1.Clinical characteristics of congenital atresia of the oval window

Objective:To investigate the clinical features of patients with congenital atresia of the oval window (CAOW).Methods:A retrospective analysis was conducted on 7 cases (8 ears) of surgically confirmed CAOW treated at our department from July 2018 to July 2024. Among the cases, 1 patient had bilateral CAOW, and 4 patients had unilateral CAOW combined with other types of ossicular chain malformations in the contralateral ear. We collected and analyzed the clinical data, audiological features, and temporal bone HRCT results of all patients.Results:The 7 patients were diagnosed at ages ranging from 8 to 19 years, with a mean age of (13.2±6.9) years. None of the patients exhibited significant auricular deformities. All presented with conductive hearing loss or mixed hearing loss predominantly of the conductive type, with an intact tympanic membrane. The diagnosis of CAOW was confirmed via endoscopic tympanotomy, revealing a concave oval window area on the medial wall of the tympanic cavity, sealed by a bony plate. All 8 ears exhibited additional ossicular chain deformities. Stapes absence was present in all 8 ears. Partial absence of the incus long process was observed in 3 ears, while, abnormal bony connections between the incus long process and the promontory were seen in 4 ears, 1 ear had a short malleolar handle, 1 ear had a smaller than normal malleus volume. In addition, facial nerve deformities were found in 6 ears, with 4 ears showing bifurcation of the facial nerve and 2 ears showing facial nerve obscuration of the oval window. Pure-tone audiometry revealed that 62.5% (5/8 ears) of patients had air conduction (AC) thresholds≥60 dB preoperatively, with a mean pure-tone average (PTA) of (69.0±11.8) dB HL and a mean air-bone gap (ABG) of (52.0±7.0) dB. The mean AC threshold and ABG were higher in the low-frequency (125-1 000 Hz) range compared to the high-frequency (2 000-8 000 Hz) range (both P<0.05). Preoperative HRCT showed abnormalities in all patients, with 7 ears being diagnosable as CAOW. Although the remaining 1 ear could not be diagnosed as CAOW, stapes and incus long process absence were detected. Conclusion:CAOW is rare in clinical, as the patients with non-progressive conductive hearing loss (AC≥60 dB, ABG≥50 dB) since childhood, intact tympanic membrane without malformations of auricle and external auditory canal, and thick bony plate covered the oval window of the HRCT imaging, CAOW should be highly suspected, which could be confirmed by the exploratory tympanotomy.

SWOT analysis is used to identify the strengths and external opportunities of scientific research management in hospitals.It facilitates the establishment of a systematic and rational approach to scientific research project management,help-ing hospitals to mitigate internal weaknesses and address external threats.This article chooses Y Hospital to carry out a case stud-y.SWOT analysis was done to investigate the hospital's strengths,weaknesses,opportunities,and threats.Based on the analysis results,it proposes targeted management strategies of"SO,""WO,""ST,"and"WT".After the use of the strategies in the management,the number of funded scientific research projects,published papers,registered invention patents,and achievements transferred from the scientific research remarkably increases,driving the improvement of research quality.
4.Summary of the best evidence for cosmetic beauty in breast cancer patients with self-image disorder

Objective:To retrieve, evaluate and integrate evidence related to cosmetic beauty in breast cancer patients with self-image disorder, so as to provide a basis for clinical practice.Methods:According to the evidence-based resource "6S" model, the guideline websites and Chinese and English databases were systematically searched from top to bottom for clinical decisions, guidelines, systematic evaluation, expert consensus and original researches on cosmetic beauty in breast cancer patients with self-image disorder. The search period was from establishment of the databases to November 2023. The included literature was evaluated by two researchers and evidence was extracted from the literature that met the quality criteria.Results:A total of nine articles were included, including one guideline, two systematic reviews, three expert consensus and three original studies. A total of 24 best pieces of evidence were formed from eight aspects, including team building and educational guidance, body image assessment and referral, basic skin care and precautions, daily makeup, hair loss care, clothing matching, prosthetic breast selection and health education.Conclusions:The best evidence summarized in this study is comprehensive and can be used by clinical medical staff in practice, but the evidence should be personalized according to patients' wishes and clinical practical problems to improve the self-image of breast cancer patients.
5.Association of related knowledge level and compliance of young and middle-aged hypertensive patients

