1.Mechanism of horizontal transfer of linezolid resistance gene optrA media-ted by plasmids in Enterococcus faecalis

Objective To study the horizontal transfer mechanism of optrA gene-carrying linezolid-resistant En-terococcus faecalis(LREf)among clinical Enterococcus faecalis mediated by plasmids.Methods Non-repeated LREf from a tertiary first-class hospital in Kunming City from August 2022 to August 2023 were collected and iden-tified by matrix-assisted laser desorption/ionisation-time of flight mass spectrometry(MALDI-TOF MS).Antimi-crobial susceptibility testing was performed by VITEK 2 Compact,disc diffusion method and microbroth dilution method,optrA gene was detected by polymerase chain reaction(PCR),molecular biology characteristics of LREf was analyzed by whole-genome sequencing(WGS),LREf as the donor strain and clinically isolated Enterococcus faecalis as the recipient strain for conjugation test.Results A total of 17 LREf strains were collected,optrA gene was detected from plasmids of 12 LREf strains.Multiple resistance genes and virulence genes were detected.12 LREf strains were mainly ST16 type(50.0%).Among the 24 transconjugates in the conjugation test,8 were suc-cessfully conjugated,with a conjugation rate of 33.3%.Further analysis revealed that IS1216E insertion sequences presented at upstream and downstream of the optrA gene on the plasmid before and after conjugation of strains,and formed IS1216E-fexA-optrA-erm(A)-IS1216E-like transposon units.Conclusion The optrA gene can be horizon-tally transferred among clinical Enterococcus faecalis by mediating of plasmid of genes carrying both erm(A)and fexA,and the insertion sequence IS1216E plays an important role in its horizontal transfer process.
2.Clinical evaluation of bonding effectiveness of different adhesives to sclerotic dentin

Objective To compare the fifth-generation total-etch adhesive system,the sixth-generation self-etching adhesive system and the eighth-generation universal adhesive system in terms of the bonding effectiveness for sclerotic dentin.Methods A total of 150 patients with caries in the occlusal surface of the mandibular right first molar who were examined between January 2022 and June 2022 at the dental department of some hospital were selected prospectively,and then enrolled into an ASB group,a SEB group and a SBU group and underwent Class Ⅰ cavity restorations on the mandibular right first molar,with 50 affected teeth in each group.The adhesives involved in included 3MTM AdperTM Single Bond 2(the fifth-generation total-etch adhesive system)for the ASB group,ClearfilTM SE Bond(the sixth-generation self-etching adhesive system)for the SEB group and 3MTM Single Bond Universal(the eighth-generation universal adhesive system)for the SBU group.Postoperative sensitivity was evaluated using a visual analog scale(VAS)1 week,6 months,1 year and 2 years after the operation in the three groups,and clinical performance—including color match,anatomic form,surface texture,marginal adaptation,marginal discoloration and secondary caries—was assessed based on the modified United States Public Health Service(USPHS)criteria at the same follow-up intervals.SPSS 27.0 software was used for data analysis.Results All the three groups showed a 100%restoration retention rate over the 2-year follow-up period.At 1 week postoperatively,2 cases of mild allergic reactions occurred in both the ASB group and the SEB group;these reactions resolved by 6 months postoperatively.No allergic reactions were observed in the SBU group.The three groups showed no statistically significant differences in sensitivity(P>0.05)and color match,anatomic form,surface texture,marginal adaptation,marginal discoloration and secondary caries(P>0.05).Conclusion The eighth-generation universal adhesive 3MTM Single Bond Universal demonstrates comparable clinical effectiveness to the fifth-and sixth-generation adhesives in Class Ⅰ cavity restorations,and can be used for dental restorations with high requirements for operational efficiency and universal adaptability.[Chinese Medical Equipment Journal,2025,46(8):72-78]
3.Summary of the best evidence for the prevention and management of chyle leak after thyroid cancer surgery

