1.Application effects and safety of continuous negative pressure drainage device in postoperative drainage of congenital microtia reconstruction of auricle

Objective:To analyze the application effects and safety of continuous negative pressure drainage devices in postoperative drainage of congenital microtia reconstruction of auricle.Methods:A retrospective study was conducted, including 1 211 patients with congenital microtia treated at the Plastic Surgery Hospital of Chinese Academy of Medical Sciences and Peking Union Medical College from January 2018 to August 2021. All patients underwent auricular reconstruction surgery and were divided into two groups based on the type of negative pressure drainage device used. The study group consisted of 624 patients [315 males and 309 females, aged 6-24 (15.1±2.2) years], while the control group consisted of 587 patients [286 males and 301 females, aged 5-23 (14.7±2.3) years]. The study group used a novel negative pressure drainage ball, while the control group used a conventional negative pressure syringe for drainage. The preoperative and postoperative 1, 7 days scores of the modified Yale preoperative anxiety scale (mYPAS) and the quality of life scores postoperative 3 days (including functional activity, self-image, psychological health, satisfaction and pain) were compared between the two groups. All patients were followed up at 4, 8, 12, and 16 weeks postoperatively to compare the surgical scar area between the two groups. The incidence of adverse reactions within 12 weeks after surgery in two groups were recorded.Results:There was no significant difference in preoperative mYPAS scores between the two groups ( P=0.897). The mYPAS scores at 1 and 7 days after surgery were lower than the preoperative scores in both groups (all P<0.05), and the study group had significantly lower mYPAS scores at all time points than those of the control group (all P<0.001). The study group had higher scores in functional activity, self-image, psychological health, and satisfaction than those of the control group (all P<0.001), and a lower pain score than that of the control group ( P<0.001). Follow-up at 4, 8, 12, and 16 weeks showed that the surgical scar area was significantly smaller in the study group at all time points than in the control group (all P<0.001). The incidence of adverse reactions within 12 weeks after surgery was 9.0% (56/624) in the study group, which was lower than the 19.4% (114/587) in the control group ( P<0.001). Conclusion:The use of continuous negative pressure drainage devices in congenital microtia reconstruction of auricle is more effective and can reduce the incidence of postoperative adverse reactions.

3.Application of refined nursing intervention in prevention of postoperative urethral stenosis in patients undergoing hypospadias

Objective To study the application of refined nursing intervention in prevention of postoperative urethral stricture in patients with hypospadias.Methods A total of 144 patients who underwent hypospadias surgery in our hospital were selected as study subjects,and were divided into control group and observation group by random number table method,with 72 cases per group.Both groups were treated with targeted surgery according to severity and different sites of hypospadias.The patients in the control group performed routine nursing intervention during the perioperative period,while the patients in the observation group were given refined nursing intervention on the basis of the control group.The response rate,incidence rates of postoperative complications and hypospadias were evaluated.Results The total response rate in the observation group was higher than that in the control group (P < 0.05).After nursing,the incidence rate of postoperative complications such as bleeding,urinary fistula,infection,and urine extravasation was lower in the observation group than that in the control group[6.94% (5/72) vs.(18.06%) (13/72),P <0.05).After nursing,the total incidence rate of mild and severe urethral stricture in the observation group was lower than that in the control group [(2.78% (2/72) vs.12.50% (9/72),P<0.05).Conclusion Refined nursing intervention for patients with hypospadias can effectively enhance treatment efficacy,reduce postoperative complications,and prevent urethral stricture,thereby to improve the prognosis of the disease.

5.Application effect of PRECEDE nursing model in elderly patients with chronic hepatitis B

Objective To explore the effect of PRECEDE nursing model in elderly patients with chronic hepatitis B.Methods Eighty-six elderly patients with chronic hepatitis B in 302 military hospital from June 2015 to June 2016 were selected. The participants were divided into two groups according to the random number table. The intervention group received PRECEDE model nursing while the control group received routine nursing care. The therapeutic effect, compliance, and quality of life were compared between the two groups. Results The intervention group had significantly higher total effective rate compared with the control group (97.67% vs. 88.37%; χ2=6.604,P<0.05). The intervention group had better diet, exercise, compliance, smoking and drinking control, and regular review compared with the control group (P<0.05). There was no significant difference in the quality of life between two groups before the intervention (P>0.05). However, after the intervention, the body function, social function, psychological function, substance function and the total score of the quality of life were significantly higher in the intervention group compared with the control group (P<0.05).Conclusions PRECEDE model can significantly improve the liver function of elderly patients with chronic hepatitis B. It can promote patients' compliance and quality of life, which should be further developed in clinical practice.
