1.New progress of refractive enhancements for residual refractive error after cataract surgery

Cataract surgery is one of the most common ophthalmologic procedures. Advances in technology and medical policies have made it more precise. Residual refractive errors and deviation of target diopters are a main cause of dissatisfaction among patients. Refractive enhancement after cataract surgery can correct or eliminate these errors, improving patients' visual quality of life. There are multiple options for correcting residual refractive errors. The best approach depends on factors like the cause of the error, degrees of residual refractive errors, type of intraocular lens, ocular comorbidities, and patient preference. This paper summarizes the incidence and types of residual refractive errors, advancements in refractive enhancement surgeries, and provides practical solutions for clinical practice.
2.Management strategies of scientific research projects based on SWOT analysis

SWOT analysis is used to identify the strengths and external opportunities of scientific research management in hospitals.It facilitates the establishment of a systematic and rational approach to scientific research project management,help-ing hospitals to mitigate internal weaknesses and address external threats.This article chooses Y Hospital to carry out a case stud-y.SWOT analysis was done to investigate the hospital's strengths,weaknesses,opportunities,and threats.Based on the analysis results,it proposes targeted management strategies of"SO,""WO,""ST,"and"WT".After the use of the strategies in the management,the number of funded scientific research projects,published papers,registered invention patents,and achievements transferred from the scientific research remarkably increases,driving the improvement of research quality.
3.Right ventricle modified myocardial performance index for evaluating maternal obstetric antiphospholipid syndrome involving fetal right ventricular function

Objective To observe the value of right ventricle modified myocardial performance index(RV-Mod-MPI)for evaluating maternal obstetric antiphospholipid syndrome(OAPS)involving fetal right ventricular function.Methods Forty-five pregnant women with maternal OAPS(OAPS group)and 60 healthy pregnant women(control group)were prospectively enrolled.Fetal RV-Mod-MPI was obtained with tricuspid and pulmonary valve flow images by applying pulsed wave Doppler(PW).Late pregnancy conditions and data of newborns after delivery were recorded.The indexes were compared between groups.Receiver operating characteristic curve was drawn,and the area under the curve(AUC)was used to assess the efficacy of fetal RV-Mod-MPI for predicting adverse pregnancy outcomes in OAPS group.Results Compared with those in control group,OAPS group had higher fetal RV-Mod-MPI values,lower newborn birth weight and lower Apgar score at 1 min after birth,as well as higher probability of adverse pregnancy outcomes(all P<0.05).The AUC of fetal RV-Mod-MPI for predicting adverse pregnancy outcome in OAPS group was 0.726.Conclusion RV-Mod-MPI could be used to evaluate maternal OAPS involving fetal right ventricular function and predict adverse pregnancy outcomes.
4.Current status and influencing factors of beliefs in taking antituberculosis drugs among young patients with newly treated pulmonary tuberculosis

Objective:To investigate the level of beliefs about medication in young patients with newly treated pulmonary tuberculosis and analyze its influencing factors.Methods:This was a cross-sectional study. Using the convenient sampling method, a total of 320 young newly treated pulmonary tuberculosis patients who visited the designated tuberculosis hospitals in Kashgar Prefecture and Hotan Prefecture of Xinjiang Uygur Autonomous Region from January 2022 to April 2023 were selected as the research objects. The investigation was carried out with the General Information Questionnaire, Beliefs about Medicines Questionaire Specific (BMQ-S), 8-Item Morisky Medication Adherence Scale (MMAS-8), Brief Illness Perception Questionnaire (BIPQ) and Tuberculosis-related Stigma Scale (TSS). Multiple linear regression was used to analyze the influencing factors of medication beliefs in young patients with newly treated tuberculosis.Results:A total of 320 questionnaires were distributed in this study, and 302 valid questionnaires were collected, with an effective response rate of 94.38% (302/320). The total score of BMQ-S of 302 young patients with newly treated tuberculosis was -1.00 (-2.00, 1.00), MMAS-8 score was 5.38 (4.75, 5.75), BIPQ score was 37.00 (24.00, 44.00) and TSS score was 48.00 (44.00, 52.00). The results of multiple linear regression analysis showed that comorbidities, medication adherence, disease perception and stigma were the influencing factors of medication beliefs in young newly treated pulmonary tuberculosis patients ( P<0.01) . Conclusions:The medication belief level of young patients with newly treated pulmonary tuberculosis needs to be improved. Medical staff should correct the negative cognition of the patient's disease, emphasize the benefits of drug treatment and enhance the patients' beliefs in the necessity of medication.
5.Summary of the best evidence for cosmetic beauty in breast cancer patients with self-image disorder

