1.Efficacy analysis of an improved radiofrequency ablation method for primary great saphenous vein varicose

Objective:To investigate the safety and efficacy of a modified radiofrequency ablation(RFA)treatment method for primary great saphenous vein varicose.Methods:Clinical data of 90 patients with primary great saphenous vein varicose treated with ultrasound-guided RFA from January 2021 to April 2024 in the Ultrasound Department of the First Affiliated Hospital of Zhengzhou University were retrospectively analyzed. Among them,45 patients were treated with traditional RFA treatment method(traditional group)and 45 patients were treated with improved RFA treatment method(improved group). Number of punctures,operation time,foam hardener dosage,intraoperative and postoperative complications were recorded in the two groups. The preoperative and postoperative venous clinical severity score(VCSS)and chronic venous insufficiency questionnaire(CIVIQ-14)were compared. The closure rate and recurrence rate of great saphenous vein varicose were followed up and the efficacy of the two methods were analyzed.Results:The success rate of the improved group and the traditional group was 100%.The number of punctures in the improved group was less than those of the traditional group[1(1,1) vs. 2(2,3), Z = -7.431, P<0.001],and the operation time of the modified group was shorter than that of the traditional group[(15.89 ± 3.63)min vs.(30.91 ± 5.58)min, t=-15.145, P<0.001],the average volume of lauryl foam was lower than that of the traditional RFA group[(7.96 ± 2.36)ml vs.(15.69 ± 2.89)ml, t=-13.892, P<0.001]. The incidence of complications was similar between the two groups,with no statistical significance(all P>0.05). Postoperative VCSS and CIVIQ-14 scores were significantly improved compared with before(all P<0.001),with no statistical significance between the two groups(all P>0.05). At 12 months after the operation,there was no significant difference in the closure rate of the saphenous vein between the improved group and the traditional group( P>0.05),and the recurrence rate of varicose veins in both groups was 0. Conclusions:This modified RFA treatment method for the treatment of lower extremity varicose veins is minimally invasive,safe,and has the same efficacy as the traditional RFA treatment method. Compared with the traditional RFA treatment method,the modified RFA treatment method has the advantages of convenient operation,less puncture times and shorter operation time,and is worthy of clinical promotion.

This article summarized Professor Liu Qiquan's clinical experience in the treatment of acne from the perspective of"internal retention and stagnation of heat pathogen".Professor Liu believes that the core pathogenesis of acne lies in the internal retention and stagnation of heat pathogen,which manifests as external skin inflammation,with the primary focus being on the heat pathogen.With the pathogen inside the body,the progress of disease and syndrome is related to"toxin,depression,blood stasis and deficiency".At the same time,the disease is also closely related to the dysfunction of the five-zang organs.Professor Liu points out that"to resist the outside,one must first settle the inside",and treats acne from the five internal organs.Based on the idea of"internal retention and stagnation of heat pathogen",according to the symptoms of the patients,the comprehensive use of methods to guide the treatment,such as penetrating turbidity,removing pathogenic factors and detoxifying heat,promoting qi and dispersing stagnant heat,cooling collaterals to control blood and breaking stasis heat,and restraining qi,softening yin and supporting deficiency heat,has been proved to have a good clinical effect.One medical case was attached as evidence.
3.Treatment of Recurrent Aphthous Ulcers from the Perspective of "Heart"

Based on the traditional Chinese medicine theory that "all pain, itching, and sores are related to the heart", this paper proposes treating recurrent aphthous ulcers from the perspective of the heart. It suggests that excessive heart fire and tissue erosion due to flaming fire in the heart meridian constitute the core pathogenesis of this condition. Hyperactive heart fire is identified as the key pathogenic factor, while heart yin deficiency, obstruction of the heart collaterals, and malnourishment of the heart spirit are considered significant contributing factors. Clinically, the treatment follows the principle of clearing heart fire as the main strategy, supplemented by nourishing yin, activating collaterals, and calming the spirit. The self-formulated Qingxin Yuchuang Formulation (清心愈疮方) serves as the base prescription, with flexible modifications incorporating the Yuyin Formulation (育阴方), Huoxue Formulation (活血方), and Yu'an Formulation (郁安方) to address specific syndromes involving heart yin deficiency, collateral blockage, and emotional disturbance.
4.Mechanism of Ethoxysanguinarine in Regulating IRE1/RIDD Signaling Pathway to Inhibit Endoplasmic Reticulum Stress and Alleviate Cardiomyocyte Apoptosis

