1.Correlation study between the expression level of SFRP5 and the condition and prognosis of patients with severe pneumonia

Objective To explore the correlation between the expression level of secreted curl associated protein 5(SFRP5)and the condition and prognosis of patients with severe pneumonia.Methods A total of 168 patients with pneumonia admitted to the Department of Respiratory and Critical Care Medicine of this hospital from June 2022 to June 2024 were selected as the research subjects,and 78 healthy individuals who underwent physical examinations in this hospital during the same period were selected as the control group.According to the acute physiology and chronic health evaluation Ⅱ(APACHEⅡ)score,the patients were di-vided into the common pneumonia group(APACHE Ⅱ score<15,n=82)and the severe pneumonia group(APACHE Ⅱ score≥ 15,n=86).Peripheral venous blood was collected from the three groups after fasting for 8 to 12 hours and peripheral blood mononuclear cells were extracted by centrifuging.Real-time fluores-cence quantitative PCR was used to detect the expression level of SFRP5 mRNA in peripheral blood mononu-clear cells,which was Used to determine the expression level of SFRP5.Pearson analysis was used to analyze the correlations between the expression level of SFRP5 and the levels of inflammatory factors,pulmonary function indices and the condition in patients with severe pneumonia.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficiency of the expression level of SFRP5 for the prognosis of patients with severe pneumonia.Results There were also significant differences in the levels of WBC,C re-active protein(CRP),procalcitonin(PCT),interleukin(IL)-6,and IL-8 among the three groups(P<0.05),with the highest levels observed in the severe pneumonia group and the lowest in the control group.There were significant differences in the levels of forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),and FEV1/FVC among the three groups(P<0.05),with the highest levels observed in the severe pneumonia group and the lowest in the control group.The expression level of SFRP5 in the severe pneumonia group was the lowest(0.46±0.13),followed by the common pneumonia group(0.78±0.15),and the control group was the highest level(1.23±0.22),with statistically significant differences(P<0.05).The results of Pearson correlation analysis showed that the expression level of SFRP5 in patients with severe pneu-monia was negatively correlated with the levels of WBC,CRP,PCT,IL-6,IL-8,and the APACHE Ⅱ score(P<0.05),and positively correlated with the levels of FEV1,FVC,and FEV1/FVC(P<0.05).The area un-der the curve(AUC)of the expression level of SFRP5 for predicting poor prognosis in patients with severe pneumonia was 0.843(95%CI:0.748-0.912),with a sensitivity of 68.18%and a specificity of 89.06%.Conclusion The expression level of SFRP5 is related to the condition of patients with severe pneumonia,and SFRP5 can predict the prognosis of patients with severe pneumonia.
2.Analysis of the efficacy and influencing factors of myofunctional therapy in the treatment of adult obstructive sleep apnea

OBJECTIVE To evaluate the efficacy of oral and facial muscle functional training in treating adult obstructive sleep apnea(OSA)and to identify clinical indicators influencing treatment outcomes.METHODS Through a prospective cohort study,patients diagnosed with OSA in the study unit were recruited to undergo a 3-month myofunctional therapy,including soft palate-related muscles,tongue muscles,buccal muscles,and labial muscles in multiple muscle groups,once a day,five times a week,with the use of offline clinic guidance,and the APP program video follow up training for effective training.Data were collected on multiple dimensions including physical signs,sleep breathing monitoring parameters,and airway measurements from imaging studies.Treatment efficacy was assessed by comparing subjective and objective sleep indicators before and after training.Patients were categorized into effective and ineffective groups based on treatment outcomes.Differences in baseline clinical indicators between these groups were analyzed using univariate and multivariate regression analyses.RESULTS The study finally included 58 people,51 males and 7 females,age(38.36±8.96)years,BMI(27.14±3.68)kg/m2,AHI of the enrolled patients was reduced from(31.27±22.28)times/h pre-training to(26.27±21.38)times/h post-training,the minimum oxygen saturation was increased from(78.43±10.07)%to(80.50±10.06)%,snoring index decreased from(62.80±75.20)times/h to(36.40±43.19)times/h,and ESS score decreased from 7.00±5.31 pre-training to 5.50±3.17.By comparing the effective and ineffective groups,it was found that there was a statistically significant difference in the tongue position and ESS scores between the two groups(both P<0.05),while no significant differences were found in gender,age,neck circumference,posterior soft palate area,uvula area,posterior tongue area,or posterior epiglottic area(all P>0.05).Univariate logistic regression analysis indicated that tongue position,AHI,and ESS scores were factors affecting the efficacy of oral and facial muscle function training.Multivariate regression analysis revealed that AHI was an independent prognostic factor for this training in OSA patients.CONCLUSION Oral and facial muscle function training can improve both subjective and objective sleep breathing indices in OSA patients.Tongue position,AHI,and ESS scores may serve as prognostic factors for OSA treatment,aiding in guiding subsequent individualized intervention therapies.
3.Value of MRI in the prediction of breast cancer with high expression of transcription factor E2F2

