Objective:To explore the effect of hyperthermic intraperitoneal chemotherapy (HIPEC) on postoperative immune function and tumor marker levels in patients with advanced gastric adenocarcinoma. Methods:A prospective cohort study was conducted, enrolling 72 patients with stage ⅢA-ⅢC gastric adenocarcinoma admitted to Jinjiang Hospital from August 2022 to December 2023. Patients were divided into the HIPEC group ( n=36, radical resection+ HIPEC within 3 days after surgery) and the control group ( n=36, radical resection alone) using the random number table method. The HIPEC protocol was perfusion with raltitrexed (4 mg in 4, 000 ml normal saline, at 43 ℃) for 60 minutes. Peripheral blood immune cells (CD3 +, CD4 +, CD8 +, CD4 + /CD8 + ratio) before treatment and 7 days after treatment, and serum levels of carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA19-9), and carbohydrate antigen 72-4 (CA72-4) 30 days after treatment were compared between the two groups. Results:Before treatment, there were no statistically significant differences in CD3 +, CD4 +, CD8 +, or CD4 + /CD8 + levels between the two groups (all P>0.05). Seven days after treatment, the HIPEC group had higher levels of CD3 +, CD4 +, and CD4 + /CD8 + ratio, and a lower CD8 + level than the control group, with statistically significant differences (all P<0.05). Before treatment, there were no significant differences in CEA, CA19-9, or CA72-4 levels between the two groups (all P>0.05). Thirty days after treatment, the HIPEC group had lower levels of CEA, CA19-9, and CA72-4 than the control group, with statistically significant differences (all P<0.05). Conclusions:HIPEC can significantly improve the postoperative immune function of patients with advanced gastric adenocarcinoma (by increasing the CD4 + /CD8 + ratio and immune cell activity) and effectively reduce tumor marker levels, which may provide a new strategy for preventing postoperative recurrence.

The construction of the National Medical Center and the National Regional Medical Center in the Chengdu-Chongqing Economic Circle (referred to as the dual medical center construction) faces the challenges of insufficient integration of industry, academia, research, and medicine, as well as limited joint development and sharing of high-quality resources. It is urgent to promote the integration and coordinated development of regional education and technology talent resources through the integration of higher medical education, and to enhance the "four chains" synergy with the integration of industry and education as the core, thereby promoting the dual medical center construction. This study investigated and summarized the key issues in the dual medical center construction and the integration of higher medical education in the Chengdu-Chongqing Economic Circle. Based on the theoretical framework of four chain coordination (industry chain-talent chain-education chain-city chain), this study clarified the mechanisms by which integrated higher medical education can promote dual medical center construction. The study found that the integration of higher medical education can effectively promote the optimization of resources for dual medical center construction by constructing a collaborative mechanism of "industry education integration-joint talent cultivation-coordinated discipline construction-intercity cooperation". Based on the strengths in higher education resources in Chengdu and Chongqing, a five-dimension collaborative path was proposed: integrated development; technological leadership; talent oriented; government guidance; and market assistance. The research conclusion not only enhances the understanding of the integration of higher medical education, but also provides theoretical basis and practical reference for the dual medical center construction in the Chengdu-Chongqing Economic Circle. The proposed coordinated development also has reference value for the construction of medical centers in urban agglomerations such as the Beijing-Tianjin-Hebei Urban Agglomeration and the Yangtze River Delta.

Objective To successfully get the full PCR alleles of the Insertion/Deletion marker rs10644346 in which a SNP-binding exists at the 3'end region of the primer. Methods Based on the AS-PCR,a common forward primer and two reverse primers with allele-specific oligonucleotides at the last second 3'end instead of the terminus were tentatively designed for typing 150 unrelated individuals and 10 father-mother--child trios from Htnan province in South-central China.Simultaneously,9 samples were typed with all the above three primers (the two primer sets which consist of the common forward primer and one of the reverse primers). Results PCR amplicons were well detected in the 150 unrelated individuals after being typed with the three primers, and the amplified fragments of parental and filial generations among 10 father-mother-child trios conformed to Mendel's principles.Allele missing was found in the two-primer group. Conclusion The primers designed by locating the specific nucleotide at the last second 3'end rather than terminal position were demonstrated also effective in getting specific alleles if perfect mismatch and PCR conditions are guaranteed, and the design strategy can provide an optional reference to rescue markers of SNP-binding primers for forensic practice.

Objective To identify the potential risks in the process of nursing care for children with tracheal foreign body removal and improve the skills and the process, to promote the quality of nursing care and the success rate of tracheal foreign body removal. Methods The failure mode and effect analysis (FMEA) were used to analyze the potential risks in children with tracheal foreign body removal, preoperative, intraoperative and postoperative care, and to find out the possible risks in each medical treatment. For pediatric venous puncture technology was not skilled, surgical position placement inappropriate (especially in the case of foreign body removal surgery posture), surgical instruments and equipment management, intraoperative nursing cooperation and observation of the disease, postoperative transfer and other aspects of the transfer of training and improvement. Results The medical risk value (RPN) in the treatment of pediatric tracheal foreign body removal in the process of nursing in the control group was reduced from 1 284.2 to 213.2 of the experimental group. The average value of doctors' satisfaction was increased from 79.33% in the control group to 93.33%in the experimental group, P<0.05, the difference was statistically significant. Conclusions The use of FMEA in the treatment of pediatric tracheal foreign body removal surgery care and medical risk management improve the quality of surgical care and help improve the success rate of tracheal foreign body removal. It also ensures the safety of the children in the operation process and improves the quality of nursing work.
