1.Posttraumatic superior mesenteric artery syndrome following a chance fracture in a child: a case report

Superior mesenteric artery syndrome (SMAS) is a rare but important cause of acute upper gastrointestinal obstruction resulting from compression of the duodenal third portion between the superior mesenteric artery and aorta. While SMAS has been described after corrective spinal surgery, it is rarely reported following pediatric trauma. We describe an 8-year-old boy involved in a high-speed motor vehicle collision who sustained multiple severe injuries, such as traumatic diaphragmatic hernia, mesenteric vascular injury, hepatosplenic laceration, and L1–L2 seatbelt-related Chance fracture-dislocation. After surgical stabilization of the spine on day 6, he presented on day 12 with bilious vomiting and abdominal distension, indicating the presence of intestinal obstruction. Contrast-enhanced computed tomography and an upper gastrointestinal series demonstrated narrowing of aorto-mesenteric angle with compression of the duodenal third portion, confirming the diagnosis of SMAS. Through nasojejunal tubal feeding, he was able to meet nutritional requirements without parenteral nutrition. This case underscores the need for heightened awareness of SMAS in pediatric trauma patients who develop postoperative duodenal obstruction symptoms, as prompt recognition and conservative management in the postoperative setting can lead to excellent outcomes.
2.Analytical Interference of Exemestane with Androstenedione Immunoassays

Background:
Exemestane, an aromatase inhibitor commonly used for breast cancer treatment, shares structural similarities with sex steroids analyzed in clinical laboratories. We aimed to investigate the influence of exemestane cross-reactivity in the measurement of sex steroids across various immunoassays.
Methods:
We conducted a multicenter study involving measurements of androstenedione, testosterone, estradiol, progesterone, and 17-hydroxyprogesterone in serum samples from women undergoing exemestane therapy (N = 15; 25 mg/day). Measurements were performed using liquid chromatography-mass spectrometry (LC-MS) and various commercially available chemiluminescence immunoassays, ELISA, and radioimmunoassay. In-vitro cross-reactivity was assessed by adding exemestane and 17-hydroexemestane to serum samples.
Results:
Patients undergoing exemestane therapy had markedly falsely elevated androstenedione results in all immunoassays evaluated (N = 4), which correlated with serum exemestane levels. In-vitro experiments confirmed this interference to be caused by cross-reactivity with exemestane. Additionally, one immunoassay yielded falsely elevated estradiol results in 20% of patients. However, in-vitro experiments did not confirm this to be caused by cross-reactivity with exemestane or 17-hydroexemestane.
Conclusions
Exemestane cross-reacts with androstenedione immunoassays, causing falsely elevated results in treated patients. This analytical interference may raise unnecessary concerns, leading to expensive diagnostic workups.
3.Roadmapping technique in the hybrid operating room for the microsurgical treatment of complex intracranial aneurysms

Objective:
To describe the roadmapping technique and our three-year experience in the management of intracranial aneurysms in the hybrid operating room.
Methods:
We analyzed all patients who underwent surgical clipping for cerebral aneurysms with the roadmapping technique from January 2017 to September 2019. We report demographic, clinical, and morphological variables, as well as clinical and radiological outcomes. We further describe three illustrative cases of the technique.
Results:
A total of 13 patients were included, 9 of which (69.2%) presented with subarachnoid hemorrhage, with a total of 23 treated aneurysms. All patients were female, with a mean age of 47.7 years (range 31-63). All cases were anterior circulation aneurysms, the most frequent location being the ophthalmic segment of the internal carotid artery (ICA) in 11 cases (48%), followed by posterior communicating in 8 (36%), and ICA bifurcation in 2 (8%). Intraoperative clip repositioning was required in 9 aneurysms (36%) as a result of the roadmapping technique in the hybrid operating room. There were no residual aneurysms in our series, nor reported mortality.
Conclusions
The roadmapping technique in the hybrid operating room offers a complementary tool for the adequate occlusion of complex intracranial aneurysms, as it provides a real time fluoroscopic-guided clipping technique, and clip repositioning is possible in a single surgical stage, whenever a residual portion of the aneurysm is identified. This technique also provides some advantages, such as immediate vasospasm identification and treatment with intra-arterial vasodilators, balloon proximal control for certain paraclinoid aneurysms, and simultaneous endovascular treatment in selected cases during a single stage.
4.Anti-inflammatory and utero-relaxant effect of α-bisabolol on the pregnant human uterus.

The aim of this study was to evaluate the in vitro anti-inflammatory and utero-relaxant effect of α-bisabolol on the pregnant human myometrium. Samples from the pregnant human myometrium were used in functional tests to evaluate the inhibitory effect of α-bisabolol (560, 860, 1,200 and 1,860 µM) on spontaneous myometrial contractions. The intracellular cyclic adenosine monophosphate (cAMP) levels generated in response to α-bisabolol in human myometrial homogenates were measured by ELISA. The anti-inflammatory effect of α-bisabolol was determined through the measurement of two pro-inflammatory cytokines, tumor necrosis factor-α (TNFα) and interleukin (IL)-1β, and the anti-inflammatory cytokine IL-10, in pregnant human myometrial explants stimulated with lipopolysaccharide (LPS). Forskolin was used as a positive control to evaluate the cAMP and cytokine levels. α-Bisabolol was found to induce a significant inhibition of spontaneous myometrial contractions at the highest concentration level (p < 0.05). α-Bisabolol caused a concentration-dependent decrease in myometrial cAMP levels (p < 0.05) and a concentration-dependent decrease in LPS-induced TNFα and IL-1β production, while IL-10 production did not increase significantly (p>0.05). The anti-inflammatory and utero-relaxant effects induced by α-bisabolol were not associated with an increase in cAMP levels in pregnant human myometrial samples. These properties place α-bisabolol as a potentially safe and effective adjuvant agent in cases of preterm birth, an area of pharmacological treatment that requires urgent improvement.
