Medical therapy and surgical intervention are the two primary approaches for treating myocardial bridge. However, there remains controversy regarding the use of coronary artery bypass grafting(CABG)and myocardial bridge unroofing. Here,we report a case of myocardial infarction following CABG in a patient with a myocardial bridge. The patient was admitted to Lanzhou First Peopie's Hospital with persistent chest pain, chest tightness, and shortness of breath lasting 2 hours.Physical examination revealed no significant abnormalities.Electrocardiography(ECG)indicated extensive anterior wall myocardial infarction.Laboratory findings showed myoglobin levels of 140.1 ng/ml and troponin Ⅰ levels of 2.59 ng/ml,with no other significant abnormalities. The initial diagnosis was acute extensive anterior wall myocardial infarction.Emergency coronary angiography revealed a myocardial bridge in the mid-segment of the left anterior descending artery(LAD).Emergency CABG using the left internal mammary artery to the LAD was performed, leading to symptomatic improvement, and the patient was discharged in stable condition. However, the patient experienced a recurrent myocardial infarction seven years post-surgery and received secondary preventive medical therapy. The patient is currently under ongoing follow-up care.CABG is an effective treatment for myocardial bridge. However, based on the case reported in this study,we recommend careful evaluation of whether a patient may benefit from CABG.

The success of"New Medical Sciences"in higher education requires effective tools in evalua-ting students'performance in courses.Previously,we reported a teamwork(T),critique(C)and ap-preciation(A)(TCA)ideological and political model,a teaching model widely applied in Basic Medical courses.TCA is an abbreviation derived from Tricarboxylic Acid Cycle in Biochemistry as an analogy for nurturing the abilities of thinking and teamwork(T),critique(C)and appreciation(A),which hope-fully could provide students with moral norms for cognition, science and life. This paper further explores the tools to assess the educational outcomes of the TCA model,by which teachers can collect feedback and reflect on teaching quality and effectiveness.Addressing the challenges of individual differences in large classes, fragmented learning feedback, and the difficulty of measuring meta-cognition in educational evaluation, this study employs strategies of value-added assessment, matrix assessment and norm-transfer-able assessment to evaluate the TCA abilities from the aspects of thinking quality, thinking creativity,co-operation ability, iterative thinking, dialectical thinking and job responsibility.By modifying/using 18 e-valuation tools in Education and Psychology,we have established a rubric system composed of 30 primary indicators(with 11 newly designed,10 partly modified and 9 directly adopted),along with 49 secondary indicators and 98 tertiary indicators to enhance the feasibility of the evaluation process. This rubric sys-tem was applied to Biochemistry teaching among the five-year-program undergraduates at Air Force Medi-cal University.Specifically, thinking and teamwork are evaluated by creative works from"the magic bio-chemical-circle",while critiques are assessed in large classes under the guidance of basic and clinical teachers, coupled with appreciation measured by job responsibility in a task-driven virtual reality(VR)project. The results indicate that Biochemistry teaching not only accumulates knowledge in students, but also achieves the goals in nurturing values and cognition. The inclusion of creative performance evalua-tion,cooperative learning and clinical case studies, can enhance students'interpersonal skills,coopera-tion,quality of thinking, creative thinking, iterative thinking and dialectical thinking to varying degrees.TCA-based Biochemistry teaching has a long-lasting impact on character education, and is capable of in-ducing positive long-term changes in students'cognition and lifestyle.Taken together, with the help of this rubric system, teachers can promptly acknowledge the effectiveness of their teaching, thereby facilita-ting their teaching strategies.

