Objective:To explore the correlation between periodontitis (PD) and chronic kidney disease (CKD) in adults, as well as the potential mechanisms involved.Methods:Data on PD and CKD from the National Health and Nutrition Examination Survey (NHANES) database between 1999 and 2014 were downloaded. Weighted univariate and multivariate logistic regression analyses were conducted to investigate the risk factors associated with PD and CKD, considering demographic and clinical indicators. Using publicly available genome-wide association study (GWAS) summary datasets for CKD and PD as outcome variables, as well as 731 immune cell phenotypes and 91 inflammatory proteins as exposure factors from the OPEN GWAS database, a two-sample Mendelian randomization (TSMR) analysis was performed using the inverse-variance weighted (IVW) method.Results:Seven demographic indicators including gender, age, race, education level, marital status, income, and health are related to the incidence of CKD and PD. Among them, the elderly (≥60 years old), poverty (poverty-income ratio <1.3), divorce or widowhood, and male ratio in the comorbidity group of CKD and PD [67.12% (833/1 241), 36.83% (457/1 241), 34.41% (427/1 241), and 57.78% (717/1 241) respectively] were significantly higher than those in the control group [23.71% (4 179/17 623), 29.17% (5 141/17 623), 18.16% (3 200/17 623), and 48.73% (8 587/17 623) respectively] (all P<0.001). Those with high educational level (university and above) and self-rated excellent health accounted for a relatively small proportion in the comorbidity group [14.10% (175/1 241) and 8.22% (102/1 241) respectively]. The prevalence of PD increased among individuals with abnormal renal function indices, including glomerular filtration rate, urine protein/creatinine ratio, serum creatinine, serum uric acid, and blood urea nitrogen. Univariate logistic regression analysis showed a positive correlation between the incidence of PD and CKD ( OR=2.14, 95 %CI: 1.90-2.42, P<0.001). Multivariate logistic regression analysis also indicated that PD and CKD were potential risk factors for each other (PD for CKD: OR=1.22, 95 %CI: 1.07-1.40, P=0.004; CKD for PD: OR=1.19, 95 %CI: 1.04-1.37, P=0.012). Furthermore, after adjusting the model based on demographic indicators, there was still a significant correlation between PD and CKD ( P=0.010). Mechanistically, the results of the TSMR analysis support the existence of a common risk factor mediated by immune cells between CKD and PD, namely the expression of CD64 on multiple innate immune cells mediates the occurrence of CKD and PD. The absolute count of CD64 + monocytes is associated with an increased risk for both CKD ( HR=1.11) and PD ( HR=1.07), while same tendency showed in the absolute count of CD64 + neutrophils for CKD ( HR=1.22) and PD ( HR=1.23). Conclusions:There is a positive correlation between CKD and PD, particularly moderate to severe PD, and the shared pathogenesis involves CD64 + monocytes in the circulatory system. Targeted interventions focusing on CD64 molecules or monocyte subsets may be beneficial.

Methods The typical sampling method was used to select 18 stations from Wuhan Metro Line 2, Line 3, and Line 7 for the measurement and evaluation of radon in the air. Using RAD7 electronic radon detector and continuous radon measurement method, radon concentrations in station platforms and station halls were investigated. Analysis of variance and t test were used for statistical analysis. Results The concentration of radon in Wuhan subway ranged from 8.8 to 16.2 Bq/m³, with an average of 11.5 Bq/m³, which is far lower than the limit set by Requirements for Radiological Protection for Radon in Public Underground Structure and Using Geothermal Water (WS/T 668-2019). There was no significant difference in radon concentration between station platform and station hall (t = −0.081, P > 0.05). There was no significant difference in radon concentration between different lines (F = 0.338, P > 0.05). On the whole, the radon concentration in summer was lower than that in winter, and there was no significant difference between months (F = 1.280, P > 0.05). Conclusion The radon concentrations in Wuhan subway result in low-dose radiation, and there is no significant health risk of radon exposure.

