1.Prognostic evaluation of liver transplantation for acute-on-chronic liver failure

Acute-on-chronic liver failure (ACLF) is an acute deterioration of liver function occurring on the basis of chronic liver disease, accompanied by failure of the liver and extrahepatic organs, and is associated with a high short-term mortality rate. Liver transplantation is the only curative treatment for patients with ACLF. However, the shortage of donor livers and limitations of the organ allocation system mean that only a minority of patients can receive transplants. The current organ allocation system based on the model for end-stage liver disease (MELD) score may underestimate the urgency of liver transplantation for ACLF patients. Therefore, it is urgent to develop better assessment tools to determine which ACLF patients are most likely to benefit from liver transplantation. This article reviews the current mainstream definitions of ACLF, the selection of candidates for liver transplantation in ACLF, and the prognostic scoring systems for liver transplantation in ACLF, both domestically and internationally, in order to provide a reference for the prognostic assessment of liver transplantation in ACLF patients.
2.Dynamic changes of prognostic scores and related clinical indicators in hepatitis B virus-related acute-on-chronic liver failure patients without underlying liver cirrhosis and their relationship with clinical outcomes

ObjectiveTo investigate the dynamic trajectories of prognostic scores and key clinical indicators in hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF) patients without liver cirrhosis, to clarify their association with outcomes, and to provide new evidence for individualized prognostic assessment. MethodsA prospective study was conducted for the data of 154 non-cirrhotic HBV-ACLF patients who attended Beijing YouAn Hospital of Capital Medical University from January 2016 to December 2023, including prognostic scores and key biochemical indicators on Days 3, 7, 14, 21, and 28 of the disease course. According to the outcome of patients at 1 year, they were divided into death/liver transplantation group with 43 patients, liver cirrhosis group with 23 patients, and non-liver cirrhosis group with 88 patients, and the trajectory heterogeneity of different outcome subgroups was analyzed. A one-way analysis of variance was used for comparison of normally distributed continuous data among the three groups; the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data among the three groups; the Wilcoxon test was used between two groups. the chi-square test was used for comparison of categorical data between groups. The mean and its 95% confidence interval (CI) were calculated for each indicator at difference time points; the linear interpolation method was used to connect the means at adjacent time points and construct the group-specific longitudinal trend curve; the 95%CI was visualized using the semi-transparent ribbon area, with the transparency parameter (α=0.2) optimized to enhance the visual discrimination of overlapping intervals across multiple groups. A linear mixed-effects model was used to compare the longitudinal changing trend of each indicator between the patients with different outcomes; likelihood ratio was used to evaluate the significance of the interaction effect between time and group, and in case of the significant interaction effect, the slope based on the estimated marginal mean was used for comparison between two groups. ResultsThere were significant differences between the three groups in the incidence rates of ascites and grade Ⅲ — Ⅳ hepatic encephalopathy, MELD score, MELD-Na score, CLIF-C ACLF score, COSSH-ACLF Ⅱ score, total bilirubin (TBil), international normalized ratio (INR), alpha-fetoprotein, blood sodium, alanine aminotransferase, and procalcitonin at the baseline(all P0.05). The analysis of dynamic trajectories showed that the death/liver transplantation group had high levels of prognostic scores and the biochemical parameters of TBil and INR (TBil400 μmol/L, INR2.5), as well as a low level of platelet count (PLT) (100×10⁹/L). The non-liver cirrhosis group had rapid improvements in indicators, with TBil200 μmol/L, INR1.5, and PLT100×10⁹/L by day 28, while the liver cirrhosis group showed a trend of recovery, with TBil200 μmol/L, INR2.0, and PLT 100×10⁹/L on day 28, with significant global heterogeneity in the temporal trends of the above indicators across the three groups (all P0.01). ConclusionDynamic monitoring of prognostic scores and key clinical indicators can effectively stratify the 1-year outcomes of non-cirrhotic patients with HBV-ACLF. Patients with poor prognosis were typically characterized by INR 2.5 and TBil 400 μmol/L. Among those who survived beyond 1 year, individuals who subsequently progressed to cirrhosis were frequently identified by the presence of INR 1.5, TBil 200 μmol/L, and PLT 100×10⁹/L at day 28.

Metastasis serves as an indicator of malignancy and is a biological characteristic of carcinomas. Epithelial-mesenchymal transition (EMT) plays a key role in the promotion of tumor invasion and metastasis and in the enhancement of tumor cell aggressiveness. Prostaglandin E synthase 3 (p23) is a cochaperone for heat shock protein 90 (HSP90). Our previous study showed that p23 is an HSP90-independent transcription factor in cancer-associated inflammation. The effect and mechanism of action of p23 on lung cancer metastasis are tested in this study. By utilizing cell models in vitro and mouse tail vein metastasis models in vivo, the results provide solid evidence that p23 is critical for promoting lung cancer metastases by regulating downstream CXCL1 expression. Rather than acting independently, p23 forms a complex with RNA-binding motif protein 14 (RBM14) to facilitate EMT progression in lung cancer. Therefore, our study provides evidence for the potential role of the RBM14-p23-CXCL1-EMT axis in the metastasis of lung cancer.
5.Post-aging comparative study of snowplow and conventional techniques for the resin restoration of defective primary teeth.