Objective:To investigate the value of MRI in diagnosing the high expression of transcription factor E2F2 in breast cancer, in order to further evaluate the biological aggressiveness of breast cancer.Methods:The study was cross-sectional. The clinical and imaging data of 92 patients with breast cancer who were pathologically diagnosed in the Second Affiliated Hospital of Nantong University from October 2014 to December 2020 were retrospectively analyzed. All patients were female, aged 32 to 82 (56±11) years. All patients underwent MRI, including T 1WI, T 2WI, diffusion-weighted imaging (DWI), and dynamic enhanced MRI. According to the immunohistochemical results, the patients were divided into E2F2 high expression group and E2F2 low expression group. Independent sample t test, Mann-Whitney U test or χ2 test were used to compare the clinical data, MRI features and pathological features of the two groups. Characteristics with P≤0.1 from univariate analyses were included in multivariate logistic regression to screen for independent risk factors predicting high expression of the transcription factor E2F2 in breast cancer. Results:Among 92 patients, 68 were in the high E2F2 expression group and 24 were in the low E2F2 expression group. There were significant differences between the two groups in peritumoral edema, type composition ratio of time-signal intensity curve, estrogen receptor expression, human epidermal growth factor receptor 2 expression, endovascular cancer embolus and molecular type composition ratio (all P<0.05). Multivariate logistic regression analysis showed that peritumoral edema alone ( OR=7.061, 95% CI 1.837—27.144, P=0.004) was an independent risk factor for predicting the high expression of transcription factor E2F2 in breast cancer. Conclusion:MRI show that peritumoral edema is an independent risk factor for predicting high expression of the breast cancer transcription factor E2F2, which is associated with the aggressiveness of breast cancer.
4.Construction and validation of a risk prediction model for the delayed healing of venous leg ulcers

Objective To construct and validate a risk prediction model for delayed healing of venous leg ulcer(VLU),so as to provide a reference basis for early identification of people at high risk of delayed healing.Methods Using a convenience sampling method,331 VLU patients attending vascular surgery departments in 2 tertiary A hospitals in Sichuan Province from January 2018 to December 2022 were selected as a modeling group and an internal validation group,and 112 patients admitted to another tertiary A hospital were selected as an external validation group.Risk factors for delayed healing in VLU patients were screened using univariate analysis,LASSO regression,and multivariate logistic regression analysis,and a risk prediction model was constructed using R software,and the predictive effects of the models were examined using the area under the receiver operating characteristic curve,the Hosmer-Lemeshow test,decision curve,and the bootstrap resampling for internal validation and spatial external validation were performed,respectively.Results The predictors that ultimately entered the prediction model were diabetes(OR=4.752),deep vein thrombosis(OR=4.104),lipodermatosclerosis(OR=5.405),ulcer recurrence(OR=3.239),and ankle mobility(OR=5.520).The model had good discrimination(AUC:0.819 for internal validation and 0.858 for external validation),calibration(Hosmer-Lemeshow test:χ2=13.517,P=0.095 for internal validation and χ2=3.375,P=0.909 for external validation)and clinical validity.Conclusion The model constructed in this study has good differentiation and calibration,and it can effectively predict people at high risk of delayed healing of VLU,which facilitates targeted clinical interventions to improve ulcer outcomes and reduce the risk of delayed ulcer healing.

