1.Qualitative research on anxiety status and coping strategies of clinical postgraduates

Objective To explore the anxiety status and coping strategies of clinical postgraduates,and to provide practical approaches to promote their mental health.Methods Using the phenomenological research method of qualitative research,12 subjects were interviewed in a semi-structured way.Using the 7-step analysis method of Co-laizzi phenomenology,the original data were analyzed and summarized.Results Five themes were extracted,in-cluding stress sources and psychological condition,interpersonal relationship,adjustment to cope with stress,self-understanding,career understanding and future vision.Clinical work,scientific research and economic pressures were the main causes of anxiety to the interviewees,who made cognitive,psychological,attitudes and behaviors ad-justments,they hope to achieve a good work-life balance in the future.Conclusions Effective measures should be taken in view point of individual,school,family and social dimensions to alleviate anxiety and promote their physi-cal and mental health.
2.Protective mechanisms of Leontopodium leontopodioides extracts on lipopolysaccharide-induced acute kidney injury viathe NF-κB/NLRP3 pathway.

Sepsis-induced uncontrolled systemic inflammatory response syndrome (SIRS) is a critical cause of multiple organ failure. Acute kidney injury (AKI) is one of the most serious complications associated with an extremely high mortality rate in SIRS, and it lacked simple, safe, and effective treatment strategies. Leontopodium leontopodioides (Willd.) Beauv (LLB) is commonly used in traditional Chinese medicine for the treatment of acute and chronic nephritis. However, it remains unclear whether lipopolysaccharide (LPS) affects LPS-induced AKI. To identify the molecular mechanisms of LLB in LPS-induced HK-2 cells and mice, LLB was prepared by extraction with 70% methanol, while a lipopolysaccharide (LPS)-induced HK-2 cell model and an AKI model were established in this study. Renal histopathology staining was performed to observe the morphology changes. The cell supernatant and kidney tissues were collected for determining the levels of inflammatory factors and protein expression by ELISA, immunofluorescence, and Western blot. The results indicated that LLB significantly reduced the expression of IL-6 and TNF-α in LPS-induced HK-2 cells, as well as the secretion of IL-6, TNF-α, and IL-1β in the supernatant. The same results were observed in LPS-induced AKI serum. Further studies revealed that LLB remarkably improved oxidative stress and apoptosis based on the content of MDA, SOD, and CAT in serum and TUNEL staining results. Notably, LLB significantly reduced the mortality due to LPS infection. Renal histopathology staining results supported these results. Furthermore, immunofluorescence and Western blot results confirmed that LLB significantly reduced the expression of the protein related to the NF-κB signaling pathway and NLRP3, ASC, and Caspase-1 which were significantly increased through LPS stimulation. These findings clearly demonstrated the potential use of LLB in the treatment of AKI and the crucial role of the NF-κB/NLRP3 pathway in the process through which LLB attenuates AKI induced by LPS.

3.Effects of perinatal personalized self-efficacy intervention on breastfeeding six months after delivery in primiparas

Objective To evaluate whether perinatal personalized self-efficacy can improve the rate of breastfeeding six months after delivery in primiparas by applying it in the breastfeeding by primiparas. Methods Totally 76 pregnant women who were admitted by the outpatient service of the Obstetrics Department from January to February 2016 were selected as the object of intervention by convenience sampling. Pregnant women admitted in January 2016 were divided into the control group (n=38) and received conventional nursing, while those admitted in February 2016 were divided into the observation group (n=38) and received perinatal personalized breastfeeding self-efficacy intervention. Then the breastfeeding self-efficacy one week and one month after delivery and the rate of pure breastfeeding one week, one month, three months and six months after delivery were compared between two groups.Results The pregnant women in the observation group showed higher breastfeeding self-efficacy than the pregnant women in the control group (t=25.43,22.78;P<0.05). The rates of pure breastfeeding one week, one month, three months and six months after delivery were 88.57%, 85.71%, 80.00% and 77.14%, respectively, higher than those of the pregnant in the control group, which were 66.67%, 55.56%, 47.22% and 36.11%, respectively (χ2=4.878, 7.749, 8.215, 12.146;P<0.05). Conclusions Perinatal personalized self-efficacy intervention can improve the breastfeeding self-efficacy and the rate of pure breastfeeding six months after delivery.
4.Biocatalytic desymmetric hydrolysis of 3-(4-chlorophenyl)-glutaronitrile to the key precursor of optically pure baclofen.

We produced (S)-4-cyano-3-(4-chlorophenyl)-butyrate by highly stereoselective biocatalyst in this study. A nitrilase-producing strain, named Gibberella intermedia WX12, was isolated by 3-(4-chlorophenyl)-glutaronitrile as substrate in the screening with phenol-sodium hypochlorite method. The fermentation conditions and catalytic properties of this strain were investigated. The preferred carbon and nitrogen sources for nitrilase production were lactose (30 g/L) and peptone (20 g/L). After being cultivated for 96 h, the cells were collected for use in biotransformation. The hydrolysis of 3-(4-chlorophenyl)-glutaronitrile was performed at 30 degrees C in phosphate buffer (pH 8.0, 50 mmol/L) for 24 h to give (S)-4-cyano-3-(4-chlorophenyl)-butyric acid with 90% yield and > 99% of ee, which can be used for the synthesis of (R)- and (S)-baclofen. The configuration of product was determined by chemically converting it to baclofen and comparison with the authentic sample by chiral HPLC analysis.

5.Expression of p16, FHIT genes in cervical carcinoma and its clinical significance

Objective To study the expression of pl6, FHIT genes in cervical carcinoma and its clinical significance. Methods By immunohistochemistry SP method, the expression of pl6, FHIT in different 118 cases of cervical lesions were detected and the results were analyzed in combination with clinical pathological features. Results Of 118 patients, 15 cases suffered cervicitis;38 cases took place cervical tumor-like changes;65 cases caught cervical cancer. p16 expression rates were 0, 33.3 %, 70.0 %, 87.5 %,and 92.3 % respectively;while FHIT expression rates were 73.3 %, 75.5 %, 60.0 %, 37.5%, and 30.8 % respectively. Compared with cervicitis, pl6 and FHIT expression rates in the cervix tumor-like changes,cervical carcinoma had significant difference (P <0.05). There was positive correlation in protein expression between p16 and FHIT (x2 =33.33, P <0.001). Conclusion Combination of p16, FHIT detection can be used as early diagnostic tool of cervical lesions and cervical cancer markers;meanwhile, the method can serve as a clinical evaluation of tumor biological behavior and prognosis of auxiliary indexes.
