Objective:To identify the relevant factors for the first-course remission of acute myeloid leukemia (AML) and to develop a predictive model as well as assess its predictive capability. Methods:Clinical data of 749 patients newly diagnosed with AML admitted to the Department of Hematology, the First Affiliated Hospital, Zhejiang University, School of Medicine from January 1, 2019, to April 30, 2023, were collected and randomly divided into training and validation sets. Multivariate logistic regression analysis was conducted to determine variables associated with complete remission in the first course of induction therapy, and a predictive model was established based on these variables. The receiver operating characteristic (ROC) curve of the predictive model was plotted, and the area under the curve (AUC) was calculated. Results:The indicators predicting the first remission course included peripheral blood white blood cell count during onset, CBF::MYH11 fusion gene, CEBPA bZIP region mutation, myelodysplastic syndrome-related gene mutation, and induction chemotherapy regimen selection as independent factors for the first remission course. The model’s area under the training and validation curves was 0.738 (95% CI: 0.696-0.780) and 0.726 (95% CI: 0.650-0.801), respectively. The Hosmer-Lemeshow test results yielded P-values of 0.993 and 0.335, respectively. Conclusion:In this study, the developed model demonstrates a strong predictive capability for the efficacy of the first course of patients with AML, providing valuable guidance to clinicians in assessing patient prognosis and selecting appropriate treatment strategies.

Objective:To analyze the incidence rate and disability-adjusted life year (DALY) rate of Chlamydia trachomatis ( C. trachomatis) infection in women aged 15-49 years in China from 1990 to 2021, identify the vulnerable populations, and provide evidence for the prevention and control of C. trachomatis infection. Methods:The data from Global Burden of Disease Study 2021 were used to analyze the incidence rate and DALY rate of C. trachomatis infection in women aged 15-49 years in China from 1990 to 2021 by using Joinpoint regression model. The age-period-cohort analysis method was used to decompose the age effect, period effect and cohort effect of incidence rate and DALY rate of C. trachomatis infection. Results:From 1990 to 2021, the incidence rate of C. trachomatis infection increased from 5 892.02/100 000 in 1990 to 7 402.23/100 000 in 2021 in women aged 15-49 years in China. The DALY rate increased from 1.92/100 000 person-years in 1990 to 2.00/100 000 person-years in 2021. Joinpoint regression showed that, from 1990 to 2021, the annual incidence of C. trachomatis infection in Chinese women age aged 15-49 years increased by 0.74% [average annual percent change (AAPC) =0.74%, 95% CI: 0.67%-0.81%, t=21.94, P<0.001] and the annual average DALY rate increased by 0.11% (AAPC=0.11%, 95% CI: -0.09%-0.30%, t=1.06, P=0.287). Age-period-cohort model analyses showed that women aged 30-34 years were at high risk to be infected with C. trachomatis and women aged 40-44 years were at high risk to have high DALY. The period effect showed that the risk for C. trachomatis infection began to increase in 1990, peaked during 2000-2004, then gradually decreased. During 2015-2019, the infection risk increased again, while the risk for high DALY showed a decreasing trend. The cohort effect showed that the risks for C. trachomatis infection and DALY were lower in later birth cohorts. Conclusions:From 1990 to 2021, the incidence rate and DALY rate of C. trachomatis infection showed fluctuations in women aged 15-49 years in China. It is necessary to pay close attention to the prevention and control of C. trachomatis infection in women aged 30-34 years.

Objective:To evalaute the prevalence and determinants of thyroid nodules in children and adolescents in Jiangsu Province. Methods:This study included in-school students aged 8-17 years and selected through stratified cluster random sampling in Jiangsu. Thyroid nodule was diagnosed when its diameter was ≥3 mm. Random urine samples were collected for the detection of urinary iodine concentration with arsenic-cerium catalytic spectrophotometry. Data were analyzed by using χ2 test and logistic regression model. Results:In the 8 201 children and adolescents selected, the thyroid nodule detection rate was 16.10%. Multivariate logistic regression analysis revealed that, compared with those with urinary iodine levels of 100-299 μg/L, boys, those with normal body weight, those who were satisfied with their school performance, urinary iodine concentration ≥300 μg/L ( OR=1.15, 95% CI: 1.01-1.31), being girls ( OR=1.42, 95% CI: 1.26-1.60), being overweight ( OR=1.27, 95% CI: 1.07-1.50), being obese ( OR=1.23, 95% CI: 1.03-1.47), and dissatisfied with school performance ( OR=1.22, 95% CI: 1.04-1.43) were associated with higher likelihood of thyroid nodule detection. Children and adolescents who had solid snacks 2 times per week to 2 times per month ( OR=0.86, 95% CI: 0.74-0.99) or less than 2 times per month ( OR=0.80, 95% CI: 0.68-0.93) were more likely to have lower detection rate compared with those who had solid snacks more than 2 times per week. The detection rate of thyroid nodule increased with age ( OR=1.09, 95% CI: 1.06-1.11). Conclusion:The main factors influencing the detection rate of thyroid nodule in children and adolescents aged 8-17 years in Jiangsu included gender, age, urinary iodine concentration, BMI, self-assessed school performance and dietary habit.

