OBJECTIVE: To explore the research history, hotspots and development trends of acupuncture and moxibustion in the treatment of gastroesophageal reflux disease（GERD）based on knowledge graph technology, and to provide references for clinical and basic research in this field. METHODS: The literature of acupuncture and moxibustion for gastroesophageal reflux disease was searched from the CNKI, Wanfang, VIP and SinoMed, from the establishment of the databases to December 31th, 2023. CiteSpace 6.2.R6 Advance was used to draw the knowledge graph of authors, institutions, keywords and other elements, and then perform the visual analysis. RESULTS: A total of 341 articles were included, with the number of publications showing an upward trend and the research types continually diversifying. A total of 832 authors and 308 institutions were analyzed, with XIE Sheng from the First Affiliated Hospital of Guangxi University of CM and BAI Xinghua from the Beijing University of CM as representative figures, forming core research teams. However, there was a lack of close collaboration between institutions, and no significant cross-regional research networks had been formed. A total of 192 keywords were included, forming 8 cluster labels, which mainly included 4 categories：treatment methods, disease types, TCM syndrome types, and literature types. The burst analysis showed that the methods of acupuncture and moxibustion in the treatment of gastroesophageal reflux disease had gradually become more integrated, the treatment methods had transitioned from simple acupuncture therapy to combined therapies with proton pump inhibitors or TCM decoctions, the disease types had become more refined, the focus of mechanism research had shifted from lower esophageal sphincter pressure and esophageal motility to changes in gastrointestinal hormone levels, and the research hotspots had gradually shifted from improving clinical symptoms to considering both mental and psychological states. Twenty-three high-frequency acupoints were obtained, forming 8 clusters of "acupuncture techniques-acupoints" for the treatment of gastroesophageal reflux disease with acupuncture and moxibustion, indicating a gradual enrichment of acupuncture and acupoint treatment protocols. CONCLUSION The research on acupuncture and moxibustion in the treatment of gastroesophageal reflux disease has gradually deepened, in the future, the cooperation among research teams should be strengthened, the quality of clinical research should be improved, more multi-dimensional mechanism research and horizontal comparative research of different acupuncture and moxibustion methods should be made, to provide a basis for clinical promotion and deeper exploration.

Antibody (Ab) humanization is critical to reduce immunogenicity and enhance efficacy in the preclinical phase of the development of therapeutic Abs originated from animal models. Computational suggestions have long been desired, but available tools focused on immunogenicity calculation of whole Ab sequences and sequence segments, missing the individual residue sites. This study introduces Site-specific Immunogenicity for Therapeutic Antibody (SITA), a novel computational framework that predicts B-cell immunogenicity score for not only the overall antibody, but also individual residues, based on a comprehensive set of amino acid descriptors characterizing physicochemical and spatial features for antibody structures. A transfer-learning-inspired framework was purposely adopted to overcome the scarcity of Ab-Ab structural complexes. On an independent testing dataset derived from 13 Ab-Ab structural complexes, SITA successfully predicted the epitope sites for Ab-Ab structures with a receiver operating characteristic (ROC)-area unver the ROC curve (AUC) of 0.85 and a precision-recall (PR)-AUC of 0.305 at the residue level. Furthermore, the SITA score can significantly distinguish immunogenicity levels of whole human Abs, therapeutic Abs and non-human-derived Abs. More importantly, analysis of an additional 25 therapeutic Abs revealed that over 70% of them were detected with decreased immunogenicity after modification compared to their parent variants. Among these, nearly 66% Abs successfully identified actual modification sites from the top five sites with the highest SITA scores, suggesting the ability of SITA scores for guide the humanization of antibody. Overall, these findings highlight the potential of SITA in optimizing immunogenicity assessments during the process of therapeutic antibody design.

