1.Early screening strategies for metabolic associated fatty liver disease

Metabolic associated fatty liver disease (MAFLD) is a common chronic liver disease worldwide, and timely and precise intervention can delay disease progression and significantly reduce the risk of serious complications such as liver fibrosis, liver cirrhosis, and liver cancer. Although traditional liver biopsy combined with metabolic markers is the gold standard, it may cause complications such as pain and bleeding as an invasive examination, which has promoted scientific research to shift its focus to the construction of noninvasive assessment systems. In recent years, noninvasive diagnostic technologies based on multi-dimensional detection strategies have been continuously updated, including serological models, imaging techniques, and clinical algorithms. This article systematically reviews the screening methods for MAFLD during the fibrotic stages F1—F3, especially deep learning models based on artificial intelligence, in order to provide ideas for the early screening of MAFLD, as well as a scientific reference for optimizing disease management strategies.
2.Epidemiological characteristics of human brucellosis in Baoshan City, Yunnan Province, 2020‒2023

ObjectiveTo investigate the epidemiological characteristics of human brucellosis (hereinafter referred to as brucellosis) in Baoshan City, Yunnan Province, and to provide scientific evidence for adjusting prevention and control strategies. MethodsBased on the surveillance data of reported brucellosis cases in Baoshan City from 2020 to 2023 and the information collected through individual epidemiological questionnaire surveys, the epidemic status and clinical characteristics of brucellosis in Baoshan City were analyzed using descriptive epidemiological methods. ResultsA total of 85 brucellosis cases were reported in Baoshan City from 2020 to 2023, and detailed individual information was obtained for 83 of them. Brucellosis in Baoshan City showed a clear seasonal pattern, with peak incidence from May to September. The average annual incidence rate was 0.80/100 000, with a male-to-female ratio of 7.5∶1. And 82.35% of the cases aged 30 to 60 years, with farmers being the predominant affected group. The main clinical manifestations of the cases were myalgia and arthralgia. Regarding transmission routes, 87.95% of the cases had a contact history with cattle, with livestock rearing and grazing being the main exposure modes. Most infections occurred at home. ConclusionFrom 2020 to 2023, the incidence of brucellosis in Baoshan City exhibited a fluctuating upward trend, with a peak period from May to September. Males and farmers were identified as the primary affected populations. It is recommended to strengthen livestock surveillance and control, and to enhance both awareness and self-protection capacity among high-risk groups.
3.Mechanism of Astragaloside Ⅳ in Regulating PI3K/Akt Molecular Pathway in Prevention and Treatment of Diabetes Complications: A Review

Diabetes mellitus (DM) is a metabolic disease caused by absolute or relative insulin deficiency and reduced insulin sensitivity in peripheral cells, posing a serious threat to global health. Chronic complications arising in the later stages of DM can lead to the decline or even loss of function in multiple organs, including the eyes, heart, liver, kidneys, nerves, and feet, making them the primary cause of mortality in DM patients. Although modern medicine has made some progress in the treatment of these complications, challenges such as high costs and adverse drug reactions remain. Thus, identifying highly effective drugs with minimal adverse effects has become a top priority. Astragalus membranaceus is a shining gem in the treasure trove of Chinese medicine. Numerous studies have shown that its primary active component, astragaloside Ⅳ, possesses various biological activities, including anti-inflammatory, antioxidant, and antiviral effects, as well as benefits for cardiac and cerebral function, nerve conduction, and myocardial protection. Meanwhile, the phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) signaling pathway plays a crucial role in regulating oxidative stress, inflammatory responses, apoptosis, and autophagy. Extensive research has highlighted the significant role of this pathway in various DM complications, leading to widespread studies on its interaction with astragaloside Ⅳ. This review summarizes research findings on how astragaloside Ⅳ alleviates pancreatic cytotoxicity in DM patients by modulating the PI3K/Akt pathway. Additionally, it highlights its protective effects on basic cardiac function, inhibition of retinal cell damage, improvement of cerebral nerve dysfunction, reduction of chronic kidney and liver damage, and mitigation of neurovascular toxicity in the lower limbs. These insights provide a valuable reference for the clinical application of A. membranaceus and its active monomer, astragaloside Ⅳ, in the treatment of DM and its complications.
4.Medication rules and mechanism of Xin'an physicians in the treatment of diabetes mellitus based on data mining and network pharmacology

