A 70-year-old man presented with a 1-year history of a solitary asymptomatic papule over the left infraorbital area, which exhibited sudden enlargement into an erythematous hyperkeratotic plaque with erosion and crust formation, with slight tenderness and occasional bleeding episodes for the past month. A skin punch biopsy was done, and showed squamous cell carcinoma in situ. The patient is a known hypertensive with cardiac disease who is not amenable to surgical treatment.
