Hepatocellular carcinoma (HCC) is a leading cause of cancer-associated death globally. Liver transplantation (LT) has emerged as a key treatment for patients with HCC, and the Milan criteria have been adopted as the cornerstone of the selection policy. To allow more patients to benefit from LT, a number of expanded criteria have been proposed, many of which use radiologic morphological characteristics with larger and more tumors as surrogates to predict outcomes. Other groups developed indices incorporating biological variables and dynamic markers of response to locoregional treatment. These expanded selection criteria achieved satisfactory results with limited liver supplies. In addition, a number of prognostic models have been developed using clinicopathological characteristics, imaging radiomics features, genetic data, and advanced techniques such as artificial intelligence. These models could improve prognostic estimation, establish surveillance strategies, and bolster long-term outcomes in patients with HCC. In this study, we reviewed the latest findings and achievements regarding the selection criteria and post-transplant prognostic models for LT in patients with HCC.

Oropharyngeal squamous cell carcinoma(OPSCC)is a malignant tumor originating from the squamous epithelium of the oro-pharyngeal mucosa, accounting for more than 90%of oropharyngeal malignancies. In recent years, human papillomavirus(HPV)infec-tion has become one of the primary etiological factors of oropharyngeal squamous carcinoma. The incidence of HPV-associated oropharyn-geal squamous carcinoma has been rising annually, with a noticeable trend toward younger populations, posing a significant threat to hu-man health.Due to the distinct biological behavior and clinical characteristics of HPV-associated oropharyngeal squamous carcinoma com-pared to its non-HPV-related counterpart, the diagnostic and treatment strategies for oropharyngeal squamous carcinoma have undergone substantial changes.Prevention and screening for oropharyngeal squamous carcinoma are of critical importance. The diagnostic and treat-ment process involves multi-disciplinary collaboration, including oral and maxillofacial surgery, otolaryngology, head and neck surgery, oncology, radiology and pathology.Based on evidence from clinical practice,a comprehensive, integrated diagnostic and therapeutic ap-proach has been established, centered around the concept of"prevention, screening, diagnosis, treatment, and rehabilitation",covering the entire patient lifecycle and providing a valuable reference for clinical practice.

Objective To investigate the effects of icariin on myocardial cell apoptosis and transforming growth factor-β1(TGF-β1)/Smad3 signaling pathway in rats with septic myocardial damage. Methods A septic myocardial damage model was established in SD rats via intraperitoneal injection of lipopolysaccharide. The rats with septic myocardial damage were randomly divided into model,low-dose icariin,high-dose icariin, and high-dose icariin+SRI-011381 groups(n=10 per group). An additional 10 rats that received intraperi-toneal injection of an equal volume of normal saline served as the control group. The left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(LVEF),and left ventricular systolic pressure(LVSP)were detected using ultrasound instrument.Hematoxylin and eosin(HE)staining was used to detect the morphology of myocardial tissue.Trans-mission electron microscopy was used to measure the ultrastructure of myocardial tissue. The enzyme-linked immunosorbent assay was used to detect the serum markers of myocardial damage including cardiac troponin T(cTnT)and creatine kinase isoenzyme-MB(CK-MB)and inflammatory cytokines including interlenkin-6(IL-6)and tumor necrosis factor α(TNF-α).Western blotting was performed to assess the expression of TGF-β1/Smad3 pathway-related proteins in the myocardial tissue of rats in each group. Results Compared with the control group,LVEDD,LVESD,myocardial pathological score, myocardial cell apoptosis rate, the cTnT,CK-MB,IL-6,and TNF-α levels,TGF-β1 protein expression, and p-Smad3/Smad3 in the model group were higher(P<0.05),while LVEF and LVSP were lower(P<0.05).Compared with the model group, the pathological indictors of rats in the low-and high-dose icariin groups were improved, and high-dose icariin had a stronger effect.SRI-011381 could weaken the improvement effect of icariin on various pathological indicators in rats with septic myocardial damage. Conclusion Icariin can inhibit the inflammatory response in rats with sepsis by blocking the activation of the TGF-β1/Smad3 signaling pathway, thereby reducing myocardial cell apoptosis and myocardial damage.

Objective:To explore the impact of plant and animal dietary behaviors on type 2 diabetes mellitus (T2DM) in older adults aged ≥65 in 18 longevity areas of China. Methods:The subjects were 5 223 older adults over 65 years old from the Healthy Ageing and Biomarkers Cohort Study (HABCS) in 18 longevity areas in China. Through a questionnaire survey and physical examination, information about their demographic characteristics, lifestyles, daily activities, self-health status, current diseases, and fasting venous blood were collected. Food Frequency and Questionnaire (FFQ) was used to collect data on food intake frequency. Based on the prior method, the plant-based diet index (PDI) and animal-based diet index (ADI) of 5 223 older adults were calculated. Subjects were divided into three groups (low-level group: PDI<39 or ADI<31, middle-level group: 39≤PDI≤42 or 31≤ADI≤34, high-level group: PDI>42 or ADI>34) by tertiles of PDI and ADI. Multivariate logistic regression was used to analyze the association between PDI and ADI and the risk of T2DM.Results:The average age of 5 223 subjects was (84.8±11.5) years, with the median ( Q1, Q3) of PDI about 41(38, 43) and the median ( Q1, Q3) of ADI about 33 (30, 35). The prevalence rate of T2DM was 16.41% (857/5 223). After adjusting for covariates, multivariate logistic regression showed that PDI was negatively associated with T2DM. Compared with the low-level group, the OR (95% CI) for T2DM in the high-level group was 0.83 (0.69-0.99). ADI was positively associated with T2DM, and compared with the low-level group, the OR (95% CI) for T2DM in the high-level group was 1.28 (1.06-1.55). For every one-point increase in PDI and ADI, the risk of T2DM decreased by 2% and increased by 3%, respectively, with the OR (95% CI) of 0.98 (0.96-1.00) and 1.03 (1.01-1.06), respectively. Conclusion:In Chinese older adults ≥65 years in 18 longevity areas, higher adherence to the plant-based behavior may be negatively associated with the risk of T2DM, while higher adherence to the animal-based behavior may be positively associated with the risk of T2DM.