Objective To explore the medication rules and mechanism of Xin'an physicians in the treatment of diabetes mellitus.Methods The prescriptions for the treatment of diabetes mellitus in Xin'an medical writings were collected.The core drug prescriptions were obtained by using IBM SPSS Modeler 18.0 for dispensing association rules as well as complex network analysis,and the cluster analysis of high-frequency traditional Chinese medicines was carried out by using IBM SPSS Statistics 25.0.The active ingredients and targets of the core drugs were extracted from the BATMAN-TCM database.The GeneCards database was used to search for diabetes gene targets,and after obtaining the intersecting targets,the STRING online platform was imported to construct the protein interactions network,and screened the core genes by using the Cytoscape 3.9.1 software,and imported into the DAVID database for GO function and KEGG pathway enrichment analysis.Results A total of 135 valid prescriptions were included,involving 184 flavors of traditional Chinese medicines,with cold and mild properties.Sweet,bitter and pungent flavors,and the main attributes of lung,spleen,kidney,stomach and heart meridians.The association rules mined 14 pairs of commonly used medicine pairs.Cluster analysis clustered the top 20 traditional Chinese medicines into five cluster groups.Complex network visualization analysis formed a core prescription consisting of honey-fried licorice root,ginseng,rhizoma anemarrhenae,ophiopogon japonicus,poria cocos.The core prescription drugs were screened for 164 effective active ingredients,1,498 action targets,1,995 diabetes gene targets,404 intersecting targets,10 core targets were extracted,and a total of 1,363 items covering BP were obtained from GO functional enrichment analysis,129 items involving CC,264 items containing MF,and 206 items of KEGG signaling pathway.Conclusion The prescriptions and medicines used by Xin'an doctors in the treatment of diabetes mellitus reflect the academic idea of"treating both the symptoms and the root cause"as well as the therapeutic thought of"using cold and warmth,supplementing with sweetness and warmth,opening up and lowering with bitterness,cultivating the earth and generating gold,and consolidating the root and cultivating the elements",which provide references for the treatment of diabetes mellitus in today's clinics.

6.Literature Case Analysis of 44 Patients with Thrombocytopenia Induced by Clopidogrel

Objective To investigate the characteristics of clopidogrel-associated thrombocytopenia to provide references for clinically safe drug use.Methods The case reports of thrombocytopenia induced by clopidogrel published in PubMed,Embase,CNKI,Wanfang and VIP were searched from the establishment of each database to November 2022,and their occurrence was collated and analysed.Results A total of 44 cases from 43 articles were identified and included in the analysis.There were 30 males(68.2%)and 14 females(31.8%).Ages ranged from 37 to 88(65.0±11.4)years,of which 30(68.2%)were ≥60 years old.Thrombocytopenia was found from 8 h to 9 months after medication,of which 29 cases(65.9%)appeared within two weeks.There were 31 cases(70.5%)with severe thrombocytopenia and 38 cases(86.4%)with complications,of which 24 cases(63.2%)with bleeding and 19 cases(50.0%)with thrombotic thrombocytopenic purpura(TTP).The platelet countof41 cases(93.2%)returned to normal after drug withdrawal and symptomatic treatment,and 3 cases(6.8%)died finally.Conclusion Clopidogrel related thrombocytopenia is mainly severe thrombocytopenia,and often accompanied by bleeding or thrombotic thrombocytopenic purpura(TTP),but the overall outcome is good.Platelet count should be regularly monitored within the first two weeks after medication.Clopidogrel should be stopped and symptomatic treatment should be given in case of any abnormality.
7.Macroscopic Clinical Medication Pattern of Traditional Chinese Medicine for Esophageal Cancer in Recent Ten Years Based on Data Mining

ObjectiveTo explore the macroscopic medication pattern of traditional Chinese medicine (TCM) in treating esophageal cancer (EC) and provide medication references for the clinical application of TCM in EC treatment. MethodRelevant literature on TCM treatment of EC was retrieved from three major Chinese databases: China National Knowledge Infrastructure (CNKI), Wanfang Data, and VIP. Information about Chinese herbal medicines was entered into Excel to establish a prescription database for EC. The data were standardized, summarized, and subjected to frequency analysis, association rules, and cluster analysis of medication in the prescriptions. Based on the TCM classification of EC syndromes, clinical indications corresponding to each syndrome were identified, and high-frequency drugs and drug pairs were analyzed correspondingly with syndromes. ResultA total of 136 prescriptions containing 240 Chinese herbal medicines were screened, with a cumulative frequency of 1 853 times. The top 5 frequently used Chinese herbal medicines were Glycyrrhizae Radix et Rhizoma, Poria, Atractylodis Macrocephalae Rhizoma, Astragali Radix, and Pinelliae Rhizoma. In terms of functions, the Chinese herbal medicines were mainly deficiency-tonifying, urination-promoting and dampness-draining, deficiency-tonifying, deficiency-tonifying, and phlegm-resolving and cough and dyspnea-relieving ones. The statistical analysis of flavor, property, and meridian tropism showed that Chinese herbal medicines were mainly bitter and sweet, warm, cold, and neutral, and acted on the spleen, lung, and stomach meridians. Association rule analysis yielded nine potential drug combinations, and cluster analysis of high-frequency drugs resulted in four combination categories. The four TCM syndromes for EC corresponded to respective clinical indications, treatment drugs, and drug pairs. ConclusionTonifying deficiency, reinforcing healthy Qi, descending adverse Qi, resolving phlegm, activating blood, and resolving stasis are the basic principles of TCM treatment for EC, which are supplemented by clearing heat and dissipating mass while focusing on regulating and smoothing the qi movement. The drug combinations obtained from high-frequency drug and association rule analysis provide references for different TCM syndrome treatments of EC, offering valuable insights for clinical medication.
8.Efficacy and safety of 125I intraluminal irradiation combined with lenvatinib in treatment of progressive extrahepatic cholangiocarcinoma