Objective To evaluate the relevant knowledge level and compliance of young and middle-aged hypertensive patients,and explore the association of them. Method A total of 276 young and middle-aged hypertensive patients were surveyed using a hypertension knowledge level scale and treatment compliance questionnaire. Results The average score of young and middle-aged hypertensive patients on relevant knowledge was (62.55 ± 17.79). From different dimensions, the hypertensive patients got the highest score in the knowledge of their lifestyle,while they got the lowest score in their knowledge for complication.The average score of young and middle-aged hypertensive patients on treatment compliance was (71.53±9.87).From different dimensions,the hypertensive patients got the highest score in medication adherence,while they got the lowest score in exercise and pressure-relieving compliance.It showed that medication and drug compliance knowledge of middle-aged hypertensive patients was closely associated with treatment compliance(r=0.648, P<0.001)). Conclusions Both knowledge level and treatment compliance of young and middle-aged hypertensive patients should be improved.We should strengthen the population of knowledge about hypertension among young and middle-aged patients,especially the guidance of drug knowledge to improve their compliance.
6. Clinical value of anti-liver/kidney microsomal-1 antibody in patients with liver disease

Objective:
To investigate the clinical and laboratory features of patients with liver disease and positive anti-liver/kidney microsomal-1 (anti-LKM-1) antibody, and to provide a reference for clinical diagnosis and differential diagnosis.
Methods:
The clinical data of patients with positive anti-LKM-1 antibody who were treated in our hospital from 2006 to 2016 were collected, and clinical and laboratory features were analyzed and compared. An analysis was also performed for special cases.
Results:
The measurement of related autoantibodies was performed for about 100 thousand case-times, and 15 patients were found to have positive anti-LKM-1 antibody. Among the 15 patients, 7 were diagnosed with type 2 autoimmune hepatitis (AIH) with an age of 11.0 ± 9.0 years and were all adolescents with acute onset; 8 were diagnosed with hepatitis C with an age of 51.5 ± 9.0 years, among whom 7 were middle-aged patients and 1 was a child aged 12 years, and all of them had an insidious onset. 7.Analysis on risk factors of thyroid nodule in Harbin City, Heilongjiang Province

Objective To analyze the risk factors of thyroid nodule (TN) in Harbin City,Heilongjiang Province.Methods From November 2014 to January 2015,residents lived in Nangang,Daoli,Daowai districts and villagers of Xubao Town of Hulan District of Harbin City were selected as research subjects,who lived in the local regions for more than 5 years,a total of 2 650 people were surveyed.Influence factors of TN were surveyed by general survey,clinical examination and thyroid ultrasound examination.Logistic regression analysis was used to analyze relationships between various factors and TN in single factor analysis and multiple factors analysis.Results Excluding 36 cases with thyroid surgery history,the total prevalence of TN was 22.92% (599/2 614),single factor Logistic regression analysis found that females [odds ratio (OR) =1.597,95% confidence interval (CI) =1.328-1.919],elderly (OR =3.356,95%CI =2.699-4.173),living in cities and towns (OR =1.677,95%CI =1.391-2.021),obesity (OR =1.245,95%CI =1.036-1.497),history of hypertension (OR =1.766,95%CI =1.414-2.205) were the risk factors of TN.In multiple factors Logistic regression analysis,risk factors from high to low in the order were elderly (OR =3.076,95%CI =2.436-3.884),females (OR =1.819,95%CI =1.493-2.215),living in cities and towns (OR =1.701,95%CI =1.400-2.066),history of diabetes (OR =1.644,95%CI =1.235-2.189),obesity (OR =1.325,95%CI =1.085-1.618).Conclusions The risk factors of TN include living in cities and towns,females,elderly,obesity,history of hypertension or diabetes in Harbin City of Heilongjiang Province.