Objective: To investigate the association of sugar sweetened beverage (SSB) consumption with insomnia and depressive symptoms among first grade middle school students from Yunnan Province, so as to provide evidence to guide interventions for the treatment of these symptoms in this population. Methods: From October to December 2022, 8 500 firstgrade middle school students from 11 counties in Yunnan Province were selected by cluster random sampling. Depressive symptoms, SSB consumption, and insomnia symptoms among students were evaluated by the Child Depression Scale (CDI), dietary frequency questionnaire, and Insomnia Severity Index (ISI). A Logistic regression model was employed to analyze the relationship between SSB consumption, insomnia, depressive symptoms, and their interaction effects among students. Results: The detection rate of depressive symptoms was 28.3%, and having insomnia symptoms ( OR=6.07, 95%CI =5.46-6.75), consuming carbonated beverages ( OR=1.20, 95%CI =1.08-1.34), tea ( OR=1.13, 95%CI =1.02-1.25), energy drinks ( OR=1.36, 95%CI =1.23-1.50), and other beverages ( OR=1.32, 95%CI =1.19-1.45) were positively correlated with depressive symptoms among first grade middle school students ( P < 0.05). Carbonated beverages (additive effect: OR=2.96, 95%CI =2.72-3.22, multiplicative effect: OR=4.75, 95%CI =4.25- 5.32 ), fruit drinks (additive effect: OR=2.61, 95%CI =2.40-2.82; multiplicative effect: OR=4.43, 95%CI =3.94-4.97), tea (additive effect: OR=2.70, 95%CI =2.47-2.89; multiplicative effect: OR=4.34, 95%CI =3.86-4.89), energy drinks (additive effect: OR=2.82, 95%CI =2.61-3.05; multiplicative effect: OR=4.48, 95%CI =3.92-5.12), sweetened milk (additive effect: OR= 2.73, 95%CI =2.06-2.96; multiplicative effect: OR=4.61, 95%CI =4.12-5.17) and other beverages (additive effect: OR= 2.73 , 95%CI =2.53-2.95; multiplicative effect: OR=4.56, 95%CI =4.00-5.20) had both additive and multiplicative effects with insomnia, and increased the risk of depressive symptoms in first grade middle school students ( P <0.01). Conclusions The interaction between the consumption of SSB and insomnia symptoms may increase the risk of depressive symptoms among first grade middle school students in Yunnan Province. It is necessary to advocate middle school students to reduce SSB intake, in order to decrease the occurrence of depressive symptoms among this population.

Objective:To explore the relationship between weight change and metabolic control of type 2 diabetic patients after 1 year treatment under the standardized metabolic disease management model.Methods:From November 2017 to November 2021, 950 patients with T2DM in the Standardized Metabolic Management Center of Suzhou Municipal Hospital were recruited in this research, who were divided into normal(BMI 18.5-23.9 kg/m 2), overweight(BMI 24.0-27.9 kg/m 2), and obese(BMI≥28 kg/m 2)groups according to baseline BMI. The metabolic indicators and control rates were compared. The overweight and obese 599 patients were divided into weight reduction(<-3 kg), maintenance(-3-1 kg) and gain(>1 kg)groups according to weight changes by one year, and metabolic indicators and control rates were compared to analyze the influence of weight change on metabolic control of overweight and obese patients. Results:(1) The overweight and obese groups had worse metabolic control at baseline and the obese group was younger. After 1 year, the metabolic indicators of three groups were significantly improved. (2) After 1 year, the metabolic control was better in weight reduction group and worse in weight gain group. The control rates were increased in weight reduction and maintenance groups. (3) For every 1 kg reduction in body weight of overweight and obese patients, blood glucose, blood lipids, blood pressure and comprehensive control rate increased by 1.206, 1.046, 1.069 and 1.107 times, respectively. There was a positive linear relationship between weight reduction and metabolic improvement.Conclusion:Standardized metabolic disease management model promotes metabolic control in type 2 diabetic patients, especially in overweight and obese patients who actively lose weight. In the future we should strengthen the management of overweight and obese patients, enhance self-management ability, and manage body weight scientifically and effectively.

