1.Study on the inhibitory effect and mechanism of Modified qifang weitong granules on gastric cancer

OBJECTIVE To investigate the inhibitory effect and mechanism of Modified qifang weitong granules on gastric cancer based on in vitro and in vivo experiments. METHODS Human gastric cancer HGC-27 cells were divided into the following groups: control group (treated with fetal bovine serum), 10% drug-containing serum group, 15% drug-containing serum group, 20% drug-containing serum group, and 5-fluorouracil (5-Fu) group (positive control, 3.90 μg/mL). After culturing the cells in each group with the corresponding serum/drug solution, their proliferation, migratory and invasive abilities, as well as the cell cycle, were assessed. Additionally, the expression levels of epithelial-mesenchymal transition (EMT)-related proteins [E-cadherin, N-cadherin, and vimentin] in the cells were measured. Logarithmic-phase HGC-27 cells were harvested and subcutaneously injected into the right axillary region of nude mice to establish a subcutaneous xenograft tumor model in nude mice. The successfully modeled tumor-bearing nude mice were randomly divided into model group, low-, medium- and high-dose groups of Modified qifang weitong granules (17.65, 35.29 and 70.58 g/kg, respectively), and 5-Fu group (25 mg/kg), with 5 mice in each group. After 14 days of treatment with the corresponding drugs in each group, the histopathological morphology of the tumor tissues in the nude mice was observed. Immunohistochemistry and Western blot assay were employed to detect the expression levels of EMT- related proteins in the tumor tissues of the nude mice. RESULTS In the cell experiment, compared with the control group, the cell proliferation rate, migration rate, number of invasive cells, as well as the expression levels of N-cadherin and vimentin proteins, and the percentage of cells in the G2/M phase were all significantly decreased/reduced in the 15% drug-containing serum group, 20% drug-containing serum group (P<0.05). Conversely, the percentage of cells in the G0/G1 phase and the expression level of E- cadherin protein were significantly increased (P<0.05). In animal experiment, compared with the model group, the high-dose group of Modified qifang weitong granules exhibited significantly reduced tumor mass and expression levels of N-cadherin and vimentin proteins in the tumor tissues of nude mice (P<0.05), while the expression level of E-cadherinprotein in the tumor tissues was significantly increased (P<0.05). Additionally, the tumor cells varied in size and showed extensive necrosis. CONCLUSIONS Modified qifang weitong granules effectively inhibit gastric cancer in both in vitro and in vivo models, and the mechanism of action is related to the suppression of EMT.

Nitric oxide synthase(NOS)and its product nitric oxide are involved in learning and memory functions.Increasing evidence shows that NOS plays an important role in the pathogenesis of Alzheimer's disease,influencing β-amyloid protein(Aβ)deposition,neuroinflammation,oxidative stress,abnormal microglia activation,synapse damage,autophagy,abnormal mitochondrial function of nerve cells,and cerebral hypoperfusion or vascular endothelial cell injury.This review summarizes the recent evidence for the role of NOS in the pathogenesis of Alzheimer's disease and provides new feasible targets for the prevention and treatment of Alzheimer's disease.

4.Executive function and clinical characteristics in children with attention-deficit hyperactivity disorder and emotional dysregulation

