1.Research on the Construction Path of the Integrated Eye Health Service System in China

Objective:To explore the necessity and basic connotation of building an integrated eye health service system and propose a systematic const ruction pathway.Methods:A multi-dimensional analytical framework for an integrated eye health service system was constructed using literature review,interviews,and the rainbow model theory.Results:Considering the current state of eye disease prevention and control in China,along with the practical need to address the imbalance in the supply and demand structure of ophthalmic resources,it is necessary to build an integrated eye health service system that is fair,accessible,systematic,continuous,high-quality and efficient.Conclusion:It is needed to further improve the top-level design framework,enhance system integration,consolidate the service network at the grassroots level,strengthen organizational coordination,establish specialized eye health prevention and treatment centers,promote professional collaboration,deliver integrated and proactive service models,optimize service integration,and reinforce enabling factors through functional unification and standardized protocols.

3.Exploring the protective mechanism of Jiedu Tongluo Tiaogan decoction on pan-creatic islet cells in SD rats based on autophagy regulation

The aim of this study is to explore the protective mechanism of Jiedu Tongluo Tiaogan decoction(JTTD)on islet cells of type 2 diabetic SD rats based on autophagy regulation.8-week-old SD male mice were fed with high-fat diet for 4 weeks and then induced with a single intraper-itoneal injection of streptozotocin 40 mg/kg.The Chinese medicine group was given the low(1.5 g·kg-1·d-1),medium(4.5 g·kg-1·d-1)and high(13.5 g·kg-1·d-1)doses of JTTD,and the western medicine group was given metformin hydrochloride(0.15 g·kg-1·d-1).Fasting blood glucose(FBG),glycated serum protein(GSP),triglycerides(TG),cholesterol(CHO),high-densi-ty lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),free fat(FFA)were measured.Moreover,the fasting serum insulin was determined by ELISA method and insulin resistance index(HOMA-IR)was calculated,glucose tolerance test was performed.The pancreatic tissues of SD mice were stained with HE,and the expression of Beclin-1 and LC3 were checked by immunohistochemistry,the expression of Beclin-1,LC3 and P62 was determined by Western blot,the expression of Beclin-1 and LC3RNA was determined by RT-PCR.The results showed that both the JTTD group and metformin hydrochloride group had better hypoglycemic and lipid-lowering effects(P<0.05,P<0.01).HE staining of pancreatic tissues showed that disorganized structure,unclear boundary,irregular arrangement,less cytoplasm,and lighter cytoplasmic staining or vacuo-lation in the islets in the JTTD group and metformin hydrochloride group were improved.In terms of autophagy regulation,immunohistochemistry,Western blot and RT-PCR,compared with the model group,the expression of Beclin-1 and LC3 mRNA was significantly increased(P<0.01)and the level of p62 protein was decreased(P<0.05,P<0.01)in the JTTD group and metformin hydrochloride group.In conclusion,the JTTD can alleviate the trend of weight loss and improve ex-cessive drinking effectively in type 2 diabetic mellitus SD mice,as well as have better effects on im-proving glucose and lipid metabolism,moreover,it can alleviate the damage of pancreatic his-topathological structure and protect islet cells,which may be achieved by regulating the level of au-tophagy.
4.Meta-analysis of the effect of information management on safe in-hospital transfer of patients in China

Objective:To conduct a Meta-analysis on the application effect of information management on safe in-hospital transfer of patients in China.Methods:Using computers to search for randomized controlled trials and quasi experimental studies on the effect of information management on safe in-hospital transfer of patients in China from China National Knowledge Infrastructure, VIP, Wanfang Database, China Biomedical Mediline disc, PubMed, Embase, Web of Science, CINAHL and Cochrane Library. The search period was from establishment of databases to May 17, 2022. Literature screening, quality evaluation and data extraction were conducted independently by two trained researchers. Stata 15.1 software was used for Meta-analysis.Results:A total of 14 articles were included, involving a total of 130 670 patients. The results of Meta-analysis showed that the incidence of adverse event in in-hospital transfer of patients in the information management group was shorter than that in the control group ( OR=0.24, 95% CI: 0.17-0.35, P<0.01), duration of in-hospital tranfer was longer than that in the control group ( WMD=-5.76, 95% CI: -8.30-3.22, P<0.01), and patients' satisfaction ( OR=1.11, 95% CI: 1.05-1.17, P<0.01) and satisfaction of medical personnel responsible for transfer ( OR=1.37, 95% CI: 1.13-1.66, P<0.01) were higher than those of the control group. Conclusions:Information management can effectively control the incidence of adverse events in in-hospital transfer of patients in China, shorten the time required for hospital transfers and improve the satisfaction of patients and medical staff in hospital transfers.
5.Effect of Suanzaoren Tang Combined with Ziwu Liuzhu Acupuncture on Vertebral Artery Hemodynamics, Inflammatory Cytokines, and Neurotrophic Factors in Patients with Cervical Insomnia

