ObjectiveTo construct a risk prediction model for frequent acute exacerbations of chronic obstructive pulmonary disease (COPD) under disease-syndrome combination, thus providing decision support for precise clinical intervention. MethodsA total of 2 029 patients with acute exacerbations of COPD admitted to the First Affiliated Hospital of Anhui University of Chinese Medicine from January 2020 to August 2024 were retrospectively included. These patients were classified into groups of frequent acute exacerbations (≥2 times/year) and infrequent acute exacerbations (<2 times/year) according to the hospitalization times per year. Risk factors were screened by LASSO regression combined with logistic regression, and a nomogram model was constructed. The model performance was assessed based on the area under the curve (AUC), calibration curves, and decision curve analysis (DCA). ResultsThe differences in baseline characteristics between the frequent acute exacerbations group (1 196 cases) and infrequent acute exacerbations group (833 cases) were not statistically significant. LASSO regression combined with multivariate logistic regression screened the following independent risk factors: body mass index (BMI), hospitalization days, number of smoking years, place of residence, use of noninvasive ventilators, oxygen-demanding therapy, liver cirrhosis, use of systemic glucocorticosteroids, and traditional Chinese medicine syndrome (phlegm and stasis obstructing the lung). The nomogram model showed good discrimination and calibration in both the training set (AUC=0.748) and validation set (AUC=0.774). ConclusionThe risk prediction model for frequent acute exacerbations of COPD, integrating traditional Chinese medicine syndrome, constructed in this study has high accuracy. It can provide a scientific basis for early clinical identification of high-risk patients and individualized intervention.

Aim To explore the mechanism of Lycium barbarum glycopeptide(LbGP)improving aging in rat primary ovarian granulosa cells. Methods This study divided the cells into a normal group,a DOX group, and four different LbGP concentration treatment groups post-DOX intervention. Results Cell proliferation was assessed using CCK-8,EDU,and Ki67 assays, while aging markers and mitochondrial function-related fac-tors were detected using immunofluorescence and West-ern blotting. The results showed that, compared to the DOX group,LbGP treatment significantly increased cell viability(P<0.05)and promoted proliferation(P<0.05).Post LbGP treatment, the β-galactosidase-posi-tive area in cells was significantly reduced compared to the DOX group(P<0.05).Immunofluorescence re-sults indicated that, compared to the DOX group, levels of p21 and γH2AX significantly decreased(P<0.05),while pRB increased(P<0.05)after LbGP treatment.Western blot results showed that, compared to the DOX group, the aging phenotype proteins p21 and p53 significantly decreased(P<0.05),and pRB notably increased(P<0.05)in the LbGP treatment group. The release of cytC into the cytoplasm and the activated caspase-9 significantly decreased(P<0.05);levels of CAMKK2,pAMPK,and mitochondrial calcium homeostasis regulator MCU increased(P<0.05);nuclear energy metabolism-related proteins SirT1,PGC1α/β and ATP5A1 significantly increased(P<0.05);compared to the DOX group,ROS levels significantly decreased after LbGP treatment(P<0.05).Conclusions The results suggest that LbGP can ameliorate DOX-induced aging in rat primary ovar-ian granulosa cells, potentially through the upregulation of the CAMKKβ/AMPK signaling pathway, thereby im-proving mitochondrial calcium homeostasis and increas-ing the expression levels of cell energy metabolism-re-lated regulatory proteins. This provides an experimen-tal basis for LbGP's potential role in supporting the im-provement of ovarian function.

Objective To investigate the effects of different hemostatic devices on the safety and comfort of compression hemostasis in the patients undergoing transradial coronary intervention(TRI).Methods From March to September 2023,900 patients undergoing TRI in the cardiology department of some hospital were divided into three groups according to the randomized numerical table method:STEPTY P hemostatic patch group(group A),spinning hemostatic group(group B)and balloon hemostatic group(group C),with 300 cases in each group. The three groups were compared in terms of the hemostatic effect, local complications and comfort of the puncture sites. Results The three groups had no statistical differences in hematoma grade and incidence rates of hematoma and hemorrhage(all P>0.05).There were significant differences in the occurrence of skin injury among the three groups(P=0.011),and group A had the skin injury incidence rate(1.0%)statistically lower than those of group B(4.3%)and C(5.3%)(P<0.05).There were obvious differences among the three groups in the incidence rates of pain on the operative side, palm swelling and limb numbness(P<0.05),and group A had the incidence rates significantly lower than those of group B and C(all P<0.05).Conclusion All the three compression hemostatic devices can achieve safe and effective post-TRI hemostasis. When compared with the other two devices,STEPTY P hemostatic patch is more effective in reducing the probability of related complications and improving patient comfort while effectively compressing the puncture site for hemostasis.[Chinese Medical Equipment Journal,2025,46(1):49-54]

Objective To establish a stable, reliable, and clinically relevant porcine model of endotoxin-induced acute respiratory distress syndrome(ARDS).Methods Ten 8-month-old male Bama minipigs were deeply sedated, followed by invasive mechanical ventilation and electrocardiographic monitoring.Lipopolysaccharide(LPS)was intravenously pumped at 600 μg/(kg·h)for 3 hours, then maintained at 15 μg/(kg·h)thereafter.Dynamic monitoring was performed at five time points after LPS injection(LPS 0,1,3,5,and 8 h),including arterial blood gas analysis and chest computed tomography(CT)scans.Pathological examination of lung tissues obtained via bronchoscopic biopsy(HE staining and transmission electron microscopy)was conducted. These indicators were comprehensively used to evaluate the success of the animal model. Results At 5 hours after LPS administration,8 minipigs developed symptoms such as skin cyanosis, elevated body temperature, and respiratory distress. The oxygenation index decreased to<300 mmHg.Chest CT scans showed diffuse pulmonary infiltrates.Histopathology revealed alveolar edema and hyaline membrane formation.Transmission electron microscopy demonstrated disruption of pulmonary blood-air barrier, depletion of lamellar bodies in type Ⅱ pneumocytes, inflammatory cell infiltration, and exudation of plasma proteins and fibrin.Compared with LPS 0 h,at LPS 8 h,the oxygenation index and arterial blood pH were significantly decreased(P<0.001),while blood lactic acid and serum potassium were significantly increased(P<0.05);serum calcium and base excess were significantly decreased(P<0.05),and the lung injury score based on HE-stained lung sections was significantly increased(P<0.01).Conclusion The porcine ARDS model established by continuous LPS injection can dynamically simulate the pathophysiological characteristics and typical pathological manifestations of clinical septic ARDS,making it an effective tool to study the pathogenesis, prevention, and treatment strategies of septic ARDS.