Ferroptosis is a nonapoptotic form of cell death and differs considerably from the well-known forms of cell death in terms of cell morphology,genetics,and biochemistry.The three primary pathways for cell ferroptosis are system Xc-/glutathione peroxidase 4(GPX4),lipid metabolism,and ferric metabolism.Since the discovery of ferroptosis,mounting evidence has revealed its critical regulatory role in several diseases,especially as a novel potential target for cancer therapy,thereby attracting increasing attention in the fields of tumor biology and anti-tumor therapy.Accordingly,broad prospects exist for identifying ferroptosis as a potential therapeutic target.In this review,we aimed to systematically summarize the activation and defense mechanisms of ferroptosis,highlight the therapeutic targets,and discuss the design of nanomedicines for ferroptosis regulation.In addition,we opted to present the advantages and disadvantages of current ferroptosis research and provide an optimistic vision of future directions in related fields.Overall,we aim to provide new ideas for further ferroptosis research and inspire new strategies for disease diagnosis and treatment.
6.Study on the mechanism of Yigong Powder in the treatment of anemia of chronic disease by means of formula components analysis based on activating spleen method

Objective To observe the role of dried tangerine peel in Yigong Powder improves iron metabolism and promotes red blood cell generation in anemia of chronic disease (ACD).Methods With a two-by-two factorial design,the Yigong Powder was divided into dried tangerine peel and Chenpi absent Decoction. According to the random number table method,32 zymosan-induced generalized inflammation (ZIGI) mice were randomly divided into the model group,the dried tangerine peel group,the Chenpi absent Decoction group,and the Yigong Powder group. The dried tangerine peel group,Chenpi absent Decoction group and the Yigong Powder group were given dried tangerine peel(3.083 g/kg),Chenpi absent Decoction(12.33g/kg),and Yigong Powder(15.413g/kg)by gavage to the corresponding group of mice. The model group was given an equal amount of physiological saline by gavage,and treated continuously for 7 days. After the completion of administration,the body weight of each group of mice was recorded. The hemoglobin content of each group of mice was detected using a fully automatic cell counter,the serum iron content was detected using colorimetry,the serum ferritin content was detected using enzyme-linked immunosorbent assay (ELISA),and the spleen index was calculated. The liver tissue inflammatory factors interleukin-1β (IL-1β),interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α),interferon-γ (IFN-γ),interleukin-4 (IL-4),and interleukin-10 (IL-10) levels were detected using Luminex method. The mRNA expressions of liver tissue hepcidin gene (HAMP) and membrane iron transporter ( Fpn) were detected using real-time fluorescence PCR method. Results Dried tangerine peel and Chenpi absent Decoction both showed interactive effects in regulating hemoglobin,serum iron,serum ferritin content,improving spleen index,and regulating the mRNA expressions of HAMP,Fpn,as well as IL-1β and IFN-γ (P<0.05). Compared with the model group,dried tangerine peel significantly increased hemoglobin,serum iron content,and Fpn mRNA expression in ZIGI model mice,while decreasing ferritin content,spleen index,HAMP mRNA expression,and the levels of IL-1β,IL-6,TNF-α,and IFN-γ (P<0.05). Chenpi absent Decoction significantly increased serum iron content and Fpn mRNA expression in ZIGI model mice,while reducing spleen index,ferritin content,HAMP mRNA expression,and the levels of IL-1β and IFN-γ、IL-4 (P<0.05). Conclusion The effects of dried tangerine peel on inflammatory factors (IL-6 and TNF-α) and Fpn may play a key role in the improvement effects of Yigong Powder on ACD and iron metabolism.
7.Research status of common recurrence risk prediction tools for hormone receptor-positive breast cancer