Objective:To retrieve and obtain relevant evidence at home and abroad on the prevention and management of chylous leakage after thyroid cancer surgery, and provide reference for clinical practice.Methods:Based on the 6S pyramid model system, evidence related to the prevention and management of chylous leakage after thyroid cancer surgery in the databases of UpToDate, BMJ best practice, Guidelines International Network (GIN), National Guideline Clearinghouse (NGC), PubMed, EMbase, Wanfang Database, China National Knowledge Infrastructure (CNKI) and other databases, including guidelines, expert consensus, systematic reviews, evidence summary, clinical decisions, and position statements. The retrieval time was from database establishment to June 1st 2024.Results:A total of 16 articles were included, including 5 guidelines, 9 expert consensus, and 2 systematic reviews. The 30 best pieces of evidence were summarized from 6 aspects: preoperative evaluation and preparation, intraoperative prevention, diagnosis, diet and nutrition management, monitoring and treatment, and health education.Conclusions:This study summarized relevant evidence and provides reference for medical staff to formulate intervention measures and management plans for chyle leakage after thyroid cancer surgery.
4.Screening of material basis for Rhei Radix et Rhizoma on improving human umbilical vein endothelial cell injury before and after carbonization based on spectrum-effect relationship

AIM To screen the material basis for Rhei Radix et Rhizoma on improving human umbilical vein endothelial cell(HUVECs)injury before and after carbonization.METHODS The HPLC fingerprints were established,after which difference analysis was performed by orthogonal partial least squares discriminant analysis.The LPS-induced HUVECs injury model was established,then NO,MDA levels and SOD activity were detected.Gray correlation analysis and partial least squares regression were adopted in the investigation of spectrum-effect relationship,and active constituents were screened.RESULTS There were 22 and 20 common peaks in the fingerprints for 17 batches of raw products and carbonized products,respectively,along with the similarities of more than 0.8.Twelve main difference components were observable,among which gallic acid,5-hydroxymethylfurfural,catechin,aloe rhodopsin-8-O-β-D-glucoside,rhubarbic acid-8-O-β-D-glucoside and sennosides A were identified.The carbonized products demonstrated stronger effect on improving HUVECs injury than the raw product.The correlations of common peaks were more than 0.5 in the fingerprints for raw products and carbonized products,and peaks 3,5,11,12,15,19,23 exhibited significant effects on their efficacy(VIP values>1).CONCLUSION This accurate,reliable and reproducible method can provide a basis for clarifying the material basis for hemostatic efficacy of Rhei Radix et Rhizoma and its carbonized product.

Spine fracture and dislocation are common traumatic spinal conditions that often require surgical intervention due to compromised spinal stability. Surgical approaches include anterior, posterior, and combined anterior-posterior spinal procedures. According to the specific surgical requirements, patients may be placed in the prone position or repositioned between prone and supine positions during surgery. Intraoperative repositioning has become an essential step in patient positioning. However, during repositioning, patients with spinal fracture and dislocation are at increased risk for complications such as hemodynamic instability, nerve injury, and pressure injuries to the skin and soft tissue. Notably, due to the instability of the spinal cord, even minor manipulations can further exacerbate the damage, potentially leading to severe outcomes like paraplegia. Although the current clinical guidelines provide instructive recommendations for standard position, there remains no specific protocols for intraoperative repositioning in patients with spine fracture and dislocation. With a concern for the lack of clinical studies on positioning techniques, risk prevention, and operational norms for special patients, no applicable guidelines or standards are available. A consensus was required to provide clinical reference, meet the requirements of surgical treatment, and minimize the safety risks of patients caused by improper placement of positions. Professional Committee of Operating Room Nursing of Shaanxi Nursing Association organized experts in nursing management and operating room nursing from major hospitals across China to formulate Expert consensus on intraoperative repositioning for patients with spinal fracture and dislocation ( version 2025). The consensus provides 11 recommendations covering pre-repositioning preparation, intraoperative maneuvers, and post-repositioning observation, aiming to provide references for clinical standardization of the intraoperative repositioning process and protection of patients′ safety.

