Conclusion In this study,the prepared GLENs were homogeneous in appearance,stable in nature,and exhibited significant anti-aging effects on mouse skin.

Chronic refractory wound (CRW) is one of the most challengeable issues in clinic due to complex pathogenesis, long course of disease and poor prognosis. Experts need to conduct systematic summary for the diagnosis and treatment of CRW due to complex pathogenesis and poor prognosis, and standard guidelines for the diagnosis and treatment of CRW should be created. The Guideline forthe diagnosis and treatment of chronic refractory wounds in orthopedic trauma patients ( version 2023) was created by the expert group organized by the Chinese Association of Orthopedic Surgeons, Chinese Orthopedic Association, Chinese Society of Traumatology, and Trauma Orthopedics and Multiple Traumatology Group of Emergency Resuscitation Committee of Chinese Medical Doctor Association after the clinical problems were chosen based on demand-driven principles and principles of evidence-based medicine. The guideline systematically elaborated CRW from aspects of the epidemiology, diagnosis, treatment, postoperative management, complication prevention and comorbidity management, and rehabilitation and health education, and 9 recommendations were finally proposed to provide a reliable clinical reference for the diagnosis and treatment of CRW.
6.BLOC1S1 promotes proliferation of goat spermatogonial stem cells.

With the rapid development of gene editing technology, the study of spermatogonial stem cells (SSCs) holds great significance in understanding spermatogenesis and its regulatory mechanism, developing transgenic animals, gene therapy, infertility treatment and protecting rare species. Biogenesis of lysosome-related organelles complex 1 subunit 1 (BLOC1S1) is believed to have anti-brucella potential. Exploring the impack of BLOC1S1 on goat SSCs not only helps investigate the ability of BLOC1S1 to promote SSCs proliferation, but also provides a cytological basis for disease-resistant breeding research. In this study, a BLOC1S1 overexpression vector was constructed by homologous recombination. The BLOC1S1 overexpression cell line of goat spermatogonial stem cells was successfully constructed by lentivirus packaging, transfection and puromycin screening. The overexpression efficiency of BLOC1S1 was found to be 18 times higher using real time quantitative PCR (RT-qPCR). Furthermore, the results from cell growth curve analysis, flow cytometry for cell cycle detection, and 5-ethynyl-2'-deoxyuridine (EdU) staining showed that BLOC1S1 significantly increased the proliferation activity of goat SSCs. The results of RT-qPCR, immunofluorescence staining and Western blotting analyses revealed up-regulation of proliferation-related genes (PCNA, CDK2, CCND1), and EIF2S3Y, a key gene regulating the proliferation of spermatogonial stem cells. These findings strongly suggest that the proliferative ability of goat SSCs can be enhanced through the EIF2S3Y/ERK pathway. In summary, this study successfully created a goat spermatogonial stem cell BLOC1S1 overexpression cell line, which exhibited improved proliferation ability. This research laid the groundwork for exploring the regulatory role of BLOC1S1 in goat spermatogonia and provided a cell platform for further study into the biological function of BLOC1S1. These findings also establish a foundation for breeding BLOC1S1 overexpressing goats.

7.Mediating effects of social participation quality on the relationship between perceived social constraints and mental health status in first stroke patients

Objective:To explore the relationships among perceived social constraints, social participation, as well as anxiety and depression in first stroke patients, and to further analyze the mediating effect of social participation on these variables.Methods:A total of 216 first stroke patients in the Department of Neurology of Affiliated Hospital of Qingdao University from April to December 2020 were recruited by convenience sampling and investigated by general information questionnaire, Social Constraints Scale (SCS), Chinese version of Impact on Participation and Autonomy Questionaire (IPA-C) and Hospital Anxiety and Depression Scale (HADS).Results:The total score of SCS, IPA-C, depression and anxiety were 33.49 ± 6.81, 43.42 ± 9.62, 8.05 ± 4.15 and 8.61 ± 2.59. Social constraints were positively correlated with social participation as well as anxiety ( r=0.644, 0.383, both P<0.05). Social constraints were positively correlated with social participation as well as depression ( r=0.482, 0.371, both P<0.05). The quality of social participation partially mediated the relationship between social constraints and anxiety (intermediary effect was 0.119), and also partially mediated the relationship between social constraints and depression (intermediary effect was 0.270). Conclusions:First stroke patients experience high level of social constraints, low quality of social participation and severe anxiety and depression. Social constraints can affect anxiety and depression through social participation. Medical staff should build a good environment to meet the needs of stroke patients of social participation, help patients to establish a correct psychological coping style, reduce patients' avoidance of social participation due to perceived constraints and exclusion, and thus promote the mental health of patients.
8.Development and reliability and validity test of Health Promotion Behavior Intention Questionnaire in enterostomy patients

