1.Chinese experts' consensus on principles of preoperative hair removal

To formulate an expert consensus on the principles of preoperative hair removal and provide scientific guidance for standardized removal of hair before surgical procedures so as to reduce the incidence of surgical site infections.METHODS Led by the Hospital Management Institute of National Health Commission of the People's Republic of China,this consensus was reached with the joint efforts from the expects of relevant fields such as surgeries,interventional therapies,nursing,and infection prevention and control.The consensus facilitates the classification and evaluation of literatures by following the evidence grade formulated by Oxford Evidence-based Medicine Center and focuses on the association of preoperative hair removal with surgical site infection,it reaches the evidence grade of expert consensus and recommendation intensity by integrating with discussions on meetings and clinical experience of the expects from relevant fields.RESULTS A total of 6 items of consensus were reached by summarizing the latest evidence on the aspects including the indications for preoperative hair removal,tools,range,timing and places.CONCLUSION The consensus,to some extent,make supplements to and complete the exiting regulations and standards.It provides guidance for the medical institutions to carry out the preoperative hair removal.

3.Relationship between cortical fragments and avascular necrosis of femoral head in young adult femoral neck fractures based on CT imaging.

OBJECTIVE:
To explore the effect of femoral neck cortical fragments and surfacial area on avascular necrosis(AVN) after internal fixation of femoral neck fractures based on CT imaging in young adult.
METHODS:
From January 2011 to January 2021, 218 young and middle-aged patients with femoral neck fractures were reverse triangle fixed with three cannulated lag screws, including 104 males and 114 females;aged 19 to 60 years old with a mean of (48.22±10.58) years old. The preoperative waiting time≤48 h was 155 cases, and the preoperative waiting time> 48 h was 63 cases; 68 cases were comminuted cortex of the femoral neck, 32 of them with superficial area of the comminuted cortex>25 mm2. All patients were followed up for at least 2 years, the age, gender, injury side, body mass index, injury mechanism, preoperative waiting time, operation time, Garden classification, whether femoral neck cortex fracture and size by CT analysis and complications were analyzed the correlation to the definite occurrence of AVN. The AVN group and the non-AVN group were divided according to the final efficacy results.Univariate analysis yielded significant independent variables, Multiple Logistic regression analysis was further performed to identify the independent risk factors for AVN.
RESULTS:
By univariate analysis, the AVN and non-AVN groups were significant in injury mechanism(χ2=18.552, P=0.000), preoperative waiting time( χ2=2.065, P=0.033), Garden classification (χ2=12.598, P=0.006), comminuted cortex (χ2=16.357, P=0.000), and the superficial area of the comminuted cortex(χ2=23.371, 0.000, P=0.000). Multiple Logistic regression indicated injury mechanism high energy injury[Exp(B)=3.487, 95%CI(1.583, 10.464), P=0.002], comminuted cortex of the femoral neck[Exp(B)=2.494, 95%CI(1.315, 10.861), P=0.002];the superficial area of the comminuted cortex>25 mm2[Exp(B)=22.161, 95%CI(3.922, 1242.828), P=0.001] were independent risk factors for AVN after internal fixation of femoral neck fractures.
CONCLUSION
High energy injury mechanism, with comminuted cortex of the femoral neck and the superficial area of the comminuted cortex> 25 mm2 were independent risk factors for AVN.The larger the superficial area of the cortex fragment may suggest more complicated injury mechanism and worse outcome.

4.Diagnostic value of contrast-enhanced ultrasound combined with tumor markers in benign and malignant ovarian tumors

Objective To explore the diagnostic value of contrast-enhanced ultrasound combined with tumor markers in benign and malignant ovarian tumors.Methods The clinical data of 100 patients with ovarian tumors treated in our hospital from May 2016 to May 2017 were analyzed retrospectively.The parameters of contrast-enhanced ultrasonography (CEUS) for benign and malignant ovarian tumors were analyzed.The levels of tumor markers were measured and the ultrasound imaging was analyzed.Analysis of the detection rate of benign and malignant ovarian tumors by contrast-enhanced ultrasound alone or in combination with tumor markers.Results Compared with benign ovarian tumors,ovarian malignant tumors have the characteristics of uneven enhancement,early onset and peak time of contrast enhancement,and higher rate of enhancement (P < 0.05);compared with benign ovarian tumors,serum CA125 and CA199 levels in patients with ovarian malignant tumors were higher,with significant difference (P < 0.05).The detection rates of contrast-enhanced ultrasound combined with tumor markers were higher than those of ultrasound or tumor markers alone,and were in accordance with pathological criteria.Conclusions The application of combined contrast-enhanced ultrasound and tumor markers in the diagnosis of ovarian benign and malignant tumors has a high detection rate,can significantly improve the accuracy of clinical detection,and provide a strong basis for treatment.
5.Effect of Di-(2-ethylhcxyl) phthalate exposure on blood-testis barrier integrity in rats.

