1.Isolation,identification,and application of exosomes derived from mesenchymal stem cells

BACKGROUND:Exosomes derived from mesenchymal stem cells play pivotal roles in cell communication and epigenetic regulation due to their low immunogenicity and targeted delivery effects,and have been clinically applied in the treatment of various diseases.OBJECTIVE:To review the isolation,purification,identification methods,and application progress of mesenchymal stem cell-derived exosomes,and to facilitate the development of large-scale preparation techniques and clinical translation of mesenchymal stem cell-derived exosomes.METHODS:The Chinese search terms"exosome,mesenchymal stem cells,isolation,purification,characterization,clinical application"and the English search terms"exosome,extracellular vesicles,mesenchymal stem cells,isolation,characterization,application"were used to search the literature published before September 2024 in CNKI,PubMed,and Web of Science databases.Articles with poor relevance to the topic,outdated,or duplicated content were excluded,and finally,109 articles were included for review.RESULTS AND CONCLUSION:(1)This paper reviews recent methods for isolating and purifying exosomes,comparing the characteristics of ultracentrifugation,ultrafiltration,size-exclusion chromatography,polymer precipitation,immunoaffinity,microfluidic methods,and other novel approaches based on their underlying principles.(2)Methods for identifying exosomes can be categorized into physical and biochemical analyses,characterizing exosomes based on their shape,size,and characteristic proteins.(3)Mesenchymal stem cell-derived exosomes have broad applications in multiple fields such as medical aesthetics,wound repair,and cancer treatment,due to their immune-regulatory properties and ability to cross biological barriers.(4)The clinical translation of exosomes faces challenges due to their complex structure,lack of universal isolation techniques,and poor stability,making it difficult to achieve in a short period of time.

3.Jiedu-shengji ointment promotes wound repair in diabetic rats through inhibiting excessive endoplasmic reticulum stress and regulating PERK/IRE1/NLRP3 signaling pathway

AIM:This study aims to investigate the effects of Jiedu-Shengji ointment(JDSJG)on wound healing in diabetic rats by modulating the protein kinase R-like endoplasmic reticulum kinase(PERK)/inositol-requiring enzyme 1(IRE1)/nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)pathway in the context of endoplasmic reticulum stress(ERS).METHODS:Male Sprague-Dawley rats were randomly assigned to four groups:control group,model group,detoxification muscle ointment group,and sulfadiazine silver cream group.All groups,except the control,were administered an intraperitoneal injection of 45 mg/kg streptozotocin to induce diabetes.The control and model groups received daily applications of normal saline,while the detoxification myogen ointment and sulfadiazine silver cream groups received their respective treatments daily.After dressing changes,wounds were bandaged with sterile gauze.Following 14 d of continuous treatment,wound healing was assessed and healing rates calculated.Histopathologi-cal changes in wound tissues were analyzed using HE staining.Transmission electron microscopy was utilized to observe the number,morphology,and swelling of endoplasmic reticulum in the wound tissues.The expression and distribution of PERK,IRE1 and thioredoxin interacting protein(TXNIP)was assessed by immunohistochemistry,while Western blot was used to measure the levels of apoptosis-associated speck-like protein containing a caspase recruitment domain(ASC),TXNIP and NLRP3.ELISA was conducted to quantify the inflammatory cytokines interleukin-18(IL-18)and IL-1β.RE-SULTS:After 14 d of intervention,significant differences were observed in wound tissue parameters across the groups.The model group exhibited a significantly lower wound healing rate compared to the control group(P<0.01),with in-creased wound exudation,poor granulation tissue growth,and elevated the protein levels of PERK,IRE1,TXNIP,CHOP,NLRP3 and ASC(P<0.01),as well as significantly higher levels of IL-1β and IL-18(P<0.01).In contrast,the detoxification muscle ointment group showed a marked improvement in wound healing rate(P<0.01),reduced inflamma-tory exudation,improved granulation tissue growth,and significant decreases in TXNIP expression(P<0.01),along with lower levels of PERK,IRE1,CHOP,ASC and NLRP3(P<0.01).Additionally,the IL-1β and IL-18 were significantly reduced(P<0.01).CONCLUSION:Jiedu Shengji ointment alleviates excessive ERS and mitigates chronic inflammato-ry responses,thereby promoting the healing of diabetic wounds.These effects may be attributed to the inhibition of exces-sive activation of the PERK/IRE1/NLRP3 pathway.
4.Biomechanical Characteristics of Lower Limbs in Female Patients with Knee Osteoarthritis at the Beginning Stage of Learing Tai Chi Yunshou Movement

Objective To observe the differences in dynamic stability and kinematic/kinetic characteristics of the lower limbs between patients with knee osteoarthritis(KOA)and healthy individuals at the begining stage of practicing Tai Chi Yunshou movement.Methods Thirty Tai Chi beginners,including 15 patients with KOA and 15 healthy controls,were recruited to practice Tai Chi Yunshou movement for two hours under the guidance of a Tai Chi expert.A motion capture system and a three-dimensional force platform were used to collect and calculate dynamic stability parameter as well as kinematic and kinetic parameters of the left lower limb during the Yunshou movement.Results Compared with healthy controls,patients with KOA demonstrated a smaller center of mass(COM)-center of pressure(COP)inclination angle,reduced mean and peak ankle dorsiflexion angle,reduced peak hip adduction angle and increased peak knee flexion moment during the Yunshou movement(P<0.05).Conclusions Patients with KOA use adaptive postural strategies to maintain the lateral stability in Tai Chi Yunshou exercise,but a comprehensive training programme should be recommended to reduce the joint loading during flexion of the lower limbs at the benginning stages of this exercise.