Objective:To retrieve, evaluate and integrate evidence related to cosmetic beauty in breast cancer patients with self-image disorder, so as to provide a basis for clinical practice.Methods:According to the evidence-based resource "6S" model, the guideline websites and Chinese and English databases were systematically searched from top to bottom for clinical decisions, guidelines, systematic evaluation, expert consensus and original researches on cosmetic beauty in breast cancer patients with self-image disorder. The search period was from establishment of the databases to November 2023. The included literature was evaluated by two researchers and evidence was extracted from the literature that met the quality criteria.Results:A total of nine articles were included, including one guideline, two systematic reviews, three expert consensus and three original studies. A total of 24 best pieces of evidence were formed from eight aspects, including team building and educational guidance, body image assessment and referral, basic skin care and precautions, daily makeup, hair loss care, clothing matching, prosthetic breast selection and health education.Conclusions:The best evidence summarized in this study is comprehensive and can be used by clinical medical staff in practice, but the evidence should be personalized according to patients' wishes and clinical practical problems to improve the self-image of breast cancer patients.
6.Epidemiological analysis of in patients with sepsis in a large tertiary general hospital in Southwest China

Objective:To analyze the epidemiological characteristics of hospitalized patients diagnosed with sepsis in a large class Ⅲ general hospital in Southwest China in a period of 2 years, and to explore the risk factors related to death in patients with sepsis.Methods:A retrospective study was conducted to select patients with sepsis admitted to Sichuan Provincial People's Hospital from September 1, 2021 to August 31, 2023, and general characteristics such as gender, age, discharge diagnosis, discharge department, hospitalization cost, length of stay, and prognosis during hospitalization were collected. The baseline of two groups of patients were compared, and the risk factors of in-hospital cause of death in patients with sepsis were analyzed by multivariate Logistic regression.Results:A total of 3 568 patients with sepsis were included with median age of 58 (35, 74) years old. Of all patients, there were 2 147 males (60.17%). The median length of hospitalization was 13 (8, 24) days, and the median hospitalization cost was 3.98 (1.87, 8.83) ten thousand yuan. The departments with more than 100 cases of sepsis in 2 years were central intensive care unit (ICU), pediatrics department, nephrology department, emergency medicine department, emergency intensive care unit (EICU), infectious department, respiratory medicine department, hematology department, neonatal care unit and emergency surgical department. A total of 1 210 patients (33.91%) admitted to ICU (including central ICU and EICU). The hospitalization cost of ICU patients were higher [6.7 (3.1, 15.5) ten thousand yuan], the hospitalization duration was longer [9 (3, 17) days], and the mortality was higher [35.29% (427/1 210)]. Among 3 568 patients with sepsis, 448 died and 3 120 survived during hospitalization. The age, male proportion and hospitalization cost of patients with sepsis in the death group were significantly higher than those in the survival group [age (years old): 75 (60, 86) vs. 57 (30, 71), male proportion: 67.86% (304/448) vs. 59.07% (1 843/3 120), hospitalization cost (ten thousand yuan): 6.7 (3.0, 16.9) vs. 3.7 (1.8, 8.1)], the ratio of diabetes mellitus was significantly lower than that of survival group [4.91% (22/448) vs. 10.45% (326/3 120)], the length of hospitalization was shorter than that of survival group [days: 10.0 (3.0, 19.0) vs. 13.0 (8.0, 24.0)], the differences were statistically significant (all P < 0.01). Multivariate Logistic regression analysis showed that male [odds ratio ( OR) = 0.75, 95% confidence interval (95% CI) was 0.59-0.96], elder ( OR = 1.04, 95% CI was 1.03-1.05) and diabetes ( OR = 0.32, 95% CI was 0.19-0.54) were independent risk factors for in-hospital death in patients with sepsis (all P < 0.05). Conclusions:Sepsis is a heavy burden in Southwest China, especially for ICU, with high mortality, high hospitalization costs, and heavy economic burden on patients and society. Male, elder and diabetes were independent risk factors for in-hospital death of sepsis patients.
7.Current status and progress of quality and effectiveness optimization technology for refractive cataract surgery

Currently, cataract surgery has entered into the refractive cataract surgery era, and accurate control of the target refractive power and the rapid and effective improvement of patient's visual quality have become the common pursuit of cataract surgeons. Application of femtosecond laser-assisted cataract surgery, surgical navigation technology, active control fluidic phacoemulsification technology, special high-quality intraocular irrigation fluid, non-drop surgery mode, ocular surface repair and dry eye management, functional intraocular lens, and postoperative visual function training can further improve the patient's diagnosis and treatment experience, accelerate patient's recovery, improve postoperative visual quality, and enhance patient's satisfaction, thus promoting the development of refractive cataract surgery.
8.Status quo and influencing factors of medication compliance in young newly diagnosed pulmonary tuberculosis patients