OBJECTIVETo investigate mechanism of di-(2-ethylhcxyl)phthalate (DEHP) exposure in causing blood-testis barrier (BTB) impairment in rats.
METHODSTwo-months-old male SD rats were randomly divided into corn oil control group and DEHP (750 mg/kg) exposure group for daily intragastic treatment for 30 consecutive days. After the treatments the rats were examined for histomorphological changes of the testicle using HE staining and the expressions of the junction proteins N-cadherin β-catenin, occludin and connexin43 of the BTB using Western blot. In the in vitro study, the vitality and ROS generation level in Sertoli cells exposed to different concentrations of DEHP were examined with MTT and ROS assay kits, respectively, and Nrf2 and p-p38 expressions were detected with Western blot.
RESULTSCompared with the control group, the rats with DEHP exposure showed structural damage of the seminiferous tubule and polarity loss of the spermatids. DEHP exposure caused significantly decreased expressions of occludin and connexin43 but increased expressions of N-cadherin and β-catenin in the testicle tissues of the rats (P<0.05). The vitality of Sertoli cells was obviously decreased and ROS level increased significantly after exposure of the cells to increasing concentrations of DEHP, which also resulted in significantly up-regulated Nrf2 and p-p38 expressions (P<0.05).
CONCLUSIONSDEHP exposure causes increased oxidative stress in the Sertoli cells of the testis, activates p38 MAPK signaling pathway, and results eventually in impaired spermatogenesis in rats.
6.Retroperitoneoscopic adrenalectomy in semilateral supine position

Objective To discuss the semilateral supine position for retroperitoneoscopic adrenalectomy. Methods From Jan. 2006 to Dec. 2008, 36 patients (20 males and 16 females with mean age of 43 years) underwent retroperitoneoscopic adrenalectomy in 60° -70° semilateral supine position. There were adrenal cortex adenomas in 18 cases, pheochromocytoma in 6 cases, adrenal cysts in 3 cases, myelolipoma in 2 cases, gangliocytoma in 1 case, lymphangioma in 1 case, metastatic tumor in 1 case and corticohyporplasia in 4 cases. The mean diameter of the tumors was 2.6 cm( 0.5 - 7.7 cm ). The tumors were superior to the renal pole in 5 cases, anteromedial in 10 cases and superomedial in 17 cases. The three ports that were usually used in lateral position and were placed anteriorly to create retroperitoneal place: the first port was placed 2 -4 cm superior to the iliac crest along the anterior axillary line, the other two were placed just below the costal margin along the midaxillary line and at the same level along the midclavicular line, and dissected along the anterior surface of kidney to its superomedial aspect, so as to avoid the hampering of the kidney in the exposing of the diseased adrenal gland. Results The procedure was completed successfully in all of the cases with the operating time of 37 - 145 min ( mean 69 min) and intraoperative blood loss of 30 - 100 ml (mean 48 ml). Six cases had rupture of peritoneum, which were sutured and the procedure was continued to completion. The postoperative hospital stay was 3 -8 d (mean 5 d ). Thirty-five patients were available for follow-up of 3 - 28 months ( mean 14 months). The case of metastatic tumor died of the primary diseases in the 12th month postoperatively. No other complication was found. Conclusion With this alternative position and ports' location, the procedure of retroperitoneoscopic adrenalectomy could be easier and safer than the conventional position.
7.Pharmacokinetics of single-dose astromicin sulfate in Chinese healthy volunteers

Objective To evaluate the pharmacokinetics of single dose of astromicin injection in healthy Chinese volunteers.Methods A randomized,three period cross-over,latin-square design study was conducted.Twelve male volunteers were administrated intravenously 150,200 and 250 mg of astromicin within 0.5 h infusion.The concentrations of astromicin in serum and urine were determinated by precolumn derivatization-fluorescence HPLC method.Results The main pharmacokinetic parameters of three dose(150,200,250 mg)were as follows:C_(max) was(11.27±2.65),(15.83±3.14)and(21.97±3.67)mg·L~(-1);t_(1/2)β was(2.56±0.72),(2.97±1.30)and(2.66±1.39)h;AUC_(0-t)was(30.92±8.49),(39.58±11.57)and(47.31±11.59)mg·h·L~(-1);AUC_(0-∞) was(31.78±8.73),(40.50±11.56)and(48.20±11.95)mg·h·L~(-1);V_c was(11.49±2.61),(10.69±1.52)and (10.20±1.93)L;CL was(5.33±2.03),(5.49±1.51)and(5.68 ±1.35)L·h~(-1),respectively.Urinary recovery rates was(78.75±7.00)%,(78.95±12.57)%and(79.94±10.28)%,respectively.Conclusion The main pharmacokinetic parameters of astromicin in difereneut doses were nearly fit linear dynamic feature,and the drug maily excreted through kidney.