OBJECTIVES:
This study aims to evaluate the differences in shear bond strength, marginal adaptation, and nano-microleakage after aging among snowplow, layered filling, and lining techniques applied to the resin-bonded restoration of defective primary teeth.
METHODS:
In this study, 51 freshly extracted, crown-intact primary anterior teeth and 30 primary molars were collected. The experimental groups were as follows: layered filling group, lining group, and snowplow group. Experiments were performed to compare the differences in shear bond strength, marginal integrity, and silver ion nano-microleakage after aging among these groups.
RESULTS:
The median shear bond strength of the layered filling group, lining group, and snowplow group were 2.45, 5.72, and 9.43 MPa, respectively. The values for lining group and snowplow group were significantly higher than that for layered filling group (P<0.05). No statistically significant difference was found between lining group and snowplow group (P>0.05). The median overall margin integrity of the layered filling group, lining group, and snowplow group were 55.38%, 48.25%, and 65.63%, respectively. The difference among the three groups was not statistically significant (P>0.05). The median percentages of silver ion nano-microleakage in the layered filling group, lining group, and snowplow group were 11.71%, 9.47%, and 11.55%, respectively. The difference among the three groups was not statistically significant (P>0.05).
CONCLUSIONS
Applying the snowplow technique to restore defective primary teeth can improve the bond strength and margin integrity and reduce nano-microleakage.

6.A universal counter-selection strategy based on replacement of sgRNA expression cassettes targeting multi-copy genes.

Selection markers are essential tools in gene editing, the utility of such systems is inherently constrained by species-specific limitations, governed by divergent host genetic backgrounds and metabolic compatibility. To address this limitation, we leveraged the CRISPR/Cas9 system to develop a universal counter-selection tool. We designed and introduced an sgRNA expression cassettes as counter-selection markers, which directs the Cas9 protein to target and cleave genomic DNA, allowing for the selection of the strains where the sgRNA expression cassette has been replaced. Optimized to target multiple copy sites with sgRNA, this system significantly enhances cell lethality, boosting counter-selection efficiency to over 85.00%. This counter-selection tool is not limited to single strains and is suitable for various scenarios, including multi-copy plasmid assembly and plasmid editing, demonstrating broad application potential.

7.A multi-enzyme cascade process for the preparation of L-phosphinothricin.

L-phosphinothricin (L-PPT) is an efficient broad-spectrum herbicide. To realize the multi-enzyme catalytic preparation of L-PPT, we constructed an engineered strain Escherichia coli YM-1 for efficient expression of D-amino acid transaminase, which could catalyze the generation of the intermediate 2-oxo-4-[(hydroxymethylphosphonyl)] butyric acid (PPO) from D-phosphinothricin (D-PPT). In addition, E. coli pLS was constructed to co-express glutamate dehydrogenase and glucose dehydrogenase, which not only catalyzed the generation of L-PPT from PPO but also regenerated the coenzyme nicotinamide adenine dinucleotide phosphate (NADPH). A fed-batch fermentation process was then established for E. coli YM-1 and pLS, and the apparent activities of D-amino acid transaminase and glutamate dehydrogenase were increased by 22.68% and 100.82%, respectively, compared with those in shake flasks. The process parameters were optimized for the catalytic preparation of L-PPT by whole-cell cascade of E. coli YM-1 and pLS with D, L-PPT as the substrate. After reaction for 8 h, 91.36% conversion of D-PPT was achieved, and the enantiomeric excess of L-PPT reached 90.22%. The findings underpin the industrial production of L-PPT.

8.Application of"one disease,one product"rehabilitation nursing in postoperative care of carpal tunnel syndrome under follow-up nursing mode

Objective To explore the efficacy of the"one disease,one product"rehabilitation nursing under the follow-up nursing mode in the postoperative recovery process of patients with carpal tunnel syndrome(CTS).Methods A total of 100 CTS patients who were admitted to The Second Affiliated Hospital of Naval Medical University from January 2022 to December 2023 were randomly assigned to observation group or control group,with 50 patients in each group.The control group was given a routine postoperative rehabilitation nursing,while the observation group was additionally given the"one disease,one product"rehabilitation nursing under follow-up care.The clinical efficacy,visual analogue scale(VAS)score,quality-of-life score,psychological status score(self-rating depression scale[SDS]and self-rating anxiety scale[SAS]),wrist recovery(grip strength,pinch strength,patient rated wrist evaluation[PRWE]score),and nursing satisfaction were compared between the two groups.Results The total effective rate in the observation group was significantly higher than that in the control group(98.00%[49/50]vs.86.00%[43/50],P<0.05).After treatment,VAS score,SDS score,and SAS score were decreased,and quality-of-life scores,grip strength,pinch strength,and PRWE score were increased in both groups;VAS score,SDS score,and SAS score in the observation group were significantly lower than those in the control group,while quality-of-life scores,grip strength,pinch strength,and PRWE score in the observation group were significantly higher than those in the control group(all P<0.05).Patient satisfaction in the observation group was significantly higher than that in the control group(98.00%[49/50]vs.88.00%[44/50],P<0.05).Conclusion The"one disease,one product"rehabilitation nursing program under the follow-up nursing mode has a better application effect than routine postoperative rehabilitation nursing plan in postoperative nursing of patients with CTS.
9.Efficacy of group play therapy on cognitive function and negative symptoms in hospitalized female patients with schizophrenia