7.Analysis of internal exposure of some nuclear medicine staff performing 131I therapy in Hubei province from 2021 to 2023

Objective:To investigate the current status of occupational internal exposure of nuclear medicine staff performing 131I treatment, and explore the related influence factors, so as to provide a basis for protection against internal exposures. Methods:A survey was conducted using cluster random sampling method to ascertain all the nuclear medicine staff involved in 131I treatment in nuclear medicine hospitals in Hubei province for the years 2021 to 2023. The in vitro monitoring mesurement was made of the 131I activity in thyroid by using portable gamma spectrometer, and the committed effective dose was estimated. Results:The positive detection rate of 131I in treatment staff was between 18.75% and 21.12%, with an average of 20.08%. The highest value found in cleaning and nursing staff in iodine therapy workplaces. The detection rate found in two hospitals were up to 75.61% and 64.71%, respectively. The detection rate was positive for the three consecutive years. There was no statistically significant difference in the committed effective dose between different treatment positions, and the level of internal exposure was lower than the external exposure arising from nuclear medicine practice. Factors affecting detection rate of 131I in thyroid included the amount of 131I used for treatment, and disorder type for treatment, 131I dose, use or otherwise of automated radiopharmaceutical dispenser, and ventilation in treatment rooms. Conclusions:Continuous attention should be paid to the internal exposure of nuclear medicine staff for use of 131I for treatment. Ventilation should be provided regularly, as required, to the 131I treatment rooms and active rooms and the staff should wear masks during the relevant working processes.
8.Expression of serum microRNA-497 in patients with colorectal cancer and its diagnostic and prognostic values

Objective To characterize the expression patterns of serum microRNA-497(miR-497)in patients with colorectal cancer(CRC)and to investigate its associations with clinicopathological characteristics,diagnostic performance,and long-term prognostic outcomes.Methods This study retrospectively analyzed data from 122 patients with CRC admitted to the hospital between March 2020 and March 2022(CRC group),and enrolled 100 healthy individuals undergoing routine physical examinations(healthy control group)for comparison.Serum samples were collected from all participants prior to any surgical intervention,and the expression levels of miR-497 in serum were quantified using real-time quantitative polymerase chain reaction(qRT-PCR).Simulta-neously,the levels of carcinoembryonic antigen(CEA)and carbohydrate antigen 199(CA199)were measured.To investigate the association between miR-497 expression and clinicopathological characteristics,we evaluated its diagnostic performance using receiver operating characteristic(ROC)curve analysis.Furthermore,Kaplan-Meier survival analysis and Cox proportional hazards regression models were employed to assess its impact on patient prognosis.Results Compared to healthy individuals,CRC patients exhibited significantly lower serum miR-497 expression levels(P<0.001).Notably,miR-497 expression was strongly correlated with TNM stage progression and lymph node metastasis(P<0.001),but showed no significant association with tumor location,patient sex,or age.Diagnostic evaluation using ROC curves demonstrated that miR-497 achieved an AUC of 0.845 for CRC detection,outperforming CEA(AUC=0.748)and CA19-9(AUC=0.702),with DeLong's test confirming the statistically significant differences(P<0.05).Kaplan-Meier survival analysis revealed a significantly higher 3-year DFS rate among patients with high miR-497 expression(84.06%)compared to those with low expression(64.58%),with median DFS not reached in the high-expression group versus 36 months in the low-expression group(P=0.015).Multivariate Cox regression analysis confirmed that reduced miR-497 expression(HR=1.923,95%CI:1.184~3.125),advanced TNM stage(HR=2.511,95%CI:1.421~4.437),and lymph node metastasis(HR=1.753,95%CI:1.151~2.664)were independently associated with poorer disease-free survival outcomes.Conclusions Serum miR-497 is downregulated in patients with CRC and is significantly associated with tumor progression and poor prognosis.It demonstrates high diagnostic accuracy and strong potential for prognostic evalua-tion,highlighting its promise as a biomarker for auxiliary diagnosis and outcome prediction in CRC.
