Objective To analyze the newly reported HIV-1 infection in several prefectures of Hubei Province, and analyze its influencing factors. Methods The limiting antigen avidity enzyme immunoassay(LAg-avidity EIA,LAg) was conducted on HIV-1 positive samples confirmed by Western blot of Hubei in 2017-2022. The demographic characteristics of the newly infected samples were analyzed by χ2 test.Logistic regression model was used to analyze influencing factors of new infection rate and predict the factors associated with the HIV-1 recent infection. Results There were 403 new cases of HIV-1 from 2017 to 2022 in several prefectures of Hubei Province, of which 77 were newly infected sorted by LAg,with a new infection rate of 19.11%. The newly confirmed HIV-1 persons of whom aged ≤24 years (40.00% new infection ratio), unmarried (29.41%), college or above (31.37%), and from Voluntary counseling and testing testing(VCT) clinics (40.00%) had a higher proportion of new infections, and the difference was statistically significant. Multivariate Logistic regression analysis showed that age ≤24 years old (aOR=4.346,95%CI: 1.342-14.075) and screening from the VCT clinic (aOR=6.761,95%CI: 1.460-31.319) were more likely to be newly infected. Conclusion The proportion of new HIV infection in several prefectures of Hubei province is relatively low in recent years.Further effective publicity and intervention measures for young students and the construction of VCT clinic should be continuously promoted to achieve early diagnosis and treatment.

With the continuous increase in global obesity prevalence, the impact of obesity on reproduc-tive physiology has garnered widespread societal attention. As a metabolic disorder, obesity is typically accompanied by multiple abnormal physiological phenomena, such as excessive adipose accumulation and exacerbated inflammatory responses, which severely compromise the reproductive health of humans and animals.Reproductive damage induced by obesity involves a series of complex biochemical reactions and in vivo metabolic pathways, manifesting as impaired male sperm quality and female fertility. To better un-derstand the relationship between obesity and reproductive physiology, this review summarizes the repro-ductive injuries caused by obesity and their underlying mechanisms. In the obese state, conditions such as oxidative stress, insulin resistance, and hyperinsulinemia are induced, with adipokines(leptin,adi-ponectin,resistin,etc.)and inflammatory factors(TNF-α,IL-6,IL-1β,etc.)interacting synergisti-cally to affect the reproductive system.Oxidative stress activates the MAPK and NF-κB pathways,inter-fering with insulin signaling, while chronic inflammation leads to adipocyte secretory disorders and dis-rupts the hypothalamic-pituitary-gonadal regulatory axis.Studies have shown that obese males exhibit sig-nificantly decreased testosterone levels and impaired sperm quality, whereas obese females suffer from re-productive hormone imbalance, ovulation disorders, and polycystic ovary syndrome. This review discus-ses how obesity-induced metabolic disorders lead to impaired reproductive physiology in both males and females, along with the underlying mechanisms, providing a theoretical basis for the prevention and treat-ment of obesity-related reproductive disorders in the future.

The engineering application of microbially induced carbonate precipitation(MICP)is limited by pH-dependent conformational dynamics of urease.Focusing on the α-subunit urease from Sporosarcina pasteurii, this study integrated conductivity experiments and constant-pH molecular dynamics simulations to analyze active site conformational dynamics and catalytic function across pH 3-11.Results showed that under neutral conditions(pH 7-8),key histidine residues(HIS139/HIS249)exhibited minimal dis-placement(<0.5 ?),the longest hydrogen bond lifetime(>8 ps),highest conformational stability(root mean square deviation,RMSD:0.15-0.18 nm),and optimal catalytic activity(conductivity change rate:0.03 mS/cm·min-1,CaCO3 precipitation:3.84 g).Extreme pH(pH 3/11)induced structural collapse(displacement up to 1.8 ?)and complete activity loss.Simulations revealed that neutral pH sta-bilizes a protonation-dependent cooperative allosteric network by maintaining active site cavity volume(~120 ?3)and moderate conformational coherence(correlation coefficient~0.8).This work deciphers the molecular mechanism of pH-regulated urease dynamics through protonation states, providing theoreti-cal support for MICP applications in acidic mine tailing remediation and alkaline soil stabilization.

The construction of the National Medical Center and the National Regional Medical Center in the Chengdu-Chongqing Economic Circle (referred to as the dual medical center construction) faces the challenges of insufficient integration of industry, academia, research, and medicine, as well as limited joint development and sharing of high-quality resources. It is urgent to promote the integration and coordinated development of regional education and technology talent resources through the integration of higher medical education, and to enhance the "four chains" synergy with the integration of industry and education as the core, thereby promoting the dual medical center construction. This study investigated and summarized the key issues in the dual medical center construction and the integration of higher medical education in the Chengdu-Chongqing Economic Circle. Based on the theoretical framework of four chain coordination (industry chain-talent chain-education chain-city chain), this study clarified the mechanisms by which integrated higher medical education can promote dual medical center construction. The study found that the integration of higher medical education can effectively promote the optimization of resources for dual medical center construction by constructing a collaborative mechanism of "industry education integration-joint talent cultivation-coordinated discipline construction-intercity cooperation". Based on the strengths in higher education resources in Chengdu and Chongqing, a five-dimension collaborative path was proposed: integrated development; technological leadership; talent oriented; government guidance; and market assistance. The research conclusion not only enhances the understanding of the integration of higher medical education, but also provides theoretical basis and practical reference for the dual medical center construction in the Chengdu-Chongqing Economic Circle. The proposed coordinated development also has reference value for the construction of medical centers in urban agglomerations such as the Beijing-Tianjin-Hebei Urban Agglomeration and the Yangtze River Delta.

Objective:To evaluate the performance of an artificial intelligence-based endoscopic virtual ruler (EVR) for non-invasive measurement of esophageal varices (EV) diameter. Methods:Patients with liver cirrhosis and EV hospitalized at the First Affiliated Hospital of Anhui Medical University between October 2022 and May 2023 were prospectively enrolled. EV diameter was measured using visual estimation, esophageal varix manometer (EVM), and EVR, with procedure times recorded. The intraclass correlation coefficient (ICC) was used to assess the consistency of EV diameter measurement obtained from the three methods, and repeated-measures ANOVA was used to compare differences in time measurements across three methods. Results:The study included 41 patients with liver cirrhosis and EV. Inter-observer ICC for visual estimation was 0.594, versus 0.840 for EVM and 0.884 for EVR. The ICC value between the EV diameters measured by EVR and EVM was higher than that of the visual assessment. The ICC value between EV diameter measurement by EVM and EVR was 0.991. Measurement times differed significantly across methods ( P<0.001): visual estimation 18.6±2.2 s (14.7-23.3 s), EVR 41.5±4.1 s (31.7-50.3 s), and EVM 170.8±26.4 s (129.3-229.3 s). Repeated measures analysis of variance (corrected using Greenhouse-Geisser) revealed significant differences in time across the three measurement methods [ F(1.033, 41.313)=1 233.800, P<0.001]. Subsequent Bonferroni post-hoc tests revealed significant differences in time between all method pairs ( P<0.001). Conclusion:EVR provides rapid, non-invasive EV diameter measurements with excellent agreement to EVM assessment, offering an efficient alternative to conventional techniques.