Objective to establish a combined diagnosis model of scleroderma related genes based on gene expression comprehensive database(GEO)and artificial neural network(ANN)and to evaluate its effect and to predict and analyze targeted traditional Chinese medicine.Methods two scleroderma chips GSE23741 and GSE95065 were obtained from the GEO database as the training group data set.Random forest and lasso regression algorithms were used to screen the key genes of scleroderma and construct the ANN model for the diagnosis of scleroderma.The validation data sets GSE76807,GSE32413 and GSE59785 were used to verify the model,and the area under curve(AUC)analysis was used to evaluate the clinical application value of ANN model.The relative expression of key gene mRNA was verified by RT-qPCR experiment.The CIBERSORT algorithm was used to estimate the bioinformatics association between scleroderma and the screened biomarkers.Finally,the key genes were used to screen the targeted traditional Chinese medicine.Results A total of 167 differential genes were obtained.Furthermore,the five most relevant key genes(SERPINE2,SFRP4,SUGCT,FBLN5,NRXN2)were screened by machine learning,and the artificial neural network diagnosis model was constructed.The model was used to draw the subject operating characteristic(ROC)curves diagnosed by the training group and the verification group,and the AUC value of the training group was 1.000.The AUC of verification group were 0.770,0.795 and 0.872 respectively.The result of RT-qPCR experiment is consistent with that of machine learning algorithm.Immune cell infiltration analysis showed that the relative content of memory CD4+T cells was significantly increased in scleroderma group,while the relative content of γ δ T cells in normal group was significantly increased.Key genes are associated with macrophage M1,T cells,memory activated CD4+T cells,resting mast cells,CD8+T cells and so on.According to the key genes,12 traditional Chinese medicines were screened.Most of the four qi and five flavors belong to warm,cold,flat,sweet,pungent and bitter,and most of them belong to the meridians of liver,spleen and lung.Conclusion the artificial neural network diagnosis model of key genes of scleroderma is constructed,which can be used in clinical diagnosis of scleroderma,and the potential targeted traditional Chinese medicine for the treatment of scleroderma is predicted,which provides a new perspective for exploring the pathogenesis of scleroderma.

Objective: To observe the clinical efficacy of herbal cake-partitioned moxibustion for ulcerative colitis (UC) and elucidate its mechanism by targeting the vitamin D receptor (VDR) signaling pathway. Methods: A total of 63 patients with UC were randomly divided into an observation group (30 cases, treated with herbal cake-partitioned moxibustion) and a control group (33 cases, treated with sham herbal cake-partitioned moxibustion). Moxibustion treatment was performed at Qihai (CV6) and bilateral Tianshu (ST25) and Shangjuxu (ST37), 3 times per week for 12 weeks. The total effective rate, visual analog scale (VAS) score for abdominal bloating and pain, and hospital anxiety and depression scale (HADS) score were compared between the two groups. Enzyme-linked immunosorbent assay was used to detect the concentrations of serum C-reactive protein (CRP), 25-hydroxyvitamin D [25(OH)D], and interleukin-12 (IL-12)/interleukin-23 (IL-23) p40. Immunohistochemistry was used to observe the expression levels of VDR and regenerating gene Ⅳ (Reg Ⅳ) proteins in colonic mucosa. The expression levels of VDR, cytochrome p45027B1 (CYP27B1), and Reg Ⅳ mRNAs were detected by real-time fluorescence quantitive polymerase chain reaction. Results: After treatment, the total effective rate in the observation group was 86.7％, which was significantly higher than 51.5％ in the control group (P<0.05). After treatment, the VAS scores for abdominal bloating and pain in the observation group were significantly decreased (P<0.01), as well as the HADS-depression subscale (HADS-D) and HADS-anxiety subscale (HADS) scores (P<0.05), while only the VAS score for abdominal pain in the control group was reduced (P<0.05), and the improvements of the scores in the observation group were more significant than those in the control group (P<0.05). After treatment, the serum CRP concentrations in both groups and the IL-12/IL-23 p40 concentration in the observation group were significantly decreased (P<0.05), and the concentrations in the observation group were lower than those in the control group (P<0.05). The expression levels of VDR protein and mRNA in the colon in both groups were all increased (P<0.01), and the expression levels of Reg Ⅳ protein and mRNA and CYP27B1 mRNA were all decreased in the two groups (P<0.05 or P<0.01); the improvements in the observation group were more notable than those in the control group (P<0.05 or P<0.01). Conclusion: Herbal cake-partitioned moxibustion can effectively alleviate abdominal pain and diarrhea in patients with UC, improve depression and anxiety disorders, and regulate the expression of related proteins in the VDR signaling pathway. The mechanism may be related to inhibiting intestinal inflammation by reducing the release of the proinflammatory cytokine IL-12/IL-23 p40.

Objective To deeply understand the psychological experience of patients with primary tumors participating in clinical trials,providing a reference basis for meeting the psychological needs of patients and improving the mental health level of patients with primary tumors.Methods From November 2021 to May 2022,13 patients with primary tumors participating in clinical trials were selected by target sampling method,semi-structured interviews were conducted,with Colaizzi 7-step analysis method was used to analyze the data.Results Four themes were extracted from the psychological experience of patients,namely the perception and attitudinal experience of clinical trials,complex and changeable emotional responses,experimental process adaptation,personal growth and transformation.Conclusion The psychological experience and related psychological problems of primary tumors patients participating in clinical trials cannot be ignored,the research team needs to help them complete role transformation and adaptation,pay attention to the experience of patients,improve mental health education programs according to the needs of this type of patients,and improve the quality of life of patients,then promote the development of cancer diagnosis and treatment in China.