Background Comorbid attention-deficit/hyperactivity disorder(ADHD)and emotional dysregulation may represent a distinct subtype of ADHD,which is characterized by an increased risk of anxiety or depressive disorder and a poor clinical prognosis,so research is urgently required to explore its unique executive functioning profile and clinical characteristics.However,there is limited research comparing the clinical symptoms and executive function in children with ADHD in terms of the presence or absence of emotional dysregulation.Objective To explore the executive function and clinical characteristics of ADHD children with emotional dysregulation.Methods From June 2020 to December 2023,118 children aged 7 to 12 with ADHD attending the Mental Health Center of West China Hospital,Sichuan University and fulfilling the Diagnostic and Statistical Manual of Mental Disorders,fifth edition(DSM-5)diagnostic criteria were enrolled.Children were classified into emotional dysregulation group(n=68)and non-emotional dysregulation group(n=50)based on the standard T-scores of Achenbach's Child Behavior Checklist(CBCL)-anxious/depressed,aggressive behavior and attention problems subscales.All children were then subjected to complete the Chinese version of Swanson Nolan and Pelham,Version IV Scale-parent form(SNAP-IV),Chinese Wechsler Intelligence Scale for Children(C-WISC),Weiss Functional Impairment Scale-Parent form(WFIRS-P)and 4 tests of the Cambridge Neuropsychological Test Automated Battery(CANTAB):①Stockings of Cambridge(SOC)testing spatial planning.②Intradimensional-extradimensional Set Shifting(IED)testing cognitive/attentional flexibility,adjusting the total errors across the task.③Spatial Working Memory(SWM)testing spatial working memory.④Rapid Visual Information Processing(RVP)testing sustained attention.Results The SNAP-IV Inattention,Hyperactivity/Impulsivity and Oppositional Defiant Disorder domain scores and total score were all higher in emotional dysregulation group compared with non-emotional dysregulation group(t=3.206,5.088,6.316,6.553,P<0.01).The WFIRS-P family,school learning,life skills,self-concept,social activities and risky activities domain scores and total score were all higher in emotional dysregulation group compared with non-emotional dysregulation group(t=6.074,4.406,4.143,3.984,6.575,6.662,8.254,P<0.01).In CANTAB,emotional dysregulation group made more total adjusted errors across the IED task compared with non-emotional dysregulation group(t=2.168,P<0.05).Conclusion Children with ADHD who exhibit emotional dysregulation have been observed to experience more severe core symptoms,impaired social functioning and poorer performance on tests assessing executive function,particularly in the area of cognitive flexibility.
5.Exploration of the Effect and Mechanism of Shuangzhu Kangxian Prescription in Improving Carbon Tetrachloride-Induced Hepatitic Fibrosis in Rats Based on Wnt/β-catenin Signaling Pathway

Objective To investigate the effect and mechanism of Shuangzhu Kangxian Prescription(Astragali Radix,bran-fried Atractylodis Macrocephalae Rhizoma,vinegar-prepared Rhizoma Curcumae,Bupleuri Radix,Salviae Miltiorrhizae Radix et Rhizoma,Litchi Semen)on improving hepatitic fibrosis induced by carbon tetrachloride(CCl4)in rats based on the Wnt/β-catenin signaling pathway.Methods The rat model of hepatitic fibrosis was replicated by subcutaneous injection of 3.0 mL·kg-1 40%CCl4 twice a week for 8 weeks.SD rats were randomly divided into blank control group(n=8),model group(n=7),positive control group(n=7,intragastric administration of 43.19 mg·kg-1 silymarin),low-dose Chinese medicine group(n=6,intragastric administration of 4.3 g·kg-1Shuangzhu Kangxian Prescription),medium-dose Chinese medicine group(n=6,intragastric administration of 8.6 g·kg-1Shuangzhu Kangxian Prescription),high-dose Chinese medicine group(n=7,intragastric administration of 17.2 g·kg-1Shuangzhu Kangxian Prescription).The continuous intragastric administration was given once a day for 4 consective weeks,in addition to the blank control group,the other groups continued to be subcutaneously injected with CCl4 at the same time.The pathological changes of liver tissue were observed by HE and Masson staining.The levels of serum type Ⅳ collagen(Col Ⅳ),type Ⅲ procollagen(PC Ⅲ),hyaluronic acid(HA)and laminin(LN)were detected by ELISA.The protein expression levels of Wnt1,β-catenin and PPAR-γ in liver tissue were detected by Western Blot.The mRNA expression levels of Wnt1,β-catenin and PPAR-γ in liver tissue were detected by qPCR.Results Compared with the blank control group,the liver of the model group showed partial necrosis of liver cells,the normal hepatic lobule structure was destroyed,the arrangement of liver cell cords was disordered,the central vein or portal area was enlarged,and a large number of inflammatory cells infiltrated to form bridging necrosis.The collagen fibers in the central vein or portal area of the liver proliferated significantly,forming fibrous septa,and the fibrosis score were significantly increased(P<0.05).The levels of serum Col IV,PC Ⅲ,HA and LN were significantly increased(P<0.05).The protein and mRNA expression levels of Wnt1 and β-catenin in liver tissue were significantly increased(P<0.05),and the protein and mRNA expression levels of PPAR-γ were significantly decreased(P<0.05).Compared with the model group,the steatosis of hepatocytes in the positive control group and the medium-and high-dose groups of Chinese medicine was improved,and the necrosis and inflammatory cell infiltration were reduced.The degree of hepatitic fibrosis was improved,the liver collagen fibers were reduced,and the fibrosis score was significantly reduced(P<0.05).The levels of serum Col Ⅳ,PC Ⅲ,HA and LN were significantly decreased(P<0.05).The protein and mRNA expression levels of Wnt1 and β-catenin in liver tissue were significantly decreased(P<0.05),and the protein and mRNA expression levels of PPAR-γ were significantly increased(P<0.05).Conclusion Shuangzhu Kangxian Prescription has the effect of anti CCl4-induced hepatitic fibrosis in rats,and its mechanism may be related to the inhibition of Wnt/β-catenin signaling pathway and up-regulation of PPAR-γ expression.
6.Decellularized Matrices for the Treatment of Tissue Defects: from Matrix Origin to Immunological Mechanisms

