1.Comprehensive evaluation of application effect of personalized narrative nursing in elderly patients with stroke hemiplegia

We aimed to explore the value of personalized narrative intervention in the nursing care of elderly patients with stroke hemiplegia(SH).A total of 68 elderly patients with SH were selected and divided into two groups using the ball-rolling method,with 34 cases in each group.The control group received routine nursing care,while the observation group received routine care and personalized narrative nursing.The nursing outcomes of the two groups were compared.Compared with the control group,the observation group had lower scores in anxiety and depression(P<0.05),and higher scores in the Functional Evaluation of Cognitive State-Short Screening(FECS-SS)(P<0.05).The rehabilitation effects of normal rate in the observation group was higher than that in the control group(P<0.05).Personalized narrative nursing for SH patients can effectively relieve the patients'negative emotions,enhance their compliance with rehabilitation,and improve the outcomes of rehabilitation.
2.Mechanism of Congrong Shujing granules in treatment of Parkinson's disease based on network pharmacology,molecular docking and parallel reaction monitoring technology

Aim To explore the mechanism of Con-grong Shujing granule(CSGs)in the treatment of Par-kinson's disease(PD)by network pharmacology,mo-lecular docking and parallel reaction monitoring(PRM)technology.Methods The active components of CSGs and the target genes of Parkinson's disease were obtained through the database.The intersection targets of drugs and diseases were selected to construct the"drug-active ingredient-target"and protein interac-tion network.The intersection target genes were impor-ted into David database for GO and KEGG enrichment analysis,and the main components were docked with key targets.27 SD rats were randomly divided into the normal group(n=9),model group(n=9)and treat-ment group(n=9).On day 1,7 and 14 of treatment,PRM analysis was used to detect the changes in the specific peptides of key target proteins in the substantia nigra of rats.Results The main components of CSGs wereTanshialdehyde,Baicalein,Quercetin and Kaempferol.The most important targets for the treat-ment of PD were TP53,AKT1,EGFR,HSP90 AA1 and STAT3.KEGG analysis mainly enriched MAPK,PI3K-Akt and neurotrophic factor signaling pathway.The molecular docking between core components and core targets showed that the binding of drugs and targets had good activity.PRM analysis of key proteins found that the target peptide expression levels of ASK1,JNK1 and JNK3 were different among groups(P<0.05).Con-clusion CSGs can alleviate ERS,inhibit apoptosis and play a neural protective role through the ASK1-JNK pathway.
3.Impact of Ultra Early Neurorehabilitation Combined with Cognitive Intervention Training on Neurological Function,Psychological Status,and Quality of Life of Stroke Patients

Objective:To observe the impact of ultra early neurorehabilitation combined with cognitive intervention training on the psychological state,neurological function,and quality of life of stroke patients.Methods:89 stroke patients who were admitted to our hospital from April 2023 to May 2024 were divided into control group(conventional rehabilitation intervention combined with cognitive intervention training,44 cases)and study group(control group combined with ultra early neurological rehabilitation,45 cases)by using random number table method.The neurological function,daily self-care ability,psychological state,quality of life,and satisfaction between two groups were compared.Results:14 d after intervention,the study group had lower scores on the national institutes of health stroke scale(NIHSS),self rating anxiety scale(SAS),and self rating depression Scale(SDS)compared to the control group,had higher scores of the comprehensive quality of life evaluation questionnaire-74(CQOLI-74)and the modified barthel index(MBI)(P<0.05).The overall satisfaction rate of the study group was higher than that of the control group(P<0.05).Conclusion:Ultra early neurorehabilitation combined with cognitive intervention training can effectively improve the neurological function and daily self-care ability of stroke patients,reduce anxiety and depression,improve their quality of life,and achieve good patient satisfaction.
4.Effect of multiple modified process management intervention on cardiac function and psychological state in patients with severe CHD

Objective:To explore the effect of multiple modified process management intervention on cardiac func-tion,psychological state,stress level,sleep quality and adverse events in patients with severe coronary atherosclerot-ic heart disease(CHD).Methods:This randomized controlled study enrolled 130 severe CHD patients who were treated in Affiliated Beijing Shijitan Hospital of Capital Medical University between January 2020 and May 2023.Patients were randomly divided into control group(n=65)and intervention group(n=65).Patients in the control group were treated with routine management intervention,while those in the intervention group were given addi-tional multiple modified process management interventions.Both groups were intervened for 4 weeks.Cardiac func-tion,levels of norepinephrine(NE)and cortisol(COR),scores of Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS),General Comfort Questionnaire(GCQ),Pittsburgh Sleep Quality Index(PSQI),and the incidence of adverse events during intervention were compared between the two groups.Results:Compared to those in control group after intervention,patients in intervention group had significant higher left ventricular ejection fraction(LVEF)[(57.81±2.15)%vs.(50.11±2.99)%]and GCQ score[(95.88±5.37)points vs.(75.81±6.67)points](P<0.001 all),and significant lower left ventricular end-diastolic volume(LVEDV)[(109.81±5.37)ml vs.(129.26±5.17)ml],left ventricular end-systolic volume(LVESV)[(50.85±3.08)ml vs.(66.02±3.77)ml],levels of NE[(61.56±5.49)pg/ml vs.(69.86±5.03)pg/ml],COR[(85.63±5.19)ng/ml vs.(92.28±6.57)ng/ml],scores of SAS[(30.06±5.19)points vs.(49.51±5.85)points],SDS[(31.86±4.51)points vs.(40.00±5.10)points]and PSQI[(8.72±1.58)points vs.(13.89±2.40)points],and incidence of ad-verse events(4.69%vs.23.44%)(P<0.01 all).Conclusion:The multiple modified process management interven-tion may improve the cardiac function,adverse psychological state,stress level,sleep quality and reduce the inci-dence of adverse events in patients with severe CHD.