Objective:To investigate the medication adherence and its influencing factors in young newly diagnosed pulmonary tuberculosis patients during different treatment periods, in order to provide a theoretical basis for formulating precise medication management strategies and thus improving the treatment success rate.Methods:A cross-sectional survey was conducted using a stratified random sampling method to select 283 young newly diagnosed pulmonary tuberculosis patients who visited and registered in the Kashgar region of Xinjiang from September 2021 to February 2022. The patients were divided into three groups according to the treatment time of receiving standard chemotherapy regimen: A (1-2 months of medication), B (3-4 months of medication), and C (5-6 months of medication), with 77, 89, and 117 cases, respectively. The clinical data of tuberculosis patients were collected by using the general information questionnaire,Eight-Item Morisky Medication Adherence Scale and Tuberculosis Medication Adherence Scale for tuberculosis patients.Results:Those who take medication well of three groups of young newly treated pulmonary tuberculosis patients were 93.5% (72/77), 89.9% (80/89), and 82.1% (96/117), respectively. The difference among the three groups was statistically significant ( χ2=6.23, P<0.05). Logistic regression analysis showed that social support was an influential factor for the 1st to 2nd month of medication ( OR=0.536, P<0.05); treatment confidence and psychological status were influential factors for the 3rd to 4th month of medication ( OR=0.668, 2.212, both P<0.05); comorbidity, social support, psychological status, and coping style were influential factors for the 5th to 6th month of medication ( OR values were 0.428 - 9.518, all P<0.05). Conclusions:The relevant factors that affect medication adherence vary among young newly diagnosed pulmonary tuberculosis patients at different stages of treatment. Accurate medication management strategies should be developed based on the influencing factors at each stage.
9.Diagnosis, treatment and prognosis of FH-deficient renal cell carcinoma with tumor thrombus

Objective:To Explore the diagnosis, treatment and prognosis of FH-deficient renal cell carcinoma (FH-deficient RCC) with tumor thrombus, and share surgical experience.Methods:From August 2019 to October 2022, 6 cases of FH-deficient RCC with tumor thrombus were diagnosed and treated in our center, including 4 males and 2 females. The patients were aged 22 to 57 years, with 2 cases younger than 40 years, icluding 5 cases on the left and 1 case on the right. The median maximum diameter of the tumor is 8 (4.8, 14.0) cm. Operations were performed after complete examination (enhanced CT and other related examinations). One case underwent open surgery and palliative resection of the left kidney was performed because of severe adhesion of the inferior vena cava. Among the remaining 5 cases, 1 case underwent retroperitoneal laparoscopic right radical nephrectomy with inferior vena cava thrombectomy, 1 case underwent transabdominal laparoscopic left radical nephrectomy with inferior vena cava thrombectomy, and 3 cases underwent robot assisted laparoscopic left radical nephrectomy with inferior vena cava thrombectomy.Results:The median surgical time was 293 (185, 366) min, with blockage of the vena cava for 13 min and 28 min in 2 of 6 cases, respectively. The pathological report of renal tumor and tumor thrombus was FH-deficient renal carcinoma. The pathological features were as follows: the gross section of the specimen was gray yellow solid, often accompanied by necrosis, and the cystic cavity could be seen locally. Microscopically, the tumor extensively involved the renal parenchyma, with papillary, cribriform and tubular cystic structures. Immunohistochemistry showed FH (-), 2SC (+ ). The median postoperative hospital stay was 8 (4, 15) days. The median follow-up time was 13 (4, 27) months. One patient undergoing palliative resection of the left kidney underwent targeted therapy and radiotherapy after surgery (died 15 months after surgery due to gastrointestinal perforation). During the follow-up process, 4 cases experienced metastasis and received systematic treatment, with 1 death 27 months after surgery. Uterine leiomyomas were found in the remaining 1 case during follow-up.Conclusions:FH-deficient RCC with tumor thrombus is very rare. This disease is highly invasive, difficult to be diagnosed preoperatively and poor clinically prognostic. Operation combined with systemic therapy is an effective way to treat FH-deficient RCC with tumor thrombus.
10.Prenatal echocardiographic features and outcomes of congenital ventricular outpouching in ten fetuses

Objective:To summarize the echocardiographic features and outcomes in fetuses with congenital ventricular outpouching (CVO).Methods:This retrospective study enrolled ten fetuses diagnosed with CVO by fetal echocardiography in the Affiliated Hospital of Qingdao University and Qingdao Women and Children's Hospital from January 2015 to April 2022. Clinical data were analyzed, including echocardiographic features, other intracardiac and extracardiac malformations, karyotypes, and pregnancy outcomes. Data were analyzed by descriptive statistics.Results:All ten cases were single, including eight ventricular diverticula and two ventricular aneurysms. Five cases had the anomaly in the left ventricular and the other five in the right. Five cases were isolated malformations, and the other five were complicated by other intra- or extracardiac malformations. A pathogenic copy number variation was detected in one case. Three pregnancies were terminated, and one was lost to follow-up. The other six fetuses were born alive and showed no obvious clinical symptoms or abnormalities in growth and development during 3-70 months of follow-up. The right ventricular diverticulum spontaneously disappeared in one case. One case with the right ventricular aneurysm was also diagnosed with noncompaction of the left ventricular myocardium by echocardiography at six months.Conclusions:Fetal CVO presents with typical echocardiographic features and can be diagnosed prenatally. Regular follow-up during pregnancy is recommended to observe the sizes of outpouchings and the occurrence of complications in fetuses with CVO after excluding other structural and chromosomal abnormalities to avoid unnecessary termination. Attention should also be paid to postnatal follow-up.