Objective To deeply understand the psychological experience of patients with primary tumors participating in clinical trials,providing a reference basis for meeting the psychological needs of patients and improving the mental health level of patients with primary tumors.Methods From November 2021 to May 2022,13 patients with primary tumors participating in clinical trials were selected by target sampling method,semi-structured interviews were conducted,with Colaizzi 7-step analysis method was used to analyze the data.Results Four themes were extracted from the psychological experience of patients,namely the perception and attitudinal experience of clinical trials,complex and changeable emotional responses,experimental process adaptation,personal growth and transformation.Conclusion The psychological experience and related psychological problems of primary tumors patients participating in clinical trials cannot be ignored,the research team needs to help them complete role transformation and adaptation,pay attention to the experience of patients,improve mental health education programs according to the needs of this type of patients,and improve the quality of life of patients,then promote the development of cancer diagnosis and treatment in China.

Objective:To explore the influencing factors by investigating the spiritual health status of patients with lung cancer during perioperative period.Methods:A total of 208 perioperative patients who underwent radical resection of lung cancer in Cancer Hospital Chinese Academy of Medical Sciences from September 2020 to September 2021 were selected as research objects by the convenient sampling method. The General Data Questionnaire, Chinese version of Functional Assessment of Chronic illness Therapy-Spiritual (FACIT-SP-12) and Family APGAR Index (APGAR) were used to investigate the patients. Pearson correlation analysis was used to explore the correlation between spiritual health and family caring. Pearson correlation analysis was used to explore the correlation between spiritual health and family caring. Multiple linear regression analysis was used to investigate the influencing factors of spiritual health in patients with lung cancer during perioperative period. A total of 208 questionnaires were sent out and 200 were effectively received, with the effective recovery of 96.15%.Results:The total score of FACIT-SP-12 of 200 patients with perioperative lung cancer was (35.59±8.16) , indicating a moderate spiritual health level. The main factors affecting spiritual health of perioperative lung cancer patients were the degree of family care, marital status and payment of medical expenses ( P<0.05) . Conclusions:The spiritual health of perioperative lung cancer patients is in the middle level, and the spiritual health of patients with different family caring degrees, marital status and medical payment method is different. Clinical medical workers can carry out targeted nursing intervention according to individual conditions of patients to improve their quality of life.

Objective:To compare two different methods for the harvest of the costal cartilage cortical slice with perichondrium preserved for rhinoplasty patients, and to explore a simple and less invasive method for the costal cartilage cortical slice.Methods:From September 2019 to December 2020, the rhinoplasty patients using the techniques of nasal tip "double arch" stent combined with prosthesis were selected in Shanghai Time Plastic Surgery Hospital, they were randomly divided into observation group and control group. In the observation group, only the cortical slice of costal cartilage with perichondrium was cut and the continuity of costal cartilage was preserved. While in the control group, with the superficial perichondrium preserved, the whole costal cartilage was dissected first, and then pruned it and harvested the cortical slice of costal cartilage with perichondrium. The operation were performed by the same clinicians in both groups. Different variables were compared between the two groups, including the operation time, intraoperative blood loss, length of incision, the degree of chest pain after postoperative 6, 24, 72 h, 1 week, the incidence of intraoperative complications and so on. The severity of chest pain was assessed by visual analogue scale (VAS). The measurement data of the two groups were analyzed by t-test, and the enumeration data were analyzed by χ2 test. Results:For both observation group and the control group, 61 patients were included. In the observation group, there were 3 males (4.9%) and 58 females (95.1%), aged (28.6±5.4) years old, and BMI (21.76±1.65) kg/m 2. In the control group, there were 2 males (3.3%) and 59 females (96.7%), aged (45.8±5.7) years old, and BMI (22.25±1.47) kg/m 2. There were no significant differences in gender composition, age and BMI between the two groups ( P>0.05). The double arch stent of nasal tip could be made by the costal cartilage cortical slices obtained in the two groups. In the control group, two cases of pleural rupture occurred and their prognosis was good after effective treatment, with no complications such as hematoma, infection or pleural injury. In the observation group, there were no complications such as hematoma, infection or pleural injury. The operation time, incision length and intraoperative blood loss in the observation group were significantly lower than those in the control group [(18.54±3.62) min vs. (25.75±3.75) min; (1.68±0.26) cm vs. (2.16±0.32) cm; (16.79±7.86) ml vs. (25.46±8.49) ml; P<0.05]. The VAS score in the observation group was lower than that in the control group after postoperative 6, 24, 72 h, 1 week [(2.76±0.62) vs. (3.87±0.68)scores; (2.37±0.45) vs. (3.12±0.55) scores; (1.76±0.45) vs. (2.57±0.53) scores; (1.17±0.44) vs. (1.85±0.51) scores; P<0.05]. Conclusions:Compared with the conventional method of whole costal cartilage harvest, the method of harvesting the costal cartilaginous cortical slice with the costal perichondrium preserved and preserving the continuity of the costal cartilage can reduce the trauma and bleeding, shorten the operation time, simplify the operation process, and significantly reduce the postoperative chest pain.