ObjectiveTo investigate the efficacy and safety of 125I intraluminal irradiation combined with lenvatinib in the treatment of progressive extrahepatic cholangiocarcinoma. MethodsA retrospective analysis was performed for 25 patients with progressive extrahepatic cholangiocarcinoma who attended Department of Interventional Radiology, The First Affiliated Hospital of Zhengzhou University, from January 2018 to November 2021, and according to the treatment modality, they were divided into combination group with 13 patients (125I intraluminal irradiation combined with lenvatinib) and control group (125I intraluminal irradiation alone). The two groups were compared in terms of technical success rates, changes in liver function, stent patency, survival time, and incidence rates of adverse events. The independent-samples t test was used for comparison of normally distributed continuous data between two groups, and the Wilcoxon rank-sum test was used for comparison of continuous data with skewed distribution between two groups; the Fisher’s exact test was used for comparison of categorical data between groups. The Kaplan-Meier method and the log-rank test were used to evaluate survival time and stent patency. ResultsAll patients had successful implantation of biliary stents and 125I particles, with a technical success rate of 100%. After 1 month of treatment, both groups had significant improvements in the serum levels of total bilirubin, direct bilirubin, alanine aminotransferase, and aspartate aminotransferase (all P<0.05). There were significant differences between the control group and the combination group in the duration of stent patency (7.0 months vs 9.5 months, P=0.022) and median survival time (11.5 months vs 15.6 months, P=0.008). There were no intolerable adverse events in the combination group during treatment. ConclusionCompared with 125I intraluminal irradiation alone, 125I intraluminal irradiation combined with lenvatinib has better efficacy and is a safe and effective treatment regimen for progressive extrahepatic cholangiocarcinoma.
9.A study on sensory processing characteristics of preschool children with autism spectrum disorder

ObjectiveTo explore the sensory processing characteristics of preschool children with autism spectrum disorder (ASD), and to provide a theoretical basis for early screening and intervention training of ASD. MethodsA total of 215 preschool children with ASD and170 typically developed (TD) children were investigated with a basic situation questionnaire and sensory processing measure (SPM). The two groups were stratified according to age and gender, and the differences of scores in sensory domains were compared to analyze the sensory processing characteristics of preschool children with ASD. ResultsThe scores of social participation, vision, hearing, touch, taste and smell, body awareness, balance and motion, planning and ideas, and total sensory system in children with ASD were all higher than those in children with TD (all P<0.01). The highest degree of abnormality was found in hearing and the lowest degree in taste and smell in children with ASD. The results of Spearman correlation analysis showed that in the 4-year-old and 5-year-old children with ASD, the scores of vision (rs=-0.200, P= 0.033) and hearing (rs=-0.194, P=0.040) decreased with age. There was no correlation between the scores of other developmental quotients and age (all P>0.05). Boys and girls with ASD had higher scores in all developmental quotients than TD children (P<0.01). However, there was no significant gender difference in any developmental quotients of ASD children (all P>0.05). ConclusionSensory processing abnormalities are common in preschool children with ASD, which are different from those of TD children in multiple sensory domains. Sensory processing abnormalities may be used as an indicator for early screening of ASD, and it is necessary to conduct corresponding intervention training for sensory processing abnormalities in children with ASD.
10.Research progress of chronic endometritis and reproduction-related diseases

The diagnostic criterion for chronic endometritis (CE) is abnormal plasma membrane infiltration of the endometrial stroma. However, because most of CE patients have no obvious symptoms, some symptomatic patients are often confused with symptomatic treatment of pelvic inflammatory disease and vaginitis, so CE are often ignored in the clinic. In recent years, with the progress of the human microbiology program and human assisted reproductive technology, more and more studies suggested that there was a potential relationship between CE and reproduction-related diseases such as repeated pregnancy loss, repeated implantation failure and endometriosis. This article reviewed the research progress of CE and reproduction-related diseases.