Objective:To explore the influencing factors by investigating the spiritual health status of patients with lung cancer during perioperative period.Methods:A total of 208 perioperative patients who underwent radical resection of lung cancer in Cancer Hospital Chinese Academy of Medical Sciences from September 2020 to September 2021 were selected as research objects by the convenient sampling method. The General Data Questionnaire, Chinese version of Functional Assessment of Chronic illness Therapy-Spiritual (FACIT-SP-12) and Family APGAR Index (APGAR) were used to investigate the patients. Pearson correlation analysis was used to explore the correlation between spiritual health and family caring. Pearson correlation analysis was used to explore the correlation between spiritual health and family caring. Multiple linear regression analysis was used to investigate the influencing factors of spiritual health in patients with lung cancer during perioperative period. A total of 208 questionnaires were sent out and 200 were effectively received, with the effective recovery of 96.15%.Results:The total score of FACIT-SP-12 of 200 patients with perioperative lung cancer was (35.59±8.16) , indicating a moderate spiritual health level. The main factors affecting spiritual health of perioperative lung cancer patients were the degree of family care, marital status and payment of medical expenses ( P<0.05) . Conclusions:The spiritual health of perioperative lung cancer patients is in the middle level, and the spiritual health of patients with different family caring degrees, marital status and medical payment method is different. Clinical medical workers can carry out targeted nursing intervention according to individual conditions of patients to improve their quality of life.

Objective:To investigate the potential guiding role of fractional flow reserve(FFR) in surgical revascularization by comparing the relationship between coronary fractional flow reserve(FFR) and blood flow pattern status of bypass graft.Methods:A total of 86 patients with coronary artery disease between March 2016 to October 2019 were included in the study, with 59 males and 27 females; the age ranged from 42 to 77 years old, with an average of(58±12) years old. According to the measured FFR value of the left anterior descending artery, they were divided into severe ischemic group(FFR<0.75), boundary group(0.75≤FFR<0.80) and mild ischemic group(FFR≥0.80). Transit time flow meter(TTFM) was used to evaluate the blood flow status of the bridge vessel from the left internal thoracic artery to the left anterior descending coronary artery.Results:Mean graft flow(MGF) was measured at(21.24±5.71)ml/min, (18.25±7.72)ml/min, (16.47±7.83)ml/min in severe ischemic group, boundary group and mild ischemic group. The results of mean pulsatility index(PI) was 2.58±0.96, 3.14±1.19 and 3.53±1.34, the ratio of diastolic flow during the entire graft flow was 0.63±0.10, 0.55±0.11 and 0.53±0.11, patients appeared systolic reverse flow was 2 cases(3.6%), 3 cases(18.8%) and 3 cases (20.0%), respectively. There were statistically significant differences in MGF( P=0.027)、PI( P=0.007)、the ratio of DF( P=0.001) and the quantity of patients appeared systolic reverse flow( P=0.027) between the three groups. Conclusion:Due to increasing severity of coronary artery stenosis, MGF and the ratio of diastolic flow increased, and there appears to be an decreased PI and quantity of patients appeared systolic reverse flow. The chance of bypass graft occlusion may increase for the near and middle term in mild to moderate functional coronary artery stenosis(FFR≥0.75). For patients with severe functional coronary artery stenosis(FFR<0.75), it can obtain satisfactory flow parameters and ideal clinical outcomes.

Objective:To explore the current situation of the use of assistive devices for mobility in patients with knee osteoarthritis and analyze its influencing factors.Methods:Using the convenient sampling method, patients with knee osteoarthritis who were admitted to the Orthopedics Department of a Class Ⅲ Grade A general public hospital in Zhengzhou from January to July 2019 were selected as the research objects. The general information questionnaire, the Quebec User Evaluation of Satisfaction with Assistive Technology (QUEST) Scale and other scales were used to conduct surveys. Univariate analysis and binary Logistic regression analysis were used to investigate the influencing factors of the use of assistive devices for mobility in patients with knee osteoarthritis. In this study, a total of 347 questionnaires were distributed and 340 valid questionnaires were returned. The effective recovery rate was 97.98%.Results:The usage rate of assistive devices for mobility in patients with knee osteoarthritis was 26.47% (90/340) , 5.55% (5/90) of the patients were recommended using assistive devices for mobility by medical staff, 2.22% (2/90) of the patients obtained assistive devices for mobility through professional adaptation, and 81.11% (73/90) of the patients did not receive health education on assistive devices for mobility. The total mean score of the QUEST Scale for 90 patients with knee osteoarthritis using assistive devices for mobility was (3.17±0.34) . Binary Logistic regression analysis showed that age, education level, monthly income, pain intensity, the activity of daily living status and social support were influencing factors for the use of assistive devices for mobility in patients with knee osteoarthritis ( P<0.05) . Conclusions:Patients with knee osteoarthritis have a lower usage rate of assistive devices for mobility, receive fewer professional assistive devices for mobility services, and have lower satisfaction with them. There are many factors affecting whether or not to use assistive devices for mobility. Medical staff should pay more attention to and guide the use of assistive devices for mobility for patients with knee osteoarthritis.