ObjectiveTo explore the effect of Suanzaoren Tang combined with Ziwu Liuzhu acupuncture on the vertebral artery hemodynamics, inflammatory cytokines, and neurotrophic factors in the patients with cervical insomnia with syndrome of deficiency of both heart and spleen. MethodThe random number table method was employed to assign 164 patients with cervical insomnia with syndrome of deficiency of both heart and spleen treated in the First Clinical Medical School of Guangzhou University of Chinese Medicine from January 2018 to June 2021 into a control group and an observation group. The control group was orally administrated with 1-2 mg estazolam tablets before bed for 4 weeks, and the observation group with Suanzaoren Tang combined with Ziwu Liuzhu acupuncture for 4 weeks. The therapeutic efficacy and safety were observed. The Pittsburgh Sleep Quality Index (PSQI) score, polysomnography monitoring results, hemodynamics parameters of vertebral artery, and serum levels of inflammatory cytokines and neurotrophic factors were compared before and after treatment. ResultExcept 4 dropouts, the remaining 160 patients were included in this study, with 80 patients in each group. The observation group had higher total effective rate than the control group [92.50% (74/80) vs. 80.00% (64/80), χ2=5.270, P<0.05]. Compared with that before treatment, the therapies in both groups decreased the PSQI score, sleep latency time, awakening time, awakening times, serum levels of interleukin-1β (IL-1β), C-reactive protein (CRP), and tumor necrosis factor-α (TNF-α) (P<0.01). Meanwhile, they increased the proportion of rapid-eye-movement (REM) sleep, the diastolic blood flow velocity (Vd), systolic blood flow velocity (Vs), and mean blood flow velocity (MFV) of vertebral artery, as well as the serum levels of brain-derived neurotrophic factor (BDNF) and glial cell-derived neurotrophic factor (GDNF) (P<0.05, P<0.01). Moreover, the observation group had lower PSQI score, sleep latency time, awakening time, awakening times, and serum IL-1β, CRP, and TNF-α levels (P<0.01) and higher proportion of REM sleep, Vd, Vs, MFV of vertebral artery, and serum BDNF and GDNF levels (P<0.05, P<0.01) than the control group. ConclusionZiwu Liuzhu acupuncture combined with Suanzaoren Tang can improve blood circulation of vertebral artery, reduce the serum levels of inflammatory cytokine, and increase the serum levels of neurotrophic factors to improve the sleep quality of the patients with cervical insomnia with syndrome of deficiency of both heart and spleen.
6.Ultrasonographic phenotype and genetic analysis of fetuses with 17q12 microdeletion.

OBJECTIVE:
To analyze the ultrasonographic phenotype and result of genetic testing in six fetuses carrying a 17q12 microdeletion.
METHODS:
Chromosomal microarray analysis (CMA) was carried out for 6200 pregnant women undergoing prenatal diagnosis from December 2016 to May 2021.
RESULTS:
CMA has identified 6 fetuses with a microdeletion in the 17q12 region, which spanned approximately 1.4 Mb and encompassed at least 13 OMIM genes. All fetuses have shown bilateral renal parenchymal echo enhancement. Four fetuses also had other ultrasonographic phenotypes. The parents of 4 fetuses had refused parental verification, whilst the remaining two fetuses were confirmed to be de novo in origin.
CONCLUSION
The prenatal ultrasonographic phenotype of 17q12 microdeletion is mainly enhanced bilateral renal parenchymal echos. CMA can facilitate detection of the 17q12 microdeletion.

7.Clinical application value of human papillomavirus typing combined with cervical thin-prep cytology test in cervical cancer screening

Objective To explore the clinical value of the third-generation hybrid capture nucleic acid detection technology (DH3) typing detection of human papillomavirus (HPV) combined with thin-prep cytology test (TCT) in screening cervical cancer. Methods A total of1 582 female patients who received HPV and TCT cervical screening in Liuzhou Workers Hospital, Fourth Affiliated Hospital of Guangxi Medical University, from October 2020 to March 2021, were selected for this retrospective analysis. The cervical histopathological diagnosis was used as the gold standard to evaluate the diagnosis accuracies of HPV and TCT test results. Results Among the 1 582 patients, 334 were positive for HPV and 1 248 were negative; 234 were positive for TCT and 1 348 were negative; 180 were positive for histopathological diagnostic and 1 402 were negative. The sensitivity of HPV detection was 100%, the positive predictive value was 53.9%, the specificity was 89.0%, and the negative predictive value was 100.0%. In the HPV typing test, the positive rate for high-risk types 16/18 was 94.8%, and the positive rate for other 12 high-risk types was 73.8%. There was a little difference between TCT test and pathological test, and the detection consistency rate of high-grade squamous intraepithelial lesions and squamous carcinoma was same. Conclusion HPV testing combined with TCT to screen cervical cancer can reduce the misdiagnosis rate by a single test. The classification test is helpful for the hierarchical management of patients, and has a high clinical value for examination triage and grade screening.
8.Prenatal ultrasonographic manifestations and genetic analysis of eight fetuses with 16p11.2 microdeletions.

