1.Influencing factors of non-alcoholic fatty liver disease in aircrews based on classification tree model

Objective To establish a classification tree model for non-alcoholic fatty liver disease(NAFLD)among aircrews,screen for influencing factors of NAFLD,so as to provide scientific basis for prevention and intervention decisions for NAFLD.Methods Aircrews who underwent recuperation at a sanatorium from January 2019 to December 2023 were selected as the research objects.Their annual physical examination data were collected and the NAFLD detection rate was calculated.Age,body mass index(BMI),blood pressure,waist circumference,blood routine,biochemistry indexes,and thyroid function were incorporated,and a NAFLD risk model was constructed using classification regression tree method.The predictive performance of the NAFLD classification tree model was evaluated through model misclassification matrix,risk statistics,and receiver operating characteristic curve.Results A total of 4088 aircrews were included in the study,and NAFLD was detected in 380 persons(380/4088,9.30%).The NAFLD model consisted of three layers,and five explanatory variables affecting the onset of NAFLD were extracted,including BMI,triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),alanine aminotransferase(ALT),and total bilirubin(TBIL).BMI was located at the top of the classification tree and was the most important risk factor for NAFLD in aircrews.The area under the curve(AUC)of the model was 0.853.The predictive accuracy of NAFLD was 90.9%,indicating that the model has good accuracy and fitting effect.Conclusion In this study,the detection rate of NAFLD in aircrews was 9.30%.BMI,TG,HDL-C,ALT,and TBIL are risk factors for the onset of NAFLD.NAFLD is mainly related to weight gain and lipid metabolism disorders caused by unhealthy lifestyles.
2.Dynamic Expression of Nogo-A in Hippocampus of Rats after Carbon Monoxide Poisoning

Objective:To study the dynamic expression of Nogo-A in hippocampus of rats after carbon monoxide poisoning,and to explore the effect and influence of Nogo-A in the damage to nervous system after carbon monoxide poisoning.Methods:Thirty male SD rats were randomly divided into NC group(n=6),CO group(n=6),CO-24 h group(n=6),CO-48 h group(n=6),CO-7d group(n=6).The method of injection CO gas was used to establish the carbon monoxide poisoning model.Then immunohistochemical (IHC) and Western blot (WB) techniques were used to observe dynamic expression of Nogo-A in hippocampus of rats at several time intervals after carbon monoxide poisoning and to analyze its change law.Results:IHC results showed that the average optical density value of expression of Nogo-A in NC group,CO group,CO-24 h group,CO-48h group and CO-7d group were 0.0928± 0.0038,0.01172± 0.0042,0.1452± 0.0056,0.1271 ± 0.0057,0.1088± 0.0055.WB results showed that the expression of Nogo-A in hippocampus after carbon monoxide poisoning was significantly higher than that in NC group(P<0.05),and reached the highest level at 24 h,then had a gradual recovery after 24h.The expression of Nogo-A decreased obviously,but still higher than that of NC group by day 7 (P<0.05).Conclusions:In this study,the increase of expression of Nogo-A was associated with carbon monoxide poisoning.The expression of Nogo-A reached the highest level at 24h,then had a gradual recovery after 24 h.
3.Effect of different blood pressure modes on endothelial function and arteriosclerosis in patients with high blood pressure

Objective To investigate the effect of different blood pressure modes on endothelial function and arteriosclerosis in patients with high blood pressure.Methods A retrospective analysis was made on such clinical data as dynamic blood pressure data,carotid-femoral pulse wave velocity (CFPwV) and the levels of thrombotic TM,nitric oxide (NO) and nitric oxide synthase (NOS) in 200 patients with high blood pressure and 100 normal people.In accordance with dynamic blood pressure level and drop rate of night blood pressure,the patients with high blood pressure were divided into the dipper type hypertension group and the non-dipper type hypertension group,each consisting of 100 patients.Then,data of dynamic blood pressure,CFPwV and the levels of TM、NO、NOS were compared between the 3 groups,and multiple-factor regression analysis was made on various dynamic blood pressure data and CFPwV.Results (1) When compared with those of the control group,dynamic blood pressure data,CFPwV and the levels of TM were increased,but the levels of NO and NOS were decreased in the dipper type hypertension group and the non-dipper type hypertension group (P < 0.05).Night diastolic blood variation pressure(nDBVP),CFPwV and TM were increased,while the levels of NO、NOS were decreased (P < 0.05).As compared with those of the dipper type hypertension group,nSBP,dDBP,24hSBVP,nSBVP,nDBVP,CFPwV and the level of TM were increased,while the levels of NO and NOS were decreased in the non-dipper type hypertension group (P < 0.05).(2) Multiple-factor regression analysis revealed that 24hSBP,dSBP,nSBP,nDBP,nSBVP and nDBVP were independent influential factors for the increase of CFPwV.Conclusion (1) The damage to endothelial function in the non-dipper type hypertension group was severer than that in the dipper type hypertension group.Abnormal mode of hypertension could intensify endothelial lesion and facilitate atherosclerosis.(2) The research indicated that the level of dynamic blood pressure data,especially 24hSBP,dSBP,nSBP,nDBP,nSBVP and nDBVP were closely associated with atherosclerosis.For those non-dipper hypertension patients,it was of great clinical significance to control night blood pressure and correct abnormal blood pressure mode,in addition to the treatment for the decrease of blood pressure.
4.Effect of emergency nursing procedures on the first-aid efficacy and quality of the patients with acute coronary syndromes