Conclusions:Compared with the conventional method of whole costal cartilage harvest, the method of harvesting the costal cartilaginous cortical slice with the costal perichondrium preserved and preserving the continuity of the costal cartilage can reduce the trauma and bleeding, shorten the operation time, simplify the operation process, and significantly reduce the postoperative chest pain.

Objective:To explore the current status of gastrointestinal complications in patients after cardiac surgery and analyze its influencing factors.Methods:This study was cross-sectional study. From October 2019 to April 2020, convenience sampling was used to select 152 patients who underwent cardiac surgery in the Department of Adult Cardiac Surgery of a ClassⅢ Grade A hospital in Henan Province as the research object. We collected and recorded whether patients had gastrointestinal complications and its related factors after surgery. Binary Logistics multivariate analysis was used to explore the influencing factors of gastrointestinal complications.Results:The incidence of gastrointestinal complications in patients after cardiac surgery was 9.9% (15/152) . Logistic regression analysis showed that the influencing factor of gastrointestinal complications included the patient's preoperative left ventricular ejection fraction ( OR=0.849) , cardiopulmonary bypass time ( OR=0.974) , aortic block time ( OR=1.056) , intraoperative blood loss ( OR=1.007) , intraoperative blood transfusion ( OR=0.996) , time in Intensive Care Unit ( OR=1.463) , type of surgery [coronary artery bypass grafting (CABG) OR=0.199, valve replacement/valvuloplasty OR=0.105, CABG+ valve replacement/valvuloplasty OR=0.006] ( P<0.05) . Conclusions:There are many factors influencing gastrointestinal complications after cardiac surgery. Medical and nursing staff should strengthen the assessment of risk factors and perform predictive care in order to minimize the occurrence of complications and improve the prognosis of patients.

This study was aimed to predict active compounds,drug targets and potential diseases of Yi-Guan decoction (YGD) by network pharmacological technology,and to clarify molecular mechanisms of YGD efficiency on different diseases with liver and kidney yin deficiency syndrome (LKYDS).Chemistry compounds,targets and related diseases from YGD were collected from TCMSP,TCM Database@Taiwan,TCMID,HIT,Drugbank,PubChem and TTD databases.The YGD compounds-targets-diseases network was constructed.The network topology was analyzed by Cytoscape software.The analysis of GO biological processes and KEGG pathways enrichment were performed by DAVID website.The results showed that 849 chemical compounds were identified from Beishashen,Maidong,Danggui,Shengdihuang,Chuanlianzi and Gouqizi.There were 49 active CHM compounds that were both oral bioavailability (OB) ≥ 30％ and drug-likeness (DL) ≥ 0.18,corresponding to 200 target proteins and 264 diseases.The top three GO biological processes were response to organic substance,regulation of cell proliferation,and response to endogenous stimulus,respectively.The top three KEGG pathways were pathways in cancer,hepatitis B,and prostate cancer,respectively.It was concluded that the analysis on YGD was conducted based on network pharmacology,and the compound-target-disease network was built,which may help to clarify the mechanisms of YGD efficiency on different diseases with LKYDS.It can provide clues to find new potential clinical adaptation of disease and new drugs.
