Background and Aims:Radical resection is the only potentially curative treatment for intrahepatic cholangiocarcinoma(ICC),yet the high recurrence rate results in poor prognosis. In recent years,"textbook outcome"(TO)has been proposed as a comprehensive quality metric, but its association with prognosis remains unclear. This study aimed to analyze the risk factors influencing the achievement of TO after radical resection of ICC and to explore the relationship between TO and survival. Methods:A retrospective analysis was conducted on the clinical data of 180 ICC patients who underwent radical resection at the 904th Hospital of the Joint Logistic Support Force of the PLA between February 2018 and February 2023.Univariate and multivariate Logistic regression analyses were performed to identify risk factors associated with TO,and survival analysis was carried out using the Kaplan-Meier method and Log-rank test. Results:Of the 180 patients,66 achieved TO.Multivariate Logistic regression analysis indicated that preoperative total bilirubin>22 μmol/L,preoperative CA19-9>35 U/mL,maximum tumor diameter>5 cm,poor tumor differentiation, microvascular invasion(MVI),and lymph node metastasis were independent risk factors for failing to achieve TO(all P<0.05).Survival analysis demonstrated that patients who achieved TO had a significantly longer median survival compared with those who did not(36 months vs.16 months,P<0.001).Conclusion:Preoperative total bilirubin>22 μmol/L,preoperative CA19-9>35 U/mL,maximum tumor diameter>5 cm,poor tumor differentiation,MVI,and lymph node metastasis are independent risk factors for not achieving TO after radical resection of ICC.Patients who achieved TO exhibited markedly longer survival, suggesting that TO not only reflects perioperative treatment quality but also serves as an important prognostic indicator.Greater attention to these risk factors and optimization of perioperative management may improve the likelihood of achieving TO and enhance long-term outcomes.

OBJECTIVE:
To assess the mid-term clinical efficacy of the direct anterior approach for window decompression and bone grafting surgery in the treatment of early to mid-stage osteonecrosis of the femoral head (ONFH).
METHODS:
A retrospective analysis was performed on 40 patients (40 hips) diagnosed with osteonecrosis of the femoral head (ONFH), classified as types L1 and L2 according to the China-Japan Friendship Hospital (CJFH) classification system, and at stagesⅡ, ⅢA, and ⅢB based on the Association Research Circulation Osseous (ARCO) staging system. All patients underwent head-neck junction fenestration decompression and bone grafting via the direct anterior approach between January 2015 and May 2022, with complete follow-up data available for a minimum of three years. The ages of the patients ranged from 35 to 69 years old, with a mean of (49.13±6.14 ) years old;their body mass index (BMI) ranged from 20.02 to 27.94 kg·m-2, with a mean of (23.65±1.69) kg·m-1;the duration of the disease ranged from 13 to 36 months, with a mean of (24.55±4.14) months. Preoperative and 3-year postoperative X-ray parameters were collected, including the anterior preserved angle(APA), lateral preserved angle (LPA), and combined preserved angle (CPA). Additionally, hip joint disability and osteoarthritis outcome scores (HOOS) and Harris hip scores (HHS) were recorded.
RESULTS:
Forty patients were followed up for a period ranging from 36 to 59 months, with a mean duration of (47.18±6.18) months. At 3 years postoperative, none of the patients underwent hip replacement surgery. The APA (73.15±19.35)°, LPA (75.35 ±21.48)°, and CPA (136.25±26.78)° at the 3-year postoperative significantly improved compared to preoperative measurements (61.93±20.54)°, (59.46±22.67)°, and (116.58±32.47)°, with statistical significance (P<0.05). The HOOS (20.37±1.39) and HHS (89.74±3.28) scores at the 3-year postoperative were significantly improved from preoperative scores (12.36±1.58) and (50.27±6.15), respectively, with statistical significance (P<0.05).
CONCLUSION
The direct anterior approach for window decompression and bone grafting surgery can relieve joint pain, improve joint function, and enhance X-ray preserved angles, effectively preventing femoral head collapse, making it an effective surgical method for treating ONFH classified as L1, L2 according to CJFH and stagesⅡ, ⅢA, ⅢB according to ARCO.