Decellularized matrix transplantation has emerged as a promising therapeutic approach for repairing tissue defects, with numerous studies assessing its safety and efficacy in both animal models and clinical settings. The host immune response elicited by decellularized matrix grafts of natural biological origin plays a crucial role in determining the success of tissue repair, influenced by matrix heterogeneity and the inflammatory microenvironment of the wound. However, the specific immunologic mechanisms underlying the interaction between decellularized matrix grafts and the host immune system remain elusive. This article reviews the sources of decellularized matrices, available decellularization techniques, and residual immunogenic components. It focuses on the host immune response following decellularized matrix transplantation, with emphasis on the key mechanisms of Toll-like receptor, T-cell receptor, and TGF-β/SMAD signaling in the stages of post-transplantation immunorecognition, immunomodulation, and tissue repair, respectively. Furthermore, it highlights the innovative roles of TLR10 and miR-29a-3p in improving transplantation outcomes. An in-depth understanding of the molecular mechanisms underlying the host immune response after decellularized matrix transplantation provides new directions for the repair of tissue defects.
7.Effect of Emotion Regulation on Anxiety/Depression Symptoms in Children With Attention-Deficit/Hyperactivity Disorder:The Mediating Role of Social Problems and the Moderating Role of Family Functioning

Objective To investigate the mediating effect of social problems in the effect pathway of emotional dysregulation influencing anxiety/depression emotions in children with attention-deficit/hyperactivity disorder(ADHD)and to explore the potential moderating effect of family functionality.Methods A total of 235 children diagnosed with ADHD were enrolled in the study.The paticipants'age ranged from 6 to 12.Emotion Regulation Checklist,Achenbach's Child Behavior Checklist(CBCL)Social Problems Subscale,CBCL Anxious/Depressed Subscale,and Family Assessment Device were used to evaluate the emotional regulation,social problems,anxiety/depression emotions,and family functionality of the participants.A moderated mediation model was employed to analyze whether social problems and family functionality mediate and moderate the relationship between emotional regulation and anxiety/depression emotions.Results Social problems partially mediated the impact of emotional dysregulation on anxiety/depression emotions in ADHD children,with the direct effect being 0.26(95%confidence interval[CI]:[0.17,0.36],P<0.001),the indirect effect being 0.13(95%CI:[0.07,0.19],P<0.001),and the mediating effect accounting for 33%of the total effect.Family functionality exhibited a positive moderating effect on the relationship between social problems and anxiety/depression emotions.Conclusion This study contributes to the understanding of complex factors influencing anxiety/depression in children with ADHD,providing reference for the further development of targeted interventions for children with ADHD and the improvement of prognosis.
8.Analysis on incidence trend of breast cancer and reproductive system cancers in women in China, 2006-2017

