OBJECTIVE:
To investigate the current status of antibiotic use for very and extremely low birth weight (VLBW/ELBW) infants in neonatal intensive care units (NICUs) of Hunan Province.
METHODS:
The use of antibiotics was investigated in multiple level 3 NICUs of Hunan Province for VLBW and ELBW infants born between January, 2017 and December, 2017.
RESULTS:
The clinical data of 1 442 VLBW/ELBW infants were collected from 24 NICUs in 2017. The median antibiotic use duration was 17 days (range: 0-86 days), accounting for 53.0% of the total length of hospital stay. The highest duration of antibiotic use was up to 91.4% of the total length of hospital stay, with the lowest at 14.6%. In 16 out of 24 NICUs, the antibiotic use duration was accounted for more than 50.0% of the hospitalization days. There were 113 cases with positive bacterial culture grown in blood or cerebrospinal fluid, making the positive rate of overall bacterial culture as 7.84%. The positive rate of bacterial culture in different NICUs was significantly different from 0% to 14.9%. The common isolated bacterial pathogens Klebsiella pneumoniae was 29 cases (25.7%); Escherichia coli 12 cases (10.6%); Staphylococcus aureus 3 cases (2.7%). The most commonly used antibiotics were third-generation of cephalosporins, accounting for 41.00% of the total antibiotics, followed by penicillins, accounting for 32.10%, and followed by carbapenems, accounting for 13.15%. The proportion of antibiotic use time was negatively correlated with birth weight Z-score and the change in weight Z-score between birth and hospital discharge (r=-0.095, -0.151 respectively, P<0.01), positively correlated with death/withdrawal of care (r=0.196, P<0.01).
CONCLUSIONS
Antibiotics used for VLBW/ELBW infants in NICUs of Hunan Province are obviously prolonged in many NICUs. The proportion of routine use of third-generation of cephalosporins and carbapenems antibiotics is high among the NICUs.

3.Effects of ClC-3 over-expression on structure and function of thyroid in mice

AIM:To study the effect of ClC-3 gene over-expression on thyroid structure and function in mice. METHODS:Three-months-old FVB mice were used to study the difference of thyroid structure and function between wild-type(WT)mouse and ClC-3 transgene mice.The expression and distribution of ClC-3 in the thyroid of mice were deter-mined by the methods of qPCR,Western blot and immunofluorescence.Behavioral monitoring was performed on the daily activities of mice.Serum concentrations of total triiodothyronine(TT3), total thyroxine(TT4)and thyrotropin(TSH) were measured by ELISA.RESULTS:Compared with the WT group,the expression of ClC-3 in the thyroid of ClC-3 trans-gene group was significantly increased(P<0.05).The thyroid gland showed obvious hyperplasia and the folliculi glandu-lae thyreoideae was significantly bigger in ClC-3 transgene mice(P<0.05).The weight loss was increased in ClC-3 trans-gene mice(P<0.05).The expression of TT3 and TT4 were significantly higher than that of WT group(P<0.05),but the change of TSH was not obvious.CONCLUSION:ClC-3 over-expression results in thyroid hyperplasia and thyroid hor-mone secretion.This study suggests that ClC-3 is likely to be involved in the synthesis of thyroid hormones.
4.Fluid-attenuated inversion recovery vascular hyperintensity: Correlation with other radiologic findings in acute ischemic stroke with middle cerebral artery occlusion

Objective: To assess fluid-attenuated inversion recovery (FLAIR) vascular hyper-intensity (FVH) and
explore its relationship with CT perfusion (CTP) penumbral/infarct core mismatch ratio and diffusion
weighted imaging (DWI) final infarct volume in acute ischemic stroke (AIS) patients with middle
cerebral artery occlusion (MCAO). Methods: The CTP and MRI images of 38 AIS patients with MCAO
were reviewed. The FVH score (longitudinal direction) [FVH score (L)] and FVH score (transverse
direction) [FVH score (T)] were quantified on the FLAIR images. The FVH score (L) (range, 0-16)
was based on a rostrocaudal extension of FVH and the FVH score (T) (range, 0-3) was based on FVH
supply of the occluded MCA territory. The mismatch ratio was calculated from the ratio of the [mean
transit time - cerebral blood volume (CBV)] lesion/CBV lesion on the CTP images. The DWI infarct
volume was measured on the DWI images. Results: The mismatch ratio was larger for the group of
FVH score (L)=7~8 than those of FVH score (L)=5~6 and FVH score (L)=3~4 (p=0.03), whereas
the DWI infarct volume was smaller (p=0.04). Similarly, the mismatch ratio of FVH score (T)=2~3
group was larger than FVH score (T)=1 group (p=0.01), whereas the DWI infarct volume was smaller
(p=0.02). Both FVH score (L) and FVH score (T) correlated positively with mismatch ratio (P=0.02,
P=0.001, respectively), but negatively with DWI infarct volume (P=0.03, P=0.004, respectively).
Conclusions: Higher FVH score is associated with larger mismatch ratio and smaller DWI infarct
volume in AIS patients with MCAO. FLAIR vascular hyperintensity may represent collateral arterial
circulation, and may play a role in protecting the ischemic penumbra.

5.Gene Analysis of Thalassemia in Han and Dai Ethnic Childbearing-aged Population of Chinese Yunnan Province.