6.Correlation between Serum IGF-1,IGFBP-3,BAP and Abundance of Intestinal Flora in Children with Idiopathic Short Stature

Objective To explore the correlation between serum insulin-like growth factor1(IGF-1),insulin-like growth factor binding protein3(IGFBP-3),bone-specific alkaline phosphatase(BAP)and abundance of intestinal flora in children with idiopathic short stature(ISS).Methods A total of 86 children with ISS who were treated with short-acting recombinant growth hormone for the first time in Department of Endocrinology of Baoding Hospital,Beijing Children's Hospital,Capital Medical University,were enrolled from July 2021 to June 2023.Before treatment and at 6 months after treatment,physical indexes(height,weight)and bone age indexes[bone age index(BAI),bone age difference(BAD)]were detected with the same instrument at the same place.The fasting venous blood was collected to detect levels of serum IGF-1,IGFBP-3 and BAP by enzyme-linked immunosorbent assay.The abundances of intestinal floras were monitored by 16s rDNA high-throughput sequencing technology combined with bioinformation analysis technology.A total of 40 healthy children during the same period were enrolled as control group,and differences in different indexes within the same groups or between the two groups were compared by independent sample t test or paired t test.The correlation between abundances of intestinal floras and levels of serum IGF-1,IGFBP-3 and BAP was analyzed by Pearson correlation analysis.Results Compared with the control group.the height,weight,BAD,BAI,levels of serum IGF,IGFBP-3 and BAP,Chaol,Shannon and Pielou e of ISS patients decreased before treatment,and the differences were statistically significant(t=7.119~44.143,all P<0.05).After treatment,height,weight,BAD,BAI,levels of serum IGF,IGFBP-3 and BAP,Chaol,Shannon and Pielou e were increased in ISS children(t=3.145~39.188),the above indexes were still lower than those in control group(t=2.389~69.893),the differences were statistically significant(all P<0.05),respectively.In intestinal floras,dominant bacterial groups at phylum level were Firmicutes,Bacteroidetes,Actinobacteria and Proteobacteria,while dominant bacterial groups at genus level were Blautia,Bifidobacterium,Faecalibacterium and Bacteroides.Compared with the control group,before treatment,the abundances of Actinobacteria and Proteobacteria increased in the ISS group,while the abundances of Bacteroidetes,Bifidobacteria and Bacteroides decreased,and the differences were statistically significant(t=25.224~46.345,all P<0.05).After treatment,abundances of Bacteroidetes,Bifidobacterium and Bacteroides in the ISS group increased compared to before treatment,while abundance of Proteobacteria decreased,and the differences were statistically significant(t=10.055~39.245,all P<0.05).The abundances of Bacteroidetes,Bifidobacterium and Bacteroides in ISS group were lower than those in control group,while abundance of Proteobacteria was higher than that in control group,and the differences were statistically significant(t=6.290~20.540,all P<0.05).The abundances of Bacteroidetes,Bifidobacterium and Bacteroides were positively correlated with levels of serum IGF-1,IGFBP-3 and BAP(r=0.300~0.759,all P<0.05).Conclusion The abundances of intestinal floras are decreased,and the structure is abnormal in children with ISS.The abundances of beneficial bacteria in intestinal floras are related to levels of serum IGF-1,IGFBP-3 and BAP.In clinical treatment of ISS,combined treatment with intestinal flora disorders may be conducted.

Isoflavones which mainly distributed in leguminous plants have plenty of health benefits.Isoflavone synthase(IFS)is a membrane-associated cytochrome P450 enzyme(CYP450)which carries out the unique aryl-ring migration and hydroxylation.So far,few crystal structures of plant P450s have been obtained.We determined the crystal structure of IFS from Medicago truncatula at 1.9 ? by MAD method using a selenomethionine substituted crystal and conducted molecular docking and mutagenesis study.The structure of IFS complexed with imidazole exhibits the helix Ⅰa-loop-helix Ⅰβ motif which cor-responds to helix Ⅰ of other P450s.Compared with structures of common P450s,IFS/imidazole structure contains an extra domain,i.e.,the γ-domain.The structure reveals a homodimer in which the γ-domain of one molecule interacts with the β-domain of another.The plane of heme group makes an angle of ap-proximately 40° with the helix Ⅰa-loop-helix Ⅰβ motif.Molecular docking combined with mutagenesis study suggested that Trp-128 and Asp-300 might play important roles in substrate binding and recogni-tion.Phe-301,Ser-303 and Gly-305 from the helix Ⅰa-loop-helix Ⅰβ motif may play important roles in the aryl-ring migration.These novel structural features reveal insights into the unique reaction mechanism of IFS and provide a basis for engineering IFS in leguminous crops for health purpose.