ObjectiveTo investigate the effects of ethoxysanguinarine (ETH) on angiotensin Ⅱ (Ang Ⅱ)-mediated cardiomyocyte apoptosis and its regulatory effects of inositol-requiring enzyme 1 (IRE1)/regulated IRE1-dependent decay (RIDD) signaling pathway and endoplasmic reticulum stress. MethodsWestern blot was used to detect the establishment of the H9c2 model via Ang Ⅱ stimulation, which was identified as a cardiomyocyte apoptosis model. Subsequently, the inhibitory effect of ETH on cell proliferation was assessed using the cell counting Kit-8 (CCK-8) to determine the optimal effective dose of ETH. H9c2 cardiomyocytes were divided into a blank group, a model group (Ang Ⅱ, 1 mmol·L-1), and low-, medium-, and high-dose ETH groups (1.25, 2.5, and 5 mmol·L-1). Morphological changes in cardiomyocytes induced by Ang Ⅱ were detected using phalloidin staining. Cardiomyocyte apoptosis was assessed using terminal deoxynucleotidyl transferase dUTP nick and labeling (TUNEL) staining. The apoptosis cycle was detected by Annexin V/PI flow cytometry. Western blot was used to detect the expression levels of apoptosis-related proteins, endoplasmic reticulum stress, and IRE1/RIDD pathway-related proteins. ResultsWestern blot results showed that 1 mmol/mL Ang Ⅱ stimulation significantly increased the protein expression levels of Bip, p-IRE1, and Bid in H9c2 cells (P<0.05, P<0.01), indicating the induction of endoplasmic reticulum stress, activation of the IRE1/RIDD signaling pathway, and initiation of the apoptosis process. Compared with the blank group, the model group showed a significant increase in the surface area of H9c2 cells and the apoptosis rate of cardiomyocytes, as well as in both early and late apoptosis rates (P<0.01). The expression levels of Bid, Bax, cleaved-Caspase-3, and cleaved-Caspase-8 proteins were significantly increased, while the expression level of Bcl-2 protein was significantly decreased (P<0.01). The expression levels of Bip, p-IRE1, and p-RIDD proteins were significantly increased (P<0.05, P<0.01). Compared with those in the model group, the surface area of cardiomyocytes and the apoptosis rate of cardiomyocytes in all ETH groups were significantly decreased after drug intervention. Both early and late apoptosis rates were significantly decreased. The expression level of cleaved-Caspase-8 was significantly decreased in the low-dose ETH group (P<0.05). The expression levels of Bid, Bax, and cleaved-Caspase-8 were significantly decreased in the medium-dose ETH group (P<0.05, P<0.01). The high-dose ETH group significantly reduced the expression levels of Bid, Bax, cleaved-Caspase-3, and cleaved-Caspase-8 (P<0.05, P<0.01) and significantly increased the expression level of Bcl-2 (P<0.05). The level of p-IRE1 protein in the medium-dose ETH group was significantly decreased (P<0.01). The expression levels of Bip, p-IRE1, and p-RIDD proteins in the high-dose ETH group were significantly decreased (P<0.05, P<0.01). ConclusionETH can alleviate Ang Ⅱ-mediated cardiomyocyte apoptosis by inhibiting the IRE1/RIDD signaling pathway and further alleviate the cardiac injury caused by hypertension.

Polycystic ovary syndrome (PCOS) is a common reproductive endocrine disorder in women of childbearing age, which can cause metabolic disorders, cardiovascular disease, ovarian cancer, uterine cancer and other complications, seriously endangering the health of the body. China has become one of the countries with the fastest increasing prevalence of PCOS, but its complex pathogenesis leads to highly heterogeneous clinical manifestations, making it difficult to completely cure. Therefore, clarifying the potential pathogenesis of PCOS is of great significance for early clinical screening, diagnosis, treatment, and prognosis. Recent studies have shown that long noncoding RNA (lncRNA) plays a dual role in the pathogenesis of PCOS and is a potential novel biomarker and intervention target. The characteristics of multi-component, multi-target, and multi-pathway action in Traditional Chinese Medicine (TCM) are consistent with the biological properties of lncRNA, which have diverse types, dual roles, and diverse locations. However, research on lncRNA mediated PCOS and how TCM can improve PCOS by regulating lncRNA is relatively scattered, which is not conducive to the recognition of its clinical value. Therefore, this article provides a systematic review of the dual regulatory mechanism, clinical value, and TCM intervention research of lncRNA in the occurrence and development of PCOS, aiming to clarify how lncRNA affects the occurrence and development of PCOS and potential treatment strategies, in order to provide new ideas for the clinical prevention and treatment of PCOS.

10.Development and validation of a nutrition-related genetic-clinical-radiological nomogram associated with behavioral and psychological symptoms in Alzheimer’s disease

Background::Few evidence is available in the early prediction models of behavioral and psychological symptoms of dementia (BPSD) in Alzheimer’s disease (AD). This study aimed to develop and validate a novel genetic-clinical-radiological nomogram for evaluating BPSD in patients with AD and explore its underlying nutritional mechanism.Methods::This retrospective study included 165 patients with AD from the Chinese Imaging, Biomarkers, and Lifestyle (CIBL) cohort between June 1, 2021, and March 31, 2022. Data on demographics, neuropsychological assessments, single-nucleotide polymorphisms of AD risk genes, and regional brain volumes were collected. A multivariate logistic regression model identified BPSD-associated factors, for subsequently constructing a diagnostic nomogram. This nomogram was internally validated through 1000-bootstrap resampling and externally validated using a time-series split based on the CIBL cohort data between June 1, 2022, and February 1, 2023. Area under receiver operating characteristic (ROC) curves, calibration curves, and decision curve analysis (DCA) were used to assess the discrimination, calibration, and clinical applicability of the nomogram.Results::Factors independently associated with BPSD were: CETP rs1800775 (odds ratio [OR] = 4.137, 95% confidence interval [CI]: 1.276-13.415, P = 0.018), decreased Mini Nutritional Assessment score (OR = 0.187, 95% CI: 0.086-0.405, P <0.001), increased caregiver burden inventory score (OR = 8.993, 95% CI: 3.830-21.119, P <0.001), and decreased brain stem volume (OR = 0.006, 95% CI: 0.001-0.191, P = 0.004). These variables were incorporated into the nomogram. The area under the ROC curve was 0.925 (95% CI: 0.884-0.967, P <0.001) in the internal validation and 0.791 (95% CI: 0.686-0.895, P <0.001) in the external validation. The calibration plots showed favorable consistency between the prediction of nomogram and actual observations, and the DCA showed that the model was clinically useful in both validations. Conclusion::A novel nomogram was established and validated based on lipid metabolism-related genes, nutritional status, and brain stem volumes, which may allow patients with AD to benefit from early triage and more intensive monitoring of BPSD.Registration::Chictr.org.cn, ChiCTR2100049131.