OBJECTIVE:
To analyze the intrauterine phenotype and genotype of eight fetuses carrying a 16p11.2 microdeletion.
METHODS:
5100 fetuses undergoing routine prenatal diagnosis were subjected to single nucleotide polymorphism-based microarray (SNP-array) analysis. Fetuses harboring a 16p11.2 microdeletion were analyzed for their ultrasonographic characteristics.
RESULTS:
Eight fetuses were found to harbor a microdeletion in the 16p11.2 region. Among these, six had a typical 500-600 kb deletion, while the remaining two had an atypical 220 kb deletion at the distal part of 16p11.2. Four fetuses showed vertebral malformations, two had mild left ventriculomegaly, one had hydrocephalus, and one had pulmonary valve stenosis with regurgitation. The parents of five fetuses have accepted pedigree verification, and the results confirmed that the 16p11.2 microdeletions carried by fetuses all had a de novo origin.
CONCLUSION
The intrauterine phenotypes of fetuses carrying a 16p11.2 microdeletion may be variable, and the deletion can be effectively detected with the SNP-array assay.

9.Prenatal ultrasonic characteristics and genetic analysis of fetuses with chromosome 22q11 microdeletion syndrome.

OBJECTIVE:
To analyze the prenatal ultrasonic characteristics and genetic features of 14 fetuses with chromosome 22q11 microdeletion syndrome (22q11DS).
METHODS:
4989 fetuses were analyzed by using single nucleotide polymorphism array (SNP array) in the Fujian Maternal and Child Health Hospital from November 2016 to November 2019.
RESULTS:
SNP array showed that 11 fetuses had classic 3 Mb microdeletion in 22q11 region, one fetus had 2.0 Mb microdeletion, and two fetuses had 1.0 Mb microdeletion. The 1.0 Mb microdeletion in 22q11 region contains SNAP29 and CRKL genes, which may increase the risk of congenital renal malformation and cardiovascular malformation.
CONCLUSION
Prenatal ultrasonic characteristics of fetuses with 22q11 microdeletion syndrome vary, and SNP array is a powerful tool to diagnose such diseases, which can provide accurate genetic diagnosis and enable prenatal diagnosis.

10.Characteristics and trends of induced abortions: a retrospective study in Xiamen from 2014 to 2018

Objective:To investigate the characteristics and trends of women who underwent induced abortion in recent years, and to provide more evidences to decrease induced abortion rate.Methods:A retrospective study was conducted on women who underwent induced abortions in the Family Planning Department of Women and Children’s Hospital of Xiamen University from January 2014 to December 2018.Results:Of the 28 529 women with induced abortion, the age was (28.77±5.78) years old. Women aged 25-34 years accounted for 58.1%, 67.0% of women were married, 62.5% had a history of delivery, 24.0% were in their first pregnancies, 30.3% had induced abortion for twice, 21.5% were for three times and more (the maximum frequency of induced abortion was 14). About 16.1% induced abortion women had a history of cesarean section,and rate showed an increasing trend in the five years (16.2%, 15.0%, 15.9%, 16.6%, 16.9%), but the difference was not statistically significant ( P=0.054). From 2014 to 2018, the age of induced abortion women [(28.05±5.69) years, (28.47±5.62) years, (28.70±5.80) years, (29.03±5.81) years, (29.59±5.82) years, P<0.001] and the percentages of married women (63.2%, 66.8%, 66.9%, 67.2%, 70.9%), women with two or more children (11.8%, 15.4%, 20.2%, 25.2%, 32.2%) and women undergoing at least a second abortion over 35 years old (21.7%, 22.1%, 23.4%, 26.7%, 29.9%) increased every year (all P<0.001). However, among the induced abortion women, the percentages of unmarried women (36.8%, 33.2%, 33.1%, 32.8%, 29.1%), women who haven’t bear children (42.9%, 38.4%, 37.8%, 36.3%, 32.4%), women with the first pregnancy (26.2%, 23.8%, 24.7%, 23.4%, 21.8%) and women under 24 years old (29.3%, 25.3%, 24.7%, 24.1%, 21.5%) decreased during the five years period (all P<0.001). Conclusion:The incidence rate of induced abortion among married women with children is the highest, so special attention should be paid to postpartum women in family planning services, and it is urgent to launch postpartum contraceptive services and promote long-term reversible contraceptive measures.