Biomedical and Environmental Sciences 2025;38(10):1179-1193
OBJECTIVE:
This study aimed to explore the association between body mass index (BMI) and mortality based on the 10-year population-based multicenter prospective study.
METHODS:
A general population-based multicenter prospective study was conducted at four sites in rural China between 2013 and 2023. Multivariate Cox proportional hazards models and restricted cubic spline analyses were used to assess the association between BMI and mortality. Stratified analyses were performed based on the individual characteristics of the participants.
RESULTS:
Overall, 19,107 participants with a sum of 163,095 person-years were included and 1,910 participants died. The underweight (< 18.5 kg/m 2) presented an increase in all-cause mortality (adjusted hazards ratio [ aHR] = 2.00, 95% confidence interval [ CI]: 1.66-2.41), while overweight (≥ 24.0 to < 28.0 kg/m 2) and obesity (≥ 28.0 kg/m 2) presented a decrease with an aHR of 0.61 (95% CI: 0.52-0.73) and 0.51 (95% CI: 0.37-0.70), respectively. Overweight ( aHR = 0.76, 95% CI: 0.67-0.86) and mild obesity ( aHR = 0.72, 95% CI: 0.59-0.87) had a positive impact on mortality in people older than 60 years. All-cause mortality decreased rapidly until reaching a BMI of 25.7 kg/m 2 ( aHR = 0.95, 95% CI: 0.92-0.98) and increased slightly above that value, indicating a U-shaped association. The beneficial impact of being overweight on mortality was robust in most subgroups and sensitivity analyses.
CONCLUSION
This study provides additional evidence that overweight and mild obesity may be inversely related to the risk of death in individuals older than 60 years. Therefore, it is essential to consider age differences when formulating health and weight management strategies.

8.Research on the correlation between early nutrition and bronchopulmonary dysplasia in very preterm infants
Chinese Pediatric Emergency Medicine 2025;32(10):748-752
Objective:To explore the correlation between enteral and parenteral nutrition intake in very preterm infants during the first week after birth and the development of bronchopulmonary dysplasia (BPD).Methods:A retrospective analysis was conducted on 158 preterm infants born between October 1,2018 and September 30,2021.All of the infants were transferred to the neonatal ward of The Third Hospital of Baogang Group within 24 hours of birth. The infants had a birth weight of less than 1 500 g and a gestational age of less than 32 weeks. These infants were used as study subjects and were divided into a bronchopulmonary dysplasia (BPD) group( n=65) and a non-BPD group ( n=93).The clinical data and nutritional intake were compared between the two groups. Results:The BPD group exhibited longer birth weight recovery and mechanical ventilation times than the non-BPD group. The BPD group also had higher rates of extrauterine growth restriction, feeding intolerance, haemodynamically significant patent ductus arteriosus and bile acid accumulation associated with parenteral nutrition than the non-BPD group ( P<0.05).The BPD group received a higher volume of parenteral nutrition in the first week after birth than the non-BPD group. However, the caloric intake per unit of parenteral nutrition, the caloric-to-nitrogen ratio, and the total intake of glucose, amino acids, and lipid emulsions were all lower in the BPD group than in the non-BPD group during the first week postnatally ( P<0.05).The breastfeeding rate, enteral caloric intake,protein-to-energy ratio, and total caloric intake were all lower in the BPD group than in the non-BPD group. It took longer for the BPD group to achieve full enteral nutrition than the non-BPD group( P<0.05). Conclusion:The early attainment of total enteral nutrition, the increased energy density of parenteral nutrition, unit parenteral nutritional calories, and an emphasis on protein-energy ratio may be measures to reduce the incidence of BPD.

Chinese Journal of Analytical Chemistry 2025;53(5):842-851
The rapid and non-destructive detection of constituent content of fresh tea leaves shows an important reference value for quality identification of tea.Visible near infrared(Vis-NIR)spectroscopy has been used for qualitative and quantitative analysis of chemical components in plant samples with the advantages such as simple, rapid and non-destructive detection. In this study, residual attention convolutional neural network(RACNN)was used to predict the internal constituent content of fresh tea leaves.Firstly, the reflectance spectral data of the samples in the Vis-NIR band range and the constituent contents of gallic acid(GA),gallocatechin(GC),epigallocatechin(EGC),and epigallocatechin gallate(ECG)in fresh tea leaves were collected.Based on the preprocessing of the spectral data, the contents of the four components were predicted using a partial least squares regression(PLSR)model, and the optimal preprocessing was determined.Subsequently, the characteristic bands were extracted using the random forest(RF)algorithm. Finally, the performances of PLSR,convolutional neural network(CNN)and RACNN models were compared. The results showed that for GA,the RACNN model worked best with a validation set coefficient of determination(R2)of 0.946 and a root mean square error of the prediction set(RMSEP)of 1.173;for GC,the RACNN model works best with a validation set R2 of 0.928 and RMSEP of 6.081;for EGC,the RACNN model works best with a validation set R2 of 0.891 and a RMSEP of 15.197;for ECG,the RACNN model worked best with a validation set R2 of 0.878 and a RMSEP of 7.837.The RACNN model established by Vis-NIR spectroscopy combined with chemometrics could realize the accurate detection of the contents of components in fresh tea.
