1.Treatment of Attention Deficit Hyperactivity Disorder with Comorbid Tic Disorder in Children from the Perspective of Ministerial Fire Scorching Yin and Internal Stirring of Deficient Wind

Attention deficit hyperactivity disorder (ADHD) is often accompanied by tic disorder. The core pathogenesis is considered to be ministerial fire scorching yin and internal stirring of deficient wind, which leads to disharmony between the body and spirit, resulting in clinical manifestations. The treatment principles emphasize nourishing yin fluids, calming ministerial fire, and extinguishing endogenous wind (内风). The method of nourishing yin fluids is applied throughout the entire treatment process, commonly using ingredients such as Shudihuang (Rehmanniae Radix Praeparata), Shanzhuyu (Corni Fructus), Gouqizi (Lycii Fructus), Wuweizi (Schisandrae Chinensis Fructus), and Tusizi (Cuscutae Semen). These are combined with approaches to harmonize the zang-fu organs, primarily including extinguishing liver wind, clearing heart fire, nourishing kidney water, and strengthening spleen earth, thereby stabilizing ministerial fire and extinguishing endogenous wind. Additionally, emotional regulation and smoothing emotional constraint are essential to improve clinical symptoms in children with ADHD comorbid with tic disorder.
2.Association between warning signs of psychological and behavioral development problems with emotional and behavioral problems in preschool children

Objective:
To explore the association between warning signs of psychological and behavioral development problems with emotional and behavioral problems in preschool children in Bao an District, Shenzhen, so as to provide an empirical basis for optimizing psychological screening strategies in kindergartens.
Methods:
From September 2023 to August 2024, a total of 49 804 preschool children aged 4-6 years from all 401 kindergartens in Bao an District were enrolled as study subjects. The Warning Signs Checklist for Screening Psychological, Behavioral and Developmental Problems of Children and the parent version of the Strengths and Difficulties Questionnaire (SDQ) were used to assess children s developmental status and emotional and behavioral problems, respectively. Multivariable Logistic regression analysis was used to examine the association between warning signs and emotional and behavioral problems, stratified by sex.
Results:
The overall positive screening rate for developmental warning signs was 1.5%, and the detection rate for high risk in the SDQ total difficulties score was 6.3%. Multivariable Logistic regression analysis revealed that after adjusting for age, children who screened positive for warning signs exhibited a significantly higher risk of elevated SDQ total difficulties and subscale scores compared to those who screened negative, across both sexes (a OR boys =1.66-13.42, a OR girls =2.04-22.15, all P <0.01). The only exceptions were gross motor skills and conduct problems in boys, and personal social skills and conduct problems in girls. Notably, abnormalities in the personal social domain demonstrated the strongest association with emotional/behavioral problems (a OR boys =7.72-13.42, a OR girls =3.88-22.15), followed by the language domain (a OR boys =4.63-9.23, a OR girls =3.78-14.41) (all P <0.01).
Conclusion
A positive screening result for warning signs, particularly in the personal social and language domains, serves as a strong indicator of emotional and behavioral problems in preschool children.

The core pathogenesis of attention deficit hyperactivity disorder (ADHD) lies in deficiency, stasis and stagnation. Deficiency arises from kidney essence depletion and spleen dysfunction in transportation and transformation, leading to inadequate nourishment of the marrow sea. Stasis caused by qi deficiency leads to obstruction in channels and collaterals, resulting in obstructed marrow transport. Stagnation is associated with the excess of the five minds transforming into fire, which scorches the brain orifices and leads to loss of control over marrow utilisation. Based on this, a "supplementation-unblocking-regulation" therapeutic approach is proposed. For deficiency, the focus is on supplementing kidney and fortifying spleen, and replenishing the marrow sea. For stasis, the priority is to unblock and open the orifices, and clear the marrow channels. For stagnation, the core is to clear fire and contain the mind, regulate and restore vital activity. In clinical practice, it is necessary to identify the primary and secondary pathogenic mechanisms and apply dynamic, combined treatment, integrating Chinese herbal medicine, acupuncture, and guiding exercises throughout the process, aiming to provide a reference for the diagnosis and treatment of ADHD with traditional Chinese medical.

As new evidence emerges, treatment strategies toward the functional cure of chronic hepatitis B are evolving. In 2019, a panel of national hepatologists published a Consensus Statement on the functional cure of chronic hepatitis B. Currently, an international group of hepatologists has been assembled to evaluate research since the publication of the original consensus, and to collaboratively develop the updated statements. The 2.0 Consensus was aimed to update the original consensus with the latest available studies, and provide a comprehensive overview of the current relevant scientific literatures regarding functional cure of hepatitis B, with a particular focus on issues that are not yet fully clarified. These cover the definition of functional cure of hepatitis B, its mechanisms and barriers, the effective strategies and treatment roadmap to achieve this endpoint, in particular new surrogate biomarkers used to measure efficacy or to predict response, and the appropriate approach to pursuing a functional cure in special populations, the development of emerging antivirals and immunomodulators with potential for curing hepatitis B. The statements are primarily intended to offer international guidance for clinicians in their practice to enhance the functional cure rate of chronic hepatitis B.
5.Evaluating value of multimodal MRI combined with digital breast 3D tomography on lymphatic vascular infiltration in patients with invasive breast cancer-non special type mass type

Objective To explore the value of multimodal MRI combined with digital breast 3D tomography(DBT)in evaluating lymphatic vascular infiltration(LVI)in patients with invasive breast cancer-non special type(IBC-NST)mass type.Methods A total of 102 patients with IBC-NST mass type were divided into LVI group and non LVI group based on whether LVI occurred.The influencing factors of LVI were analyzed by using multivariate logistic regression.Clinical value of multimodal MRI combined with DBT in evaluating LVI in patients with IBC-NST mass type were analyzed by receiver operating characteristic(ROC)curve.Results Multivariate logistic analysis showed that apparent diffusion coefficient(ADC)value,maximum tumor diameter and peritumoral edema were independent risk factors for LVI in IBC-NST mass type(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of ADC value,maximum tumor diameter and peritumoral edema alone and in combination for LVI in patients with IBC-NST mass type were 0.749,0.655,0.638 and 0.791,respectively.Conclusion ADC value and its combined application model have high efficacy in evaluating LVI in patients with IBC-NST mass type.

Hematologic malignancies are a severe threat to human health.CAR-T cell therapy,as a highly effective treatment for hematologic malignancies,has become one of the fastest developing cancer immunotherapies in recent years.To obtain highly effective and low-toxicity CAR-T cells,through continuous development and exploration,CAR-T cell therapy has achieved a series of significant breakthroughs in treating hematologic malignancies,including humanized CAR-T cell technology,dual-target and multitarget CAR-T cell technology,universal CAR-T cell technology,and CAR-T cell combination strategy technology.Humanized CAR-T cell technology reduces immunogenicity through humanized modification.Dual-target and multi-target CAR-T cell technology recognizes two or more tumor antigens,minimizes antigen escape and enhance efficacy.Universal CAR-T cell technology solves problems such as high cost and accessibility issues.The CAR-T cell combination strategy technology effectively deals with CAR-T recurrence and enhances efficacy and safety by combining different treatment methods.Although CAR-T cell therapy has achieved significant breakthroughs in the treatment of hematologic malignancies,it still faces challenges such as drug resistance,antigen escape,cytotoxicity,and posttreatment recurrence.This review presents the significant breakthroughs,existing problems,and solutions for CAR-T cell precise therapy for hematological malignancies.