Following the release of the Technical Requirements on Quality Control and Standard Establishment of Traditional Chinese Medicine Formula Granules by the National Medical Products Administration in 2021, Chinese Pharmacopoeia Commission has promulgated 296 national drug standards so far, and most provinces have started the work of establishing provincial standards as supplements. The promulgation of standards fostered high-quality development of the industry. Since the implementation of national and provincial standards for more than three years, enterprises have gained deep understanding and hands-on experiences on the characteristics, technical requirements, production process, and quality control of traditional Chinese medicine(TCM) formula granules. Meanwhile, challenges have emerged restricting the high-quality development of this industry, including how to formulate quality control strategies for medicinal materials and decoction pieces, how to reduce manufacturing costs, and how to improve the pass rate and product stability under high standards. Based on the work experiences from standard management and process research, this article analyzed the distribution of sources, processing methods, dry extract rate ranges, process requirements for volatile oil-containing decoction pieces, control measures of safety indices, characteristics and trends of setting characteristic chromatograms or fingerprints, characteristics and trends of setting content ranges, and main differences between national standards and provincial standards. On the one hand, this article aims to present main characteristics for deeply understanding different indicators in standards and provide basic ideas for establishing quality and process control systems. On the other hand, from the perspective of industrial practice, suggestions are put forward on the important aspects that need to be focused on in the quality and process control of TCM formula granules.

9.Identification and expression analysis of AP2/ERF family members in Lonicera macranthoides.

The AP2/ERF transcription factor family is a class of transcription factors widely present in plants, playing a crucial role in regulating flowering, flower development, flower opening, and flower senescence. Based on transcriptome data from flower, leaf, and stem samples of two Lonicera macranthoides varieties, 117 L. macranthoides AP2/ERF family members were identified, including 14 AP2 subfamily members, 61 ERF subfamily members, 40 DREB subfamily members, and 2 RAV subfamily members. Bioinformatics and differential gene expression analyses were performed using NCBI, ExPASy, SOMPA, and other platforms, and the expression patterns of L. macranthoides AP2/ERF transcription factors were validated via qRT-PCR. The results indicated that the 117 LmAP2/ERF members exhibited both similarities and variations in protein physicochemical properties, AP2 domains, family evolution, and protein functions. Differential gene expression analysis revealed that AP2/ERF transcription factors were primarily differentially expressed in the flowers of the two L. macranthoides varieties, with the differentially expressed genes mainly belonging to the ERF and DREB subfamilies. Further analysis identified three AP2 subfamily genes and two ERF subfamily genes as potential regulators of flower development, two ERF subfamily genes involved in flower opening, and two ERF subfamily genes along with one DREB subfamily gene involved in flower senescence. Based on family evolution and expression analyses, it is speculated that AP2/ERF transcription factors can regulate flower development, opening, and senescence in L. macranthoides, with ERF subfamily genes potentially serving as key regulators of flowering duration. These findings provide a theoretical foundation for further research into the specific functions of the AP2/ERF transcription factor family in L. macranthoides and offer important theoretical insights into the molecular mechanisms underlying floral phenotypic differences among its varieties.

10.Application progress on functional insoles in the prevention and treatment of diabetic foot.

Diabetic foot (DF) is one of the most serious chronic complications of diabetes. The incidence rate among global diabetes patients is as high as 15% to 25%, and about 50% of patients will develop contralateral foot ulcers within 5 years after the first unilateral ulcer. As a non-invasive prevention and control solution, the application progress of functional insoles is mainly reflected in the following aspects:(1) Material innovation. The application of new composite materials and smart materials has significantly enhanced the pressure reduction effect and comfort. (2) Structural optimization. The development of multi-layer design and local pressure reduction structure has achieved more precise pressure distribution regulation. (3) Manufacturing process. 3D printing and parametric design have enabled the personalized customization of functional insoles. (4) Intelligent monitoring. It integrates functions such as pressure sensing and temperature monitoring, achieving real-time monitoring and early warning of foot conditions. Clinical research has confirmed that personalized functional insoles could reduce the incidence of foot ulcers and shorten the healing time of ulcers. At present, the research hotspots mainly focus on the development of smart materials, the construction of multi-functional integration and remote monitoring systems. However, in-depth research is still needed in the aspects of biomechanical mechanisms, standardized evaluation systems and long-term efficacy assessment. The development of future functional insoles should focus on the coordinated advancement of "personalization-intelligence-standardization", with the aim of providing more effective solutions for the prevention and treatment of DF.
