1.Exploration of the protection of mild moxibustion on ovarian function in mice with premature ovarian insufficiency by regulating AMPK/PGC-1α signal, thus inhibiting the response to oxidative stress

Objective:
To explore the protective effect of mild moxibustion on the ovary of mice with premature ovarian insufficiency (POI) and its mechanism.
Methods:
SPF female ICR mice with normal estrus cycle were divided into blank group, model group, moxibustion group and moxibustion + Compound C group according to the random number table method, with 20 mice in each group. The mice in the blank group were given normal saline daily. The mice in the other groups were gavaged with Tripterygium wilfordii polyglycosides[75 mg/(kg·d)] for 14 consecutive days for modeling. An hour later, the mice in the moxibustion group were treated with mild moxibustion every day, and bilateral "shenshu" and "guanyuan" "zhongwan" were alternately applied every other day, and each point was treated with mild moxibustion for 10 minutes each day. Thirty minutes after gavaging Tripterygium wilfordii polyglycosides, the mice in the moxibustion + Compound C group were given intraperitoneal injection of Compound C (10 mg/kg); an hour later, mild moxibustion was applied same as the moxibustion group. The mild moxibustion was applied once a day for 14 consecutive days. The protective effect of moxibustion on ovary was evaluated by ovarian index, rate of estrus cycle disorder, ovarian tissue morphology, the number of follicles at all levels and serum sex hormone levels. Quantitative real-time polymerase chain reaction and immunohistochemistry were used to detect the mRNA and protein expression of adenosine 5′-monophosphate-activated protein kinase (AMPK) and peroxisome proliferator-activated receptor gamma coactivator 1α (PGC-1α) in ovarian tissue. Serum sex hormones oxidative stress markers, adenosine triphosphate (ATP) and reactive oxygen species (ROS) levels in ovarian granulosa cells were detected by enzyme-linked immunosorbent assay, luciferase method and chemifluorescence method, respectively.
Results:
Compared with the model group, the ovarian index of the moxibustion group increased (P<0.05), the rate of estrous cycle disorder decreased (P<0.05), the number of atretic follicles decreased (P<0.05), and the number of primordial follicles, primary follicles, secondary follicles and antral follicles all increased (P<0.05). The levels of serum anti-Müllerian hormone and estrogen increased (P<0.05), while the level of follicle-stimulating hormone decreased (P<0.05); the expressions of protein and mRNA of AMPK and PGC-1α were upregulated (P<0.05); the activity of serum superoxide dismutase increased, the content of malondialdehyde decreased, and the ATP level of granulosa cells increased, while the mean fluorescence intensity of ROS decreased (P<0.05). However, when Compound C was given before moxibustion intervention, the protective effect on the ovary was significantly reduced, as shown by the reduction of ovarian index (P<0.05), the rate of estrus cycle disorder increased (P <0.05), the number of atretic follicles increased (P<0.05), and the number of primordial follicles, primary follicles, secondary follicles and antral follicles decreased (P<0.05).
Conclusion
Moxibustion can protect ovarian function in mice by promoting the activation of AMPK and PGC-1α signaling, inhibiting oxidative stress response and regulating hormone levels.
2.Clinical Observation of Acupuncture Based on Huang Yuanyu's Theory"Ascending and Descending of Visceral Qi Movement"for Chronic Insomnia Disorder

Objective To observe the clinical efficacy of acupuncture in treating chronic insomnia disorder based on Huang Yuanyu's theory of"central qi ascending and descending,harmony of the four dimensions"regarding visceral qi movement.Methods Sixty patients diagnosed with chronic insomnia disorder at the Preventive Treatment Department of Yangjiang Hospital of Traditional Chinese Medicine from March 2022 to February 2023.The patients were randomly divided into an observation group and a control group according to a random number table,with 30 cases in each group.The observation group received acupuncture therapy,while the control group received oral administration of Alprazolam Tablets.The treatment duration spanned 4 weeks with 1 course per week.Clinical efficacy was evaluated after 1 month,and the changes in the Pittsburgh Sleep Quality Index(PSQI)scores and Hamilton Anxiety Rating Scale(HAMA)scores were observed.The Hamilton Depression Rating Scale(HAMD)scores were compared between groups before and after treatment.Results(1)After treatment,the PSQI scores of the two groups of patients were significantly improved(P<0.05),and the observation group was significantly superior to the control group in improving the PSQI scores,and the difference was statistically significant(P<0.05).(2)After treatment,the HAMA scores and HAMD scores in the two groups of patients were significantly improved(P<0.05),an the observation group in improvement of HAMA and HAMD was significantly superior to the control group,with statistically statistical significance(P<0.05).(3)The total effective rate was 83.33%(25/30)in the observation group and 70.00%(21/30)in the control group.The efficacy of the observation group was superior to that of the control group,and the difference was statistically significant(P<0.05).Conclusion Acupuncture based on the ascending and descending of visceral qi movement theory significantly alleviates anxiety and depressive states while improving sleep quality in patients with chronic insomnia disorder,demonstrating notable clinical efficacy.

