1.Toric-ICL shows better predictability and efficacy than FS-LASIK for myopia correction in patients with moderate to high myopia and astigmatism.

OBJECTIVES:
To compare the efficacy of toric implantable collamer lens (Toric-ICL) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for myopia correction in patients with moderate to high myopia complicated with astigmatism.
METHODS:
We retrospectively collected data from 64 patients (aged 18-42 years) with moderate to high myopia complicated with astigmatism (128 eyes) undergoing either Toric-ICL (28 patients/56 eyes) or FS-LASIK (36 patients/72 eyes) at our department between January, 2019 and December, 2020. The changes of uncorrected distance visual acuity (UCVA), spherical equivalent (SE), mean astigmatism correction index (CI), corneal endothelial cell density (ECD) and intraocular pressure (IOP) following the procedures were compared between the two groups.
RESULTS:
In FS-LASIK group, all the eyes (72/72) achieved an UCVA≥1.0, similar to the rate in Toric-ICL group (55/56 eyes; P=0.2374). The postoperative SE was also comparable between FS-LASIK and Toric-ICL groups [0.43±0.06 D (range: -1.0 to 1.50 D) vs 0.38±0.05 D (range: -0.75 to 1.00 D); P=0.56]. The mean astigmatism CI was significantly higher in FS-LASIK group than in Toric-ICL group (0.8561 vs 0.7176; P<0.0001), and 88.89% of the eyes in FS-LASIK group and 69.64% in Toric-ICL group had postoperative astigmatism ≤0.50 D. No significant changes were observed in postoperative corneal ECD in FS-LASIK group, whereas ECD decreased significantly after the procedure in Toric-ICL group (P=0.0057). The patients undergoing Toric-ICL exhibited no significant changes of postoperative IOP, but the patients receiving FS-LASIK had significantly reduced IOP after the procedure (P<0.001).
CONCLUSIONS
Although the patients included in Toric-ICL group had higher myopia and astigmatism, Toric-ICL still showed better predictability and efficacy for astigmatic correction in Toric-ICL group. Toric-ICL is an effective and safe equivalent of FS-LASIK for correcting moderate myopia but can be more advantageous for correcting high myopia with astigmatism.

2.Effect of daglipzin combined with routine treatment on renal function, urinary podocyte-related proteins and inflammatory factors in patients with early diabetic kidney disease

Objective:To explore the effect of daglipzin combined with routine treatment on renal function, urinary podocyte-related protein (PCX) and inflammatory factors in patients with early diabetic kidney disease (DKD).Methods:A total of 120 early DKD patients in Baoji Traditional Chinese Medicine Hospital from March 2021 to December 2023 were prospectively collected and divided into two groups (observation group and control group) according to the random number table method, with 60 cases in each group. All patients received routine treatment. The observation group was additionally treated with daglipzin, while the control group was treated with metformin. Before and after treatment, the renal function indexes [vascular cell adhesion molecule-1 (VACM-1), creatinine (Cr), blood urea nitrogen (BUN)], urinary podocalyxin (PCX), and inflammatory factors [homocysteine (Hcy), high-sensitivity C-reactive protein (hs-CRP), macrophage migration inhibitory factor (MIF), tumor necrosis factor-α (TNF-α)] were detected in both groups. Meanwhile, the glucose metabolism levels [fasting blood glucose (FBG), glycosylated hemoglobin A 1c (HbA 1c)] and the occurrence of adverse reactions were compared between the two groups. Results:There were no significant differences in renal function, urinary PCX, glucose metabolism indexes, and inflammatory factor levels between the two groups before treatment (all P>0.05). After treatment, serum levels of VACM-1, BUN, Cr, hs-CRP, TNF-α, MIF, Hcy, FBG, HbA 1c, and urinary PCX in both groups were lower than those before treatment (all P<0.05). Moreover, the serum levels of VACM-1, BUN, Cr, hs-CRP, TNF-α, MIF, Hcy, FBG, HbA 1c, and urinary PCX in the observation group after treatment were lower than those in the control group (all P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups ( P>0.05). Conclusions:Compared with metformin, daglipzin combined with routine treatment can improve renal function, reduce urinary PCX and inflammatory factor levels in patients with early DKD, which has certain clinical application value.
3.Clinical application of sentinel lymph node dissection in the treatment of early cervical cancer