OBJECTIVETo investigate the common mutation spectrum of α- and β-thalassemia in Yunnan childbearing-aged population.
METHODSThe common mutation types of α- or β-globin genes were detected by multiple Gap-PCR and the PCR-reversed dot blotting, and the unknown mutation types were determined by DNA sequencing in DNA samples of hypochromic microcytic anemia patients and carriers who were confirmed to be positive by serologic screaning, then the mutation types of globin in Yunnan population were analyzed statistically.
RESULTSA total of 40 kinds of mutation types were detected in 685 detected persons, among them the 3 commonest mutation types of α-globin genes were --(SEA)/αα (49.09%), -α(3.7)/αα (36.67%) and α(CS)α/αα (8.79%), the 3 commonest genetypes of β-globin gene were CD26(GAG>AAG)/N (43.78%), CD41-42(-CTTT)/N (20.1%) and CD17(AAG>TAG)/N (18.9%). There were 348 Han and 212 Dai ethnic persons in 685 cases, but their mutation of globin genes were different between these 2 ethnic groups. The results also showed that the gene mutation types were mostly concentrated in Dai ethnic individuals, since 28 of 38 detected α-β-thalassemia cases were Dai ethnic individuals.
CONCLUSIONThe mutation spectrums of α- and β-globin genes in Yunnan childbearing-aged population are diverse and different from that in other areas of China.

6.Accuracy of prenatal diagnosis of congenital heart defects by fetal echocardiography:a 7-year experience in a Chinese tertiary obstetric center

Objective To evaluate the prenatal diagnostic accuracy of fetal echocardiography for congenital heart defects.Methods Fetal echocardiographic databases from 2001 to 2007 were searched for patients with a prenatal diagnosis of congenital heart defect,medical records were obtained and the prenatal echocardiographic findings were correlated with postnatal echocardiography results or autopsy findings,if the pregnancy was terminated or the fetus died in utero.Results Prenatal diagnosis of congenital heart defects was made in 113 pregnancies at a mean gestational age of 26.8 weeks.Pathology or postnatal echocardiography was available in 79 cases(70%)and the accuracy of prenatal diagnosis was 86%(68/79).Prenatal diagnosis was accurate in 24 of 31 patients(77%)with conotruncal malformations,26 of 27 patients(96%)with septal defects,9 of 10 patients(90%)with valve abnormalities,and 5 of 6 patients(83%)with univentricular hearts.There were 4 false-positives and the positive predictive value was 95%(75/79).Conclusion Fetal echocardiography is a reliable tool for prenatal diagnosis of congenital heart defects despite Limitations for correctly diagnosing some spocific fetal heart defects.
7.Observation on controlled hypotension during the peri-operation of endoscopic sinus surgery and nursing cooperation

Objective To discuss the mode of nursing cooperation of endoscopic sinus surgery for nasal sinus and nasal cavity under controlled hypotension, and to summary and accumulate the experiences. Methods The method of nursing cooperation and the operation program of 80 cases with endoscopic sinus surgery through the way of moving down the nasal endoscope under whole-anesthetization were retrospectively analyzed. These patients were divided into the controlled hypotension group and the non-controlled hypotension group. Results The effect of decreasing blood pressure was satisfied. The bleeding amount of the controlled hypotension group was smaller than that in the non-controlled hypotension group (P<0. 05). The amount of transfused liquid was smaller than that in the non-controlled hypotension group (P<0. 05). And the operation time was obviously shortened because of proper nursing cooperation. Conclusions Controlled hypotension can obviously reduce the bleeding amount and the transfused liquid during the operation, thereby shortening the operation time. The nursing measures should lay emphasis on the cycle and the bleeding amount during the operation, the breath and close observation of nervous system after the operation, and cooperation with the doctors to timely treat the symptoms.
8.Effect of interleukin-2 on the activity of Ca2+ ATPase and Na+/K+ ATPase of sarcoplasmic reticulum and sarcolemma.

The purpose of the present study was to investigate whether interleukin-2 (IL-2) changes the activity of sarcoplasmic reticulum (SR) Ca(2+) ATPase, sarcolemmal Ca(2+)ATPase and Na(+)/K(+) ATPase by measuring the Pi liberated from ATP hydrolysis with colorimetrical methods. It was shown that the activity of Ca(2+)ATPase in SR from IL-2-perfused (10, 40, 200, 800 U/ml) rat heart increased dose-dependently. After incubation of the SR with ATP (0.1 approximately 4 mmol/L), the activity of SR Ca(2+)ATPase increased dose-dependently in the control group. In the SR from 200 U/ml IL-2-perfused hearts, the activity of Ca(2+)ATPase was much higher than that in the control group. On the other hand, incubation of the SR with Ca(2+) (1 approximately 40 micromol/L) increased the activity of SR Ca(2+) ATPase in the control group. The activity of SR Ca(2+)ATPase of IL-2-perfused hearts was inhibited as the function to Ca(2+). Pretreatment with specific kappa-opioid receptor antagonist nor-BNI (10 nmol/L) for 5 min attenuated the effect of IL-2 (200 U/ml) on the activity of SR Ca(2+) ATPase. After pretreatment with pertussis toxin (PTX, 5 mg/L) or U73122 (5 micromol/L), IL-2 failed to increase SR Ca(2+)ATPase activity. The activity of SR Ca(2+)ATPase was not changed by incubation of SR isolated from normal hearts with IL-2. Perfusion of rat heart with IL-2 did not affect the activity of sarcolemmal Ca(2+)ATPase and Na(+)/K(+)ATPase. It is concluded that perfusion of rat heart with IL-2 increases the activity of SR Ca(2+)ATPase dose-dependently, which is mainly mediated by cardiac kappa-opioid receptor pathway including a PTX sensitive Gi-protein and phospholipase C. IL-2 increases the activity of SR Ca(2+)ATPase as the function to ATP, but inhibits the activity of SR Ca(2+)ATPase as the function to Ca(2+). IL-2 has no effect on the activity of sarcolemmal Ca(2+)ATPase and Na(+)/K(+)ATPase.
