1.Predicting Hepatic Fibrosis Risk in Wilson's Disease: Development and Validation of Prognostic Model Integrating TCM Syndromes and Modern Biomarkers

ObjectiveTo analyze the influencing factors of hepatic fibrosis in patients with Wilson's disease (WD) and to develop and validate a predictive model for hepatic fibrosis in WD that integrates traditional Chinese medicine (TCM) syndromes and modern biomarkers, thus providing a basis for early identification of high-risk populations and medical intervention. MethodsThe clinical data of 220 WD patients diagnosed in the Department of Encephalopathy, The First Affiliated Hospital of Anhui University of Chinese Medicine between January 2010 and December 2024 were retrospectively collected. Through a random number table, patients were assigned into a training set (154 patients) and a validation set (66 patients) in a 7∶3 ratio. Univariate COX regression, LASSO regression analysis, and multivariate COX regression analysis were performed to identify independent influencing factors for hepatic fibrosis in WD patients, and a clinical prediction model was established based on these factors. The discrimination, calibration, and clinical utility of the model were evaluated based on the area under the receiver operating characteristic curve (AUC), calibration curves, and decision curve analysis (DCA), respectively. ResultsA total of 220 patients were included. The syndrome of combined phlegm and stasis, triglycerides (TG), laminin (LN), and male gender were identified as independent risk factors for hepatic fibrosis in WD patients, while TCM intervention and high-density lipoprotein cholesterol (HDL-C) were protective factors. The C-index indicated excellent discriminative ability (training set: 3-year AUC=0.908, 5-year AUC=0.869, 7-year AUC=0.829; validation set: 3-year AUC= 0.898, 5-year AUC=0.780, 7-year AUC=0.743). Calibration curves showed good consistency. DCA demonstrated the clinical net benefit probabilities across different risk thresholds at various time points. ConclusionThe predictive model established in this study has high accuracy and can be conveniently used for the early identification and risk prediction of hepatic fibrosis in patients with WD.
2.Bioinformatics Analysis and Validation of Cuproptosis-related Genes in Wilson Disease

ObjectiveTo explore the role of cuproptosis and identify cuproptosis-related genes in Wilson disease (WD) through bioinformatics analysis and clinical validation,providing implications and directions for the diagnosis and treatment of WD. Methods(1) Screening of target genes: The differentially expressed genes (DEGs) between WD and healthy control were obtained from GeneCards,and the cuproptosis-related genes were obtained from Gene Expression Omnibus (GEO) and published literature.The cuproptosis-related genes in WD were obtained by intersection.Through gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses,the specific biological process,functions or metabolic pathways of cuproptosis-related genes in WD were predicted.Molecular docking and PyMOL visualization were then performed to analyze and verify the potential regulatory mechanism of Gandou Fumu Decoction for cuproptosis.(2)Validation of target genes: The blood samples of 15 WD patients treated in the department of encephalopathy and 15 healthy volunteers undergoing physical examinations in the health management center were randomly collected from the First Affiliated Hospital of Anhui University of Chinese Medicine.The expression levels of target genes were determined by Western blot and real-time PCR. Results(1) A total of 3 607 DEGs in WD were obtained from GSE107323 in GEO,and 68 cuproptosis-related genes were obtained from GeneCards and published literature.Twelve common target genes were obtained by intersection,including three up-related genes(SQSTM1,MIF1,and TAX1BP1) and nine down-regulated genes(CP,SERPINE1,AOC3,GPX4,SLC27A5,VEGF-A,PDHB,PDK1,and ATP7B).The common target genes were mainly enriched in monocarboxylic acid metabolism,oxidoreductase activity,negative regulation of molecular functions,which mainly involved HIF-1,ferroptosis and other signaling pathways.Molecular docking and PyMOL visualization results showed Gandou Fumu Decoction had good binding ability with the cuproptosis-related genes PDK1,SERPINE1,VEGFA,and AOC3 in WD.(2)A total of 30 blood samples were collected,including 15 WD patients and 15 health volunteers.Western blot results showed that expression levels of target genes were consistent with the results obtained by bioinformatics analysis.RT-qPCR results showed that compared with healthy volunteers,WD patients had down-regulated mRNA levels of SERPINE1,GPX4,SLC27A5,and VEGF-A and up-regulated mRNA levels of SQSTM1 and MIF1(P<0.05). ConclusionThe expression levels of cuproptosis-related genes in WD patients are consistent with the results predicted by bioinformatics analysis.The characteristic preparation Gandou Fumu Decoction of Xin'an Medicine showed good binding abilities with the cuproptosis-related genes in WD.Cuproptosis may play a key role in the pathophysiological mechanism of WD,which can provide a new target for the diagnosis and treatment of WD.
3.Clinical and imaging features of invasive Klebsiella pneumoniae liver abscess syndrome

Objective To observe the clinical and imaging features of invasive Klebsiella pneumoniae liver abscess syndrome(IKPLAS).Methods Data of 68 patients with Klebsiella pneumoniae liver abscess(KPLA)were retrospectively analyzed.The patients were divided into IKPLAS group(n=25)or non-IKPLAS group(n=43)according to extrahepatic invasive infection or not.Clinical data as well as CT and/or MRI findings were compared between groups.Results The patients'age was lower,while glycated hemoglobin and D-dimer levels were higher in IKPLAS group than those in non-IKPLAS group(all P<0.05).Hepatic venous thrombophlebitis was detected in 18 cases in IKPLAS group and 6 cases in non-IKPLAS group,while arterial phase abnormal high perfusion around abscess was noticed in 10 cases in IKPLAS group and 28 cases in non-IKPLAS group,both being significantly different between groups(both P<0.05).The extrahepatic infection in IKPLAS group mainly observed in lungs(19/25,76.00%).Conclusion The ages were lower,while glycated hemoglobin and D-dimer levels were both higher in IKPLAS than in the other KPLA patients.Often appeared hepatic venous thrombophlebitis and extrahepatic infection mainly affecting lungs were imaging characteristics of IKPLAS.
4.Clinical observation of levonorgestrel-releasing intrauterine system in treatment of endometriosis

Objective To evaluate the efficacy of levonorgestrel-releasing intrauterine system(LNGIUS) in the treatment of endometriosis. Methods Fifty-three patients suffering from endometriosis were placed LNG-IUS in uterine cavity after menstruation. The efficacy 3,6,12 months after LNG-IUS placement was observed and compared. Results The scores of dysmenorrhea, deep dyspareunia, menstrual anus pain 3,6,12 months after LNG-IUS placement were lower than those before LNG-IUS placement (P < 0.05 ).CA125 [(21.8±12.5) kU/L]and destradiol [(78.8±45.5)ng/L] 12 months after LNG-IUS placement were improved than those before LNG-IUS placement and 3,6 months after LNG-IUS placement [CA 125:(36.3±17.5), (33.5±15.6), (28.2 ± 11.7) kU/L,destradiol: (40.4 ± 28.6), (42.5 ± 31.5), (56.5 ± 36.2) ng/L](P < 0.05 ). The menstrual blood volume was less 6,12 months after LNG-IUS placement than that before LNG-IUS placement (P<0.05), and the quality of life was improved( P < 0.05 ). Conclusion LNG-IUS can be safe and effective treatment of patients with endometriosis, and less side effects.