4.Application of PDCA circulation method in critical value management in ultrasound department

Objective To analyze the current situation of the quality of critical value management in ultrasound department,and to explore the application and effect of PDCA cycle method in critical value management.Methods The quality status of emergency data during the period from January 2015 to January 2019 after the PDCA cycle was adopted was analyzed.The PDCA cycle method was applied to improve the overall quality.The reporting rate,awareness rate,qualified rate of writing,and timely rate of clinical treatment were compared before and after the PDCA cycle was adopted to evaluate the effect of PDCA cycle method.Results From January 2015 to January 2019,the reporting rate was 96.4%,98.0%,94.5%,and 96.7%,the awareness rate of critical value was 63.2%,78.6%,84.8%,and 92.2%,the qualified rate of writing was 70.4%,77.2%,85.3%,and 94.0%,and the timely rate of clinical treatment was 50.9%,71.7%,81.1%,and 85.5%,respectively.The awareness rate,the qualified rate of writing,and the timely rate of clinical treatment were significantly improved (x2=12.464,38.184,52.926,all P < 0.05).Conclusion PDCA cycle method can improve the quality of emergency management in ultrasound department.
5. Correlation between the maximum elastic modulus(Emax) shear-wave elastography and immunohistochemical profiles in breast cancer

Objective:
To investigate the correlation between the maximum elastic modulus(Emax) of breast cancer obtained by shear-wave elastography (SWE) and immunohistochemical profiles.
Methods:
One hundred and sixty patients with 171 pathologically proven breast lesions were included in this study. All of them underwent conventional ultrasound and SWE examination before surgery. The Emax and maximum diameter were obtained. Histologic grade, immunohistochemical profiles ER, PR, HER-2, Ki-67 and molecular subtypes were recorded according to pathological results after surgery. The correlation between Emax and tumor size, histological grade, ER, PR, HER-2, Ki-67 and molecular subtypes were analyzed.

Objective To investigate the value of early?phase enhancement ratio combined with peripheral vascular diameter in the differential diagnosis of benign and malignant breast lesions using 3.0 T dynamic contrast?enhanced magnetic resonance imaging (DCE?MRI). Methods Sixty seven cases of patients (35 with malignant lesions and 32 with benign lesions in the breasts) were retrospectively analyzed. Their diagnoses were confirmed by surgery and pathology and all the patients underwent breast MRI plain scan and DCE?MRI in the two weeks before surgery. Lesion ROIs were drawn and time?signal intensity curves in the DCE?MRI were generated. Early?phase enhancement rate, time to peak, early?phase enhancement ratio, numbers of tumor vessel within 3 cm of the lesion and diameter of the largest vessel were recorded. Mann?Whitney U test was used to compare the difference of DCE?MRI between benign and malignant lesions, and the ROC curve was used to evaluate the efficiency of early?phase enhancement rate, early?phase enhancement ratio and vascular diameter in differentiating benign and malignant lesions. Results With breast malignant lesions, the medians of time to peak, early?phase enhancement rate, early?phase enhancement ratio, numbers of tumor vessel and vascular diameter were 2.2 s, 176.0%, 100.0%, 4 and 2.96 mm respectively, while with benign lesions of these parameters were 4.7 s, 113.3%, 81.9%, 0 and 0.00 mm respectively, and the differences were statistically significant (all P<0.05).When early?phase enhancement rate was used for differential diagnosis of breast benign and malignant lesions, the area under the ROC curve was 0.702 and the sensitivity and specificity were 82.86%and 56.25%with a threshold of 120.0%. When early?phase enhancement ratio was used, the area under the ROC curve was 0.854 and the sensitivity and specificity were 94.29% and 68.75% with a threshold of 86.0%. When peripheral vascular diameter was used, the area under the ROC curve was 0.896 and the sensitivity and specificity were 74.29%and 84.38% with a threshold of 2.78 mm. When early?phase enhancement ratio was combined with peripheral vascular diameter, the area was 0.925 and the sensitivity and specificity were 97.14% and 62.50%. Conclusion In the differential diagnosis of benign and malignant breast lesions under DCE?MRI, early?phase enhancement ratio combining with peripheral vascular diameter has improved sensitivity.