Background Schizophrenia patients are often accompanied by negative symptoms and severe cognitive impairment,but effective interventions intended to alleviate such condition are currently limited.Existing researches on group play therapy for schizophrenia is still in its initial stages,and such therapy has the potential to contribute to symptoms improvement.Objective To explore the efficacy of group play therapy on improving cognitive function and negative symptoms in hospitalized female patients with schizophrenia,so as to provide references for clinical intervention in such group.Methods This study involved 40 female patients with schizophrenia who received inpatient treatment at the Third People's Hospital of Fuyang from April 2022 to May 2023 as well as met the diagnostic criteria of the International Classification of Diseases,tenth edition(ICD-10).They were divided into study group(n=20)and control group(n=20)according to the random number table method.Both groups received routine treatment,and the study group received 10 sessions of group play therapy for 5 weeks on the basis.At baseline,Scale for Assessment of Positive Symptoms(SAPS),Scale for Assessment of Negative Symptoms(SANS),Self-rating Depression Scale(SDS),Nurses' Observation Scale for Inpatient Evaluation(NOSIE),and Repeatable Battery for the Assessment of Neuropsychological Status(RBANS)were used for assessment.Post-therapy evaluation was conducted by using SAPS,SANS and RBANS.Patients' baseline SAPS scores,SANS scores,RBANS total scores,and various factor scores of RBANS were used as covariates,and covariance analysis was adopted to compare the scores of each scale between the two groups after treatment.Results A total of 39 patients went through the whole study.Results of covariance analysis showed that the SANS score of study group was lower than that of control group,while several scores of RBANS(including total score,immediate memory factor score,speech function factor score and attention factor score)were all higher than those in control group.Significant difference was observed between two groups in scores of both scales above(F=13.408,10.331,4.932,9.967,10.010,P<0.05 or 0.01).Conclusion Group play therapy may help improve negative symptoms and cognitive function in hospitalized female patients with schizophrenia.
10.Comparison of the effects of transperineal prostate laser ablation versus transurethral resection of the prostate in the treatment of benign prostatic hyperplasia:a single center prospective randomized controlled study

Objective To compare the clinical efficacy of transperineal prostate laser ablation(TPLA)and transurethral resection of the prostate(TURP)in the treatment of benign prostatic hyperplasia(BPH).Methods A total of 60 BPH patients diagnosed during Oct.2021 and Oct.2022 at Tangdu Hospital were selected as the research subjects and randomly divided into the TPLA group(n=30)and TURP group(n=30).The intraoperative bleeding volume,operation time,catheter indwelling time,length of hospital stay,postoperative sexual dysfunction,and surgical related complications were compared between the two groups.The international prostate symptom score(IPSS),international index of erectile function-5(IIEF-5),maximum urinary flow rate(Qmax),quality of life score(QoL),postvoid residual(PVR)and prostate volume(PV)were compared between the two groups before surgery and 1,3,and 12 months after surgery.Results The TPLA group had significantly less intraoperative bleeding volume,shorter operation time and length of hospital stay compared to the TURP group,but longer catheter indwelling time(P<0.05).Both groups showed significant improvement in IPSS and Qmax 1,3,and 12 months postoperatively compared to preoperative(P<0.05),the IPSS of the TPLA group was significantly higher than that of the TURP group 1 and 3 months after surgery(P<0.05);the Qmax of TPLA group 1,3,and 12 months after surgery was lower than that of the TURP group(P<0.05).The IIEF-5 score was significantly better in the TPLA group than in the TURP group after surgery(P<0.05).The postoperative QoL,PV,and PVR levels in both groups improved after surgery(P<0.05),the QoL of the TPLA group was lower than that of the TURP group 1 and 12 months after surgery(P<0.05),the PV and PVR of the TPLA group were higher than those of the TURP group 1,3,and 12 months after surgery(P<0.05).The incidence of surgery-related complications(3.33%vs.26.67%)and postoperative sexual dysfunction(3.33%vs.36.67%)in the TPLA group were lower than those in the TURP group(P<0.05).Conclusion TPLA shows significant efficacy in treating BPH with minimal impact on the sexual function.It provides a new approach for BPH patients and can serve as an effective complementary method in clinical practice.