Objective:To develop the Health Promotion Behavior Intention Questionnaire for enterostomy patients and test its reliability and validity.Method:Based on the theory of planned behavior, the first draft of the questionnaire was formed through literature review, group discussion, correspondence with Delphi experts and pre-test. The convenient sampling method was used to select 419 patients with enterostomy who visited Stoma Outpatient Department of the Affiliated Hospital of Qingdao University from December 2020 to June 2021 to conduct a questionnaire survey. Item analysis and reliability and validity tests were carried out on the questionnaires.Results:The Health Promotion Behavior Intention Questionnaire for enterostomy patients included 15 items. Exploratory factor analysis extracted four common factors, namely, attitude, subjective norm, perceived behavioral control and behavioral intention. The cumulative variance contribution rate was 83.166%. The content validity index of item level was 0.875-1.000 and the content validity index of questionnaire level was 0.983. The Cronbach's α coefficient of the total questionnaire was 0.921 and the retest reliability coefficient was 0.848.Conclusions:The Health Promotion Behavior Intention Questionnaire for enterostomy patients has good reliability and validity and it can be used as a tool to evaluate the health promotion behavior intention of patients with enterostomy.
9.A hierarchical vascularized engineered bone inspired by intramembranous ossification for mandibular regeneration.

Mandibular defects caused by injuries, tumors, and infections are common and can severely affect mandibular function and the patient's appearance. However, mandible reconstruction with a mandibular bionic structure remains challenging. Inspired by the process of intramembranous ossification in mandibular development, a hierarchical vascularized engineered bone consisting of angiogenesis and osteogenesis modules has been produced. Moreover, the hierarchical vascular network and bone structure generated by these hierarchical vascularized engineered bone modules match the particular anatomical structure of the mandible. The ultra-tough polyion complex has been used as the basic scaffold for hierarchical vascularized engineered bone for ensuring better reconstruction of mandible function. According to the results of in vivo experiments, the bone regenerated using hierarchical vascularized engineered bone is similar to the natural mandibular bone in terms of morphology and genomics. The sonic hedgehog signaling pathway is specifically activated in hierarchical vascularized engineered bone, indicating that the new bone in hierarchical vascularized engineered bone underwent a process of intramembranous ossification identical to that of mandible development. Thus, hierarchical vascularized engineered bone has a high potential for clinical application in mandibular defect reconstruction. Moreover, the concept based on developmental processes and bionic structures provides an effective strategy for tissue regeneration.

10. Herbert screwing directly via the anterior approach for femoral head fractures of Pipkin type Ⅱ in the youth

Objective:
To evaluate Herbert screwing directly via the anterior approach for femoral head fractures of Pipkin type Ⅱ in the youth.
Methods:
A retrospective analysis was conducted of the 16 patients who had been treated at Department of Orthopaedics, Union Hospital, Tongji Medical College for femoral head fractures of Pipkin type Ⅱ from August 2016 to June 2018. They were 9 men and 7 women, aged from 18 to 45 years (mean, 25.6 years). Of them, 10 cases who had been complicated with posterior dis-location of the hip received emergency hip reduction(<6 h) before surgical fixation. All the patients underwent Herbert screwing directly via the anterior approach. Their incision length, operation time, intraoperative blood loss, hospitalization time, Harris hip scores, therapeutic efficacy and complications were recorded.
Results:
In this series, the incision length averaged 10.4 cm, operation time 45.6 min, intraoperative blood loss 46.5 mL, and hospitalization time 4.0 d. All the 16 patients were followed up for 11 to 15 months (average, 12.7 months). Their Harris hip scores at preoperation, 3, 6 and 9 months postoperation and at the last follow-up were, respectively, 14.3±2.2, 64.8±2.4, 81.1±4.9, 88.1±4.6 and 91.9±3.4 points, showing a significant difference between any 2 time points (