Objective:To explore the safety and efficacy of intraosseous regional administration (IORA) of vancomycin for preventing infection in primary total knee arthroplasty (TKA).Methods:A total of 124 patients with knee osteoarthritis undergoing TKA between February 2024 and May 2024 at nine hospitals were enrolled. Preoperative infection prophylaxis involved either IORA (0.5 g vancomycin administered via intraosseous regional infusion before incision) or intravenous infusion (1 g vancomycin via peripheral vein). The IORA group included 15 males and 47 females with a median age of 66.5 years (range, 60.0-70.0 years), while the intravenous group included 14 males and 48 females with a median age of 66.0 years (range, 61.8-70.3 years) years. Intraoperative samples were collected including fat and synovium tissues after incision, before prosthesis placement, and after tourniquet release; distal femoral cancellous bone during femoral osteotomy; proximal tibial cancellous bone during tibial osteotomy; proximal intercondylar cancellous bone before prosthesis placement; and peripheral blood from non-infused arms at surgery initiation and after tourniquet release. Vancomycin concentrations were measured using liquid chromatography-tandem mass spectrometry. Vital sign changes were recorded from admission to 5~10 minutes post-IORA (IORA group) or post-incision (intravenous group). Follow-ups were conducted on postoperative day 1 and 3, and at 1 and 3 months, to document complications including IORA-related adverse events, periprosthetic joint infections, surgical site infections, red man syndrome, acute kidney injury, deep vein thrombosis and so on.Results:Vancomycin concentrations in bone, fat, and synovial tissue samples were significantly higher in the IORA group than in the intravenous group ( P<0.05), while vancomycin concentrations in blood samples were significantly lower in the IORA group than in the intravenous group ( P<0.05). Only 7.3%(41/558) of tissue samples in the IORA group had vancomycin concentrations below 2.0 μg/g (the minimum inhibitory concentration of vancomycin against coagulase-negative staphylococcus), compared to 59.3%(331/558) in the intravenous group (χ 2=11.285, P<0.001). In the intravenous group, 16.9%(21/124) of blood samples had vancomycin concentrations exceeding 15.0 mg/L (the threshold associated with a significantly increased risk of nephrotoxicity), while all concentrations in the IORA group were below this threshold, the difference was statistically significant (χ 2=22.943, P<0.001). There were no statistically significant difference ( P>0.05) in vital signs changes before and after vancomycin administration between the two groups. Two patients in the intravenous group experienced incision exudate, while no other related complications occurred in either group. Conclusions:Compared to the traditional intravenous infusion of 1 g vancomycin, intraosseous injection of a low dose (0.5 g) of vancomycin achieves higher local tissue concentrations in the knee joint with a lower incidence of adverse reactions and is safe for infection prophylaxis. Despite guidelines not recommending the routine use of vancomycin for preventing infection after primary TKA, intraosseous injection of 0.5 g vancomycin may be considered intraoperatively for primary TKA in the following scenarios: patients in medical institutions with a high prevalence of methicillin-resistant staphylococcus aureus (MRSA) infections, patients with potential preoperative MRSA colonization, or patients with cephalosporin allergy.

Chinese Journal of Industrial Hygiene and Occupational Diseases 2025;43(1):54-57
Objective:To investigate the relationship between serum tumor markers and the severity of silicosis. Methods:In April 2024, A cluster sampling was conducted on the pulmonary rehabilitation centers in Jiangsu Province. A retrospective analysis was carried out on 629 patients who received follow-up physical examinations for pneumoconiosis from April to November 2023 at the selected rehabilitation centers. 281 healthy volunteers who underwent occupational health examinations at our institution were selected as the blank control group, and 384 dust exposed workers from a coal mining enterprise were selected as the dust exposed control group. Detect the levels of peripheral bloodtumor markers carcinoembryonic antigen CEA, carbohydrate antigen CA199, and neuron-specific enolase NSE in specimens. Measure the patient's forced vital capacity (FVC), forced expiratory volume in one second (FEV 1), and lactate acid dehydrogenase (LDH) levels in the blood. The Jonckheere-Terpstra test, a non-parametric test for ordered independent samples, was used to compare the expression levels of tumor markers among multiple groups. Spearman's rank correlation analysis was adopted to explore the correlations between serum tumor markers and the staging of pneumoconiosis as well as lung function parameters. Results:Among the 629 patients with pneumoconiosis, 454 were in stage Ⅰ, 150 were in stage Ⅱ, 25 were in stage Ⅲ. There were significant differences in serum CEA, CA199 and NSE levels among patients with different periods of pneumoconiosis, blank control group and dust-exposed control group ( P<0.001). Serum CEA, CA199 and NSE levels were positively correlated with pneumoconiosis stage ( P<0.05). There is a negative correlation between serum CA199 levels and FEV 1% in patients with pneumoconiosis ( r=-0.082, P<0.05). The serum CA199 level was positively correlated with LDH in patients with pneumoconiosis ( r=0.108, P<0.05) .The serum NSE level of pneumoconiosis patients was negatively correlated with FVC% ( r=-0.079, P<0.05) .There is a positive correlation between serum NSE levels and LDH in patients with pneumoconiosis ( r=0.435, P<0.05) . Conclusion:The serum CA199 and NSE of pneumoconiosis patients are directly related to the severity and stage of the disease, which have practical significance in judging the treatment effect of pneumoconiosis patients and monitoring the development of the disease.
