To explore the predictive value of carotid corrected flow time (FTc) and velocity time integral (VTI) for post-induction hypotension (PIH) in elderly patients undergoing gastrointestinal tumor surgery. Elderly patients scheduled for elective gastrointestinal tumor surgery at Yantai Yuhuangding Hospital Affiliated to Qingdao University, from October 2024 to March 2025, with an American Society of Anesthesiologists (ASA) anesthesia classification of Ⅰ-Ⅲ were selected as the study subjects. Before anesthesia induction, carotid FTc and VTI were measured using ultrasound in all patients. According to whether hypotension occurred after anesthesia induction, the patients were divided into a hypotension group and a non-hypotension group. The receiver operating characteristic (ROC) curve was used to evaluate the predictive efficacy of FTc and VTI, and their combination for PIH. Multivariate logistic regression analysis was conducted to screen the influencing factors of PIH. A total of elderly 115 patients were included in this study. After excluding 2 patients with unclear carotid spectra, 2 patients with canceled surgeries, and 3 patients with multiple systolic blood pressure(SBP) measurements exceeding 180 mm Hg(1 mm Hg=0.133 kPa) after entering the operating room, 108 patients were finally included. Among them, 43 patients (39.8%) developed hypotension after anesthesia induction (hypotension group), and 65 patients (60.2%) did not develop hypotension (non-hypotension group). The proportions of patients with higher SBP, mean arterial pressure (MAP), ASA classification of Ⅲ, and concurrent hypertension in the hypotension group were all higher than those in the non-hypotension group. The FTc in the hypotension group was shorter, and the VTI was lower than those in the non-hypotension group, with statistically significant differences (all Both carotid FTc and VTI can predict the occurrence of PIH in elderly patients undergoing gastrointestinal tumor surgery, but the predictive efficacy of FTc is higher than that of VTI. The combined application of FTc and VTI has a higher predictive efficacy for the occurrence of PIH in elderly patients undergoing gastrointestinal tumor surgery than FTc or VTI alone.

Objective:To discuss the effect of pharmacist intervention on the use of antimicrobial agents in the clinical pathway of community-acquired pneumonia ( CAP) in our hospital to standardize the rational medication and promote the rational use of antimicro-bial agents. Methods:Totally 100 bacterial CAP patients in 2013 ( before the intervention) and 2014 ( after the intervention) in the pneumology department were studied. The antibacterial drug cost, total hospitalization cost, use intensity of antimicrobial drugs, hospi-talization time, therapeutic effects and so on in the two groups were observed during the treatment. Results: There were significant differences between the two groups in antibacterial drug cost, total hospitalization cost, use intensity of antimicrobial drugs, hospitaliza-tion time and so on, whereas there was no significant difference in the curative effect. Conclusion:After the pharmacist intervention, the application of antibiotics is more rational, the antibiotics use density and per capita cost are reduced, the hospitalization day is shortened and the value of pharmacists is also improved.