Objective To investigate the effect of emergency nursing procedures on the first-aid efficacy and quality of the pa-tients with acute coronary syndromes (ACSs).Methods Eighty patients with ACSs treated in the emergency department of our hospital from March 2012 to March 2014 were selected for the study.These patients were randomly divided into 2 groups:the study group and the control group, each consisting of 40 patients.The control group was given routine care, while the study group was given nursing care with emergency nursing procedures.Results The time used for all the emergency medical receptions in the patients of the study group was significantly shorter than that of the control group (P<0.05).The levels of CK-MB and hs-cTnT [(14.8 ±2.4)U/L,6.2 ±0.9) mg/L] were all significantly lower than those of the control group [(36.2 ±7.6)U/L,(14.2 ±2.4)mg/L] (P<0.05).The LVEF and LVEDD levels of the study group were all significantly higher than those of the control group ( P<0.05) .The scores of PF, GH, VT, SF, RE and MH were markedly higher than those of the control group (P<0.05), and the satisfaction scores of therapeutic nurs-ing, patient service, health education, psychological nursing care were also significantly higher than those of the control group ( P<0.05).Conclusion Emergency nursing procedures could significantly improve the efficacy of first-aid and quality of the patients with acute coronary syndromes.
5.Correlation analysis between homocysteine level and arteriosclerosis in high blood pressure patients in routine medical examination

Objective To study the correlation between homocysteine (Hcy) level and arteriosclerosis in hypertension pa-tients.Methods Four hundred and twenty hypertension patients (assigned as the experimental group) and 192 healthy examinees (as-signed as the control group) were recruited for the study, and the hypertension patients were further subdivided into the normal Hcy group, the slightly higher Hcy group and the obviously high Hcy group, in accordance with the plasma levels of Hcy.Carotid-femoral pulse wave velocity ( CFPwV) was measured by automatic pulse wave velocity detector and was used as an arteriosclerosis indicator. Blood pressure parameters, Hcy levels, CFPwV levels were compared between the 4 groups and the incidence of atherosclerosis for each group were calculated accordingly.Then, Pearson correlation analysis and multiple regression analysis were performed in all the associ-ated factors of hypertension with CFPwV.Results (1) As compared with those of the control group, the levels of systolic blood pres-sure ( SBP) , diastolic blood pressure ( DBP) , mean arterial blood pressure ( MAP) , pulse pressure ( PP) , Hcy and CFPwV increased in all the experimental groups (P<0.05).As compared with those of the Hcy normal group, the levels of SBP, DBP, MAP, PP, Hcy and CFPwV increased in the slightly higher Hcy group and the obviously high Hcy group (P<0.05).And as compared with those of the slightly higher Hcy group, the levels of SBP, Hcy and CFPwV in the obviously high Hcy group all elevated significantly ( P<0.05).(2) The incidence of atherosclerosis in the slightly higher Hcy group and the obviously high Hcy group was higher than those of the normal Hcy group (P<0.05).(3) Pearson correlation analysis revealed that CFPwV was positively correlated with age, SBP, DBP, PP, MAP and Hcy.Results of multiple linear regression analysis indicated that Hcy, SBP and PP were independent influential factors of CFPwV.Conclusion Hcy, SBP and PP were independent influential factors of CFPwV.High Hcy level and hypertension were synergistic, and high Hcy could increase the risk of atherosclerosis due to hypertension, indicating that hypertension and high Hcy level were all involved in the occurrence and development of atherosclerosis.