7.Research progress on targeted therapy of breast cancer with low expression of HER2

Breast cancer has become the malignant tumor with the highest incidence rate among women, of which human epidermal growth factor receptor 2 (HER2) positive breast cancer accounts for about 15%-20%. With the development of anti HER2 targeted drugs, the survival and prognosis of HER2 positive breast cancer has significantly benefited. About 45%-55% of breast cancer patients have low HER2 expression, and these patients usually do not receive anti HER2 treatment. However, there is a significant difference between the biological behavior and prognosis of breast cancer with low HER2 expression and breast cancer with zero HER2 expression. It is helpful to differentiate and adopt corresponding treatment strategies to improve the prognosis of patients. At present, there have been many advances in targeted therapy of breast cancer with low HER2 expression, which provides a useful reference for precision treatment of breast cancer with low HER2 expression.
8.Improving the teaching quality of cervical vertebra DR in radiology department with scenario simulation and rapid film reading

Objective:To explore the application of scenario simulation and rapid film reading training in the standardization of cervical vertebra DR examination in radiology department.Methods:Using the method of scene simulation in the examination room to take DR films of cervical vertebra, two senior technicians in charge evaluated and guided the deficiencies of junior technicians from the aspects of patient check, exposure conditions, body position placement, film printing, inspection safety and radiation protection. Every morning, DR images of cervical vertebra were randomly selected and read quickly, and the junior technicians should quickly browse the images within one minute and make quality control evaluation. Then the senior technician in charge pointed out the shortcomings of the evaluation and related improvement methods. After that, the technical specification manual of cervical vertebra DR was compiled based on the deficiencies and problems found by each technician, and 200 patients aged 18-55 years, without abnormal movement and no internal fixation before and after the establishment of standardization were randomly selected in group A (before standardization) and group B (after standardization). The image quality and repetition projection rate of group A and group B before and after the standardization were compared. SPSS 25.0 was used to analyze the data.Results:The image quality score of group B (8.83±0.33) was higher than that of group A (8.45±0.30), and the repetition projection rate of group B (2%) was lower than that of group A (9%), with statistical significance ( P<0.05). Conclusion:Through the training mode of rapid film reading and scenario simulation, the standardization of cervical vertebra DR examination technology suitable for the situation of the department has been established, which effectively solves the problem of abstraction in the technical teaching of radiology department. The standardization establishment plays an important role in standardized training, for which, the technicians can quickly grasp the work process of the department and the corresponding examination technology during participating in the standardization establishment and training process.
9.The gait patterns of youths with chronic, nonspecific low back pain

Objective:To quantify the kinematic changes in the gait cycle among youths with chronic, nonspecific low back pain (CNLBP).Methods:A Qualisys 3D optical motion capture system was used to document the gait kinematics of 11 youths with CNLBP and 11 healthy counterparts. Gait profile scores and movement analysis profiles were then calculated.Results:The young CNLBP subjects exhibited gait abnormalities, as reflected by the significant difference of the gait variable the range of motion of the dominant knee between the CNLBP and healthy groups [(5.66±1.83)° vs (3.64±1.13)°]. Gait profile scores and movement analysis profiles are applicable to evaluating them.Conclusions:There manifested gait abnormalities in youth CNLBP patients. Gait profile scores and movement analysis profiles are applicable to evaluating them.
10.Association of polymorphisms of NAPE-PLD and FAAH genes with schizophrenia in Chinese Han population.

OBJECTIVETo assess the association of polymorphisms of N-acyl-phosphatidylethanolamine-phospholipase D (DAPE-PLD) and fatty acid amide hydrolase (FAAH) genes, as well as their interaction, with schizophrenia.
METHODSPolymorphisms of NAPE-PLD rs12540583 and FAAH rs324420, rs2295633, and rs6429600 were determined with PCR - restriction fragment length polymorphism assay and Sanger sequencing. The genotypes of 345 subjects of Han Chinese origin diagnosed with schizophrenia and a 403 controls were compared. The results were analyzed with SPSS 17.0, and the interaction of the two genes was analyzed using a multifactor dimensionality reduction (MDR) method.
RESULTSThe frequency of NAPE-PLD rs12540583 polymorphism was significantly different between the two groups under both dominant and additive models (χ2=17.18 vs. χ2=18.94, P<0.0125). The frequencies of AC genotype and C allele of the patient group at rs12540583 were higher than those of the controls, and the interaction of NAPE-PLD and FAAH was associated with schizophrenia. A four-loci model (rs12540583, rs324420, rs2295633 and rs6429600) can best model the interaction between NAPE-PLD and FAAH.
CONCLUSIONThe AC genotype and C allele of NAPE-PLD rs12540583 locus are risk factors for schizophrenia, and the interaction between NAPE-PLD rs12540583 and FAAH rs324420, rs2295633 and rs6429600 is associated with schizophrenia.