BACKGROUND:The relationship between rheumatoid arthritis and coronary atherosclerosis has received extensive attention.Inflammation is related to rheumatoid arthritis and coronary atherosclerosis, indicating that there may be a common pathophysiological pathway between the two diseases. However, observational studies have not yet clarified the causal relationship.OBJECTIVE:To explore whether there is a causal relationship between rheumatoid arthritis and coronary atherosclerosis,as well as the potential causal relationship with 1 400 serum metabolites and 91 inflammatory factors through a Mendelian randomization analysis.METHODS:Coronary atherosclerosis data are from Finngen database, rheumatoid arthritis data are from IEU OpenGWAS database, serum metabolites data are from Canadian Longitudinal Study on Aging,Augsburg Cooperative Health Research and British Twin Project Research, and data of 91 inflammatory proteins are from research published in Nature Immunology in 2023.Mendelian randomization analysis was performed using data from genome-wide association studies, and causal effects were evaluated using inverse variance weighting,MR-Egger regression, weighted median, weighted model, and simple model methods, with inverse variance weighting being the primary analysis method. To enhance robustness,Cochran's Q-test MR-Egger intercept was used for sensitivity analysis.RESULTS AND CONCLUSION:(1)Inverse variance weighting results showed that rheumatoid arthritis was positively correlated with the increased relative risk of coronary atherosclerosis(odds ratio=1.002,95%confidence interval=1.001-1.003,P=0.003).There was no reverse causal relationship between coronary atherosclerosis and rheumatoid arthritis. In addition,96 serum metabolites and 9 inflammatory factors were found to have causal relationships with coronary atherosclerosis. There was a causal relationship between 51 serum metabolites and 7 inflammatory factors and rheumatoid arthritis.(2)This study provided epidemiological evidence between rheumatoid arthritis and coronary atherosclerosis, and emphasized the potential role of serum metabolites and inflammatory factors in the pathogenesis of these diseases. These findings may contribute to the development of new treatment strategies.Due to the limited inclusion of data from Asian populations, most contemporary studies used international databases and European population analyses.By collecting and analyzing the health data of European populations,it is conducive to a better understanding of the effects and potential role of Chinese medicine in Europe, and to further promote the practice of modern integration of Western and Chinese medicine.Meanwhile, through the comparative study with the European databases,it is possible to reveal the genetic differences and susceptibility to diseases among different populations, providing more dimensions and perspectives for global health research.

Objective To elucidate the mechanisms of trauma-induced coagulopathy(TIC),clarify the specific pathogenic factors and pathophysiological processes, and discover the effective diagnostic indicators and therapeutic targets. Methods Transcriptomic data of traumatic hemorrhagic shock patients were obtained from the Gene Expression Omnibus(GEO)to identify differentially expressed genes(DEGs).Coagulation-related genes(CRGs)from the Kyoto Encyclopedia of Genes and Genomes(KEGG)were intersected with DEGs.Machine learning algorithms, including least absolute shrinkage and selection operator(LASSO)and random forest(RF),were applied to identify key genes. The CIBERSORT algorithm was used to analyze the correlation between key genes and immune cell infiltration.Through consensus clustering, subtype analysis was conducted on trauma patients to compare the infiltration of immune cells.A rat model of traumatic hemorrhagic shock was established to validate coagulation function and the expression of key genes. Results The dataset included samples from 17 healthy controls and 478 patients with traumatic hemorrhagic shock.A total of 6 315 DEGs were identified under the screening criterion of corrected P<0.05.Gene set enrichment analysis(GSEA)showed that the up-regulated DEGs were significantly enriched in the glucose metabolism pathway, while the down-regulated DEGs were enriched in the immune reaction-related pathways.Through cross-analysis of DEGs and CRGs,a total of 65 differentially expressed coagulation-related genes(DE-CRGs)were screened out.GO functional enrichment showed that these genes were mainly located in secreting granular membranes and platelet α-granules,and were involved in physiological processes such as blood coagulation, regulation of body fluid levels, and wound healing.KEGG pathway analysis revealed that these genes were significantly enriched in pathways such as platelet activation, complement and coagulation cascade reactions,Rap1 signaling pathway, and human cytomegalovirus infection.Six key DE-CRGs were identified through machine learning.Receiver operating characteristic(ROC)curve analysis indicated that these genes had good diagnostic efficacy.CIBERSORT analysis revealed a significant correlation between key genes and immune cell infiltration.Patients were classified into two subtypes based on the six key genes:subtype A was rich in CD8+T cells and activated NK cells, presented an immune-active state;subtype B was mainly composed of monocytes and resting NK cells, with insufficient activation of immune pathways.Animal experiments on rats showed that hemorrhagic shock can lead to coagulation dysfunction. The results of qRT-PCR further confirmed that the expression trend of key genes was consistent with the results of bioinformatics analysis. Conclusion In this study, through transcriptomics and machine learning methods, six key genes closely related to TIC were systematically screened out, namely GNA13,PIK3R3,ITGAM,MAPK14,PPP1CC and LYN,and their close connections with coagulation function and immune infiltration were revealed.Animal experiments have further verified the value of these genes as potential diagnostic and therapeutic targets.