Objective:To investigate the efficacy and safety of sentinel lymph node dissection in the treatment of early cervical cancer.Methods:This study used a prospective study design. A total of 122 patients with early cervical cancer who received treatment at Jinhua Maternal & Child Health Care Hospital form June 2022 to June 2024 were included in this study. These patients were divided into an observation group and a control group ( n = 61/group) using a random number table method. In the observation group, sentinel lymph node resection instead of systematic lymphadenectomy was performed, while in the control group, systematic lymphadenectomy was performed. Perioperative indicators, postoperative lymph node pathology results, complications, and postoperative tumor recurrence rates were compared between the two groups. Additionally, sentinel lymph node imaging results were evaluated in the observation group. Results:After treatment, the surgical time, duration of pelvic drainage tube retention, duration of catheter retention, postoperative hospital stay, number of lymph nodes dissected, and time to postoperative bowel function recovery in the observation group were (178.33 ± 8.29) minutes, (7.19 ± 1.38) days, (11.73±2.57) days, (8.09±1.34) days, (12.48 ± 2.63), and (2.28 ± 0.69) days, respectively. These values were significantly shorter or lower than those in the control group [(239.41 ± 9.83) minutes, (9.74 ± 1.75) days, (12.88 ± 2.76) days, (9.53 ± 1.47) days, (133.27 ± 11.43) mL, (26.18 ± 2.27), (3.12 ± 0.98) days, t = 37.10, 8.94, 2.38, 5.65, 30.80, 5.47, all P < 0.05]. There were no statistically significant differences in postoperative pathological results between the two groups ( χ2 = 2.03, 0.12, both P > 0.05). The overall incidence of perioperative complications in the observation group was significantly lower than that in the control group [1.46% (1/61) vs. 8.19% (5/61), χ2 = 5.55, P < 0.05). Conclusions:Sentinel lymph node resection can significantly enhance the perioperative outcomes of patients with cervical cancer and reduce the incidence of complications. Additionally, there were no recurrences of cervical cancer during the follow-up period. Therefore, sentinel lymph node resection is a safe and feasible procedure.

Additionally, it proposes future research directions to provide references for conducting relevant studies and practices on family communication in PSA patients within the current cultural context.

Objective:To explore the clinical application value of multiple cytogenetic and molecular genetic techniques in the diagnosing of chromosomal microstructural abnormalities.Methods:The proband was a 30-year-old man. He went to Reproductive Medicine Center of Shenzhen Zhongshan Urology Hospital in July 2021 because of a 7-year history of primary infertility. Chromosome karyotype was analyzed by conventional G-banding technique. One case was found to be suspected of microstructural aberration in X chromosome. The origin and structural characteristics of this X chromosome structural aberration was identified by high-resolution G-banding, copy number variation sequencing (CNV-seq), C-banding and fluorescence in situ hybridization (FISH). The pregnancy outcome of this case was followed up. Results:Conventional G-banding karyotype of peripheral blood lymphocytes was initially diagnosed as 46, Y, ?inv(X)(p22.3p22.2). The final karyotype of proband was interpreted as 46, Y, der(X) t(X; Y)(p22.3; q12) mat by high-resolution G-banding karyotype analysis, CNV-seq, C-banding analysis and FISH detection. His spouse had conceived singleton pregnancy via in vitro fertilization and embryo transfer. Prenatal diagnosis had been performed. Karyotype of amniotic fluid was 46, X, der(X) t(X; Y)(p22.3; q12) pat. No structural malformation was detected prenatally by ultrasound. The neonate was phenotypically normal one month after birth. Conclusion:The combined application of multiple cytogenetic and molecular genetic techniques can provide a reliable technical platform for characterizing the microscopic structural aberrations of chromosomes, and an important genetic basis for exploring the phenotype, prognosis and offspring risk of such patients.

10.Effects of improved catheter extubation method on urination

Objective:To compare the effects between improved catheter extubation method and the traditional one on urination pain, urinary retention, first urination time and first urination volume, and to evaluate the advantage of the improved method.Methods:144 patients with indwelling catheters after operation in our department were randomly divided into observation group and control group, with 72 cases in each group. The control group returned to the ward after surgery and began to clamp the urinary tube to train the bladder function, the catheter was removed by traditional method. Observation group: urination reflex was evaluated before extubation, according to more than 250 ml urine in the urine bag. Pumping the saline of the gas bag and injecting back 0.5ml to keep the wall of the bag smooth, eventually the catheter was excreted when urinating. Urethral pain, urinary retention, first micturition time and first micturition volume of two groups were analyzed.Results:Urethral pain, urinary retention, first micturition time in observation group and control group were 1.47±1.48, (20.44±12.98) min, 95.8% (69/72) and 3.11±1.98, (28.03±27.00) min, 83.3% (60/72), respectively, and the difference between the two groups was statistically significant ( t value was -5.644, -2.148, χ2 value was 6.628, all P<0.05). The first micturition volume in observation group and control group were (258.6±41.57) ml and (248.14±48.82) ml, respectively, and there was no significant difference between the two groups ( t value was 1.377, P>0.05). Conclusion:The improved catheter extubation method could significantly reduce the urethral pain, shorten the time of the first urination, and improve the success rate of self-urination, which of clinical promotion.