Breast cancer is one of the malignant tumors that seriously threaten the health of women worldwide,with hormone receptor(HR)positivity being its most common molecular subtype.The heterogeneity of breast cancer leads to varying risks of recurrence after surgery for patients,thus requiring highly sensitive recurrence risk prediction tools to help clinical doctors assess patients'prognoses and to develop personalized treatment plans.Currently,widely used recurrence risk prediction tools developed based on multi-gene testing or clinical pathological factors for HR-positive breast cancer patients after surgery,such as Oncotype DX,IHC 4 scores.In addition,prognostic models based on imaging features are also gradually being applied in clinical practice.The recurrence risk prediction tools can more accurately assess the risk of recurrence and prognosis of breast cancer patients after surgery,guide individualized clinical diagnosis and treatment plans for patients,improve the quality of life of patients,and prolong overall survival.
8.Clinical value of manual massage in treatment of grade Ⅰ internal hemorrhoids under endoscopic foam sclerotherapy

Objective To evaluate the effect of manual massage on complications after endoscopic foam sclerotherapy injection for the treatment of internal hemorrhoids.Methods Consecutive 113 patients with grade Ⅰinternal hemorrhoids were prospectively enrolled and completed endoscopic foam sclerotherapy.The patients were randomly divided into a massage group(n=65)and a control group(n=68).Massage group performed manual perianal massage,Visual analogue scale(VAS)was used to evaluate perianal pain.The postoperative bleeding,short-term and long-term efficacy were also compared.Results The median VAS of 24 h postoperation was 1.0(0.0,3.0)in massage group,which was significantly lower than 2.0(1.0,4.0)in control group,the difference was statistically significant(P=0.014).The no bleeding rate of one week postoperation was 84.6%in massage group,which was significantly higher than 64.7%in control group,the difference was statistically significant(P=0.009).After 12 weeks,6 months and 12 months of follow-up,there were no significant differences in cure rate and remission rate between the two groups(P>0.05).Conclusion Manual massage after endoscopic sclerosing agent injection is beneficial to relieve postoperative pain of grade Ⅰ internal hemorrhoids and reduce bleeding.
9.Preparation of collagen-polysaccharide composite hydrogels and research progress in biomedical applications.

Collagen contains abundant cell binding motifs, which are conducive to adhesion, migration, and differentiation, maintain cell vitality and promote cell proliferation. However, pure collagen hydrogel has some shortcomings such as poor mechanical properties, poor thermal stability and fast degradation. Numerous studies have shown that the properties of collagen can be improved by combining it with natural polysaccharides such as alginate, chitosan, hyaluronic acid and cellulose. In this paper, the research status and biological application fields of four kinds of composite hydrogels, including collagen-alginate composite hydrogels, collagen-chitosan hydrogels, collagen-hyaluronic acid hydrogels and collagen-cellulose hydrogels, were summarized. The common preparation methods of four kinds of composite hydrogels were introduced, and the future development direction of collagen-based composite hydrogels was prospected.

10.Chemical Compounds Isolated from Dendrobium crepidatum and Their Bioactive Activities

Objective To study the chemical constituents from the herbs of Dendrobium crepidatum and identify its antitumor bioactive compounds.Methods The constituents were extracted by methyl alcohol and isolated by CC and Sephadex LH-20.Their structures were identified by spectroscopic analysis(1H NMR,13CNMR and EIMS).The antitumor activity of the constituents was determined by MTT assay in vitro.Results 9 compouds were obtained and identified as p-hydroxybenzaldehyde(1),p-methoxyphenol(2),p-Hydroxybenzyl alcohol(3),hydroquinone(4),(24R)-ethylcholest-5-en-3-ol-7-one(5),sinalexin(6),bis(2-ethylhexyl)benzene-1,2-dicarboxylate(7),pinoresinol(8),Chaenomin B(9).The antitumor activity of compounds 5~9 was tested,and it was found that compound 9 had good inhibitory activity against A549 cell line.Conclusion Compounds 2~5,7~8 were isolated from the herbs for the first time and compounds 6,9 were isolated from this genus for the first time.IC50 of 9 on A549 cell line is 29.35 μM.