Objective:To understand the incidence trend and temporal distribution of breast cancer and reproductive system cancers in women in different age groups and areas in China from 2006 to 2017.Methods:Based on the incidences of breast cancer, vulvar cancer, vaginal cancer, cervical cancer, uterine corpus cancer, and ovarian cancer in women, average age at diagnosis and cases in different age groups and areas in China were calculated, standardized through world population. Software Joinpoint 4.5.0.1 was used to calculate average annual percent of change (AAPC).Results:Between 2006 and 2017, the overall age-standardized incidence rate (ASIR) of the six cancers showed an increasing trend from 39.48/100 000 to 51.11/100 000 (AAPC=2.24%, 95% CI: 1.59%-2.89%). The increasing trend was more obvious in rural area (AAPC=4.65%,95% CI:3.67%-5.64%), whereas no significant increase was observed in urban area (AAPC=0.15%,95% CI:-0.26%-0.56%). Except uterine corpus cancer, the incidences of 5 cancers showed increasing trends. The incidences of cervical cancer showed similar upward trends in urban and rural areas. In urban area, the ASIRs of breast cancer, vaginal cancer and vulvar cancer showed no significant increase, while in rural area it showed significant increase. The ASIR of ovarian cancer showed a decreasing trend in urban area and an increasing trend in rural area. The average age at diagnosis increased for all the cancers, except uterine corpus cancer. However, after standardizing through world population, the increases in the standardized average age at diagnosis were observed only in cervical cancer and vaginal cancer from 49.11 and 55.15 years to 52.13 and 58.81 years, respectively. Conclusions:The overall ASIR of breast cancer and reproductive system cancers in women showed increase trend in China in 2006-2017, and the increase trend was more obvious in rural area than in urban area. Meanwhile, the accessibility to medical facilities in rural area needs to be improved to ensure medical care, early diagnosis and early treatment for the purpose of bridging the gap in female cancer incidence between rural area and urban area.
9.Bedside ultrasound diagnosis and risk factors of early thromboembolism after pancreaticoduodenectomy with vein reconstruction

Objective To investigate the risk factors of early thromboembolism after pancreaticoduodenectomy with vein reconstruction.Methods The results of bedside ultrasonography and clinical data of 90 patients from Ruijin Hospital,Shanghai Jiao Tong University School of Medicine who underwent pancreaticoduodenectomy with vein reconstruction within 7 days after operation between Janurary 2018 and September 2021 were analyzed retrospectively.According to the results of bedside ultrasound examination of the patency of portal vein,the postoperative patients were divided into portal vein pa-tency group and portal vein embolization group.The hemodynamic differences of bedside ultrasound and the influencing factors of postoperative portal vein thromboembolism were compared.Results Among 90 patients underwent bedside ultra-sound,8 patients had portal vein thromboembolism after pancreaticoduodenectomy with vein reconstruction,and the inci-dence rate was 8.89%.Bedside ultrasound showed that the velocity and flow of portal vein in patients with portal vein thromboembolism decreased significantly(P<0.0O1).Among the perioperative related risk factors,there was a statistically significant difference in the smoking history and alcohol abuse history between the portal vein patency group and portal vein thromboembolism group.Further,previous smoking history was an independent risk factor for portal vein thromboem-bolism(P=0.003).The outcomes showed that the pancreaticoduodenectomy with vein reconstruction was safe and feasible,however early portal vein thromboembolism with severe complications indicated a poor prognosis.Conclusions The history of smoking and alcohol abuse are the high risk factors of portal vein thromboembolism after pancreaticoduodenectomy with vein reconstruction,which should be prevented by preoperative education.Bedside ultrasound can effectively and safely evaluate the patency of portal vein after pancreaticoduodenectomy with vein reconstruction,which contributes to early clini-cal intervention.
10.Exploring the feasibility of endoscopic surgery by gastrointestinal surgeons: surgeon′s perspective

Due to traditional professional divisions, the practice of endoscopy by gastro-intestinal surgeons in China remains controversial. However, with the evolution of treatment philo-sophies, medical technology, and equipment advancements, a trend of integration between tradi-tional surgery and endoscopy is emerging. Gastrointestinal surgeons performing endoscopy can maxi-mize patient benefits, and they naturally possess advantages in conducting endoscopic procedures. It is recommended to further establish entry thresholds for surgeons to perform endoscopy, provide standardized endoscopic training for surgeons, and coordinate efforts at the administrative depart-ment. With the support of artificial intelligence, more patients can receive minimally invasive, indivi-dualized, and precise treatments.
